Control of cell division in hepatoma cells by exogenous heparan sulfate proteoglycan.

The effects of cell surface heparan sulfate proteoglycan (HSPG) prepared from log and confluent monolayers of a rat hepatoma cell line on hepatoma cell growth were studied. When HSPG isolated from confluent cells was added exogenously to log phase cells, it was internalized and free heparan sulfate (HS) chains appeared transiently in the nucleus. Concurrently, the growth of the treated cells was inhibited, but the cells resumed logarithmic growth as the level of nuclear HS fell, and the cells grew to confluence and became contact inhibited. When HSPG prepared from log-phase hepatoma cells was added exogenously to log phase cells, it was internalized but very little of the internalized HS appeared in the nucleus, and there was no change in the rate of cell growth. However, when the rate of cell growth was reduced by culture of the cells in serum- and insulin-deficient medium, HSPG prepared from log-phase cells stimulated the growth rate of these slow-growing cells. The cell cycle dependency of HSPG uptake and growth inhibition was studied in cultures synchronized by a thymidine/aphidicolin double block. When [35SO4]HSPG from confluent cells was added to synchronized cells just as they were released from the second block, a portion of the [35SO4]HSPG was internalized and [35SO4]HS appeared in the nucleus. However, at mitosis the [35SO4]HS disappeared almost completely from all of the cellular pools, and after mitosis, more of the [35SO4]HSPG was taken up and [35SO4]HS reappeared in the nucleus and remained in the nucleus until the cells divided again. When cultures were released from the aphidicolin block, both control and HSPG-treated cells progressed through the S, the G2, and the M phases of the cell cycle. However, the length of the G1 phase of the cycle was increased in the HSPG-treated cells. The treated cultures then progressed through the second S, G2, and M phases. Thus, the inhibition of cell division occurred in the G1 phase of the cell cycle, prior to the G1/S boundary. Addition of the HSPG to the synchronized cultures just after the first mitosis resulted in an immediate arrest of the cell cycle in G1.(ABSTRACT TRUNCATED AT 400 WORDS)

Continence and reflex pressure profile after surgery to correct the imperforate anus.

To clarify the cause of constipation which follows surgery for the supralevator type of disorder associated with the imperforate anus, rectal compliance, percentage maximum static anorectal pressure, and reflex profile were measured by anorectal manometry in 108 normal controls and 42 patients. Patients with constipation had a low percentage anorectal pressure (50%), high rectal compliance, associated with megarectum, and defecation of the staining type with constipation due to a reaction in anorectal motility.

Presence of anti-DNA antibody in diabetes mellitus: its relation to the duration of diabetes and diabetic complications.

In seven patients with insulin-dependent diabetes mellitus (IDDM) and 86 patients with non-insulin-dependent diabetes mellitus (NIDDM), serum anti-DNA antibody was measured by a semiquantitative radioimmunoassay (RIA) method. Prevalence of positive anti-DNA antibody (more than 20 U/mL) was five of seven in IDDM patients, 15 of 36 in NIDDM patients with insulin therapy, and seven of 50 in NIDDM patients without insulin therapy. None of normal subjects or patients with impaired glucose tolerance (IGT) showed positive anti-DNA antibody. The titer of anti-DNA antibody was higher in IDDM patients than in age-matched normal subjects (mean +/- SD; 22.1 +/- 15.3 v 6.5 +/- 2.2 U/mL, P less than .05). In patients with NIDDM, the antibody titer regardless of insulin treatment, was higher than in age-matched subjects with IGT (18.5 +/- 13.1 U/mL in NIDDM patients receiving insulin, 14.8 +/- 8.1 U/mL in NIDDM patients not receiving insulin, and 8.8 +/- 3.9 U/mL in IGT patients [P less than .001] for either of NIDDM groups v IGT). The titer of anti-DNA antibody was positively correlated with the duration of diabetes (r = .413, P less than .001) and with the postprandial blood glucose level (r = .311, P less than .01) in NIDDM patients when all of them were combined and analyzed as a group.(ABSTRACT TRUNCATED AT 250 WORDS)

Plasma atrial natriuretic peptide, plasma renin activity and aldosterone during treatment of hyperthyroidism due to Graves' disease.

Plasma atrial natriuretic peptide (ANP), plasma renin activity (PRA) and aldosterone were consecutively measured during methimazole treatment in patients with hyperthyroidism due to Graves' disease. ANP values of untreated hyperthyroid patients varied greatly from patient to patient, but decreased progressively with a decrease of serum thyroid hormone concentration during methimazole treatment. PRA was elevated in hyperthyroid patients but less aldosterone was secreted as evidenced by lower aldosterone/PRA ratio in these patients than in normal subjects and in hypertensive patients treated with thiazide. In addition, aldosterone/PRA ratio increased progressively with a decrease of ANP during methimazole treatment. The data indicated that ANP secretion was increased and ANP thus secreted depressed aldosterone secretion in hyperthyroid patients. Propranolol depressed pulse rate but failed to affect ANP secretion. It is suggested that thyroid hormone specifically acts on myocytes to stimulate ANP secretion but physiologic significance of such increased ANP secretion remains to be solved.

Co-occurrence of hypercalciuria and hypouricaemia in type 2 diabetic patients.

The relationship between the urinary excretion of calcium (Ca2+) and uric acid was investigated in 151 Type 2 diabetic patients and 48 normal subjects. In the diabetic patients, uric acid clearance/creatinine clearance (Clurate/Clcr) was higher and the serum level of uric acid was lower than in the normal subjects (Clurate/Clcr: 10.9 +/- 5.8 vs 8.1 +/- 2.6%, p less than 0.001; serum uric acid: 3.4 +/- 86 vs 357 +/- 89 mumol l-1, p less than 0.001). Calcium clearance/Clcr (Clca/Clcr) also increased in the diabetic patients, as did urinary excretion rate, but the serum Ca2+ level was not different to normal control subjects (Clca/Clcr: 2.29 +/- 1.59 vs 1.56 +/- 0.98%, p less than 0.001; Ca2+ excretion rate: 2.24 +/- 1.67 vs 1.63 +/- 1.11 mmol day-1, p less than 0.01; serum Ca2+ level: 2.34 +/- 0.11 vs 2.33 +/- 0.08 mmol l-1). In the diabetic patients, Clcr positively and the serum uric acid negatively correlated with the urinary excretion of Ca2+ (p less than 0.001 for both correlations in the multivariate regression analysis). These data suggest that the diabetic patients have increased fractional excretion of both Ca2+ and uric acid.

Age-related changes of thyroid function and immunologic abnormalities in patients with hyperthyroidism due to Graves' disease.

In order to clarify the reasons why hyperthyroidism due to Graves' disease is mild in aged patients compared to young patients, we examined the degree of hyperthyroidism and immune abnormalities in 371 untreated patients. The patients were divided into nine groups based on their age; groups A to I consisted of patients aged 0-9, 10-19, 20-29, 30-39, 40-49, 50-59, 60-69, 70-79, and 80-89 years, respectively. Serum thyroxine (T4), triiodo-thyronine (T3), and thyroglobulin levels were highest in group B (patients aged 10-19 years) and decreased progressively with advancing age. T3/T4 ratio was also highest in group B and decreased progressively with age. These data reconfirmed that the degree of hyperthyroidism is milder in older patients. The prevalence of positive microsomal and thyroglobulin antibodies was highest in group B (patients aged 10-19 years) and progressively lower in older groups, suggesting that an association with Hashimoto's thyroiditis is less prevalent in aged patients. We suggest that hyperthyroidism due to Graves' disease is mild in aged patients because responsiveness of the thyrocyte to the abnormal stimulator, thyrotropin-receptor antibody, may be reduced. Such reduced responsiveness may not be due to Hashimoto's thyroiditis, since the prevalence of antithyroid antibodies is lower in aged than in younger patients with hyperthyroidism due to Graves' disease.

Echocardiographic studies on the relationship between atrial fibrillation and atrial enlargement in patients with hyperthyroidism of Graves' disease.

In order to study the relationship between the left atrial size and atrial fibrillation (AF) in patients with thyrotoxicosis, 92 patients with Graves' disease were examined. Out of 92 patients (23 men and 69 women, age 7-71 years), 21% had AF: 31% of the patients older than 40 years had AF whereas none of the patients younger than 40 years had AF. Echocardiography revealed that the left atrial enlargement (more than 40 mm) exist in 7% of patients younger than 40 years, in 2% of those older than 40 years without AF and 94% of those older than 40 years with AF. Serum T3 and T4 were higher in the patients with AF than in those without AF when patients older than 40 years were analyzed.

Effects of calcium antagonists and nitroglycerin on atrial natriuretic peptide in normal subjects and patients with essential hypertension.

Acute effects of coronary vasodilators (nifedipine, nicardipine, and nitroglycerin) on atrial natriuretic peptide (ANP) and the renin-angiotensin-aldosterone system were studied in normal subjects and patients with essential hypertension. Nifedipine lowered blood pressure both in normal subjects and in patients and elevated ANP, plasma renin activity, and angiotensin II in normal subjects but not in hypertensive patients. Nicardipine lowered blood pressure but failed to elevate ANP and angiotensin II in normal subjects. Nitroglycerin failed to elevate ANP in normal subjects.

[The effect of an antihypertensive drug budralazine on cerebrovascular lesions in salt-loaded SHR].

Budralazine was evaluated for its protective effect on the onset of cerebrovascular lesions in SHR given 1.5% NaCl as drinking water. The salt-loading for 67 days rapidly accelerated the development of hypertension in SHR (from 180 to over 250 mmHg, 40 days after the loading). The acceleration of hypertension was accompanied by an increase in the incidence of brain softening, cerebral infarct, angionecrosis and hemorrhage by 30-60% following the thrombosis and necrosis of cerebral arterioles. Renal angionecrosis associated with the interstitial nephrosis was also observed by 90% in the animals. Throughout the salt-loading period, oral administration of budralazine (1, 4 and 15 mg/kg/day) resulted in a dose-dependent inhibition of the accelerated hypertension. At larger doses (4 and 15 mg/kg/day), budralazine almost completely ameliorated the cerebral and renal lesions and significantly attenuated the rise of weight in the brain and heart observed in the salt-loaded control rats. Changes in the serum biochemical findings were also inhibited by this drug. In some of the parameters measured, budralazine appeared to be more efficacious than hydralazine (1, 4 and 15 mg/kg/day, p.o.). These results suggest that budralazine attenuates the serious development of hypertension and reduces the incidence and severity of stroke in salt-loaded SHR.

[Experimental study of vibration analysis in middle ear models by holographic interferometry. Effects of the cross-sectioned area of aditus on the vibration of tympanic membrane].

To aid in the establishment of optimum conditions for postoperative function of the tympanic membrane, a model of the middle ear was experimentally prepared, the cross-sectioned area of aditus ad antrum was altered and the effects on the tympanic membrane vibrational characteristics were observed using the holographic interference method. The following results were obtained. 1) At the low frequencies below 2 kHz, the vibration mode of tympanic membrane was concentric on the whole, but the vibration pattern became fractionated at 4 kHz and above. The frequency of onset of the fractionated vibration pattern was changed only by the conditions of the membrane itself and was not affected much by the sound pressure, the volume of the middle ear cavity and the cross-sectioned area of aditus ad antrum. 2) There were resonance and anti-resonance points in the tympanic vibration pattern. These resonance and anti-resonance frequencies became higher as the diameter of aditus increased until eventually a constant value was reached. At diameters in excess of this constant value, the resonance frequency tended to fall and the anti-resonance frequency tended to rise. 3) The tympanic vibration in the middle ear cavity was not influenced very much by the diameter of aditus at 250Hz. Vibration decreased when aditus was blocked at 500Hz and the opening diameter had a major influence at 1kHz. Vibration was maximum at a diameter of about 2mm and a minimum when aditus was blocked. At 2 to 3kHz, the effects on vibration increased when aditus was blocked. 4) When aditus ad antrum was blocked, the vibration frequency characteristics of tympanic membrane deteriorated at under 1kHz and improved at 2 to 3kHz. When a comparison was made between the case with opened aditus, (which was considered to be equivalent to the closed method of tympano-plasty), and the case with closed aditus, (considered to be equivalent to the open method), the vibration frequency characteristics of the tympanic membrane improved in the low sound region up to 2kHz when the diameter was 4mm.

[Experimental study on the effect of UFT with concomitant radiotherapy of uterine cervical cancer].

In order to examine effectiveness of the chemotherapy with concomitant radiotherapy, an experimental study was designed using AMCC-1 cell line established from a human uterine cervical carcinoma in our laboratory. The growth of AMCC-1 cells cultured in plastic flask was inhibited significantly in the group treated with a half of minimal effective doses of 60Co and 5-FU in comparison with the control group. The 5-FU treatment after radiation inhibited the AMCC-1 cell growth more effectively than that before radiation. The growth of AMCC-1 tumor transplanted into nude mice was inhibited significantly in the group given a combination chemoradiotherapy, consisted of oral administration of three quarters of minimal effective dose of UFT for fourteen consecutive days and radiation of a half of minimal effective dose of 60Co. The oral administration of UFT after radiation inhibited the AMCC-1 tumor growth more effectively than that before radiation. These results suggest that the radiotherapy with administration of UFT was more effective against uterine cervical cancer than radiation alone and it is important to administer UFT orally after radiation as well as before radiation.

Clinical usefulness of sialyl SSEA-1 antigen as tumor marker for ovarian cancer as compared with CA125, CA19-9, TPA, IAP, CEA and ferritin.

In order to determine the clinical significance of sialyl SSEA-1 antigen, we compared its usefulness as a tumor marker for ovarian cancer with simultaneously measured CA125, CA19-9, TPA, IAP, CEA and ferritin. The sialyl SSEA-1 antigen in serum was measured by radioimmunoassay with an "FH-6" Otsuka Kit. The immunohistochemical localization of sialyl SSEA-1 antigen in ovarian carcinoma tissues was determined by an immunoperoxidase method using FH-6 monoclonal antibody. Among fifty-one patients with ovarian cancer, the incidence of elevated serum levels was 54.9% with sialyl SSEA-1 antigen, 90.2% with CA125, 48.8% with CA19-9, 78.0% with TPA, 73.1% with IAP, 17.1% with CEA and 63.4% with ferritin. On the other hand, among the patients with uterine malignancies and gynecologic benign tumors, the incidence of elevated sialyl SSEA-1 antigen levels in serum was lower than that of other tumour markers. In the patients with ovarian cancer, the serum levels of sialyl SSEA-1 antigen increased in accordance with the advance of the clinical stage and were also correlated with the effect of therapy. In the examination of immunohistochemical localization of sialyl SSEA-1 antigen, a positive reaction occurred in 10 out of 30 ovarian carcinoma specimens. Intense staining appeared in the secretory materials, in the luminal surface of the glands, and in the cytoplasm of cells. Thus, sialyl SSEA-1 antigen appears to be a useful tumor marker for the diagnosis of ovarian cancer, especially when measured simultaneously with CA125, CA19-9, TPA, ferritin and IAP.

[Clinical features of patients with spontaneous recanalization of the infarct-related artery during evolving acute myocardial infarction].

The clinical features of 124 patients with incompletely obstructed infarct-related arteries during the early stages of myocardial infarction (Group 1) were compared with those of 212 patients having completely occluded coronary arteries (Group 2). Coronary angiography was performed within 12 hours after onset of symptoms in all cases. Patients treated with emergency coronary angioplasty were excluded from the study. Thrombolytic therapy, performed in both groups whenever intracoronary thrombi were detected, was successful in 61% of Group 2. Results were as follows: 1. In Group 1, three-vessel disease was observed more frequently than one-vessel disease (49 vs 27%, p less than 0.005). 2. The peak level of CPK was higher in Group 2 (p less than 0.001), and left ventricular ejection fraction was higher in Group 1 (66 +/- 16 vs 56 +/- 14%, p less than 0.01). 3. Either significant ST elevation or the Q wave was more commonly absent in Group 1 (31 vs 12%, p less than 0.01; 49 vs 12%, p less than 0.001). 4. Improvement of ejection fraction was observed in Group 1, but not in Group 2 even if the infarct-related artery was recanalized within six hours. 5. Extension of an infarct area was more common in Group 1 compared to Group 2 which was successfully treated with thrombolytic therapy (12 vs 3.9%, p less than 0.05). 6. The most important cause of death was extension of an infarct area in Group 1 and pump failure in Group 2, though hospital mortality rates were similar in both groups. It was concluded that patients with myocardial infarction having incompletely obstructed infarct-related coronary arteries have better left ventricular function and higher rates of non-Q myocardial infarction compared with those who had completely obstructed coronary arteries. However, extensions of infarcted areas commonly occur in these patients.

Establishment and characterization of a new human cell-line (AMOC-2) derived from a serous adenocarcinoma of ovary.

A new cell line, designated as AMOC-2, was established from a mouse graft of human ovarian serous adenocarcinoma, and has grown well without interruption for over 12 months. The cultured cells of the AMOC-2 line were spindle or polygonal in shape and showed a monolayer growth with a pavement-like arrangement and a slight tendency to pile up, thus lacking contact inhibition. This cell line has a doubling time of 17.4h, a saturation density of 9.17 X 10(4) cells/cm2, a plating efficiency of 36.2% and a mitotic coefficient of 6.2%. The chromosomal number of this cell line is distributed in the diploid range and three of the specific marker chromosomes were identified in the cells with 49 chromosomes. This cell line was able to be transplanted into the subcutis of nude mice and produced a poorly differentiated serous adenocarcinoma resembling the original tumor. These cells have maintained their tumorigenic capacity despite prolonged maintenance in culture and are now in the 60th passage.

Expression of Schwann cell characteristics in pigmented nevus. Immunohistochemical study using monoclonal antibody to Schwann cell associated antigen.

A monoclonal antibody to the Schwann cell associated antigen (AHMY1) and a monoclonal antibody to neurofilament proteins (NFP) (AHNF1) each were produced. Using AHMY1, AHNF1, and anti-S-100 antibody, 90 pigmented nevi were examined immunohistochemically to clarify the relationship between nevus cells and Schwann cells. All nevus cells had S-100 protein, but did not have neurofilament proteins. The nevic corpuscle and Type C nevus cell were positively stained with AHMY1 indicating the presence of the Schwann cell associated antigen, while Type A and B nevus cells were entirely negative. This suggests that the nevic corpuscle and Type C nevus cell are closely related to the Schwann cell and differ from Type A and B nevus cells in their development.

Combination assay of CA125, TPA, IAP, CEA, and ferritin in serum for ovarian cancer.

The levels of CA125, TPA, IAP, CEA, and ferritin in the serum were measured simultaneously in 68 healthy nonpregnant females and 133 patients with various gynecological diseases, and were subjected to statistical discriminant analysis for the diagnosis of ovarian cancer. The usefulness and the limits for diagnosis of various gynecological diseases were investigated for each tumor marker. Also, the diagnostic usefulness of the stepwise discriminant analysis employing the values of these five tumor markers in the serum in cases of ovarian cancer was compared with that of CA125 measurements alone. Because the frequency of cases with an elevated serum CA125 level increased more specifically in the ovarian cancer group than those of other tumor makers in the serum, this parameter was considered to be more useful for the diagnosis of ovarian cancer than the levels of the other tumor markers. The frequencies of cases with the elevated serum CA125 levels, however, also increased in the groups of patients with endometriosis and at an early stage of normal pregnancy more than in the group of healthy nonpregnant females. In the ovarian cancer patients, the discriminant analysis employing the values of CA125 and four other tumor markers in sera was more useful for early diagnosis, differential diagnosis, early detection of recurrences, and the determination of complete remission after therapy than the measurement of the serum CA125 level alone.

Improvement of immunologic abnormalities associated with hyperthyroidism of Graves' disease during methimazole treatment.

In an attempt to study the effects of methimazole treatment on immunologic abnormality of hyperthyroidism of Graves' disease, TSH receptor antibody (TRab), anti-DNA antibody and HLA-DR were measured in untreated patients with hyperthyroidism of Graves' disease and treated patients with methimazole for 2 years, using peripheral blood. In untreated patients, all 3 parameters elevated above normal. Three parameters decreased 2 years after methimazole treatment, but the magnitude of decrease was more in T3-suppressible patients than in T3-unsuppressible patients. However, both anti-DNA antibody and HLA-DR were significantly more in T3-suppressible patients than in normal subjects. It is suggested that immunologic abnormalities should largely be improved before remission of Graves' disease can be obtained.
