[Changes in the criteria for planned delivery in the National Defense Medical College Hospital with statistical analysis].

Planned deliveries have been attempted with the administration of prostaglandins E2 and F2 alpha since 1978. Eight hundred and eighteen prostaglandins-induced and 729 uninduced deliveries from January, 1978 to July, 1980 were statistically analyzed. The uninduced deliveries included 465 spontaneous and 264 stimulated labors. The cervical ripening was considered the basic requirement stimulated labors. The cervical ripening was considered the basic requirement for labor induction for the first year of study. However, spontaneous labor onset or the membranes rupture occurred before the induction in 52.4% of cases, where babies were large enough (3,052.6 +/- 33.4g, mean +/- SE, n = 154). Therefore, the cervical factor was excluded from the essential requirement for induction. Since the beginning of 1979, labor induction has been performed based on gestational age (38 weeks or more) and uterine fundal length (32cm or more). Laminaria tents were inserted into the unfavorable cervix the night before induction. Delivery time in the day, duration of labor, and birth weight of babies in the induced group were 2.04 +/- 0.18 p.m., 8.0 +/- 0.3 hours and 3,138.8 +/- 12.3g, respectively. Apgar scores and the amount of postpartum hemorrhage were not significantly different from those in the uninduced group. Similar results were obtained between the laminaria subgroup with unfavorable cervix and the non-laminaria subgroup with ripe cervix in the induced primigravidas.

Effects of anisatin on the GABA action in the crayfish neuromuscular junction.

Anisatin depressed the conductance increase produced by GABA in a non-competitive manner and decreased the amplitude of the inhibitory junctional potentials. The presynaptic inhibition was depressed by anisatin in the similar concentrations to those of picrotoxin. The blockade of the presynaptic inhibition by anisatin was irreversible, whereas the effect of anisatin on postsynaptic inhibition subsided after prolonged washing of the preparation.

Secondary hyperparathyroidism with 1,25-dihydroxyvitamin D deficiency and pseudohypoparathyroidism in childhood: relationship between plasma 1,25-dihydroxyvitamin D and parathyroid hormone levels and urinary cyclic AMP response to exogenous PTH.

In order to clarify the complex interrelationship between serum calcium, 1,25-dihydroxyvitamin D (1,25(OH)2D), and parathyroid hormone (PTH), and the urinary excretion of cyclic AMP (cAMP) in response to exogenous PTH in pseudohypoparathyroidism (PHP) and related diseases, we investigated 3 patients with parathyroid disorders before and after treatment with 1 alpha-hydroxyvitamin D3 (1 alpha-OH-D3). Low plasma 1,25(OH)2D before treatment increased after giving 1 alpha-OH-D3 (0.1 micrograms/kg/day), whereas high plasma PTH measured by the c-terminal assay (C-PTH) decreased in all 3. No response in urinary cAMP was found before or after treatment in 2 patients with PHP type I, despite the fall of plasma C-PTH. However, in one patient with extremely high plasma C-PTH but normal N-PTH (measured by a homologous radioimmunoassay using 1-34 human PTH), urinary cAMP response to exogenous PTH was increased after treatment with 1 alpha-OH-D3. We suggest that he had pseudopseudohypoparathyroidism (PPHP) with Albright's hereditary osteodystrophy and a partial deficiency of renal 1 alpha-hydroxylase. In this patient secondary hyperparathyroidism is thought to be due to 1,25(OH)2D deficiency, and the decreased responsiveness to exogenous PTH before treatment due to excess PTH occupying renal receptors.

Antigenic and genetic analysis of A/Hong Kong (H3N2) influenza viruses isolated from swine and man.

Two H3N2 strains of influenza A virus isolated from pigs in Japan and Thailand were characterized antigenically and genetically. A/swine/Wadayama/5/69 (isolated in Japan in 1969) was antigenically similar to a human strain, A/Aichi/2/68, while A/swine/Bangkok/9/78 (a swine isolate in Thailand) was closely related to A/Bangkok/36/78, a contemporary human isolate. Gel electrophoresis and oligonucleotide mapping of the virus RNA revealed genetic similarity between A/swine/Wadayama/5/69 and A/Aichi/2/68, and between A/swine/Bangkok/9/78 and A/Bangkok/36/78, suggesting the derivation of swine H3N2 strains from contemporary human strains.

Plasma active vitamin D concentration in low birthweight infants with rickets and its response to vitamin D treatment.

Impairment of 25-hydroxylation may be a cause of rickets in infants of low birthweight, weighing between 2000 and 2500 g. In addition there may be impairment of 1 alpha-hydroxylation in infants weighing less than 2000 g. Our data show that a supplementary dose of vitamin D2 of at least 500 IU daily is a reasonable regimen for infants who weighed between 2000 and 2500 g at birth. However for infants who weighed less than 2000 g with a gestation of under 38 weeks, administration of 1 alpha-OHD3 may be more effective in preventing rickets.

Effect of dibutyryl adenosine 3',5'-monophosphate administration on plasma concentrations of 1,25-dihydroxyvitamin D in pseudohypoparathyroidism type I.

The effect of dibutyryl cAMP on plasma concentrations of 1,25-dihydroxyvitamin D [1,25-(OH)2D] was studied in two patients with pseudohypoparathyroidism (PHP) type I, five normal adults, and three normal children as controls. In normal adults, plasma 1,25-(OH)2D tended to increase 3 h after the infusion of 2.5 mg/kg dbcAMP and was significantly increased after 6 h. In normal children, plasma 1,25-(OH)2D increased 3 h after the infusion, then gradually decreased; in the patients with PHP type I, it increased greatly from 6.8 and 11.2 pg/ml to 264.6 and 128.2 pg/ml 3 h after the infusion. These results suggest that the renal mechanism for the response to parathyroid hormone is intact distal to the renal adenylate cyclase in patients with PHP type I.

Quisqualate action on the crayfish neuromuscular junction.

Actions of quisqualic acid on the crayfish neuromuscular junction were examined and compared with those of glutamate. The glutamate and quisqualate currents were induced in the voltage-clamped muscle. When their peak amplitudes were plotted against the current intensity of iontophoretically applied drugs, the hyperbola resulted. The amplitude of the maximum response to quisqualate was similar to that to glutamate. When the amount of quisqualate which was simultaneously applied with glutamate was changed, the amplitudes of the maximum response of glutamate plus quisqualate did not vary much at the top of the dose-response curves. Bath application of quisqualate in concentrations above 10(-6) M produced a large depolarization of the muscle fiber, but this response was maintained after it reached a maximum, unlike glutamate. On the other hand, the amplitude of excitatory junctional potential (EJPs) was gradually decreased. A quantum analysis of the extracellular EJP demonstrated that this decrease in EJP amplitude produced by quisqualic acid was due to the postsynaptic event. This decrease in EJP amplitude could not be prevent by exposure of the muscle fiber to concanavalin A, which completely blocked development of desensitization of the glutamate receptor. Based on these results, it is doubtful that the decrease in EJP amplitude produced by quisqualic acid is due to desensitization of the neuroreceptor.

An attempt at an analysis of the factors determining the time course of the glutamate response in the crayfish neuromuscular junction.

An attempt was made to analyse the factors which might determine the time course of the end-plate current. The end-plate current was measured by clamping the membrane potential of the crayfish muscle fiber. The calculated glutamate-induced current was in good agreement with the observations obtained, suggesting that the presented kinetic model explained well the practical phenomenon. The kinetic model could be applied also to explain the depolarization change induced by prolonged application of glutamate. The semilogarithmic plots of the falling phase of calculated glutamate-induced currents gave an approximately straight line. The slope of the line yielded an apparent desensitization rate constant, and the relationship between the true and apparent desensitization rate constants was determined, but it was suggested that the desensitization rate could not be determined from the slope of the decay of glutamate-induced current alone.
