[Experimental and clinical studies on cefmetazole in the field of internal medicine].

Susceptibilities in various types of clinical isolates to cefmetazole (CMZ) were determined by the disk method and the serial agar dilution method for MIC measurement. CMZ showed high antibacterial activity for all Gram-positive cocci except E. faecalis, and for H. influenzae, E. coli, Klebsiella sp. and Proteus sp.. CMZ was administered on 33 patients with infections, mainly biliary tract infections, in the field of internal medicine. The clinical efficacy of the drug was 86.7% for infections of the biliary tract, 80.0% for respiratory tract infections, 100% for urinary tract infections and 76.7% for all cases. There were no adverse reaction or changes in laboratory findings caused by CMZ in any of the patients.

[Episode secretion of LH releasing hormone and LH in peripheral plasma of women with and without ovulatory disturbances].

We studied the secretory dynamics of plasma luteinizing hormone releasing hormone (LHRH) and luteinizing hormone (LH) in 5 normal ovulatory women during the early follicular phase, and 29 cases of ovuluatory disturbances using radioimmunoassay methods. All of the normal ovulatory women demonstrated one or two pulsatile LHRH releases in 3 or 4 hours. The height of LHRH secretory spikes was observed to be between 1.4 and 9.2 pg/ml. Plasma LH episodic secretion was related to LHRH episodic secretion. In 15 out of 27 cases of of ovulatory disturbances except cases with primary ovarian dysfunction, the LH episodic secretion revealed a significant decrease in pulse frequency. In 13 out of 16 cases of ovulatory disturbances, the height of LHRH secretory spikes was not greater than 1.3 pg/ml. This study indicates that the main causes of ovulatory disturbances are decrease in LHRH and LH episodic secretion. In all cases treated with clomphene, the plasma LHRH pulse increased after treatment. But in the cases where ovulation was not induced by clomiphene citrate, the plasma LH pulse did not increase as the LHRH pulse increased.

Effects of percutaneous transluminal coronary angioplasty: intracoronary thrombolysis with urokinase in acute myocardial infarction.

Coronary angiography and percutaneous transluminal coronary angioplasty (PTCA) were performed in 32 patients with evolving acute myocardial infarction. Of the 25 patients with complete occlusion of an infarct-related coronary artery, in 18 (72%) the occluded vessel was successfully opened by an intracoronary infusion of urokinase. With a small dose of urokinase the successful recanalization was achieved in only 25%; with a larger dose it was achieved in 94%. After PTCA, all patients received glucose-insulin-potassium solution for 76 hours. Repeat angiography 42 days later showed a patent coronary artery in 12 (group A) of 18 patients with successful PTCA. In group A, left ventricular ejection fraction increased from 51 +/- 13% to 72 +/- 10% (p less than 0.01) and regional wall shortening from 4.5 +/- 9.5% to 29 +/- 19% (p less than 0.01). In contrast, these variables did not change significantly in patients with unsuccessful PTCA or late reocclusion of an infarct-related vessel (group B). These data suggest that successful PTCA with sustained patency of an infarct-related coronary artery has a beneficial effect on the salvage of the jeopardized myocardium, and glucose-insulin-potassium therapy may enhance the beneficial effect of PTCA.

Immunochemical identification of an alpha 1-acid glycoprotein-antigenic determinant on carcinoembryonic antigen (CEA) and non-specific cross-reacting antigen (NCA).

An immunochemical characterization of carcinoembryonic antigen (CEA) and NCA (non-specific cross-reacting antigen) was performed. Positive reactions of CEA and NCA (Mr 60 000) with some antibodies to alpha 1-acid glycoprotein (AG) were observed. Thus, both antigens may contain immune determinants in common with alpha 1-acid glycoprotein. CEA showed positive reactivity with anti-NCA. NCA showed positive reactivity with either polyclonal or monoclonal antibodies to CEA, but negative reactivity with auto-antibodies to CEA. 125I-Tetracosapeptide (synthetic peptide-24 corresponding to the amino terminal sequence of CEA) failed to react with any antisera against CEA, NCA and AG. 125I-AG also showed no immuno-reaction with any antibody against CEA, NCA and tetracosapeptide. These results suggest that some monoclonal antibodies to CEA are directed against a common antigenic determinant of both CEA and NCA in addition to AG and tetracosapeptide, and that the auto-antibody to CEA is directed against a unique immune determinant which is not common to NCA. Thus, CEA appears to contain a unique determinant not found in NCA. Similarities in the composition of both amino acids and carbohydrates of CEA and NCA suggest that CEA is 'big-big' AG and NCA is 'big' AG.

Activity of renal 25-hydroxyvitamin D3-1 alpha-hydroxylase in a case of X-linked hypophosphataemic rickets.

In 1974, a 2-year-old boy was diagnosed as having X-linked hypophosphataemic rickets (XLH) because of severe rickets and hypophosphataemia. The vitamin D metabolite concentrations, blood and urine chemistry and renal 25-hydroxyvitamin D3 (25OHD3)-1 alpha-hydroxylase were measured in 1982 (about 2 weeks after withdrawal of medication). 1 alpha-hydroxylase was 392 pg/mg tissue/20 min in the patient, which was high compared with aged-matched controls (69.7 +/- 28.5 pg/mg tissue/20 min, mean +/- SD, n = 7). Our present studies showed that the 1 alpha-hydroxylase activity in the patient with XLH was elevated. Therefore, the normal or low 1,25-dihydroxyvitamin D3 (1,25-(OH)2D3) concentrations in XLH patients could be due to accelerated catabolism of 1,25-(OH)2D3 or abnormally regulated 25OHD3-1 alpha-hydroxylase in response to hypophosphataemia, although significantly elevated above that in normal controls.

Glutamate inhibitory action of matrine at the crayfish neuromuscular junction.

The effect of some alkaloids from Sophora flavescens (matrine, oxymatrine and N-methylcytisine) on glutamate-induced responses was investigated using electrophysiological techniques at the crayfish neuromuscular junction. At concentrations greater than 0.1 mM, matrine depressed both glutamate-induced responses and neurally evoked excitatory junctional potentials. Oxymatrine was less powerful than matrine, and N-methylcytisine was not effective. Matrine also depressed quisqualate-induced responses at this site, but did not have an effect on gamma-aminobutyric acid (GABA)-induced responses. Matrine had no influence upon either the resting membrane potential or the input resistance of the crayfish opener muscle. The inhibition of the glutamate-induced response by matrine was not affected by the membrane potential of the muscle fibre. Matrine reduced the size of extracellularly recorded excitatory junctional potentials without affecting their quantal content. Matrine did not affect the decay of extracellular excitatory junctional currents at the resting membrane potential. The results presented clearly demonstrate that matrine has an inhibitory action on glutamate-induced responses.

Immunological and genomic analyses of two serotypes of avian paramyxovirus isolated from wild ducks in Japan.

A total of 11 avian paramyxoviruses isolated from migrating feral ducks in Niigata, Japan, were characterized by serological and genomic analyses. Hemagglutination inhibition and immuno-double-diffusion tests with antisera specific for the isolated hemagglutinin-neuraminidase polypeptides of reference strains indicated that, of these, eight isolates possessed hemagglutinin-neuraminidase antigen closely related to that of duck/Hong Kong/D3/75, and the remaining three isolates possessed antigen closely related to that of duck/Hong Kong/199/77. RNA analysis of the eight isolates identified serologically as duck/Hong Kong/D3/75 by oligonucleotide mapping revealed that these isolates were genetically very similar to each other but different from the reference strain and isolates reported previously. The oligonucleotide maps of duck/Hong Kong/199/77-like isolates appeared to be very similar to each other, suggesting the same origin, but not to the duck/Hong Kong/199/77 virus.

Digital image processing: effect on detectability of simulated low-contrast radiographic patterns.

Detection studies of simulated low-contrast radiographic patterns were performed with a high-quality digital image processing system. The original images, prepared with conventional screen-film systems, were processed digitally to enhance contrast by a "windowing" technique. The detectability of simulated patterns was quantified in terms of the results of observer performance experiments by using the multiple-alternative forced-choice method. The processed images demonstrated a significant increase in observer detection performance over that for the original images. These results are related to the displayed and perceived signal-to-noise ratios derived from signal detection theory. The improvement in detectability is ascribed to a reduction in the relative magnitude of the human observer's "internal" noise after image processing. The measured dependence of threshold signal contrast on object size and noise level is accounted for by a statistical decision theory model that includes internal noise.

Angiographic and pathologic evidence of hemorrhage into the myocardium after coronary reperfusion.

Severe myocardial hemorrhage can occur as a potential adverse effect of reperfusion therapy in evolving myocardial infarction. This report describes a 83-year-old man, who showed angiographic evidence of extravasation of contrast medium from the reperfused right coronary artery into the inferoposterior left ventricular wall. At autopsy, severe hemorrhage was transmurally observed in the inferoposterior wall of the left ventricle. The finding of extravasation is a useful angiographic sign of the production of hemorrhage during coronary reperfusion therapy, and great attention should be focused to the existence of this sign to prevent further hemorrhage.

Roles of calcium in aggregation of rat ascites hepatoma cells by cell surface-associated adhesive factor.

Our previous studies have shown that a cell surface-associated adhesive factor (AF), separated from rat ascites hepatoma AH136B cells of a differentiated type and highly purified by chromatography, induces not only aggregation of dissociated AH136B cells or rat ascites hepatoma AH109A cells of an undifferentiated type but also adhesiveness characterized by the development of junctional complexes; the AF-induced aggregation of the cells was Ca2+-dependent. Further analysis of the roles of Ca2+ in cell aggregation was performed using AH109A cells (present as single cells in vivo). (1) AF clearly enhanced 45Ca uptake by the cells; (2) calmodulin was isolated from the cells; (3) calmodulin inhibitor, W-7 (N-(6-amino-hexyl)-5-chloro-1-naphthalenesulphonamide), strongly inhibited aggregation of the cells; (4) W-7 also inhibited the clustering or capping of AF-binding sites on the cell surface; (5) binding of 125I-labelled AF to the cells was independent of Ca2+ concentration; (6) binding of 125I-labelled AF to AF-conjugated beads was not observed, independently of the presence of Ca2+. These findings suggest that Ca2+ and Ca2+-activated calmodulin may play a key role in the process of aggregation of the cells by controlling the microfilament components and that Ca2+ may not be involved either in the interactions between AF and its cellular receptor or in linkages of AF molecules.

Pulmonary embolism as a complication of transfemoral arteriography--incidence, symptoms, and prevention.

In order to evaluate the pulmonary embolism as a complication of transfemoral arteriography, we performed lung perfusion scintigrams before and after arteriography in consecutive 120 patients. Of the initial 60 patients who did not receive subcutaneous low-dose heparin, 19 (32%) demonstrated new pulmonary perfusion defects. There was no significant difference in regard to the incidence of new defects with or without right heart catheterization. On the other hand, in the latter 60 patients who received prophylactic heparin, the incidence of new defects decreased to 10%, without increasing clinically important bleeding. We believe that the source of emboli is from the leg vein thrombosis due to compression of the groin and subsequent bed rest. These data suggest that pulmonary embolism is a more common complication of transfemoral arteriography than previously appreciated and low-dose heparin is useful in reducing this complication.
