[Epstein-Barr virus-infected T-cell malignancy in an adult patient with Behçet's disease-like symptoms].

A 20-year-old woman was hospitalized on November 11, 1994 with Behçet's disease-like symptoms (fever, genital ulcer and aphtha in the oral cavity). Bilateral cervical lymph node swelling was also noted and diagnosed as lymphadenitis on biopsy. Chronic active Epstein-Barr virus infection (CAEBV) was diagnosed based on the high titer of antibodies to the EBV capsid antigen, early antigen, and nuclear antigen. She was treated with prednisolone and acyclovir and all symptoms improved. However, ten months after onset of symptoms, T-cell malignancy was diagnosed on bone marrow aspiration, which revealed 34.9% blast cells that had rearrangement of TCR-beta. She died on May 8, 1995, despite anticancer therapy. In analyzing the blast cells, the monoclonal junctional DNA structure of the EBV terminal repeat was analyzed by Southern blotting and provided definitive evidence for the monoclonality of EBV-infected T cells. These findings strongly suggest that EBV plays a pathogenic role in T-cell malignancy. EBV-infected T-cell malignancy, such as this case, is very rare in Japan, especially in adult.

High plasma concentrations of nitrite/nitrate in patients with hepatocellular carcinoma.

OBJECTIVES: Nitric oxide (NO) is considered to play a central role in macrophage or Kupffer cell-induced tumor cytotoxicity. Hepatocytes also produce NO in response to several inflammatory stimuli. Thus, there is a possibility that NO production by hepatic tissue is accelerated in patients with hepatocellular carcinoma (HCC). We, therefore, measured plasma nitrite/nitrate levels as an index of in vivo NO production in patients with HCC. METHODS: Plasma nitrite/nitrate levels were measured using Griess reaction in 95 patients with chronic hepatitis (CH) and compensated liver cirrhosis (LC) with (n = 48) or without HCC (n = 47), as well as 45 healthy control subjects. Possible factors related to nitrite/nitrate levels were evaluated for each subject. RESULTS: Plasma nitrite/nitrate levels in patients with HCC based on CH (mean +/- SD, 71.7 +/- 23.1 microM) and LC (52.4 +/- 20.2 microM) were significantly higher than those without HCC (CH, 31.1 +/- 15.0 microM; LC, 34.6 +/- 16.1 microM) (p < 0.01). Plasma nitrite/nitrate levels in patients with HCC based on CH were significantly higher than those in patients with HCC based on LC (p < 0.05). Simple regression analysis showed that plasma nitrite/nitrate levels significantly correlated with both tumor surface area (r = 0.577, p = 0.001) and tumor volume (r = 0.532, p = 0.003). CONCLUSION: Patients with HCC have elevated plasma nitrite/nitrate levels correlating to tumor volume as well as tumor surface area.

[Usefulness of continuous venous daily chemotherapy of 5-fluorouracil and low-dose cisplatin for patients undergoing noncurative surgery].

We examined the usefulness of continuous venous daily chemotherapy of 5-FU and low/I dose CDDP for patients undergoing noncurative surgery. Twelve patients with carcinoma (1 esophageal, 6 gastric, 4 colorectal carcinoma cases and 1 carcinoma case of unknown origin) underwent resection of primary lesion and postoperative adjuvant chemotherapy. The schedule for the chemotherapy was as follows: 2-7 courses of 24-hours continuous venous infusion of CDDP (5mg/day on day 1, 2, 3, 4, 5) and 5-FU (250 mg/day on day 1, 2, 3, 4, 5 or 1, 3, 5). Several patients received a bolus infusion of Epirubicin (10 mg/1 week or 30 mg/2 week). Mean total volumes of CDDP and 5-FU were 120 mg and 5.38 g, respectively. Nine cases could be evaluated. Results of the chemotherapy were 1 CR, 1 PR, 3 NC and 4 PD, and the overall response rate was 22%. Side effects were found in 6 cases, but were not so severe. One-year survival rate was 47%, and 50% survival time was 9 months. The prognosis of the patients seemed to be relatively good. These results suggest that continuous chemotherapy of 5-FU and low dose CDDP may be useful for patients undergoing noncurative surgery.

[A case of esophagopleural fistula successfully cured by conservative therapy after middle and lower lobectomy of right lung].

We experience a case of esophagopleural fistula successfully cured by conservative therapy after the lung cancer operation. A 46-year-old man was received middle and lower lobectomy for adenoid cystic carcinoma of the right lung. Complication of empyema associated with an esophagopleural fistula occurred on postoperative 4th day. Conservative therapy of nothing by mouth, intravenous hyperalimentation and antibiotics was started. Three thoracic drains were inserted and the thoracic irrigation of total 3,000 ml warm saline per day twice on one day was continued. The esophagopleural fistula was closed on 6th week and the patient was discharged on 11th week after the therapy start. This complication is much rare, but recent advance in the diagnostic methods seemed to increase the indication of conservative therapy in future.

[The present state of the visiting nurse support system of our hospital].

In our hospital, a Visiting Care Service System was started in 1985, and now we have four nurses and one public health nurse. In the first half of 1996, we visited 116 patients. Their average age was 79.5 years-old, and most of them were almost bedridden all day. In 63.8% of these cases, the underlying disease was cerebrovascular. Frequency of visits was once a week in 45.7%, but in the terminal state we visited more frequently almost all days. In 79.3% of these cases, the duration of this service was less than three years, but in a few cases more than 10 years. Some patients were given the following medical care: gastrostomy (39.7%), care of bed sores (29.3%), necessity of suction of sputa (26.7%), urinary drainage tube (13.0%), tracheostomy, home oxygen therapy, home parenteral nutrition therapy, bile drainage tube insertion and so on. Persons providing care were mostly female (87.8%), and most of them were 40 to 60-year-old women. It is important to create a suitable support system to continue treatment at home.

[Psychosocial evaluation on the correct recollection of periodic medical checkups of workers. 1. The ratio of correctly recalled results of medical checkups].

To investigate the usefulness of a periodic medical checkup at the workplace from the standpoint of remembering the results, a survey was performed by a self-rating questionnaire on 424 industrial workers in a certain manufacturing company. The questionnaire included several items, such as their recalled abnormal medical findings and follow-up toward them, Breslow's 7 health practices, Goldberg's 12 selected items from the Japanese version of the General Health Questionnaire, and other original health related items. The study was conducted a month after the workers received their results. A total of 391 (290 males and 101 females) or 92.2% responded to the questionnaire. Several factors, such as age, gender, the type of occupation, and perceived health status were significantly correlated to the recalled results of medical checkups, but, only age and the way of observing the results were in accordance with the actual results. Only forty-nine percent of the responders correctly remembered their results. Age and perceived health status were significantly correlated to the exact recollection of the results. In particular, those who had poor perceived health status tended to have a wrong understanding of their medical results. Furthermore, there were those with the highest averaged GHQ scores who responded as having some abnormal findings even though no abnormalities were discovered in the medical checkups. On the other hand, there were those with the lowest averaged GHQ scores who answered that they had no abnormal findings even though some abnormalities were revealed in the medical checkups. The frequency of correct follow-up of the results was lower than the exact recalling of the results. In addition, those who made a mistake in the follow-up were inclined to have an optimistic view. Therefore, comprehensive health care for precise recalling of the results of medical checkups should be required to improve abnormal findings or maintain a good health status.

[Helical CT for lung-cancer screening: fifth report: evaluation of image quality at low-dose CT using healthy volunteers].

In order to reduce radiation dose in the application of CT for lung-cancer screening, we investigated the potential of low-dose screening helical CT (SHCT) by reducing the tube current and using an additional 10 mm aluminum filter. Six healthy volunteers were scanned at various tube currents (50 mA, 30 mA, 20 mA) with and without the filter. In comparison with the images at 50 mA without the filter, we evaluated the visualization of normal anatomical structures and the detection of simulated nodules at low-dose SHCT. Although a bias toward unclearness in the visualization of small anatomical structures was noted at 20 mA with use of the filter, no structure was assessed as having no visualization at any low-dose scan. Detection of simulated nodules was not degraded significantly by reducing the tube current to 20 mA and using the filter. Further reduction of radiation dose at SHCT may be achieved by the application of small tube current and an additional filter.

Urban-rural comparison of HBV and HCV infection prevalence among adult women in Shandong Province, China.

In order to make urban-rural comparison of the prevalence of hepatitis B and C virus (HBV and HCV, respectively) infection in China, a twin survey was conducted in 1996 on two groups (50 subjects each) of 21-55 year-old, apparently healthy women for infection markers and serum enzyme levels; one group (the urban group) was in Jinan, the provincial capital of Shandong Province, and the other (the rural group) was in a farming village in Zhangqiu area some 30 km away from Jinan City. Comparison between the two groups showed that there was no significant (p > 0.10) difference in the prevalence of HBsAg+, anti-HBs+, anti-HBc+ and the cases positive to any of three HBV infection markers (i.e. HBV+). No age-dependent difference was detected within or between the groups. Thus the overall positivity rate was 8% for HBsAg+, 47% for anti-HBs+, 48% for anti-HBc+, and 64% for HBV+. No anti-HCV+ case was found either in the urban group or in the rural group. Liver function remained normal in all cases studied. The results together with perusal of published data suggest the urban-rural difference will not be remarkable, if present, in HBV and HCV infection in the regions studied, and possibly in China as a whole.

Hypnotic activities of N3-benzyl and xylyl substituted arabinofuranosyluracil.

N3-Substituted derivatives of arabinofuranosyluracil (1), methyl (2), ethyl (3), propyl (4), butyl (5), allyl (6), benzyl (7), o-, m-, p-xylyls (8, 9, 10) and alpha-phenethyl (11) derivatives, were synthesized and their CNS depressant effects were evaluated by intracerebroventricular (i.c.v.) injection in mice by using hypnotic activity and pentobarbital-induced sleep prolongation as indices. At a dose of 2.0 mumol/mouse, the values of mean sleeping time induced by 7, 8, 9, and 10 were 144, 154, 117, and 33 min, respectively. Although the alkyl (2-6) derivatives did not cause any hypnotic activity, some derivatives tested (5, 7-10) significantly prolonged the pentobarbital-induced sleeping time.

Hepatitis B and C virus infection prevalence among women in Manila, the Philippines.

A seroepidemiological survey of HBsAg, HBeAg, anti-HBs, anti-HBc, anti-HBe and anti-HCV positivities was conducted in 1997 in Manila, The Philippines. Adult women (21 to 59 years of age) were invited to donate peripheral blood for the study, and 50 people volunteered. They were nonsmokers, nonhabitual drinkers, and mostly married; none of them was a professional blood donor. The serum assay showed that all subjects were negative for HBsAg and HBeAg. Overall, a half (50%) of the study subjects were positive for any one of the HBV infection markers (i.e. HBV+). The older group (25 women at 36-59 years of age) showed significantly higher prevalence than the younger group (25 women at 21-35 years of age) of positivity to all three HBV antibody markers and therefore HBV positivity also. None of the study subjects was positive for anti-HCV. Comparison of the present findings with the results reported in the literature for the 1980s suggests that the risk of HBV infection and most probably that of HCV infection also has decreased in this 10-year period in the Philippines.

The consensus motif for phosphorylation by cyclin D1-Cdk4 is different from that for phosphorylation by cyclin A/E-Cdk2.

Cyclin D-Cdk4/6 and cyclin A/E-Cdk2 are suggested to be involved in phosphorylation of the retinoblastoma protein (pRB) during the G1/S transition of the cell cycle. However, it is unclear why several Cdks are needed and how they are different from one another. We found that the consensus amino acid sequence for phosphorylation by cyclin D1-Cdk4 is different from S/T-P-X-K/R, which is the consensus sequence for phosphorylation by cyclin A/E-Cdk2 using various synthetic peptides as substrates. Cyclin D1-Cdk4 efficiently phosphorylated the G1 peptide, RPPTLS780PIPHIPR that contained a part of the sequence of pRB, while cyclins E-Cdk2 and A-Cdk2 did not. To determine the phosphorylation state of pRB in vitro and in vivo, we raised the specific antibody against phospho-Ser780 in pRB. We confirmed that cyclin D1-Cdk4, but not cyclin E-Cdk2, phosphorylated Ser780 in recombinant pRB. The Ser780 in pRB was phosphorylated in the G1 phase in a cell cycle-dependent manner. Furthermore, we found that pRB phosphorylated at Ser780 cannot bind to E2F-1 in vivo. Our data show that cyclin D1-Cdk4 and cyclin A/E Cdk2 phosphorylate different sites of pRB in vivo.

Change in ascorbate radical production in an irradiated experimental tumor with increased tumor size.

We have reported that ascorbate radical (Asc.-) could serve as an indicator of the amount of hydroxyl radical and superoxide produced by irradiation in vivo. Using this method, we investigated the relationship between tumor size and Asc.- production after irradiation (10 Gy) and between tumor size and the radical-scavenging ability of WR-2721 (300 mg/kg). Asc.- was measured in normal muscle and SCC-VII tumors transplanted into mice (n = 6). In tumors, the increase in Asc.- significantly decreased with increasing tumor size (r = -0.483; P < 0.05). The increase in Asc.- production after irradiation was more inhibited by WR-2721 in normal muscle tissue than in tumor tissue at various sizes. In tumors, the increase in Asc.- was less inhibited by WR-2721 with increasing tumor size. These results demonstrate that the increase in radical production after irradiation and drug distribution decreased with increasing tumor size and that WR-2721 has excellent differential protection. This method is expected to measure changes in the amounts of local hydroxyl radical and superoxide modified by a change of tumor environment or drug administration.

[The effect of brain-derived neurotrophic factor on regeneration of nerve fibers after crush injury--morphometric evaluation].

Brain-derived neurotrophic factor (BDNF) is produced in Schwann cells and fibroblasts in the peripheral nerve, and is considered to play an important role in survival, maintenance, development and repair of the peripheral neuron. In this study, the effect of human recombinant BDNF on the regeneration of nerve fibers following a crush injury to the sciatic nerves of Sprague-Dawley rats was evaluated. In the experimental group, 20 mg/kg of BDNF was injected subcutaneously three times a week for 4 weeks in seven rats. In the control group, only the dissolving solution used in the experimental group was injected similarly in seven rats as in the experimental group. The various morphometric parameters were evaluated in the peroneal, sural and tibial nerves and the nerve to soleus muscle distal to the crush site. There were no statistically significant differences in the change of body weight, the improvement of the width between the first and fifth toes on the side of the crush, the density of total myelinated fibers (numbers per mm2), the total number of myelinated fibers (number per nerve), maximum and median diameters of myelinated fibers, the density of unmyelinated fibers, and the electron microscopic parameters of axon and myelin sheath. On the other hand, the number of myelin lamellae in relation to the transverse axonal area was significantly greater in the experimental group than in the control group. Therefore, BDNF may promote the myelination. However, such an effect on myelination seems not to be clinically significant, because such an effect was not demonstrated in other morphometric evaluations reflecting the myelination condition. Therefore, taking all the data obtained in this study into consideration, we concluded that there was no definite evidence that BDNF promoted the regeneration of nerve fibers at least under these experimental conditions.

A fusion protein library: an improved method for rapid screening and characterization of DNA binding or interacting proteins.

A rapid method for screening and characterization of DNA binding or protein-interacting molecules is described. The method relies on a fusion protein library in which randomized DNA fragments are inserted into pGEX-3X and its derivatives to generate collections of GST-fusion proteins. After inducing the expression of the fusion proteins by addition of IPTG, the colonies can be screened either with radioactively labeled DNA/RNA fragment for specific clones encoding DNA/RNA binding proteins or with an antibody for clones encoding proteins of interest. They can also be screened with a radioactively labeled protein for cloning of interacting molecules. The fusion proteins encoded by the isolated clones can be readily purified by conducting the lysis of the cells and an affinity column in the presence of an alkyl anionic detergent, N-laurylsarcosine (sarkosyl), and can be further characterized.

Lead content of rice collected from various areas in the world.

Rice samples consumed by local populations were collected between 1990 and 1995 in 17 areas in the world, mostly from Asia (10 areas), but also from 7 areas outside of Asia. In total, 1528 samples were obtained (about one half from Japan) and were analyzed for lead (Pb) by electrothermal atomic absorption spectrometry. When the Pb contents in rice samples were classified into 10 areas in Asia, the highest and the lowest geometric means (GMs) were for Indonesia (38 ng/g) and Australia (2 ng/g), respectively. Among 7 regions outside Asia, the GM was highest for Spain (58 ng/g) and the lowest for the USA. (3 ng/g). Moreover, inter-regional differences were significant, when more than 100 samples from one area were divided into 3-9 regions and GMs for regions were compared. There was a marked difference in Pb contents in rice by areas and regions in the world. Nevertheless, comparison by area of the present results with those reported in literature show that there has been no substantial reduction in Pb in rice in the past decade either in Japan or elsewhere in the world.
