Urban-rural comparison of nutrient intake by adult women in Shandong Province, China.

In 1996, 24-hour food duplicate samples were collected from two groups of 50 nonsmoking women each; one group was in Jinan, the capital city for of Shandong Province in China, and the other in a farming village in Zhangqiu area some 30 km away from the city. They had also physical examinations including hematology and serum biochemistry tests. Nutritional analysis of the duplicate samples were conducted utilizing standard food composition tables for Chinese populations. The urban-rural comparison between the two groups showed that women in Zhangqiu took significantly less lipid (54 g/day) and more carbohydrate (335 g/day) than Jinan counterparts (76 g lipid and 254 g carbohydrate/day), although there was no significant difference in total energy and protein intake (1968 kcal/day in Jinan vs. 2071 kcal/day in Zhangqiu, and 67 g protein/day in Jinan vs. 61 g/day in Zhangqiu). The intakes of the three major nutrients were sufficient in majorities of members in both groups. Iron intake was also sufficient (22 and 25 mg/day in Jinan and Zhangqiu, respectively), but intake of calcium was insufficient in more than a half of the subjects in both groups. Zhangqiu women heavily depended on plant-based foods both for protein and lipid, whereas the dependency was less remarkable among Jinan women.

Urban-rural difference in cereal consumption by people in Shandong Province, China.

In October, 1996, 24-hour food duplicate samples were collected from two groups of 50 adult women each; one group was in Jinan, the capital city of Shandong Province in China, and the other in a farming village of Baiquan in Zhangqiu area some 30 km away from the city. Perusal of food menus showed that wheat, rice, foxtail millet and maize were major cereals for the two groups. Nutritional analysis of the food duplicate samples were conducted taking advantage of weight of each food item and the standard food composition tables for Chinese populations. Whereas wheat was the leading source in both groups (73% of cereal-based energy in the city and 64% in the village), further urban-rural comparison showed that women in the village took substantial amounts of millet (19%) and maize (12%) in contrast to small consumption by the city people (millet by 5% and maize by 2%). Rice consumption was larger in the city (20%) than in the village (5%). The differences between the two groups were all significant for each of the four cereals. Nutritional significance of consumption of maize in particular and also that of millet were discussed.

[A long-term follow-up of patients with Meniere's disease--comparative study of unilateral and bilateral Mènière's disease].

A retrospective study was carried out on 46 patients with Meniere's disease (MD). Twenty of them were followed up for 20-40 years (long-term follow-up cases), and 26 of them for 5-12 years (middle-term follow-up cases). Thirteen of the patients suffered from fluctuating sensorineural hearing loss in one ear (unilateral MD), and 33 of them suffered from it in both ears (bilateral MD). Comparing the results of repeated audio-vestibular examinations of patients with unilateral MD with those of patients with bilateral MD revealed the following differences between them: 1) The range of fluctuation of the hearing level of the patients with unilateral MD was greater than that of the patients with bilateral MD. 2) Most patients with unilateral MD showed three audiometric configurations, but most patients with bilateral MD showed two audiometric configurations during the first 5 years of the follow-up period. 3) The patients with unilateral MD showed unilateral weakness on the caloric test more often than the patients with bilateral MD. But other clinical features in the patients with unilateral MD and those with bilateral MD were similar. We confirmed that there was an increase in bilaterality with time, but it is impossible to speculate whether the patients with unilateral MD will also suffer it in the contralateral ear or not. Therefore, it is possible that in all patients who have suffered from unilateral MD for a long time the disease may become bilateral in the future. Clinical observation of patients with MD as long as possible is the only way to clarify the relationship between unilateral and bilateral MD. It is stressed that to gain the patient's confidence is not only the basis of long-term follow-up of the patient, but also of effective treatment of MD.

[Possible ectopic cerebral tissue in the middle ear--a case report].

A 58-year-old woman suffered from chronic otitis media with possible ectopic cerebral tissue on the aditus ad antrum and the epitympanum. This 6 x 2 mm ectopic brain mass was removed carefully after discontinuity between this mass and intracranial cerebral tissue was detected. By histopathological examination, this possible ectopic cerebral mass was found to include mucous glands of middle ear mucosa. An exposed area of the dura mater 2 mm in diameter was found at the tagmen of the mastoid cavity and the aditus ad antrum, but there was neither perforation of the dura nor cerebrospinal fluid leakage from my part of the dura. This patient has not experienced any trouble during the one year post-operative course, and she obtained about a 15 dB hearing improvement in the ten months after surgery when compared with her preoperative status.

[A case report and an experimental study of esophageal foreign body of lithium battery].

In recent years an increase in the incidence of esophageal injury due to battery ingestion by infants with miniaturization of batteries has been noted. A case of esophageal ulceration due to lithium battery ingestion is reported. Moreover, an experimental model of esophageal injury due to lithium battery ingestion was produced in dogs, and the tissue damage was evaluated histologically. When the battery was inserted and left in the esophagus for one hour, the structure of the esophagus was well preserved. Minor injury was histologically found in the group in which the battery was retained for two hours. Esophageal injury from the epithelium to the muscle layer was found when the battery was retained for four hours. On the basis of our experimental findings, early extraction within four hours is desirable in the case of an esophageal foreign body of electric battery.

[The mechanisms of the inhibition of gastric acid secretion induced by intraduodenal amino acids in rats].

We investigated the effect of intraduodenal amino acids (AA) on gastric acid secretion and gastrin release, and possible role of endogenous secretin on this phenomenon in vivo in rats. Intraduodenal administration of mixed AA solution (140 mg/hr, pH6.5) resulted in significant inhibition of gastric acid secretion and gastrin release stimulated by intragastric perfusion of peptone (0.5%). Intravenous infusion of secretin (0.05CU/kg/hr) also inhibited peptone-stimulated gastric acid secretion and gastrin release. Furthermore, the AA-induced suppression of gastric acid secretion was significantly blocked by intravenous injection of rabbit anti-secretin serum. In conclusion, intraduodenal AA inhibit peptone-stimulated gastric acid secretion and gastrin release, and endogenous secretin released by AA is attributed to this suppression. Thus, the result of this study indicates that intestinal AA regulate gastric secretory function mediated by AA-released endogenous secretin in the intestinal phase.

[The role of surgery for chronic empyema of the advanced ages].

Chronic empyema, a sequelae of pulmonary tuberculosis, is now a only tuberculosis-related disease which was remained to be treated surgically. The candidates who have basically poor respiratory function are now attained advanced age. Over a 15 years period (1980-95), 22 patients 70 years of age or older underwent surgical intervention for chronic empyema at our hospital. There were 17 men and 5 women, ranging from 70 to 80 years of age (median age 75.0). They were 15.3% of all 145 surgically treated patients during same period. The empyema continued latent from 25 to 58 years (average 39.8 years). On admission they complained of productive cough (9), fever (9), hemosputam (5) and mass on the chest wall. Their Hugh-Johnes classification for dyspnea was I.: 4, II.: 6, III.: 11, IV.: 1 respectively. Their %VC ranged from 31.5 to 79.0 (average 54.8). In fifteen patients, tubercle bacilli (5), aspergillus (3) and other bacteria (9) were discovered in the empyema space. Surgical procedures consisted of 1 pneumonectomy (4.5%), 12 decortication or curettage of empyema wall (54.5%), 4 extraperiosteal air plombage (18%) and 5 other procedures (muscle or omental plombage, thoracoplasty, fenestration and others) (22.7%). There were no operative death and no lethal postoperative complication. In contrast, lethal postoperative complications such as GVIID, MOF and gastrointestinal bleeding occurred in the younger group. There were 2 cases of late respiratory failure in 70 years or older and 6 cases in younger group. Seventy-four years man who, preoperative %VC 33.0, underwent pneumonectomy died of asphyxia 6 month postoperatively. Another 74 years man who, preoperative %VC 76.1, developed respiratory failure after relapse of pulmonary tuberculosis. Four patients of younger group who developed late respiratory failure had all received thoracoplasty as a second operation. Other 2 patients, preoperative %VC 33.0 and 27.4 respectively, had undergone pneumonectomy. The risk of lethal postoperative complication or late respiratory failure were dependent mainly on preoperative respiratory function or surgical procedure selected rather than the age of patients.

[Inhibitory effect of ketamine on depolarization-induced norepinephrine release in the rat ventral hippocampus investigated by in vivo microdialysis].

The release of norepinephrine (NE) in the ventral hippocampus was studied in rats with microdialysis method. The basal release of NE with perfusion of normal artificial cerebrospinal fluid (ACSF) was 1.58 +/- 0.37 x 20 microliters-1 sample. The NE concentration increased significantly with perfusion of high potassium (60 mM) ACSF indicating that depolarization-induced release was up to 5 times higher than the basic level. Ketamine (20 mg.kg-1 and 80 mg.kg-1 im) significantly inhibited the depolarization-induced increase of NE, but did not affect the basal release. Neither 5 mg.kg-1 im of ketamine nor MK-801 had any effect on the basic or the depolarization-induced release. These results suggest that the inhibitory effect of ketamine on the depolarization-induced NE release was not due to the NMDA channel blocking properties of ketamine.

Alteration of white matter MR signal intensity in frontotemporal dementia.

PURPOSE: To determine the diagnostic potential of MR imaging to show white matter involvement in frontotemporal dementia. METHODS: We evaluated MR signal intensity in cerebral white matter by visually inspecting and by quantitatively measuring signal intensity on MR images in 22 patients with frontotemporal dementia. The findings were compared with those in 22 age- and sex-matched patients who had had Alzheimer disease for the same length of time and with 16 age- and sex-matched healthy control subjects. RESULTS: Patients with frontotemporal dementia had a significant increase in white matter signal intensity in the frontal and/or temporal lobes on T2- and proton density-weighted images. Visual inspection of regular proton density-weighted images and measurements made on the T2- and proton density-weighted images were sensitive to changes in white matter signal. CONCLUSION: Increased MR signal intensity in the frontotemporal white matter on T2- and proton density-weighted MR images is a useful diagnostic sign of frontotemporal dementia and distinguishes this condition from Alzheimer disease.

[A successful surgical treatment of coronary to pulmonary artery fistula with coronary steal phenomenon due to large volume of shunt: a case report].

A successful surgical treatment of right and left coronary to pulmonary artery fistula was presented. The patient was 61-year-old female, who was admitted for evaluation of cardiac murmur and chest pain. Cardiac catheterization and coronary artery angiogram revealed a large fistula both from RCA and LAD to the main pulmonary artery and distal LAD flow was sealed by this fistula. The left to right shunt ratio was calculated to be 55.1%. Preoperative BMIPP scintigram showed low uptake in the apex of the heart. Operation was indicated, because it was considered that angina pectoris was originated from coronary steal phenomenon. At the operation each right and left coronary shunt flow was measured to be 100 ml/min respectively. The fistulas were ligated and an opening of fistulas draining into the main pulmonary artery was closed through pulmonary arteriotomy under total cardiopulmonary bypass. Postoperative angiogram showed complete disappearance of the fistulas, postoperative BMIPP scintigram became to be normal, and her chest pain disappeared.

[Japanese version of the Neuropsychiatric Inventory--a scoring system for neuropsychiatric disturbance in dementia patients].

BACKGROUND AND PURPOSE: Neuropsychiatric disturbances are common and burdensome symptoms of dementias. Assessment and measurement of neuropsychiatric disturbances are indispensable to the management of patients with dementia and for clinical research. No standardized instruments have been developed for these purposes in Japan. The Neuropsychiatric Inventory (NPI), developed by Cummings et al., is a standardized, validated, and reliable tool to assess a wide range of neuropsychiatric derangements in patients with dementia, and has been used in clinical studies in North America. In this study, we prepared a Japanese version of the NPI and tested its reliability and usefulness in dementia patients. SUBJECTS AND METHODS: The subjects were 174 patients with dementia, including Alzheimer's disease (140), frontotemporal dementia (12), vascular dementia (19), and normal pressure hydrocephalus (3), whose diagnosis was made on the basis of the results of extensive examinations including MRI and PET/SPECT of the brain and appropriate clinical diagnostic criteria for each disorder. The subjects consisted of 133 women and 41 men; their mean age was 72.6 +/- 7.7 years (S.D.; range: 49 to 88) years. An examiner (neurologist) used the NPI to interview a caregiver familiar with the patient's everyday behavior. In 24 randomly selected patients interview was repeated by another examiner (psychiatrist) one month later, and test-retest reliability was evaluated. The Clinical Dementia Rating (CDR), the Mini-Mental State Examination (MMSE), and the Alzheimer's Disease Assessment Scale (ADAS) were used to assess the severity of the dementia and cognitive impairment. RESULTS: The MMSE scores ranged from 5 to 29 (mean +/- SD = 19.0 +/- 4.8) and the ADAS scores ranged from 7 to 63 (24.3 +/- 10.9). The CDR score was 0.5 in 17 patients, 1 in 95 patients, 2 in 52 patients, and 3 in 10 patient. Test-retest reliability of each score was acceptably high, and the correlation coefficient for each score obtained by repeated assessments in this study was similar to the coefficient in the original report. Apathy was the most common behavior in the ten abnormal behavioral domains, and euphoria was the least common. The prevalence of abnormal findings was also comparable to the results in the original report. The total NPI scores and many of the NPI subscale scores were significantly correlated with both the severity of dementia and cognitive impairment. CONCLUSIONS: This Japanese version of NPI, whose reliability and competency are comparable to those of the original version, is a reliable and useful tool for measuring neuropsychiatric disturbances in dementia patients.

Evaluation of combined therapy with chemoembolization and ethanol injection for advanced hepatocellular carcinoma.

To evaluate the therapeutic efficacy of transcatheter arterial chemoembolization (TACE) combined with percutaneous ethanol injection therapy (PEIT) for advanced hepatocellular carcinoma, we studied the effectiveness of TACE therapy combined with PEIT (50 cases) and TACE alone (50 cases). In both groups, patients had multiple lesions, or a single lesion with a diameter exceeding 2 cm or with vascular invasion (stages II, III, and IV in the tumor staging classification of the Liver Cancer Study Group of Japan). The clinical features in the two groups were comparable. The cumulative survival rates with TACE-PEIT were 95.0% for 1 year, 72.5% for 2 years, and 50.0% for 3 years, whereas the rates with TACE alone were 92.5% for 1 year, 57.5% for 2 years, and 20.0% for 3 years. The survival rate in the TACE-PEIT group was significantly higher than that in the TACE alone group. Moreover, the survival rate of patients with stage II or III disease in the TACE-PEIT group was significantly better than that in the TACE alone group, and the survival rate of patients with Child's classification B or C in the TACE-PEIT group was significantly higher than that in the TACE alone group. Multivariate analysis using Cox's proportional hazard regression model showed that the most significant prognostic factors in the TACE-PEIT group were tumor embolus in the portal vein and the number of tumors. These results suggest the effectiveness of combining TACE and PEIT for the treatment of advanced hepatocellular carcinoma.

[Clinical efficacy of GG032X tablets, a new dosage form of ondansetron (fast dispersing tablet), on cisplatin-induced nausea and emesis].

The inhibitory effects of GG032X tablets, a new dosage form (fast dispersing tablet) of ondansetron, 5-HT2 receptor antagonist, on nausea and emesis induced by cisplatin (CDDP), were investigated along with safety and usefulness. Subjects were chemotherapy patients starting CDDP administration for the first time, who were receiving a high single dose of CDDP (50 mg/m2 or more and intravenous drip infusion of less than 4 hours), or lower multiple doses of CDDP (a single dose of 10 mg/m2 or more, administered intravenously for 3-5 consecutive days). GG032X tablets were administered orally 1-2 hours before CDDP administration. In lower multiple doses of CDDP, GG032X tablets and CDDP were administered, as much as possible, at the same respective time when they were administered on the first day. Efficacy of GG032X tablets was evaluated in terms of inhibitory effect on nausea and emesis 24 hours after administration of a high single dose of CDDP, and of the inhibitory effect on nausea and emesis during the study period (3-5 days) in lower multiple doses of CDDP. Efficacy, safety and usefulness were evaluated in accordance with the evaluation criteria used in the clinical study of already-approved ondanstron tablets. In a high single dose of CDDP, the cases judged "effective" or better in the investigation of the inhibitory effect of the drug on nausea and emesis, accounted for 52.9% (63/119 cases). As for the overall safety rating, the cases judged as "safe" accounted for 87.0% (107/123 cases), and as a "minor safety problem" accounted for 13.0% (16/123 cases). As for the usefulness rating, the cases judged "useful" or better accounted for 52.1% (62/119 cases). Major adverse effects included headache, fever, atrial fibrillation and increases in total bilirubin, GOT and GPT values. None of these was serious, and the patients recovered without any treatment or by nosotropic therapy. Meanwhile, in lower multiple doses of CDDP, the inhibitory effect judged "effective" or better accounted for 70.6% (12/17 cases). As for the overall safety rating, all cases were judged "safe". In terms of usefulness, those cases judged "useful" or better accounted for 70.6% (12/17 cases). No adverse effect was observed. Study results of these two groups were almost the same as those for already-approved ondansetron tablets. According to the results of questionnaires for the patients who participated in the study and took GG032X tablets, the drug was found to be easy to take and had favorable results. Based on the above results, GG032X tablets were evaluated as having the same inhibitory effect as the already-approved ondansetron tablets against CDDP-induced nausea and emesis, and were considered safe and clinically useful.

[The relationship between workers' attitudes towards health, lifestyle and mental health].

In order to investigate the relationship between health practices, various psychosocial factors, and mental health, a survey was conducted by means of a self-rating questionnaire on 424 industrial workers. The questionnaire included items concerning Goldberg's 12 selected items from the Japanese version of the General Health Questionnaire, the Type A scale used in the Framingham study, Breslow's 7 health practices, and other original psychosocial, health practicing items. A total of 391 or 92.2% responded to the questionnaire. Among them, 354 (266 males and 88 females) complete responders to the GHQ were analysed for the present study. As a result, several factors such as decrease in age, perceived present illness, worsening of perceived health status, increase in anxiety concerning health, exacerbation of alcohol and smoking related behaviour, decrease in physical exercise, reduction of Breslow's 7 health practices, perceived stress and difficulty in dealing with stress, increase in stress content, negative and malfunctional coping strategies, and decrease in relaxation were, according to the GHQ scores, related to negative mental health. Furthermore, Type A scores were significantly correlated to GHQ scores, the number of stress factors, and Breslow's 7 health practices in males, but they were weak or had no correlation in females. It is considered that favorable health practices and attitudes towards health might help to maintain positive mental health, but further analysis is needed to determine their causal relationships because of the cross-sectional design of the present study.

Time series analysis of the course of Menière's disease.

Subjective symptoms of Menière's disease (vertiginous attacks, nonparoxysmal dizziness, tinnitus, hearing loss, headache, stiff neck, nausea etc.) were graded respectively according to severity (0-3 or 0-5) and were recorded daily by affected patients. These recordings, from 20 cases of Menière's disease, were collected and analysed by an original program of time series analysis including autocorrelation, cross-correlation, daily sum of inner ear related factors and general factors, and their moving average. Cluster occurrence of dizzy spells in the active stage and a marked cross-correlation between inner ear factors and general factors were found to constitute a characteristic pattern in Menière's disease in relation to its pathophysiology. Time series analysis was also useful for clinical decision-making, such as evaluation of the effect of treatment, and estimation of prognosis.
