Well-defined poly(ethylene oxide) (PEO) with a formyl group at one end and a hydroxyl group at the other terminus was synthesized by the anionic ring opening polymerization of ethylene oxide (EO) with a new organometallic initiator possessing an acetal moiety, potassium 3,3-diethoxypropyl alkoxide. Hydrolysis of the acetal moiety produced a formyl group-terminated heterobifunctional PEO with a hydroxyl group at the other end.
Well-defined poly(ethylene oxide) (PEO) with a cyano group at one end and a hydroxyl group at the other terminus was synthesized by the anionic ring opening polymerization of ethylene oxide (EO) initiated with (cyanomethyl)potassium (CMP) which was prepared by the metalation reaction of acetonitrile with potassium naphthalene in THF. Primary amino-terminal heterotelechelic PEO was obtained by the reduction of the cyano group at the end of the polymer chain by lithium aluminum hydride.
Lymph node enlargement is common in active systemic lupus erythematosus (SLE), a disease that is characterized by well-defined clinical criteria. Histologically, although lymphadenopathy associated with SLE exhibits marked histological diversity and occasionally shows atypical lymphoproliferative disorders, there has not been any description of the histopathological features of reactive lymph node hyperplasia with giant follicles (RHGF). We here report three such cases. The subjects were a 23-year-old Japanese female, a 44-year-old Japanese female and a 49-year-old Japanese male. All three patients initially presented with systemic lymphadenopathy. They also had systemic symptoms and abnormal laboratory findings indicating active disease, although two patients did not fulfill the diagnostic criteria for SLE at lymph node biopsy. Histologically, three lesions were characterized by numerous enlarged, coalescing lymphoid follicles with distortion rather than effacement of the lymph node architecture. By in situ hybridization, Epstein-Barr virus (EBV) genomes were demonstrated in two cases. The present three cases indicate that lymphadenopathy associated with SLE representing RFGH should be differentiated from the early stage of HIV-related lymphadenopathy as well as follicular lymphoma, particularly the floral variant. The authors would like to stress that the RHGF which is described in the present study should be listed in the pathohistology of SLE lymphadenopathy.
BACKGROUND: In Japan, vascular dementia (VD) has been responsible for the majority of all dementia cases in both epidemiological and neuropathological studies. Recently, however, several epidemiological investigations have shown that the rate of VD has decreased and senile dementia of the Alzheimer type (SDAT) is now the major cause of dementia, though few neuropathological investigations show that the majority of dementia is SDAT. OBJECTIVE: The purpose of the study was to clarify the rates of dementia types in Japanese nursing home residents and to compare the ratio of SDAT to VD in the first and second halves of the study. METHODS: One hundred and twenty-two demented nursing home residents (48 men and 74 women) were evaluated neuropathologically over a period of 17 years in Shimane prefecture, Japan. All subjects died between 1976 and 1992. The average age at death was 81.4 +/- 9.4 years for men, 85.0 +/- 7.2 for women and 83.6 +/- 8.3 for total samples. RESULTS: In classifying dementia type, SDAT accounted for 34% (41 cases); VD 35% (42); mixed dementia 11% (14); and 'other' dementia 20% (25) of all samples. Comparison of the first 9-year period with the second 8-year period revealed the ratio of SDAT to VD tended to increase (from 0.83 to 1.15). This increasing tendency was found both in men (from 0.53 to 0.7) and in women (from 1.13 to 1.44). These tendencies, however, were not statistically significant. CONCLUSION: There was no change statistically in the ratio of SDAT to VD between the first half of the study and the second half. However, there was an increasing tendency of the ratio in our study.
BACKGROUND: Several recent epidemiological investigations have shown that the rate of vascular dementia (VD) has decreased and the rate of senile dementia of the Alzheimer type has increased in Japan. In our previous neuropathological examination, such a trend but no significant differences were shown in nursing home residents in Shimane Prefecture, Japan. OBJECTIVE: The aim of this study was to clarify whether or not the frequency of cerebral infarctions (CI) and the ratio of CI [large infarction type (LI) and small multiple infarction (MI)] types changed between the first half of this study and the second half, corresponding to the decreasing trend of the rate of VD. METHODS: Three hundred and ten aged nursing home residents (146 men and 164 women) were evaluated neuropathologically in Shimane prefecture, Japan. All subjects died between 1976 and 1992. The average age +/- standard deviation at death was 79.1+/-8.7 years for men, 83.4+/-7.8 for women, and 81.4+/-8.5 overall. They were placed into the first or second study group and compared. RESULTS: (1) The incidence of hypertension (HT) decreased significantly from the first half of the study to the second half in both men and women. (2) The frequencies of CI and CI with HT were not significantly different between the first and second half (both men and women). (3) There was a decreasing trend in the ratio of LI to MI from the first to the second period in men. CONCLUSION: There may be no effect of the incidence of HT on CI frequency and some effects on CI types. The incidence of CI in total lifespan (both men and women) has not changed in our study. The recent decreasing trend in LI in men may have contributed to the decreasing tendency in the prevalence of vascular dementia.
Single smooth muscle cells isolated from guinea pig ileum using collagenase and papain produce contractile response to muscarinic agents, while the cultured cells do not. Using fluo-3/AM and a confocal laser scanning fluorescence microscope, it was observed that carbachol, a muscarinic agent, caused an increase in the intracellular Ca2+ of both single and cultured cells. SDS-PAGE and Western Blot analyses revealed the expression of myosin heavy chain isoforms of SM1 (204 kDa) and SM2 (200 kDa) in single smooth muscle cells, and non muscle isoform (196 kDa) of myosin heavy chain only in the cultured cells. With respect to actin isoforms, alpha-actin was predominant in single cells and beta-actin was major in the cultured cells. Two types of tropomyosin monomer, 39 kDa and 41 kDa, were detected in single cells, while the 41 kDa monomer was lost in cultured cells. These differences in contractile protein profiles between single and cultured cells were collaborated with the observation of cells using immunofluorescence microscope with responsible antibodies to isoforms of myosin heavy chain, actin and tropomyosin. These results suggest that the loss of contractility in cultured smooth muscle cells is profoundly related to changes in contractile protein profiles from smooth muscle type to non muscle type.
The movement of the body's center of gravity was calculated in normal subjects and in vertiginous patients by using a strain gauge platform system and a ditigal computer. The total length of the loci traced by the center of gravity in 40 normal subjects during normal standing for 1 minute was 52.0 +/- 18.5 cm with eyes open, and 81.5 +/- 52.3 cm with eyes closed. The ratio of the total length with the eyes closed compared with that with the eyes open during normal standing for 1 minute was 1.56 +/- 0.56. The time course of the length of the locus was calculated every 10 seconds, and characteric curves were obtained. The dependability of this test appeared to be fairly accurate and the usefulness of this technique for the diagnosis of vertiginous or ataxic diseases is recommended.
Cardiotrophin-1 (CT-1) is a novel cytokine which is involved in the growth and survival of cardiac cells. We examined whether CT-1 plays a role in the development of cardiac hypertrophy in carnitine-deficient juvenile visceral steatosis (JVS) mice. The CT-1 mRNA level was quantitatively measured by the competitive RT-PCR method. In contrast to other models including spontaneously hypertensive rats, CT-1 mRNA in the ventricles of JVS mice was comparable to the control at 5 days and was less than half the control value at 2 and 8 weeks when the ventricles of the JVS mice were highly hypertrophied. There were no significant differences in CT-1 mRNA levels in the lung, liver, kidney, small intestine and skeletal muscle between the JVS and control mice at 2 weeks. We did not find any difference between JVS and control mice at 2 weeks in the mRNA level of ventricular leukemia inhibitory factor (LIF) which binds to the same receptor as CT-1. Furthermore, almost no phosphorylated STAT3 (signal transducer and activator of transcription 3), downstream of the LIF receptor and the gp130 signaling subunit, was observed in the ventricles of JVS and control mice. These data show that the CT-1 signaling pathway does not play a significant role in the development of cardiac hypertrophy in JVS mice. Furthermore, we could not detect any differences in insulin-like growth factor I and II mRNA levels. All these data suggest distinct differences in the mechanisms of cardiac hypertrophy between JVS mice and other model animals.
We present a case of spontaneous rupture of bladder diverticulum three years after postoperative whole pelvic irradiation (50.4 Gy) for carcinoma of the uterine cervix. The patient had suffered from a neurogenic bladder after hysterectomy, but excretory urography revealed no abnormalities. Bladder diverticulum was found two years later. Spontaneous rupture of the urinary bladder is one of the late complications associated with radiotherapy, although it is very rare. Postoperative neurogenic bladder may also be associated with rupture. We should be aware of this rare complication in patients who receive pelvic irradiation.
We reported a 74-year-old male case of progressive supranuclear palsy (PSP) who responded to tandospirone citrate, a serotonin receptor (5-HT1A) agonist. The patient manifested postural instability and gait disturbance at 71 years. Additionally, he showed vertical gaze paresis, regidity of the neck, extremities and trunk, bradykinesia and mild cognitive impairment. A brain MRI revealed moderate atrophy of bilateral frontal/temporal lobes and of midbrain tegmentum one year after the onset. The patient had been diagnosed as PSP and treated with L-DOPA. However, L-DOPA therapy showed only transient response for a few months. His symptoms deteriorated gradually, and he became unable to sit, stand up or walk by himself. Tandospirone citrate was additionally administered at 30 mg/day. Rigidity and bradykinesia were remarkably improved in two weeks after the start of tandospirone treatment. He became able to stand up and walk a short distance with supports in four weeks. Cognitive disturbance was also slightly improved. Tandospirone citrate was effective on our case of PSP, especially on rigidity. Our findings suggest that combination of levodopa and tandospirone citrate is a useful therapy for PSP.
Three autopsy cases of panencephalopathic type of familial Creutzfeldt-Jakob disease (CJD) were investigated. Cases 1 (51-year-old male) and 3 (54-year-old female) were siblings and Case 2 (68-year-old female) was their aunt. In cases 1 and 3, the age of onset (Case 1:51, Case 3:53), duration of illness (Case 1:9 months, Case 3:8 months) and neuropsychiatric symptoms (pyramidal and extrapyramidal tracts involvements, blindness and dementia in chronological order) were similar, but in Case 2, the onset was later (66 years old), duration was longer (32 months) and the initial symptom was dementia. Myoclonus and apallic state in the terminal stage were common to all 3 cases. Neuropathologically, all 3 cases had characteristics that indicated panencephalopathic type of CJD. Cases 1 and 3 had similar neuropathological findings with characteristic circumscribed necrotic foci in the subcortical white matter. In Case 2 in contrast, diffuse demyelination and fibrillary gliosis in the cerebral white matter were observed without circumscribed necrotic foci. In the cerebellum of Case 3, granular cell loss was very slight. The other lesions in the cerebral cortex and striatum of the 3 cases were common. In conclusion, the clinical symptoms and neuropathological findings of our familial CJD cases were different from one another.
A rare case of neuro-Behçet disease with diffuse demyelination and gliosis of the frontal white matter is reported clinico-pathologically. The disease began with genital ulcer and recurrent oral aphthosis when the patient was 42 years of age. There was erythema, moderate fever, CSF (cerebrospinal fluid)-pleocytosis and elevated CSF-globulin. He was diagnosed as having neuro-Behçet disease and treated with prednisolone. He gradually became euphoric, disinhibited, indifferent and demented. His cranial CT showed diffuse low density areas in the bilateral frontal white matter. He became bedridden, akinetic mute and died from respiratory dysfunction 3 1/2 years after onset. The following neuropathological findings were observed: 1) Moderate demyelination and gliosis was present mainly in the frontal and parietal white matter. 2) There were many micro-spongious necrotic foci in the gray and white matters of the cerebrum, basal ganglia, thalamus, midbrain and pons, some of which were accompanied by gliosis. 3) From 1/2 to 1/3 of all micro-necrotic foci in the frontal white matter were old and accompanied by gliosis. The white matter containing numerous micro-necrotic foci had myelin pallor and gliosis. 4) There was neither micro-necrosis nor gliosis in the occipital lobe. The pathogenetic correlation of white matter lesions with primary and secondary circulatory disturbances is discussed.
Combined administration of buthionine sulfoximine (2.5 mmol/kg, i.p.) and diethyl maleate (1.0 ml/kg, i.p.) resulted in a near-complete depletion of hepatic glutathione (0.02 mumol/g liver vs 5.17 mumol/g in saline-treated controls) in male Sprague-Dawley rats. Bile flow was markedly reduced in the rats as compared with the controls and glutathione was not detected in the bile. The linear regression of the correlation between bile flow and endogenous bile-acid excretion rates revealed that no bile acid-independent bile flow was produced in the glutathione-depleted rats. The bile flow was partially restored by an intravenous infusion of glutathione isopropyl ester (1.17 mmol/kg/hr). Glutathione levels were increased in the bile (16 nmol/kg/min) and in the liver (0.55 mumol/g) at the end of the 100 min infusion period of the ester. The increments in bile flow rates were not proportional to the biliary excretion rates of bile acids or glutathione, and the flow rates suddenly increased when glutathione levels in the bile reached an apparent threshold. The increments, not accompanied with an excretion of diethyl maleate-glutathione conjugate, were much greater than expected from the osmotic choleresis of glutathione in the bile. These results indicate that hepatic glutathione above a certain level is required for the formation of a portion of bile flow, and that an intravenous administration of glutathione isopropyl ester is effective in partially restoring the bile formation impaired by glutathione depletion.
