Neoplastic transformation arising in Peutz-Jeghers polyposis.

PURPOSE AND METHODS: To clarify the potential for malignancy of Peutz-Jeghers polyposis, we investigated 75 gastrointestinal polyps resected surgically or endoscopically from seven patients with this syndrome. RESULTS: There were 19 polyps in the stomach, 18 in the duodenum, 22 in the small intestine, and 16 in the large intestine, and these were histologically composed of 1 pyogenic granuloma, 1 cancer in adenoma, 2 adenomas, and 71 Peutz-Jeghers polyps. Nine of these Peutz-Jeghers polyps were accompanied by an adenomatous component, and, in addition, two of these showed a cancerous transformation with stalk invasion. A total of 12 neoplastic polyps (16 percent) were found in three relatively young patients (aged 20, 25, and 43 years), all of which were pedunculated and located either in the duodenum or in the jejunum. There was no statistical significance in size between the neoplastic polyps (mean +/- SD, 20.1 +/- 10.8 mm) and the completely hamartomatous polyps (mean +/- SD, 15.8 +/- 9.0 mm). Moreover, the configuration of these types of polyps seemed similar. CONCLUSION: Neoplastic transformation is not a rare event, and our results may indicate evidence of a hamartoma-adenoma-carcinoma sequence in Peutz-Jeghers polyposis.

An animal model of longitudinal ulcers in the small intestine induced by intracolonically administered indomethacin in rats.

The ulcerogenic effect of intracolonically administered indomethacin was evaluated in rats. Conventionally fed rats aged from 5 to 10 weeks were treated by 8, 16, 24, or 32 mg/kg of intracolonic indomethacin for two days, and any damage to the stomach, small intestine and the colon was investigated. Longitudinal ulcers and scattered small ulcers were found in the small intestine at all doses of indomethacin, and the length of the longitudinal ulcers increase dose-dependently, but this was unrelated to the body weight of the rats. The cecum was frequently affected by irregularly shaped ulcers, and the incidence increased as the dose of indomethacin increased. The colon, other than the cecum, was not involved macroscopically. In contrast, the stomach was affected by only large doses of indomethacin (24 or 32 mg/kg), and the size of gastric ulcers increased according to the body weight of the rats. These findings suggest that intracolonic indomethacin in relatively young rats causes ulcers predominantly in the small intestine and the cecum, which are the frequent site of involvement of human Crohn's disease, and that this animal model may be suitable for investigation of the pathophysiology of inflammatory bowel disease.

Stereomicroscopic examination of surface morphology in colorectal epithelial tumors.

The mucosal surface structure of epithelial neoplasms in the large intestine was examined by a stereomicroscope, and the correlation between histologic and macroscopic features was studied. The surface structure of the colonic mucosa could be readily and clearly observed using the stereomicroscope after superficial staining, and neoplastic lesions were identified as well-defined foci of the unique structures that were conspicuously different from those in the normal mucosa. Five hundred two adenomas and 159 carcinomas were examined and divided into five types according to stereomicroscopic appearance: long ellipsoid (359 lesions), cerebriform (141 lesions), leaf-like (16 lesions), dense tiny pits (89 lesions), and devastated (56 lesions). In the first three types benign adenomas predominated in the majority of cases (94.7%, 72.3%, and 68.8%, respectively) and the variation in the surface features correlated well with the histologic types. In the latter two types the frequency of malignancy was strikingly high (49.4% and 92.9%, respectively), with the devastated mucosal appearance being highly indicative of invasive carcinoma. Macroscopically, exophytic polypoid configurations were frequently found in the first three types, whereas flat nonpolypoid configurations predominated in the cases with dense tiny pits. Our results indicate that there is a close relationship between the surface structure of epithelial neoplasms in the large intestine and their histologic features and growth patterns.

Natural course of jaw lesions in patients with familial adenomatosis coli (Gardner's syndrome).

Twenty-three patients suffering from familial adenomatosis coli (FAC) were followed for an average of 7 years (0.11-16.11 years). In 22 patients, jaw lesions including osteomas and/or odontomas were present at the first examination. In 12 patients, some changes occurred; that is, the number and size of the lesions increased. These findings indicate that changes of jaw lesions in FAC patients occur gradually even in adulthood.

Intestinal pseudo-obstruction in patients with amyloidosis: clinicopathologic differences between chemical types of amyloid protein.

A clinicopathologic study was made of 16 patients with amyloidosis and with clinical signs of intestinal pseudo-obstruction. amyloid deposits in the small intestine were proved in all cases by endoscopic or intra-operative biopsies, and immunohistochemical study identified the chemical types of amyloid protein: amyloid A protein (AA) in 13 cases, light chain protein (AL) in two, and beta 2-microglobulin (AH) in one. Clinically, an acute self limiting obstructive condition was evident in 13 cases with AA, and 12 of them returned to normal bowel function after receiving total parenteral nutrition. Two cases with AL and one with AH presented chronic, intermittent, obstructive symptoms, and medical treatment, including total parenteral nutrition, was ineffective with no recovery of intestinal propulsion. Pathological examination of the necropsy specimens in seven cases showed considerable differences in the preferential sites of gastrointestinal deposits between the chemical types of amyloid; extensive infiltration and replacement of the muscularis propria by amyloid deposits throughout the gastrointestinal tract, especially the small intestine, were found in the AL and the AH cases, while amyloid deposits in the myenteric plexus without appreciable muscle infiltration were shown in the AA cases. These results show that intestinal pseudo-obstruction in patients with amyloidosis is caused by either myopathy or neuropathy, and that chemical types of amyloid may determine which of the two factors has the dominant affect on the bowel function.

Minute nonpolypoid adenomas of the colon depicted with barium enema examination.

To evaluate the usefulness of barium enema examination for detection of small nonpolypoid adenomas, the barium enema study findings were reviewed in patients with flat or depressed adenomas of the colon 3-5 mm in diameter that had been initially found during colonoscopy and that had been diagnosed as adenomas at histologic analysis. Eleven of the 21 nonpolypoid adenomas were depicted on radiographs; they tended to be located on the left side of the colon. The 10 other lesions were not detected retrospectively on barium enema radiographs. The radiographic findings in the depicted lesions included smooth, round, radiolucent areas; in seven of these lesions, round or irregular barium flecks were also seen. The radiolucent areas were attributed either to slightly elevated growth of adenomas or to surrounding hyperplasia of nonneoplastic glands; the central barium fleck seemed to conform to depressed areas within adenomas. Barium enema examination can demonstrate nonpolypoid adenomas of the colon, even though colonoscopy seems superior to barium enema examination for detection of these lesions.

Intestinal Behçet disease: serial changes at radiography.

To clarify the characteristic radiographic findings and serial radiographic changes in nonsurgical cases of intestinal Behçet disease (BD), seven patients were followed up for an average of 4 1/2 years (range, 8 months to 11 years 4 months). They underwent repeated examination with barium meal studies and double-contrast barium enema meal studies and double-contrast barium enema radiography. Initial radiography revealed one to three deep ulcers in the terminal ileum or ileocecal region in five patients. The ulcers healed while patients were treated with medical therapy but recurred during the follow-up period. In four of these five patients, follow-up radiography demonstrated various changes within each case: At each examination, an enlarged ulcer or a newly developed ulcer was often found in conjunction with a healed ulcer. In the other two patients, multiple shallow ulcers or innumerable aphthoid ulcers were seen throughout the entire colon, and these ulcers showed a good response to medical treatment. These results indicate that serial radiographic changes in patients with intestinal BD may mimic those in patients with Crohn disease, with findings in the terminal ileum and colon that wax and wane with medical treatment.

Natural history of fundic gland polyposis without familial adenomatosis coli: follow-up observations in 31 patients.

PURPOSE: To clarify the natural history of fundic gland polyposis (FGP) without familial adenomatosis coli (FAC). MATERIALS AND METHODS: Thirty-one FGP patients without FAC (six men and 25 women) were followed up with radiography and endoscopy for 1-13 years (mean, 4.3 years). RESULTS: In eight of 11 patients who initially had a single polyp, the polyp disappeared, probably due to endoscopic forceps biopsy. Of 20 patients with multiple polyps, seven (one man and six women with a mean age of 40.6 years at initial diagnosis) had obvious changes in the number of polyps, including complete disappearance in three patients, a decrease in one, transient disappearance followed by recurrence in two, and an increase in one. These patients were younger and had a greater number of polyps at initial diagnosis than the remaining 16 patients without any remarkable change. CONCLUSION: FGP without FAC is characterized by spontaneous decrease or increase in the number of polyps, especially in middle-aged female patients with a large number of polyps.

Plasma fibrinogen and its correlates in Japanese and US population samples.

In a preliminary 1987 study, we reported that the plasma fibrinogen level was significantly higher in Caucasian American men than Japanese men. To confirm this finding, we used data from 1,020 Japanese men and women in a 1989-1991 Akita, Japan, population study and from > 15,000 men and women from the 1986-1989 Atherosclerosis Risk in Communities (ARIC) Study. To examine further the correlates of plasma fibrinogen level, subsamples of nonsmoking Akita Japanese (n = 150) and Minneapolis Caucasians (n = 150) were also studied separately in 1990. Compared with the Japanese in the Akita study, Caucasians and African Americans in ARIC had a 23-40 mg/dL higher age-adjusted fibrinogen level for men and a 25-67 mg/dL higher level for women. In the subsample, the mean plasma fibrinogen value was 288 mg/dL in Caucasian men and 248 mg/dL in Japanese men (test for difference: p < 0.001). Women showed a similar racial difference: 300 mg/dL in Caucasians and 257 mg/dL in Japanese (p < 0.001). There were weak but positive correlations of plasma fibrinogen with age and body mass index and weak inverse correlations with alcohol intake, high density lipoprotein cholesterol, and triglycerides in most of the sex-race groups. For women, plasma fibrinogen was positively associated with menopause and inversely associated with the use of hormone replacement therapy. Total fish intake was inversely associated with plasma fibrinogen in all sex-race groups, and the association was statistically significant for Caucasian men.(ABSTRACT TRUNCATED AT 250 WORDS)

Associations of serum total cholesterol, different types of stroke, and stenosis distribution of cerebral arteries. The Akita Pathology Study.

BACKGROUND AND PURPOSE: The relation between serum total cholesterol levels and stroke is controversial. The Akita Pathology Study provides data on the association of serum total cholesterol, different types of stroke, and distribution of stenosis in cerebral arteries. METHODS: The data are based on 750 autopsied men aged 30 years and older who were admitted to a local hospital in northeast Japan between 1966 and 1984. The overall autopsy rate was 88%. The grade of stenosis in the cerebral arteries was determined blindly by one pathologist using Baker's method for basal cerebral arteries (atherosclerosis scores) and using microscopic examination of a single basal ganglion slide for the intracerebral penetrating arteries (arteriolosclerosis scores). RESULTS: The age-adjusted mean value of serum total cholesterol concentration was 164 mg/dL for cerebral hemorrhage, 177 mg/dL for infarction in penetrating artery regions, and 200 mg/dL for infarction in cortical artery regions. Mean serum cholesterol was lower in deaths caused by cerebral hemorrhage than in those caused by myocardial infarction and other cardiovascular disease. Mean atherosclerosis score of basal cerebral arteries was low for cerebral hemorrhage, intermediate for penetrating artery infarction, and high for cortical artery infarction. Stenosis of both basal and penetrating arteries was minimum or absent in cases of cerebral hemorrhage. Only the basal arteries were stenotic in cases of cortical artery infarction, whereas both basal and penetrating arteries were stenosed in cases of penetrating artery infarction. There were positive associations of serum cholesterol with stenosis of basal and penetrating arteries. Among cases of cerebral hemorrhage, serum total cholesterol levels were even lower in men with no significant stenosis in either basal or penetrating arteries than in men with stenosis in either type of artery. CONCLUSIONS: The association of serum cholesterol with pathogenesis varies among stroke types. Elevated serum cholesterol levels were associated with the presence of cortical artery infarction, while low serum cholesterol levels were associated with cerebral hemorrhage.

[Population-based comparative study on dietary habits and serum fatty acid compositions].

Appropriate dietary instruction on fat intake is required for Japanese to prevent coronary heart disease because there has been a consistent increase in fat intake during the past two decades. To gain fundamental information for dietary instruction, we examined the relation between dietary habits related to fat and serum fatty acid compositions in women aged 40-69 of four Japanese populations (fishing, coastal farming, inland farming and urban populations) and a Caucasian population in which different dietary habits have been reported. Among Japanese, dietary intake of monounsaturated and polyunsaturated fatty acids (in particular omega 6-polyunsaturated fatty acid) were higher in the urban population than in the other populations. Intake of omega 3-polyunsaturated fatty acids did not vary among the Japanese populations, but intakes of eicosapentaenoic acid and docosahexaenoic acid were higher in fishing and coastal farming populations than in the other two populations. Serum concentrations and compositions of omega 6-polyunsaturated fatty acids (mostly linoleic acid) were higher in the urban and inland farming populations than in the other two populations whereas those of omega 3-polyunsaturated fatty acids (in particular eicosapentaenoic acid) showed the opposite trend. The proportion of omega 6-polyunsaturated fatty acids was higher and that of omega 3-polyunsaturated fatty acids was lower in Caucasian women than in the Japanese populations. The ratio of omega 3 (polyunsaturated fatty acids) to omega 6 was highest in the fishing population (ratio = 0.38), intermediate in coastal farming (0.35), urban (0.27) and inland farming (0.24) populations, and lowest in Caucasians (0.07). Within populations, the percentage of serum omega 6-polyunsaturated fatty acids increased with the increase of frequency of meat intake and oil intake. The percentage of serum omega 3-polyunsaturated fatty acids increased with the increase in frequency of fish intake. Because there are evident associations between the frequency of fish, meat and oil intake and serum fatty acid compositions, a frequency questionnaire on these foods would be useful for dietary instruction in the prevention of coronary heart disease.

[Clinical and microbiological study concerning effects of scaling and root planing. 2. The effects of mechanical debridement until one year and of minocycline application topically].

This study was performed to assess the effect of scaling and root planing clinically and microbiologically during one year. Nine patients with moderate to severe periodontitis were studied. After the process of plaque control, the selected single rooted teeth with 5mm probing depth were debrided mechanically under non-local anesthesia four times biweekly. Observing the results of the treated teeth until three months, the experimental teeth were divided into two groups. The standard of division was whether they showed good response to the mechanical debridement or not. Observation was continued until one year after the debridement according to the following criteria: 1. The sites which showed good responses at three months were continued to be observed biweekly. 2. The sites which showed no improvement at three months were applied the gel of minocycline (Periocline) four times biweekly. 3. The untreated sites used as control until three months received mechanical debridement four times biweekly. The following results were obtained: The sites that showed good responses to the mechanical debridement at three months were maintained in good condition continuously during one year. The topical application of Periocline was effective in reducing the inflammation and probing depth clinically and microbiologically until three months.

Results and prognostic factors in the radical sinus operation for chronic sinusitis.

Radical surgical procedures of the maxillary sinus with or without involvement of the ethmoid, sphenoid and frontal sinuses were performed in 382 adult patients in the Department of Otorhinolaryngology. Tokai University Hospital in the 8-year period from 1984 to 1991. Of 382 patients, the postoperative symptomatology was studied in 189 patients by mcans of questionnaires. The postoperative results were finally analyzed in 63 patients whose subjective symptoms and objective findings were completely recorded. The follow-up period was over 3 months and subjective postoperative improvement was achieved in 83% and objective postoperative improvement in 75%; the overall improvement rate was 73%. In an analysis of results of the radical surgical operation, it appeared that poor results were closely related to the following factors: (1) maxillary sinus mucosa with cellular infiltration or edematous changes; (2) multiplicity of bacterial species including Gram-negative rods isolated from maxillary sinus secretion; (3) poor mucociliary clearance function; and (4) unsatisfactory postoperative treatment.

[Dietary habits and serum fatty acid compositions of women college students--changes in serum lipids and fatty acids by dietary education].

To obtain fundamental information for prevention of coronary heart disease in young adults, frequency of dietary intake, blood lipid and serum fatty acid compositions were examined for 114 female students. The relationship of selected food intake to serum fatty acid composition was also examined. Results of a nutritional survey were used to identify students who had a frequency of fish intake of "none" or "1-2 times/week" from whom random selection into two groups was accomplished: control group (n = 31) and education group (n = 31). The education group received a recommendation for increasing fish intake for 3 months. The effect of the education program was evaluated by examination of blood lipids, serum fatty acid compositions and frequency of dietary intake. Fifty-four percent of the students reported "none" or "1-2 times/week" for the frequency of fish intake, and 76% of them reported "1 and more times/day" for oil intake. The proportion of serum saturated fatty acids, monounsaturated fatty acids, polyunsaturated fatty acids were 28.9%, 24.4%, 46.8%, respectively. Of the polyunsaturated fraction, 41.4% was omega 6-polyunsaturated fatty acids, and that of omega 3-polyunsaturated fatty acids was 5.4%. The omega 3/omega 6 ratio was 0.13. Serum omega 3-polyunsaturated fatty acids was directly related to frequency of fish intake, and serum omega 6-polyunsaturated fatty acid, especially linoleic acid, was directly related to frequency of oil intake. After a 3-month education program, the frequency of fish intake and meat intake increased, while frequency of oil intake decreased in the education group. Serum omega 3-polyunsaturated fatty acid (in particular eicosapentaenoic acid, docosahexaenoic acid) increased in the education group, but not in the control group. The omega 3/omega 6 ratio increased from 0.13 to 0.18 in the education group. These results indicate that dietary education for fish intake is effective in increasing serum omega 3-polyunsaturated fatty acids, and the omega 3/omega 6 ratio for female students with initial low intake of fish and a high intake of omega 6-polyunsaturated fatty acids.

[Asynchronous Bell's phenomenon in progressive supranuclear palsy].

A 60-year-old man suffering from progressive supranuclear palsy showed asynchronous Bell's phenomenon and apraxia of eye-lid opening. He showed synchronous upward rotation of the eyeball at closing eyes. Although apraxia of eye-lid opening was observed in both sides, eye opening of the right side took longer duration than that of the opposite side. When he tried to open his eyes, unilateral Bell's phenomenon was sustained in the side of involuntary eye closing for a several seconds. These findings indicated disorders of the coordination of eye opening-and-closing, which regulate the orbicural muscle of eye, levator muscle of upper eyelid and external ocular muscles. And the mechanism of synchronous eye movement between the both sides was also disturbed. The administration of L-droxydopa was effective to gait disturbance, but not to ocular movement disorder including impaired Bell's phenomenon. The pharmacological results suggested different neuron system between locomotion control and conjugation affected in this disease.

[Long-term efficacy of low residue diet for the maintenance of remission in patients with Crohn's disease].

The long term efficacy of low residue diet (LRD) for the maintenance of remission in patients with Crohn's disease was studied. The patients consisted of 60 with quiescent Crohn's disease who were given LRD (Clinimeal or Ensure Liquid) for maintenance therapy for more than one year (average 2.4 years) after discharge from our institute. On the basis of the quantity of LRD given per day, the patients were classified into three groups: A; O to 799 kcal a day, B; 800 to 1599 kcal a day and C; more than 1600 kcal a day. The remission rate and the avoidance rate of admission were calculated and compared among three groups. The remission rate in group C was significantly higher than those in groups A and B. The avoidance rates of admission in groups B and C were significantly higher than that in group A. These results suggest that drinking of LRD at home is useful for the maintenance of remission in patients with Crohn's disease and that the effect of LRD depends on the quantity of LRD given per day.

[A clinico-MRI study of extrapyramidal symptoms in multiple system atrophy--linear hyperintensity in the outer margin of the putamen].

We did a clinico-MRI study concerning extrapyramidal symptoms and T2-weighted MRI findings of the putamen in twenty patients with multiple system atrophy (MSA) as well as twenty-five with idiopathic Parkinson's disease. Nine out of twenty MSA patients showed extrapyramidal symptoms. And we could not observe cerebellar ataxia in two of these patients because of severe rigidity and akinesia. Eight out of nine MSA patients with extrapyramidal symptoms showed linear hyperintensity in the outer margin of the putamen. This abnormal intensity was bilateral and symmetric in most patients. However, in MSA patients without extrapyramidal symptoms, only one patient showed the linear hyperintensity. We could not find such abnormal intensity in any patients with Parkinson's disease. On proton density MRI, the signal intensity in the lesion was higher than that in the gray matter, which leads the speculation that the hyperintensity is gliosis of the putamen or increased extracellular fluid space caused by severe shrinkage of the putamen. This characteristic MRI finding is useful to distinguish MSA with extrapyramidal symptoms from Parkinson's disease.
