[Randomized controlled trial of amino acid based diet versus oligopeptide based diet in enteral nutritional therapy of active Crohn's disease].

This study compared the short-term effect of semi-elemental diet in treatment of active Crohn's disease with elemental diet which has been reported to be successful in inducing remission. The important differences between semi-elemental diet and elemental diet in composition are the form of nitrogen and fat content. Forty patients with active Crohn's disease were randomized to receive for 6 weeks either elemental diet (n = 21) which is composed of amino acid based with low fat content or semi-elemental diet (n = 19) which is oligopeptide-based and has higher fat content. Both groups showed similar improvement in Dutch-AI, Crohn's disease activity index (CDAI), erythrocyte sedimentation rate, C-reactive protein, serum albumin, body mass index and radiographic findings of bowel lesion. The present study suggests that semi-elemental diet is equal to elemental diet for treating active Crohn's disease. We conclude the compositional differences between the two diets did not affect the short-term effect of enteral nutritional therapy for active Crohn's disease.

[A basic and clinical study of CA 19-9-MP-Mitsui kit for determination of CA 19-9].

Recently, the CA 19-9-MP-Mitsui kit (MP-kit) by a sandwich enzyme immunoassay using two mouse monoclonal antibodies of C192 and C241 for the determination of CA 19-9 was developed by Mitsui Pharmaceutical Co., Ltd.. We carried out basic and clinical studies of MP-kit for the serum CA 19-9 in comparison with the Immunoclone CA 19-9 EIA kit (I-kit) which was provided by Fuji-Rebio Company. It was found that MP-kit appeared to have excellent reproducibility with high sensitivity and specificity for CA 19-9. Furthermore, the coefficient of positive correlation was as high as 0.92 in serum CA 19-9 level between MP-kit and I-kit in the determination of serum CA 19-9 in 67 cases by each of two kits. On the other hand, with two kits, it was found that employment of a cut off value more than 70 U/ml brought a highest diagnostic efficiency for pancreatic and bile duct cancers, while employment of a cut off value of 37 U/ml which was used routinely resulted in a high false positive incidence, particularly in liver cirrhosis or hepatitis. From the results, it is concluded that a pertinent cut off value of CA 19-9 in serum was about 70 U/ml and MP-kit was available for determination of CA 19-9 in serum such as the other known kits on the market for CA 19-9.

[Significance of serum myosin light chain-1 level in neuroleptic malignant syndrome].

In Parkinson disease, the interruption of medication often results in neuroleptic malignant syndrome, which is caused by acute inhibition of the dopaminergic system. It is a serious question in the management of Parkinson disease, whether episode of pyrexia is derived from neuroleptic malignant syndrome or other origins. In this syndrome, muscle damage enhances serum titers of myogenic enzymes and proteins. Myosin light chain-1 (MLC-1), which is a small fragment of myosin, has been reported to show long lasting elevation compared with CK in neuromuscular diseases and cardiac infarction. Then, we evaluated the clinical significance of serum MLC-1 in six patients with neuroleptic malignant syndrome, i.e., four cases of Parkinson disease, one multiple system atrophy and one schizophrenia with acute administration of haloperidol. Muscle breakdown was observed by the elevation of serum MLC-1 titer, which sustained several days after normalization of serum CK titer. In two cases of Parkinson disease, high level of serum MLC-1 was observed in spite of normal body temperature, which suggested pre-clinical stage of neuroleptic malignant syndrome. Thus, we concluded that the estimation of serum MLC-1 is useful in those patients with high risk of neuroleptic malignant syndrome.

[Family make-up is associated with the admission rate of stroke patients--difference between Kochi and Shimane Prefectures and in a rural community].

Kochi Prefecture had a higher admission rate for patients with cardiovascular disease, particularly for stroke, than Shimane Prefecture which has a similar socio-economic level. Kochi Prefecture had a smaller average number of persons in a household, a lower proportion of three generation households, and a lower proportion of households with six persons or more than Shimane Prefecture. The decline in number of persons in a household was greater in Kochi Prefecture than in Shimane Prefecture from 1960 to 1985, especially between 1970 and 1975. The association between family make-up and hospital discharge one year after stroke was examined in a rural community in Kochi Prefecture. Two hundred forty nine patients aged 40 years and older who were admitted immediately after stroke events were registered between 1969 and 1988, and they were followed for one year. According to family make-up, the subjects were divided into four groups: 1) persons who lived with spouse and daughter-in-law, 2) persons who lived with spouse, 3) persons who lived with daughter-in-law, 4) persons who lived without spouse and daughter-in-law. Although mean age and the proportion of serious cases were not different between group 1 and group 4, the proportion of admitted patients of group 1 was significantly lower than that of group 4. About 60% of group 1 had households with six persons or more. Group 2 had the lowest proportion of admitted patients, but this group was significantly younger than other groups. Within group 3, the proportion of admitted patients was higher in households in which the daughter-in-law had a job than in households in which she did not. These results suggest that the high admission rate in Kochi Prefecture is attributable to households which are short of personnel for home care.

Evaluation of the efficacy of nicorandil in patients with ischemic heart disease by exercise Tl-201 myocardial SPECT.

The effect of nicorandil on myocardial perfusion in ischemic heart disease was studied using exercise-load Tl-201 myocardial SPECT (Ex-SPECT). ExSPECT was carried out in 12 patients with previous myocardial infarction (OMI) and 9 with angina pectoris (AP) before and after administration of nicorandil 15 mg/day, for three or more weeks; %Tl uptake and the washout rate in infarcted or ischemic areas were calculated from short axial images using the Bull's eye method. In the OMI group, %Tl uptake and washout rates in the infarction areas improved significantly from 52.4% and 0.25 before nicorandil to 60.4% and 0.38 after nicorandil. In the AP group, too, %Tl uptake and washout rates showed a significant improvement from 56.9% and 0.10 before to 69.1% and 0.33 after administration of nicorandil. Six subjects who had not received the drug, and who showed negative washout rates, had high improvement rates when nicorandil was administered, suggesting that the drug could increase myocardial perfusion during exercise loading as well as suppressing coronary spasm. Ex-SPECT was carried out in 4 subjects before and after the administration of nicarandil and after subsequent surgical treatment (PTCA or CABG) and the effects of the two therapies were compared. The washout rate was improved from 0.01 to 0.34 by the administration of nicorandil, and a notable increase in coronary artery blood flow was achieved compared to the level after surgical treatment, i.e. 0.50. It is concluded that normal dosages of nicorandil have a powerful direct effect of dilating the coronary arteries without any influence on preload or afterload.

[Secondary leukemia after etoposide treatment involved MLL gene rearrangement].

The 11q23 chromosomal abnormality is frequently observed in infantile leukemia and secondary leukemia, and the translocation associated gene in infantile leukemia is called mixed-lineage leukemia (MLL) gene. A 50-year-old man was admitted because of left axillary lymphadenopathy and IBL like T cell lymphoma was diagnosed by lymph node biopsy. The patient responded to the LSG-9 protocol with complete remission. After 10 months he was readmitted because of fever and was diagnosed acute myeloblastic leukemia by bone marrow aspiration. Chromosome analysis revealed 11q23 abnormality, suggesting that the leukemia was induced by etoposide treatment. Southern blot analysis demonstrated DNA rearrangement in the MLL gene at 11q23. It was suggested that the breakpoint region of the MLL gene in secondary leukemia is the same as that of infantile leukemia.

[Two cases of trochlear nerve neurinoma].

Two cases of trochlear nerve neurinoma without Recklinghausen's disease are described. A 60-year-old male had a left-side sensory disturbance and a hemiparesis. Computed tomography (CT) scan and magnetic resonance image (MRI) showed a cystic enhancing mass in the right tentorial incisura. A 57-year-old male had a gait disturbance and a left-side facial numbness. CT scan and MRI showed a solitary enhancing mass in the right tentorial incisura. These tumors were subtotally removed, and the intraoperative diagnosis of two patients was trochlear nerve neurinoma. Unique clinical manifestations of trochlear nerve neurinoma are discussed with a brief review of 11 cases reported in the literature.

Cancer in Peutz-Jeghers syndrome.

BACKGROUND: Peutz-Jeghers (P-J) syndrome has been found to be associated with an increased risk of malignant neoplasia. METHODS: The authors reviewed the clinical courses of eight patients with P-J syndrome (four male and four female patients, ranging in age from 8 to 55 years), who had been followed up for as long as 12 years. The diagnosis of this syndrome was based on the evidence of characteristic mucocutaneous pigmentations and gastrointestinal hamartomatous polyposis. RESULTS: Four cases of malignant neoplasm among the eight patients were found. In a 25-year-old woman, an extremely well-differentiated adenocarcinoma of the uterus cervix was found at the initial examination. A gastric cancer in a 32-year-old woman, duodenal cancers in a 43-year-old man, and a pancreatic cancer in a 60-year-old man also were identified 12, 10, and 5 years after the time of initial examinations, respectively. Two of these patients died of the effects of the tumor soon after. CONCLUSION: The authors' experience confirms a veritable malignant potency in P-J syndrome and suggests that an intensive follow-up of gastrointestinal and extra-gastrointestinal sites is needed in patients with this syndrome.

Antigens of tissue plasminogen activator and plasminogen activator inhibitor 1: correlates in nonsmoking Japanese and Caucasian men and women.

We reported in a 1987 preliminary study that tissue plasminogen activator antigen was significantly higher in American Caucasian men than in Japanese men. To further examine possible differences in fibrinolytic activity between the two races, an expanded study was conducted in a total of 300 nonsmoking men and women aged 47-69 years in two population-based samples: rural Japanese living in Akita and Caucasians living in Minneapolis-St. Paul, MN. Antigens of tissue plasminogen activator (t-PA) and plasminogen activator inhibitor 1 (PAI-1) were measured. Mean t-PA antigen was 2.3 ng/ml higher in Caucasian men than in Japanese men (P < 0.001), but no race difference was seen for women (P = 0.59). Mean PAI-1 was higher in Caucasians than in Japanese for both sexes, and the race difference in mean was 1.8 ng/ml for men (P = 0.07) and 4.4 ng/ml for women (P < 0.001). Both t-PA and PAI-1 were associated positively with body mass index and blood triglycerides for all sex-race groups, and positively with alcohol intake for Japanese and Caucasian men. Compared to Japanese, Caucasians of both sexes had higher levels of body mass index and blood triglycerides, and lower average intake of alcohol among men. Even when adjusted for body mass index, triglycerides, alcohol and other cardiovascular risk factors, the race difference in mean t-PA antigen persisted for men (P < 0.001), as did the difference in mean PAI-1 for men (P = 0.03) and women (P = 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

Chemoattractants for neutrophils in lipopolysaccharide-induced inflammatory exudate from rats are not interleukin-8 counterparts but gro-gene-product/melanoma-growth-stimulating-activity-related factors.

Potent chemotactic activity for neutrophils was detected in rat inflammatory exudate induced by a subcutaneous injection of lipopolysaccharide in a carboxymethyl-cellulose suspension. We purified and characterized chemoattractants from the exudate by the following procedures: carboxymethyl-Sephadex C-25 ion-exchange chromatography; G3000SW gel-filtration chromatography; preparative reverse-phase high-pressure liquid chromatography; rechromatography on reverse-phase HPLC. Two chemotactic factors were purified and their N-terminal amino acid sequences were determined. One factor was a protein in which the first 20 N-terminal amino acids were identical to those of rat cytokine-induced neutrophil chemoattractant (CINC), a counterpart of human gro/melanoma growth-stimulating activity (MGSA). The other factor was highly similar to mouse macrophage inflammatory protein 2 (MIP-2). Mouse MIP-2, a chemotactic factor for neutrophils, is a member of the interleukin-8 family; however the protein we purified had higher similarity to human gro/MGSA than to human interleukin-8. These results indicate that, in rats, chemotactic factors for neutrophils induced by lipopolysaccharide stimulation are not counterparts of interleukin-8, but are gro/CINC-related peptides.

Diagnosis of primary early gastric lymphoma. Usefulness of endoscopic mucosal resection for histologic evaluation.

BACKGROUND AND METHODS: Eight patients thought to have primary early gastric lymphoma were studied to evaluate the usefulness of endoscopic mucosal resection for the diagnosis of reactive lymphoid hyperplasia (RLH) or malignant lymphoma. RESULTS: In these patients, conventional endoscopic forceps biopsy did not provide a definite histologic diagnosis. Endoscopic ultrasonography (EUS) demonstrated that the disease was limited to the mucosa and the submucosa in all patients, without any paragastric lymph node involvement, so endoscopic mucosal resection was performed to obtain specimens large enough to include the submucosa. As a result, primary early gastric lymphoma was diagnosed in five patients, for whom subsequent gastrectomy confirmed the diagnosis. CONCLUSIONS: The authors recommend that endoscopic mucosal resection be performed in patients thought to have primary early gastric lymphoma and for whom a definite diagnosis can not be made by conventional endoscopic forceps biopsy.

Effect of lentinan on lymphocyte subsets of peripheral blood, lymph nodes, and tumor tissues in patients with gastric cancer.

The effect of lentinan on lymphocyte subsets of peripheral blood, lymph nodes, and tumor tissues in gastric cancer patients was investigated. A 2-mg dose of lentinan was administered to 12 patients intravenously twice, the first at 3-9 days before and the second the day before surgery. The results were then compared with a control group without lentinan administration comprising 12 patients. Regarding peripheral blood and lymph nodes without metastasis, lymphocyte subsets defined with anti-CD3, anti-CD4, anti-CD8, anti-Leu7, and anti-Leu11 were analyzed by flow-cytometry. As for tumor tissues, lymphocyte subsets defined with anti-CD3, anti-CD4, anti-CD8, anti-Leu11, and anti-M3 were analyzed after immunohistochemical staining. There were no significant changes in the lymphocyte subsets of peripheral blood between the two groups. In the lymph nodes, the CD4 cell ratios increased; otherwise in regard to tumor-infiltrating lymphocytes (TILs), the number of CD4, Leu11 and LeuM3 cells showed a prominent increase. Therefore, lentinan was observed to produce different effects in accordance with the subjective organs.

Multiple vitamin status in Crohn's disease. Correlation with disease activity.

We measured serum, blood, or red cell concentrations of various vitamins in 24 patients with Crohn's disease who had been free from any nutritional treatment, and compared them with those in 24 healthy controls. Twelve of the patients were affected in the small bowel only, two in the large bowel only, and the remaining 10 in both the small and large bowel. The fat-soluble vitamins A and E were significantly decreased in patients with Crohn's disease compared to controls. Among the water-soluble vitamins, vitamins B1, B2 and B6 and folic acid were more depleted in patients with Crohn's disease than in the controls, whereas vitamins B12 and C, nicotinic acid, and biotin were not different between the two groups, and pantothenic acid was increased in patients with Crohn's disease. In addition, vitamin B2 and nicotinic acid showed a negative correlation with the Crohn's disease activity index. These findings suggest that there is a variety of vitamin deficiencies in Crohn's disease prior to treatment and also that concentrations of some vitamins, such as vitamin B2 and nicotinic acid, may reflect the severity of the disease.
