Epidemiological study of lathyrism in northwestern districts of Bangladesh.

Two northwestern districts of Bangladesh with a population of 629,752 were surveyed from June 1991 to March 1993 to detect and follow up lathyrism patients. Out of 2,567 neurological patients, 882 were diagnosed as having lathyrism, giving a prevalence rate of 14.0/10,000. This prevalence was higher among young males: only 12.9% of the patients were female, and only 19.3% of the patients were over 30 years of age at onset of the disease. The average family size was 4.6 members. In the surveyed area, 730 families were evaluated: 622 families had only 1 affected member, and 108 families had 2-8 affected members. Most of the patients were working, while only 4% were not. The majority of the patients had a very low intake of animal protein. A few cases of human T-lymphotropic virus infection and osteolathyrism were found during this study.

Immunological analysis of organized pneumonia with eosinophilic pleural effusion.

A 73-year-old man developed organized pneumonia with severe right-sided eosinophilic pleural effusion (PE). CD69+ and HLA-DR+ eosinophils in PE were 90 and 31%, respectively, but were undetectable in peripheral blood (PB). CD4+, CD45RA+ (naive) and CD4+, CD45RO+ (memory) cells in PB, PE and bronchoalveolar lavage (BAL) were 10.9, 8.4, 2.5 and 22.7, 38.3 and 16.4%, respectively. CD8+, CD45RA+ (naive) and CD8+, CD45RO+ (memory) cells in PB, PE and BAL were 3.5, 4.7, 1.0, and 8.9, 11.3 and 46.0%, respectively. The concentrations of interleukin-5 (IL-5) and IL-6 in PE were 1,680 and 2,797 pg/ml, respectively; however, these cytokines were undetectable in PB. The patient died 1 month after surgery to remove right thickened pleura. Microscopic findings showed right fibrinous pleuritis and organized pneumonia.

Effect of gamma radiation on fetal haemopoiesis of mouse.

Effect of a single acute exposure to gamma-rays during the late fetal period on the fetal haemopoietic tissue of mouse was studied. Pregnant Swiss albino mice were exposed to 1 Gy of gamma-rays on day 17 post coitus (late fetal period) and 24 hr after exposure the fetuses were observed for changes in the liver weight, cytogenetic damage in liver cells by micronucleus (MN) induction and stem cell survival by spleen colony (CFU-S) assay. Irradiation resulted in a significant (P < 0.01) decrease in fetal liver weight. A significant (P < 0.001) increase in MN count was observed after exposure, while CFU-S displayed a significant (P < 0.001) reduction in the cell survival as compared to the sham-treated control. These results demonstrate the high susceptibility of mouse fetal haemopoietic system to radiation.

Immunohistochemical study of proliferating cell nuclear antigen in hepatocytes of biliary atresia: a parameter to predict clinical outcome.

The hepatic lobulus was studied histologically and immunohistochemically using the monoclonal antibody for proliferating cell nuclear antigen (PCNA/Cyclin, a cell-cycle-related nuclear protein) in 27 patients with biliary atresia (BA) and six normal infants. The study showed that the labeling index (LI) for PCNA-positive hepatocytes was 37.21 +/- 17.75% in the patients with BA and 3.14 +/- 1.5% in the normal infants (P < .0001). LI for PCNA-positive cells was higher in the periportal area than the pericentral area (P < .01). LI was not related to the patients' age at the time of hepatic portoenterostomy. LI was 20.80 +/- 7.03% for patients whose jaundice cleared postoperatively and 48.49 +/- 13.43% for patients who had persistent jaundice (P < .001). Conventional histological studies of the same specimens showed common findings of BA, such as hepatocellular degeneration, necrosis, inflammatory cell infiltration, and giant cell transformation. Most patients with BA had 8.94 +/- 13.55% giant cell transformation among 1,000 hepatocytes. Patients who exhibited high giant cell transformation had an unfavorable outcome. Only 0.42% of giant cells were immunoreactive for PCNA. In conclusion, the PCNA expression of hepatocytes is closely related to the prognosis of patients with BA, and thus could be used as a prognostic indicator.

Adhesion molecules on eosinophils in acute eosinophilic pneumonia.

We describe the clinical features, eosinophil surface analysis, and concentration of interleukin-5 (IL-5) in bronchoalveolar lavage (BAL) and in peripheral blood (PB) of a young male patient with acute eosinophilic pneumonia. Eosinophilic pneumonia was diagnosed by BAL and by its clinical course. There was no evidence of an infectious etiology, and the patient showed rapid improvement with methylprednisolone pulse therapy (1 gm/d for 3 d). Significant differences in the expression of surface molecules such as CD54, HLA-DR, and CD69 were observed between BAL and PB eosinophils; IL-5 concentrations in BAL and in PB before treatment were 450 pg/ml and 700 pg/ml, respectively. After treatment, IL-5 concentration in PB was reduced to below 100 pg/ml. The patient has shown no recurrence after treatment.

Periodic angioedema with eosinophilia: increased serum level of interleukin 5.

A 48-year-old man developed episodic non pitting edema and eosinophilia. Symptoms were alleviated promptly when treated with prednisolone. However, major basic protein (MBP), eosinophil cationic protein (ECP), and eosinophil-derived neurotoxin (EDN) levels in the serum as well as the total number of eosinophils remained high. During periods of attack serum levels of interleukin-5 (IL-5) were elevated, but the levels were lowered following treatment, suggesting that IL-5 is involved in periodic angioedema with eosinophilia.

IL-5 predominant in bronchoalveolar lavage fluid and peripheral blood in a patient with acute eosinophilic pneumonia.

We describe an acute eosinophilic pneumonia (AEP) patient with bronchoalveolar lavage fluid (BALF). Eosinophil cell number (47%), content of interleukin (IL)-5 (8.22 x 10(2) pg/ml) and eosinophil cationic protein (9.25 micrograms/ml) were high in BALF. No eosinophilia was seen in peripheral blood on admission; however, content of IL-5 was 9.47 x 10(2) pg/ml. After methylprednisolone pulse therapy, he improved rapidly with a reduction in eosinophil cell number (7%) and the content of IL-5 (< 100 pg/ml) in BALF. However, a high content of IL-5 (6.9 x 10(2) pg/ml) and transient eosinophilia (17.5%) were seen in peripheral blood. It is important to distinguish between AEP and infectious pneumonia, because of the differing treatments. If the diagnosis of AEP is doubtful, BALF should be performed early.

Study of HTLV-I antibodies in CSF and serum of neurolathyrism patients in Bangladesh.

Neurolathyrism is a form of human spastic paraparesis related to the overconsumption of the legume Lathyrus sativus or grass pea (Khesari in Bangladesh) containing the neurotoxin 3-N-oxalyl-2,3-diaminopropanoic acid (beta-ODAP). The clinical symptoms of neurolathyrism are similar to those of Tropical Spastic Paraparesis. In order to eliminate the proposed causative agent of TSP (HTLV-I) as a potential cause of the symptoms ascribed to neurolathyrism, a total of 444 diagnosed lathyrism patients were screened for HTLV-I antibodies. 50 CSF and 394 serum samples were collected from male (415) and female (29) patients. Only 4 serum samples were found sero-positive for HTLV-I. This agrees with the assumption that overconsumption of beta-ODAP containing Lathyrus seeds, and not HTLV infection, is the causative agent for neurolathyrism.

New findings and symptomatic treatment for neurolathyrism, a motor neuron disease occurring in north west Bangladesh.

Neurolathyrism is a form of spastic paraparesis caused by the neuroexcitatory amino acid 3-N-oxalyl-L-2,3-diaminopropanoic acid (beta-ODAP) present in the seeds and foliage of Lathyrus sativus. The disease is irreversible and usually nonprogressive. Tolperisone HCl, a centrally acting muscle relaxant, has been shown to reduce significantly the spasticity in neurolathyrism patients. Sporadic occurrence of HTLV-1 infection (0.9%) and of osteolathyrism was found among the neurolathyrism patients. Osteolathyrism is linked to the consumption of the green shoots of Lathyrus sativus.

A longitudinal study of risk factors associated with the formation of pressure ulcers in nursing homes.

OBJECTIVE: To determine risk factors associated with the formation of stage II-IV pressure ulcers in nursing homes. DESIGN: Since the incidence rate for pressure ulcer formation varies among nursing homes, the homes were divided into tertiles based on these rates. Pooled logistic regression was used to model which factors are associated with the formation of pressure ulcers in both high and low incidence homes. SETTING: 78 National HealthCorp nursing homes. SUBJECTS: We studied 4232 nursing home residents free of pressure ulcers on admission to a nursing facility and at 3-months follow-up. All remained in the home for at least 3 additional months to a maximum of 21 months. MEASUREMENTS: The effects of age, gender, race, antipsychotic drug use, urinary incontinence, fecal incontinence, body mass index, diabetes mellitus, disorientation, ambulation, physical restraints, activities of daily living of bathing, feeding, or transferring, and nursing home bed size on the formation of a stage II-IV pressure ulcer while the subject was a resident in the nursing home were studied. MAIN RESULTS: Significant factors associated with the formation of pressure ulcers in high incidence homes (21-month incidence = 19.3%) were ambulation difficulty (OR = 3.3; CI = 2.0, 5.3), fecal incontinence (OR = 2.5; CI = 1.6, 4.0), diabetes mellitus (OR = 1.7; CI = 1.2, 2.5), and difficulty feeding oneself (OR = 2.2; CI = 1.5, 3.3). In the low incidence homes (21-month incidence = 6.5%), significant factors associated with pressure ulcer incidence were ambulation difficulty (OR = 3.6; CI = 1.7, 7.4), difficulty feeding oneself (OR = 3.5; CI = 2.0, 6.3), and male gender (OR = 1.9; CI = 1.2, 3.6). CONCLUSIONS: Although low and high incidence homes share similar risk factors, such as ambulation and feeding activities of daily living, the main difference was that diabetes and fecal incontinence played a major role only in high risk homes, while male gender was an important discriminator only in low incidence homes. Yet, it is unclear if these factors explain the three-fold difference in the incidence rates for pressure ulcers in these facilities. Baseline or resident clinical characteristic differences of any one factor between the high and low incidence homes varied by no more than 5%. While we identified certain conditions which are associated with pressure ulcer formation, there may be unknown or unmeasured facility effects in addition to the characteristics of a given resident in a particular home.

Cytokine production in patients with mite-sensitive bronchial asthma.

We examined the eosinophil viability-enhancing activity (EVEA) of peripheral blood mononuclear cells (PBMNCs) obtained from mite-sensitive bronchial asthma (BA) and normal control subjects. Mite concentrations of 1 and 10 micrograms/ml significantly increased EVEA in BA patients as compared with normal controls (p < 0.05 and p < 0.05, respectively). The level of IFN-gamma in PBMNC culture supernatants was higher in BA patients than in normal controls. Dexamethasone, cyclosporin A and FK506 significantly inhibited EVEA in BA patients (p < 0.05 to p < 0.001).

Eosinophil viability-enhancing activity in mite-sensitive bronchial asthma.

We examined the eosinophil viability-enhancing activity (EVEA) of peripheral blood mononuclear cells (PBMNCs) obtained from 6 patients with mite-sensitive bronchial asthma (BA) and 9 normal control subjects. Mite concentrations of 1 microgram/ml and 10 micrograms/ml significantly increased EVEA in PBMNC culture supernatants from BA patients compared with PBMNCs from normal control subjects (76.1 +/- 11.0% at 10 micrograms/ml and 56.3 +/- 16.0% at 1 microgram/ml vs 20.6 +/- 12.6% at 10 micrograms/ml and 7.4 +/- 2.3% at 1 microgram/ml; p < 0.05). The level of IFN-gamma in PBMNC culture supernatants in BA patients was 2.3 +/- 0.9 IU/ml and in normal control subjects 0.7 +/- 0.3 IU/ml. A combination of mAbs (anti-IL-3, anti-IL-5 and anti-GM-CSF, with or without anti-IFN-gamma) neutralized the EVEA (p < 0.001, p < 0.001, respectively). Dexamethasone (10(-8) M to 10(-5) M), cyclosporin A (10(-7) M to 10(-5) M) and FK506 (10(-8) M to 10(-6) M) significantly inhibited EVEA in BA patients (p < 0.05 to p < 0.001). The release of eosinophil cationic protein (ECP) from eosinophils in the presence of mite-stimulated PBMNC culture supernatants was higher in patients with bronchial asthma (569 +/- 147 micrograms/l) than in normal control subjects (203 +/- 99 micrograms/l; p < 0.05).

Effects of various drugs (staurosporine, herbimycin A, ketotifen, theophylline, FK506 and cyclosporin A) on eosinophil viability.

Eosinophils are known to play an important role in the pathogenesis of asthma and other allergic diseases. This study demonstrated the effects of various drugs on eosinophil viability in vitro, which might help clinicians and researchers in treating and studying eosinophilic diseases. Staurosporine, a protein kinase C inhibitor, and herbimycin A, a tyrosine kinase inhibitor, at 10(-6) M and 10(-7) M significantly lowered eosinophil viability in a dose-dependent fashion (p < 0.002, p < 0.02 and p < 0.001, p < 0.002, respectively). Both staurosporine and herbimycin A reduced eosinophil survival in a time-dependent fashion at 10(-6) M and 10(-7) M. Ketotifen at 10(-4) M and theophylline at 10(-3) M, significantly decreased eosinophil viability (p < 0.001 and p < 0.001, respectively) in the presence of 100 pg/ml of recombinant human interleukin-5 (rhIL-5). Both FK506 and cyclosporin A at 10(-4) M significantly reduced eosinophil viability (p < 0.001 and p < 0.005, respectively) in the presence of 100 pg/ml of rhIL-5. Our data show that ketotifen, theophylline, FK506, cyclosporin A reduced eosinophil viability at a high concentration. Furthermore, it is suggested that protein kinase C and tyrosine kinase are involved in eosinophil survival.

The bimodal mortality pattern of acute renal failure in children.

A total of 1784 children suffering from acute nonobstructive anuric renal failure requiring dialysis were studied in the Pediatrics Nephrology unit of Institute of Postgraduate Medicine and Research, Dhaka, Bangladesh, for the 10-year period 1981-1990. The patients were divided into two groups: Group I, the majority of the children (94.5%), presented with acute renal failure without evidence of hemolysis and were labeled as acute ischemic renal failure (AIRF), and the remaining 5.5% of the children with acute renal failure (ARF) showing features of severe hemolysis were labeled as hemolytic uremic syndrome (Group II.) The children of group I had mean age 4 (0.5-10) years, mean period of anuria 5 (2-13) days, and had mean serum creatinine 701 +/- 267.98 mumol/L. The overall mortality in this group was 89%. Children in Group II had mean age 2 (0.2-6) years, mean duration of anuria 3 (2-5) days, and mean serum creatinine level 500 +/- 100 mumol/L. The mortality rate in this group was 15%. In spite of some differences between the two groups the difference in the mortality rate between the two groups was significant (p < 0.001). It is concluded that AIRF in children bears a much poorer prognosis compared to that of ARF due to HUS.

Pharmacokinetics and pharmacodynamics in the design of controlled-release beads with acetaminophen as model drug.

Relationships between pharmacodynamics (drug concentration and effect) and pharmacokinetics were used to develop an oral, controlled-release-bead dosage form. Reported pharmacodynamic data were used with pharmacokinetic curves to identify effective therapeutic drug concentrations for optimum therapy for a drug with a "deep tissue" effective compartment. The commonly used, over-the-counter, non-narcotic, analgesic-antipyretic acetaminophen (APAP) was used as the model drug. Data reported in the literature were used to compare analgesic and antipyretic efficacy. Computer simulations were performed with MAXSIM (version 3.01) to suggest a zero-order drug release useful for a 12-h, oral, sustained-dosage form for antipyretic therapy in children, on the basis of current pediatric dosing of APAP. Coated APAP beads with the desired release rate were then developed with fluid-bed coating technology.

Studies on virulence determinants of Plesiomonas shigelloides.

Virulence determinants of nineteen strains of Plesiomonas shigelloides isolated from stool samples of diarrhoeal children were studied. Heat-labile toxin was detected in seven strains using rat ileal loop model and in none of the strains using Chinese hamster ovary cell assay system and by Enzyme-linked immunosorbent assay. Rat ileal loop model was used for the first time to assay toxin in P. shigelloides and is suggested to be a cheap and effective method of detecting labile toxin in the organism. Heat stable toxin, cytotoxin, hemolysin and hemagglutinin were not detected in the strains tested. Invasiveness could not be established by using HEp-2 cell assay system. The results of this study provide some experimental support for an etiological role for P. shigelloides in the production of diarrhoea.
