Macro-fingerprinting analysis at the chromosomal genomic DNA level of isolates of thermophilic Campylobacter coli and C. jejuni, by pulsed-field gel electrophoresis.

Chromosomal genomic DNA from 25 strains of thermophilic Campylobacter coli and C. jejuni were analysed after pulsed-field gel electrophoresis (PFGE) of restriction fragments generated by SalI and SmaI to evaluate the utility of macro-fingerprinting analysis and to assess the possible subtyping of the isolates. Two rare-cutting restriction enzymes, SalI and SmaI, allowed the subclassification of eight strains of C. coli into six types. SalI facilitated the subclassification of seventeen strains of C. jejuni into thirteen types and SmaI the subclassification of seventeen strains into twelve types. The diversity in the PFGE profiles was demonstrated and, therefore, the two enzymes were found to be potentially useful for detecting variability of PFGE profiles among strains of C. coli and C. jejuni. The two enzymes and PFGE may be useful in molecular epidemiology.

[Coronary artery spasm after mitral valve replacement: a case report].

A 50-year-old female with a malfunctioning bioprosthetic mitral valve which was implanted 7 years earlier underwent reoperation. She had no history of angina pectoris. Calcium channel blockers and nitrates had not been taken by the patient. Coronary arteriograms were normal. About 2 hours after the reoperation, EKG monitor showed abrupt ST segment elevation which was immediately followed by ventricular tachycardias and fibrillations. This life threatening circulatory collapse recurred until a drip infusion of diltiazem was started. Maximum CPK-MB was 145 IU/l. Postoperative coronary arteriography, which was performed after 12 hours of withhold of diltiazem, showed a spasm in the proximal segment of the right coronary artery. Coronary artery spasm should be considered among the causes of abrupt and unexpected circulatory collapse after open heart surgery even in the absence of coronary artery disease.

[Quantitative analysis of 123I-metaiodobenzylguanidine myocardial imaging: assessment of its usefulness in patients with congestive heart failure].

To investigate the usefulness of the quantitative analysis of 123I-metaiodobenzylguanidine (123I-MIBG) myocardial uptake, we studied 9 normal subjects and 18 patients with congestive heart failure (CHF). Rest myocardial imaging with 123I-MIBG was performed at 20 minutes and 3 hours (delayed image) after 123I-MIBG injection. Rest 201Tl imaging was obtained at 20 minutes after 201Tl injection. In addition to ordinary tomograms, a planar anterior image and a whole body image were supplemented in each imaging. In patients with CHF fractional shortening (%FS) was calculated from echocardiography and left ventricular ejection fraction was obtained from cardiac blood pool imaging with 99mTc at rest. We calculated H/M (heart to mediastinum count ratio) from the anterior planar image and %Uptake (percentage of cardiac uptake of the isotope to total injected dose) from the whole body image. H/M of 123I-MIBG in delayed images separated patients with CHF from normal subjects (2.00 +/- 0.19 vs. 2.56 +/- 0.13, p < 0.01). H/M Ratio (H/M of 123I-MIBG divided by H/M of 201Tl) in delayed image could distinguish these two groups poorly (0.72 +/- 0.12 vs. 0.88 +/- 0.14, p < 0.05). On the other hand, %Uptake of 123I-MIBG was not different between two groups (3.49 +/- 0.60% in CHF, 3.54 +/- 0.34% in normal). But %Uptake of 201Tl was greater in CHF than in normal (5.96 +/- 1.09% vs. 4.70 +/- 0.30%, p < 0.05). When myocardial 123I-MIBG uptake was normalized by myocardial perfusion (%Uptake of 123I-MIBG divided by %Uptake of 201Tl, Uptake Ratio), Uptake Ratio in delayed image could distinguish theses two groups as same as H/M (0.60 +/- 0.05 in CHF, 0.75 +/- 0.05 in normal, p < 0.01). In patients with CHF, H/M of 123I-MIBG did not reflect LV function and serum norepinephrine (NE) level. But Uptake Ratio and H/M Ratio in delayed image correlated well with %FS (r = 0.88, r = 0.65), EF (r = 0.80, r = 0.68) and NE level (r = -0.77, r = -0.75). Although the calculation of Uptake Ratio is time consuming and expensive, it was assumed that Uptake Ratio is an useful index to quantitate myocardial 123I-MIBG uptake.

[Quadricuspid aortic valve: three case reports and review of the literature].

Three patients with grade 3 aortic regurgitation caused by rare congenital quadricuspid valve underwent aortic valve replacement. Patients were two females (51 and 45 years old) and a 51 year-old male. The first case showed 2 equal larger and 2 equal smaller valves. In the latter two cases, 4 equal sized cusps were noted. Fibrous trabeculations bridging the aortic wall and the commissures, giving an appearance of a hammock, were noted in the third case. Small fenestations were also noted in two cusps in this case. These findings may suggest dysplastic feature of the quadricuspid aortic valve. Hypertension seemed to have played an important role in the occurrence of regurgitation in their 4th or 5th decade of life. Their postoperative courses were uneventful.

[A case of emergent surgical salvage for the mitral prosthetic perivalvular leakage diagnosed only by the transesophageal Doppler echocardiography].

A case of the successful emergency surgery for the mitral prosthetic perivalvular leakage (PVL) which was diagnosed only by the transesophageal Doppler echocardiography (TEE) is reported. A 59-year-old male who had been diagnosed MS + TR + severe PH underwent mitral valve replacement with St. Jude medical valve 27 M. On the eighteenth postoperative day hemoglobinuria was noticed suddenly. Subsequently hemolysis and nonoriguric renal failure was aggravated, then we suspected PVL, but heart murmur was not audible or PVL could not been detected by the transthoracic echocardiography (TTE). PVL was detected for the first time by the TEE, then open heart resuture was emergently performed. The site of the PVL was the portion where the anterior mitral annulus adjoined the aortic annulus, that is, adjacent to the right fibrous trigone. This site is the portion which a surgeon should treat very carefully and where the PVL is very difficult to detect by the TTE. In regard to the detection of the PVL in mitral position, the TEE, which has ultrasonically no flow masking by the mitral prosthesis, is the very useful diagnostic means.

Chemopreventive effects of tamoxifen in ethyl methanesulphonate-induced rat mammary carcinogenesis.

Ethyl methanesulphonate (EMS), an alkylating agent and a potent mutagen, has been shown to be an effective carcinogen for the induction of mammary carcinoma in female Wistar King A rats. We therefore utilized this new system to assess the effects of tamoxifen (TAM) on mammary carcinogenesis. In Group A rats, given EMS orally for a period of 12 weeks, mammary carcinomas were first detected at the 13th week and were found in all surviving rats at the 20th week. The concomitant administration of TAM for 4 weeks, in Group B rats, retarded the development of the tumors significantly. There was a significant reduction in the incidence of estrogen receptor (ER)-positive tumors in the rats previously exposed to TAM; 100% in Group A versus 50% in Group B. Neither the progesterone receptor (PgR) nor androgen receptor (AR) status of the tumors were significantly different between these two groups. The inhibitory effects of TAM on tumor induction was also observed when TAM treatment started after EMS administration, though the intensity was smaller than that in Group B. These findings suggest the preventive action of TAM on EMS-induced mammary carcinogenesis, and indicate that this tumor system may provide a feasible model for research on chemoprevention and hormone therapy using an antiestrogen for human mammary carcinoma.

Coronary neovascularity: a possible sign of new and growing mural thrombus.

This is a case report of a coronary artery fistula through a mural thrombus on a left ventricular aneurysm into the left ventricle. Serial coronary arteriography disclosed a vessel exhibiting a "smokelike" appearance originating at the anterior descending artery. This abnormal finding faded away over a period of 10 years with a decrease in size of the mural thrombus. This finding strongly suggests that the abnormal vessel represents an acquired neovascularity related to a newly developed thrombus.

A monoclonal antibody disrupting cell-cell adhesion of rat ascites hepatoma cells.

A cell surface-associated adhesive factor (AF) separated from differentiated rat ascites hepatoma AH136B cells (forming cell islands in vivo) has been highly purified by chromatography. AF is assumed to mediate the cell-cell adhesion essential to island formation of the hepatoma cells. A substance, immunologically cross-reactive with AF, is present in the ascites fluid or culture medium of the AH136B cells. Because the substance is almost identical to AF in molecular weight and aggregation-promoting activity, it has been concluded that AF is released into the ascites fluid where it is concentrated. Monoclonal antibodies have been raised against AF purified from ascites fluid of AH136B cells. We have obtained a monoclonal antibody, coded MoAF-6D6, that strongly abolishes the aggregation-promoting activity of AF. When AH136B cell islands are incubated in the presence of Fab fragments of MoAF-6D6, cell detachment from the islands is evident within 24 h. Cell islands following 36-h culture show a distinct dissociation and islands completely lose their organization 48 h after culture. The dissociating effect of MoAF-6D6 is neutralized by the addition of AF. These results suggest that AF plays a significant role in the maintenance of cell islands.

Sequence analysis of putative structural regions of hepatitis C virus isolated from 5 Japanese patients with hepatocellular carcinoma.

The nucleotide sequences of the 5'-portion of the HCV genome, consisting of 2672 nucleotides, were determined by analysis of clones derived from 5 different patients with liver cirrhosis and hepatocellular carcinoma. Comparison of 5 genomes and 8 isolates already reported demonstrated the significant genome diversity in the structural regions. Genetic divergence was outstanding in the envelope regions (E1 and NS1/E2, > 59.7% identity) in contrast to the sequence conservation of the 5' non-coding (> 93.8% identity) or core regions (> 82.0% identity). Although hypervariable regions were found in the NS1/E2 region as reported, hydropathicity was maintained for all clones, reflecting the biological functions of these regions.

Mycoplasmas stimulate replication of human immunodeficiency virus type 1 through selective activation of CD4+ T lymphocytes.

Because mycoplasma infection has often been observed in individuals infected with human immunodeficiency virus type 1 (HIV-1), we investigated the influence of mycoplasmas on HIV replication in vitro. Replication of HIV-1 assessed by reverse transcriptase activity and the p24 antigen level in peripheral blood mononuclear cells (PBMCs) was enhanced by the stimulation with the cell lysates of human mycoplasmas such as Mycoplasma pneumoniae, M. genitalium, and M. fermentans. The most drastic increase of HIV-1 replication was shown in M. pneumoniae, which induced an approximately 10-fold higher replication level than phytohemagglutinin. Stimulation with the mycoplasmas accompanied the induction of blastotransformation of CD4+ but not CD8+ T lymphocytes in PBMCs and the most predominant alterations were induced by the stimulation with M. pneumoniae among the species examined. Production of IL-2 was significantly increased by stimulation with all three species of Mycoplasma used. These results suggest that mycoplasmas induce vigorous replication of HIV-1 through the activation of CD4-positive T lymphocytes.
