Importance of mechanical damage to urinary red blood cells by the glomerular basement membrane.

The reasons for morphological changes of urinary red blood cells (RBC) in patients with glomerulonephritis are still controversial. In order to evaluate the importance of mechanical damage by the glomerular basement membrane (GBM), we examined urinary RBC taken from the patients with two different diseases which have characteristic GBM changes. Urinary RBC taken from 20 patients with Alport syndrome and nine with thin GBM disease were examined using a scanning electron microscope. Nineteen out of the 20 patients (95.0%) with Alport syndrome showed 'glomerular type', while five of the nine patients (55.6%) with thin GBM disease showed 'glomerular type'. These results suggest that more complicated GBM abnormalities cause more severe RBC distortion. Therefore, we conclude that mechanical damage by the GBM may be the major factor in dysmorphism of urinary RBC.

Determination of sagittal tongue shape from the positions of points on the tongue surface.

This paper describes a method for determining the shape of the midsagittal tongue contour from the positions of points on the tongue surface. The positions of the points and the tongue shape were measured simultaneously by using an alternating magnetic field device and an ultrasonic B-mode scanner for continuous speech utterances. A comparison between the magnetic and the ultrasonic data revealed that the average measurement difference between the two types of data was 1.16 mm. The shape of the tongue contour was then represented by multivariable linear regression of the magnetically determined positions. The results of the regression analysis showed that the tongue contour was estimated, from four positions on the tongue, with an average estimation error of 1.24 mm. This estimation error could be reduced to 0.84 mm when there was no measurement error between the magnetic and the ultrasonic data, and it was further reduced to 0.43 mm when the receiver coils of the magnetic device were positioned optimally on the tongue. It was also shown that the number of data frames for calculating the regression coefficients could be reduced, while maintaining the estimation accuracy, by appropriately selecting data frames. Finally, the tongue shape was estimated successfully for several phonemes from the magnetically determined positions, thus demonstrating the usefulness of this method for observing the articulatory configuration of the tongue.

Detection of human immunodeficiency virus, hepatitis B virus, and hepatitis C virus in donor eyes using polymerase chain reaction.

Detection of human immunodeficiency virus (HIV), hepatitis B virus (HBV), and hepatitis C virus (HCV) in donor eyes was performed. DNAs were extracted from the uvea, and they were amplified using the polymerase chain reaction (PCR). Amplified viral DNAs were detected with liquid hybridisation and chemiluminescent assay in which no radioactive materials were used. This method was shown to have a sensitivity limit of fewer than 10 copies of HIV, making it much more sensitive than the current techniques employed in eye banks. The method was applied to 120 donor eyes, including four from donors seropositive for HBV. The HBV gene was detected in one case in which the donor's blood had not been tested for HBV. HIV and HCV genes were not detected in any of the samples. The assay could be an effective screening test for the detection of these viruses in eye bank eyes.

Methemoglobin formation and reduction in canine erythrocytes characterized by inherited high Na+, K(+)-ATPase activity with normal and high glutathione concentrations.

The methemoglobin formation and methemoglobin reduction in canine erythrocytes characterized by inherited high potassium (K+) and normal reduced glutathione concentrations (HK-low GSH cells) were compared with those in canine erythrocytes with inherited high K+ and high GSH concentrations (HK-high GSH cells) and normal canine erythrocytes with low K+ and low (= normal) GSH concentrations (LK-low GSH cells). The rate of methemoglobin formation induced by sodium nitrite (NaNO2) was in the order; LK-low GSH > HK-low GSH > HK-high GSH cells, and the difference among groups was significant at 7 and 15 min. Methemoglobin reduction in a medium containing glucose occurred rapidly in both HK-high GSH and HK-low GSH cells, and the rate of reduction was 1.7-fold higher than in LK-low GSH cells. Accumulation of pyruvate equivalent to the amount of methemoglobin reduced indicated that methemoglobin was predominantly reduced by NADH-methemoglobin reductase coupled to glycolysis. HK-low GSH cells showed an increased glycolytic rate and high pyruvate kinase activity similar to the levels in HK-high GSH cells. It is therefore evident that HK-low GSH cells offer greater protection against oxidation of hemoglobin to methemoglobin than LK-low GSH cells because of the increased glycolytic rate in HK-low GSH cells attributable to high pyruvate kinase activity in these cells.

Selective staining of the superficial cells of mouse urinary bladder epithelium by horseradish peroxidase (HRP).

A simple method for staining the superficial cells of mouse urinary bladder epithelium with horseradish peroxidase (HRP) is presented. HRP solution was instilled into the bladder lumen in vivo. After fixation with glutaraldehyde, the bladder was treated with Triton X-100 and then reacted with diaminobenzidine (DAB). Intense DAB reactions were detected throughout the whole of the cytoplasm of superficial cells by both light and electron microscopy. No reaction was observed in intermediate or basal epithelial cells. This simple method will be useful for the three-dimensional reconstruction of cell shape and identification of the stratification of transitional epithelial cell layers.

Different effects of an angiotensin converting enzyme inhibitor and a calcium antagonist on protein metabolism in rats with right ventricular hypertrophy.

OBJECTIVE: We examined the effects of a calcium antagonist and an angiotensin converting enzyme (ACE) inhibitor on contractile and non-contractile protein metabolism and cardiac function in a monocrotaline-induced right ventricular hypertrophy model, in order to define the effects of these drugs on cardiac hypertrophy. METHODS: One week after monocrotaline injection, male Sprague-Dawley rats were given either a calcium antagonist (nilvadipine; 3 mg/kg per day) or an ACE inhibitor (delapril-HCl; 30 mg/kg per day) for 2 weeks. Right ventricular pressure, the right ventricle: (left ventricle + interventricular septum) ratio, myosin isoenzymes, collagen concentration, collagen types and contractility of right ventricular free wall were examined. RESULTS: In untreated rats significant monocrotaline-induced right ventricular hypertrophy with an increase in the proportion of collagen types III and V was observed. There were no significant changes in collagen concentration. Both drugs reduced right ventricular pressure to the same degree and decreased right ventricular hypertrophy. However, the inhibitory effect of delapril on right ventricular hypertrophy was stronger than that of nilvadipine. Nilvadipine reduced the collagen concentration and reversed changes in collagen types, whereas delapril did not have any significant effect on collagen concentration or collagen types. Cardiac contractility was improved by delapril, but not by nilvadipine. CONCLUSIONS: The results show that a calcium antagonist disproportionately inhibited contractile and non-contractile protein metabolism, whereas an ACE inhibitor proportionally inhibited them and improved cardiac function in a model of right ventricular hypertrophy. The improvement in cardiac function may be due partly to the proportional inhibition of contractile and non-contractile proteins elicited by an ACE inhibitor.

[A successful repair of ventricular septal defect and patient ductus arteriosus associated with pulmonary hypertension in an infant with left lung agenesis].

We report the first case of successful repair of ventricular septal defect (VSD) and patent ductus arteriosus (PDA) associated with pulmonary hypertension in an infant with left lung agenesis. A 70-days-old infant was referred to our hospital with the diagnosis of congenital heart disease and abnormalities of the left lung. Two-dimensional echocardiogram showed ventricular septal defect and patent ductus arteriosus associated with pulmonary hypertension. Computed tomogram of the chest and bronchogram confirmed agenesis of the left lung. The operation was performed at age 114 days with the body weight 4333 g. VSD was closed with the patch and PDA was ligated under cardiopulmonary bypass. Postoperative course was uneventful. Cardiac catheterization, 50 days after operation, revealed normal pulmonary arterial pressure and good cardiac function. Agenesis of the lung is a rare condition and often associated with congenital abnormalities of the skeletal, gastrointestinal, genitourinary and cardiovascular system. As the prognosis is poor especially in those with congenital heart disease, the cardiac surgery is mandatory for the patient to survive.

[Is myocardial fatty acid metabolism different between hypertrophic cardiomyopathy and hypertensive hypertrophy?].

To investigate the characteristics of fatty acid metabolism in hypertrophic cardiomyopathy (HCM), we performed myocardial imaging with 123I-iodophenyl-3-methylpentadecanoic acid (BMIPP) in 24 patients with HCM, 13 patients with hypertensive hypertrophy (HT) and 10 normal subjects. Rest myocardial imaging with 123I-BMIPP was obtained at 20 minutes and 3 hours after 123I-BMIPP injection. Rest 201Tl imaging was also performed. In addition to ordinary tomography, whole body imaging was performed to calculate % Uptake (percentage of cardiac uptake of the isotope to total injected dose). As global indexes of fatty acid metabolism, we calculated two parameters; 1) Uptake Ratio (%Uptake of 123I-BMIPP normalized by myocardial perfusion) and 2) WOR (percent reduction of myocardial 123I-BMIPP within 3 hours). Regional abnormality was evaluated by visual assessment of ordinary tomograms and by BMIPP/T1 map. BMIPP/T1 map was made from Bull's-eye maps of 123I-BMIPP and 201Tl, and it represented 123I-BMIPP uptake normalized by myocardial perfusion of each pixel which constructed the image. %Uptake of 123I-BMIPP was not different among three groups. But Uptake Ratio was significantly (p < 0.001) different among three groups; normal (1.13 +/- 0.08) > HT (1.03 +/- 0.08) > HCM (0.87 +/- 0.09). WOR of 123I-BMIPP was accelerated in HCM (12.7 +/- 4.7%) and HT (10.2 +/- 2.9%) comparing with that in normal (5.1 +/- 3.1%) (p < 0.01). In patients with HCM, by visual assessment, regional abnormality of 123I-BMIPP distribution was found in 17 of 24 patients (71%) including 3 patients with equivocal abnormality. But in patients with HT, only equivocal abnormality was observed in 23%. In BMIPP/T1 map, abnormality was observed in 92% of HCM and 8% of HT. Although global myocardial fatty acid metabolism was equally disturbed both in HCM and HT, regional abnormality of fatty acid metabolism was observed preferentially in HCM. This indicated myocardial fatty acid metabolism was not identical between HCM and HT.

Degree of diversity of hepatitis C virus quasispecies and progression of liver disease.

We examined the quasispecies of the hepatitis C virus genome in 28 patients with liver disease of varying severity. Nucleotide sequences of the hepatitis C virus genome spanning the region from the core to envelope were used to calculate the nucleotide diversity: 0.58% +/- 0.88% in 5 patients with acute hepatitis, 0.85% +/- 0.62% in 5 patients with chronic persistent hepatitis, 1.79% +/- 0.92% in 11 patients with chronic active hepatitis, 3.05% +/- 1.26% in 4 patients with cirrhosis and 2.71% +/- 1.47% in 3 patients with cirrhosis complicated by hepatocellular carcinoma. Thus the intrapatient variation in nucleotides increased significantly with severity of liver disease (p < 0.01), except in those cases of cirrhosis complicated by hepatocellular carcinoma. Multivariate analysis including the histology, duration of infection, age, sex, history of blood transfusion and serum level of ALT at diagnosis as variables showed that the histological finding was the strongest independent factor of the nucleotide diversity (p = 0.003). Serial analysis of the genome in three patients demonstrated that the intra-patient variation in nucleotides increased with the progression of liver disease. The magnitude of the intrapatient variation in nucleotides deduced from the observed changes in the patients was correlated with the mean serum levels of ALT. These findings suggest that the degree of diversity of HCV quasispecies is related to the progression of liver disease.

[Myocardial imaging with 123I-metaiodobenzylguanidine (123I-MIBG) in essential hypertension: does the 123I-MIBG imaging have the ability to predict its prognosis?].

To study myocardial norepinephrine (NE) activity in essential Hypertension 9HT) and to clarify its prognostic significance, we performed myocardial imaging with 123I-metaiodobenzylguanidine (123I-MIBG) and 201Tl at rest in 16 patients with HT and 9 normal subjects. In addition to ordinary tomograms, whole body images were obtained in both 123I-MIBG and 201Tl imaging. From the whole body image, a ratio of myocardial radionuclide accumulation to total injected dose was calculated (%Uptake). And Uptake Ratio (%Uptake of 123I-MIBG/%Uptake of 201Tl) was used as an index of myocardial 123I-MIBG uptake. Reduction of myocardial 123I-MIBG during 3 hours was calculated and expressed as washout rate (WOR). From the Bull's-eye map, the extent of defect was quantitatively assessed (Defect Score) and the homogeneity of the radionuclide distribution within the myocardium (CV) was calculated. In 123I-MIBG imaging, defect appeared in 12 patients with HT (75%), but in normal subject no one showed defect. Patients with HT were divided into two groups according to left ventricular ejection fraction (EF) responses to exercise stress which were obtained by 99mTc blood pool imaging. Group 1 consisted of 8 patients who showed EF elevation (5-10%) by exercise and group 2 consisted of 6 patients with depression of EF (-4-6%) by exercise. Rest EF, peak filling rate, exercise heart rate, exercise blood pressure and left ventricular mass were identical between two groups. Uptake Ratio in Group 2 (0.67 +/- 0.04) was significantly smaller (p < 0.05) than those in Group 1 (0.75 +/- 0.05) and normal subject (0.75 +/- 0.06). And in Group 2, WOR of 123I-MIBG was greater than that of Group 1. In Group 2, defect of 123I-MIBG appeared in all patients, but in Group 1 defect developed in 50%, besides Defect Score was significantly greater in Group 2 than in Group 1 (3.8 +/- 1.4 vs. 1.5 +/- 1.8, p < 0.05). The unhomogeneity of 123I-MIBG distribution was significantly greater in Group 2 (CV; 30 +/- 5%) than in Group 1 (21 +/- 4%) and in normal subjects (18 +/- 4%). These results suggested that quantitative analysis of 123I-MIBG imaging may be helpful for assessing the prognosis of HT.

Biventricular endomyocardial biopsy findings in essential hypertension of graded severity.

We performed endomyocardial biopsies from both ventricles in 29 patients with graded severities of hypertension. The patients (53.8 +/- 8.1 (mean +/- s.d.) years old) were either symptomatic or asymptomatic, and were grouped into three grades according to the severity of hypertension, that is, slight, moderate and severe. Breadth of the left ventricular myocytes (expressed as mean +/- s.d. microns) were 14.6 +/- 2.0 in Group I (n = 5), 15.5 +/- 1.4 in Group II (n = 15) and 18.7 +/- 2.4 in Group III (n = 9) showing a gradual increase. A noteworthy increase in the right ventricular myocyte breadth was also observed (12.5 +/- 0.9 microns in Group I, 13.6 +/- 1.7 microns in Group II and 14.7 +/- 1.4 microns in Group III, respectively). Increase in the interstitial fibrosis which was determined by the point counting method and expressed in percentage was also observed in both ventricular myocardiium (14.2 +/- 2.4 microns in Group I, 15.7 +/- 1.8 microns in Group II, and 16.2 +/- 3.1 microns in Group III in the left ventricle and 12.7 +/- 2.4 microns in Group I, 14.6 +/- 2.7 microns in Group II, and 17.5 +/- 3.5 microns in Group III in the right ventricle). It is concluded that, in the hypertensive heart, increase in the breadth of the myocytes as well as the interstitial fibrosis occurs in both the left and the right ventricle, and both changes progress in accordance with the severity of hypertension and electrocardiographic abnormalities.

A case of peculiar plantar warts. Human papillomavirus type 60 infection.

BACKGROUND: Various kinds of viral warts have their own typical clinical appearances and histologic properties. From each of them, a corresponding unique type of human papillomavirus (HPV) has been detected. More than 60 types of HPV have been identified from the warts and other squamous cell epithelial tumors. Human papillomavirus type 60 has been identified in an epidermal cyst on the sole. We present a case of a peculiar plantar wart infected with HPV type 60. OBSERVATIONS: A 21-year-old male student complained of yellowish hyperkeratotic slightly elevated nodules on the right plantar arch. The surface of the nodules was not papillary, but a normal ridge pattern of the sole could be traced. Histologic findings included moderate acanthosis, papillomatosis, and eosinophilic intracytoplasmic large granules with or without surrounded vacuoles. These were noted in the middle and upper epidermis, including the hypertrophic stratum corneum. Virologic findings included HPV antigen detected by anti-HPV polyclonal antibody. The DNA of HPV type 60 was detected by in situ hybridization. CONCLUSION: A new type of plantar wart infected with HPV type 60 is reported. It shows unique clinical and histologic findings that can be distinguished from those of ordinary verruca or myrmecia on the sole.

[Injury of the coronary artery following non-penetrating chest trauma--a case report].

A previously healthy 71-year-old man sustained a non-penetrating chest trauma. The chest CT scan revealed cardiac tamponade and then emergent operation was performed. We opened the pericardium through median sternotomy and found a laceration of the distal left anterior descending coronary artery (seg. 8). We ligated the artery and he showed signs of a limited myocardial infarction on the postoperative ECGs, but echocardiography revealed symmetric contraction and normal ejection fraction. His postoperative course was uneventful. We think that injuries of distal coronary arteries could be successfully ligated.

Production of interleukin-6 at operative wound sites in surgical patients.

BACKGROUND: Interleukin-6 (IL-6) plays a central role in the acute phase of inflammation after surgical injury. The serum concentration of IL-6 increases during an operation. The mechanisms of this increase in the serum IL-6 level, however, has not yet been fully clarified. STUDY DESIGN: To determine the possibility of production of IL-6 at the operative wound site and its regulation by humoral factors in surgical patients, the IL-6 secretion of biopsied skin obtained from an operative wound both before and after the operation were quantitated by using organ culture techniques. RESULTS: When skin explants obtained from the uninjured skin were cultivated and the amounts of IL-6 secreted into the culture medium were measured, IL-6 secretion increased exponentially during culture, which indicated that the stress of the skin incision induced IL-6 production. The skin specimens obtained from the operative wounds postoperatively secreted a significantly larger amount of IL-6 than those obtained from uninjured skin either preoperatively or postoperatively, implying that skin at the site of the operative wound had been more sensitized to produce IL-6 because of the surgical injury. The IL-6 secretion by skin explants was significantly enhanced either by tumor necrosis factor or interleukin-1, while it was inhibited by corticosteroids. CONCLUSIONS: Interleukin-6 production at the site of the operative wound is partly responsible for the elevation of the serum IL-6 level during the operation. Organ cultures of the skin explants may provide a feasible system for research on the cytokine networks in surgical patients.
