[Clinical evaluation of a combination therapy using cefmenoxime and cefsulodin on infections complicated by hematological disorders. Tohkai Research Group on Infections in Hematopoietic Disorders].

Infected patients with hematological disorders were treated with the combination of cefmenoxime (CMX) and cefsulodin (CFS). This therapy was done on 74 patients, of whom 38 (51%) had acute myelocytic leukemia, 14 (19%) malignant lymphoma, 7 (9%) acute lymphocytic leukemia, 5 aplastic anemia, 4 adult T cell leukemia, 4 chronic myelocytic leukemia, 1 multiple myeloma and 1 histiocytic medullary reticulosis. Complicated infections included 5 cases of septicemia, 41 cases of suspected septicemia, 19 cases of respiratory tract infection, 2 with anal abscess, 1 with urinary tract infection and others. The obtained results were as follows: Clinical effectiveness of the combination therapy was excellent in 17 cases (23.0%), good in 24 (32.4%) and poor in 33 (44.6%). Total clinical efficacy rate was 55.4%. Clinical efficacy rate was 40% against septicemias, 51.2% against suspected septicemias and 57.9% against respiratory tract infections. Causative pathogens were isolated in only 21 cases (28.4%): Gram-positive bacteria in 9 cases, Gram-negative bacteria in 11 and fungus in 1. About half of the Gram-negative bacteria belonged to Pseudomonas sp. The efficacy rate of this combination therapy against Gram-negative bacterial infections was 72.7% but the rate against Gram-positive bacterial infections were only 33.3%. Only in 1 case, this combination therapy was discontinued because of drug eruption. Abnormal laboratory findings were observed in 5 cases: Elevation of BUN in 3, GOT and GPT in 1 and prolongation of activated partial thromboplastin time in 1. In conclusion, this combination therapy of CMX and CFS is useful and safe against infections complicated by hematological disorders.

Differentiation of astrocytes and oligodendrocytes from germinal matrix cells in primary culture.

Primary cultures derived from neonatal rat forebrain grow almost entirely as glial cultures, with a large astrocytic preponderance and smaller numbers of oligodendrocytic cells. Although both astrocytic and oligodendrocytic characteristics are acquired in vitro, the origins of both types of glia in primary cultures have not been determined. We tested the hypothesis that glia differentiate in vitro from immature neuroectodermal cells by following the fate of germinal zone cells in primary cultures. A monoclonal antibody that binds GD3 ganglioside was used as a marker for cells of the subventricular zone (SVZ), since antibody binding in newborn rat forebrain could be detected by immunofluorescence only in the SVZ of newborn rats (Goldman et al., 1984). We followed the expression of glial fibrillary acidic protein (GFAP), an astrocytic marker; galactocerebroside (GC), an oligodendrocytic marker; and GD3 during the first several weeks of culture. Both GFAP and GC expression were first detected in cells that bound the GD3 antibody. Astrocytes developed during the first week in vitro; eventually, they lost the ability to bind the GD3 antibody and most became GD3-/GFAP+ cells. In high-density cultures, a population of small cells that resided on top of the astrocytic monolayer retained GD3 expression. GC-antibody binding was first observed in these cells of the upper layer, although it was not readily apparent until the second week of culture. Few GC+ cells were seen in cultures grown at low density, however.(ABSTRACT TRUNCATED AT 250 WORDS)

[Bestrabucil-induced complete remission in a case of follicular lymphoma, medium-sized cell type, stage IV (leukemic)].

Bestrabucil, a chlorambucil-estradiol conjugate known to accumulate in malignant cells irrespective of the presence or absence of estrogen receptor, was administered to a 41-year-old man with follicular lymphoma, medium-sized cell type (LSG classification) stage IV (leukemic). A daily dose of 100 mg was given for two weeks and then 200 mg for a week. The WBC dropped from 43,100/mm3 with 82% abnormal lymphocytes to 12,800/mm3, and the drug was discontinued. It was resumed two weeks later at a daily dose of 50 mg. Four weeks thereafter, complete remission was obtained with the disappearance of all peripheral adenopathies and abnormal cells from the bone marrow. The patient remains disease-free for more than eight months on a maintenance dose of 50 mg per day. Transient erythema and gynecomastia have been the only side effects so far observed.

[A new anticancer preparation, 5-fluorouracil-poly-L-lactic acid microcapsule, and its therapeutic evaluation].

An attempt was made to develop the new anticancer drug inducing enhanced tumor response to arterial chemoembolization therapy. 5-Fluorouracil (5-FU) was microencapsulated with poly L-lactic acid by the organic phase separation technique. This granular preparation containing 5-FU (FU-PLA-mc) was about 200 micron in diameter and was about 40% in 5-FU content. The release of 5-FU in vitro from FU-PLA-mc gradually increased, extending over 50 hours to reach maximum level. When 75 mg of FU-PLA-mc (5-FU content, 30 mg) was infused into normal rabbits through femoral arteries, maximum level of 5-FU in blood was 1.03 +/- 0.16 microgram/g after 15 minutes. Meanwhile, arterial infusion of the same dosage of FU-PLA-mc into VX2 tumor bearing rabbits resulted in the production of high level of 5-FU (2.1 +/- 0.5 micrograms/g) in tumors even after 72 hours. In vivo anticancer effect of FU-PLA-mc was significantly demonstrated; when 75 mg of FU-PLA-mc was administrated to VX2 tumor bearing rabbits through arterial infusion, T/C ratio of mean tumor weight was 0.01% on the 14th day after infusion. After hepatic arterial embolization with FU-PLA-mc in normal rabbits, elevation of serum transaminases was observed, but it was transient. Hepatic arterial chemoembolization with FU-PLA-mc was carried out for 8 patients with hepatic malignancies. Of these 8 patients, partial response was obtained in 3 patients and no severe complication was observed.

[Phase II study of mitoxantrone in patients with acute leukemia].

A phase II study of acute leukemia with mitoxantrone (MIT) was conducted by the Tokai Blood Cancer Study Group. The drug efficacy was evaluable in 31 cases (14 of ALL and 17 of ANLL) out of 37 entered at 13 institutions. Five cases were for first induction remission and 26 cases for re-induction remission; ages ranged from 7 to 69 (median: 43); 18 males and 13 females. MIT dosage was intravenous injection of 3-6 mg/m2/day X 5 consecutive days as a rule. Of the 14 cases of ALL, 2 achieved CR and 3, PR; the efficacy rate was 36%. Of the 17 cases of ANLL, 4 achieved CR and 3, PR: the efficacy rate was 41%. Of the 5 first induction remission cases, 3 achieved CR, and 1, PR, the efficacy rate being 80%, whereas out of the 26 re-induction remission cases, 3 achieved CR, 5 PR, and the efficacy rate was 31%. In 3 of 6 cases of CR, large cumulative doses of anthracyclines such as DNR 140 mg plus ACR 410 mg, DNR 360 mg plus ADR 120 mg plus ADR 120 mg, and DNR 240 mg plus ADR 540 mg, had been administered previously in each case. As to complications, sepsis and other infections were observed at the rates of 15% and 32%, respectively, from which it was inferred that in therapy with mitoxantrone, leukopenia should be observed carefully. The major non-hematological toxicity was gastrointestinal symptoms, but the degree was mild. From the results of this trial, it was concluded that mitoxantrone was an effective form of therapy for acute leukemia. Further clinical trials on mitoxantrone in combination with other drugs are scheduled.

Difficulty in high-pitched phonation by laryngeal trauma.

We treated a case of laryngeal trauma in which difficulty in high-pitched phonation was the main feature of a voice problem. We emphasize the importance of phonatory function tests in determining the mechanism of voice disorders and in monitoring the effects of treatment. Physical examination, phonatory function tests, computed tomographic scan, and electromyogram disclosed that the voice problem was caused by a fracture of the thyroid cartilage and its fixation against the cricoid cartilage. An open reduction yielded recovery of the phonatory function.

Results of HMG (Humegon)-HCG therapy in 6096 treatment cycles of 2166 Japanese women with anovulatory infertility.

The effects of HMG (Humegon)-HCG therapy in 6096 cycles in 2166 Japanese women with anovulatory infertility were examined. The rates of ovulation, pregnancy, the ovarian hyperstimulation syndrome, multiple pregnancy, abortion, and malformations in the newborn were recorded, and the possible factors of multiple pregnancies were analyzed. Ovulation occurred in 73.2% of the cases and 64.5% of the treatment cycles. Pregnancy occurred in 23.0% of the cases and 8.6% of the cycles. Ovarian hyperstimulation syndrome with grade I of WHO definition or more was observed in 10.3% of the cases and 5.3% of the cycles. The incidence of the ovarian hyperstimulation syndrome was high in amenorrheic patients, who respond to progestin with bleeding. The multiple pregnancy rate was 20.5%, of which 13.0% was twins and 7.5% triplets or more. The abortion rate was 22.0%, and the abortion rate in multiple pregnancy was significantly higher (P less than 0.05) than that in singleton pregnancy. The external malformation rate was 1.68% in the 594 newborn who could be examined. No significant differences were found in maternal factors, the treatment schedule, or the ovarian response to treatment in singleton and multiple pregnancy groups. This survey revealed that the efficacy and the incidence of adverse effects of Humegon-HCG therapy in a large number of Japanese women were not different from those in Caucasians except for a lower rate of multiple pregnancy, and no special causative factors for multiple pregnancy were found.
