Sensitivity and accuracy in recording His-Purkinje activity by surface-averaged electrocardiography.

A clinical evaluation of the surface-averaged ECG (SAE) to record His-Purkinje activity (HPA) was made on 70 patients who underwent His bundle electrograms (HBE). The recorded signals first judged as HPA in 43 patients by the noninvasive method alone were later verified in 37 patients by HBE; the accuracy of the HPA recordings (predictive value) was 86.0%. The HPA-V interval measured noninvasively had a high correlation with the HV interval by HBE (r = 0.89, p less than 0.01). The verified detection rate in all 70 patients was 52.9%: HPA was detected in 12 of 18 patients (66.7%) with sclerotic and hypertensive heart disease (Group I), five of 19 patients (26.3%) with rheumatic heart disease (Group II), 11 of 17 patients (64.7%) with congenital heart disease (Group III), and nine of 16 patients (56.2%) with miscellaneous conditions (Group IV). The detection rate was markedly lower in Group II than in other groups (Group II vs Group I or III, p less than 0.025). The PR segment was significantly longer in the patients in whom HPA was detected than in those in whom it was not detected (71.5 +/- 22.3 msec vs 43.9 +/- 19.5 msec, p less than 0.001). His-Purkinje activity (HPA) was detected in 32 of 52 recordings (61.5%) with sinus rhythm and seven of 20 recordings (35.0%) with atrial fibrillation, including two recordings in each of two cardioverted patients (p less than 0.05). We conclude that the surface-averaged ECG (SAE) has clinically acceptable sensitivity and accuracy except in patients with rheumatic heart disease, short PR segments or atrial fibrillations.

Partial laryngopharyngectomy for piriform sinus carcinoma. Technique and preliminary results.

This paper presents a technique for partial laryngopharyngectomy followed by a one-stage reconstruction and its preliminary results. This surgery is indicated for carefully selected cases in which the lesion is confined to the ipsilateral piriform sinus, aryepiglottic fold, arytenoid eminence and paraglottic space at the level of the false fold. The hyoid bone, thyroid ala, arytenoid cartilage, epiglottis, aryepiglottic fold, arytenoid eminence and false fold are removed on the affected side. Reconstruction is performed with the use of a pectoralis major myocutaneous (PMMC) flap. The surgery was performed on four cases: two were successful; one suffered from persistent postsurgical aspiration because the reconstructed hypopharynx was too wide; and one developed necrosis of PMMC flap and a secondary reconstruction procedure was performed.

A new ion-coating surface treatment of alloys for dental adhesive resins.

4-META and new phosphate-methacrylate resins adhere strongly to dental alloys. However, for strengthening the water durability of the adhesive interface, the oxidation of the alloy surface is indispensable. A new oxidation method using ion-sputtering was developed, and the effectiveness of this surface treatment on two dental alloys--a type IV gold alloy and Ni-Cr-Be alloy--was investigated. As an endurance test, thermocycling for a maximum of 100,000 cycles was adopted, and the tensile adhesive bond strength was then measured. Ion-coating the surface of the alloys resulted in strong bonds with adhesive resins, and after 100,000 thermocycles, a bond strength of above 20 MPa was maintained.

Mortality, major cause of moribundity, and spontaneous tumors in CD-1 mice.

Mortality, major causes of moribundity, and spontaneous tumors in CD-1 mice were studied in 891 males and 890 females, which were used as controls in 11 different 2-year chronic and oncogenicity studies during the past 5 years. Average mortality of males and females at 83 weeks of age was 32.6% and 28.6%, respectively, and at 109 weeks of age was 66.4% and 63.3%, respectively. Mortality was significantly lowered in males and females born after 1980 in accordance with an abruptly decreased occurrence of systemic amyloidosis in these animals. The major cause of death or moribundity included systemic arteritis, systemic amyloidosis, auricular thrombosis, glomerulosclerosis, lymphoma, and pulmonary adenocarcinoma in both sexes. Dysuria and hepatocellular carcinoma in males and mammary adenocarcinoma in females were also critical lesions. The major tumors occurring at more than 3% incidence were systemic lymphoma, adenoma/adenocarcinoma of the lung, adenoma/carcinoma of the liver and adenoma/adenocarcinoma of the Harderian gland for males, and systemic lymphoma, adenoma/adenocarcinoma of the lung, adenoma/carcinoma of the liver, leiomyoma/leiomyosarcoma of the uterus, adenoma/adenocarcinoma of the pituitary (anterior), adenoma/adenocarcinoma of the mammary gland and adenoma/adenocarcinoma of the Harderian gland for females. Intralaboratory heterogeneities in the incidence were recorded as follows: systemic lymphoma in 1 of 11 control groups (1/11) and adenoma/adenocarcinoma in 1/11 for males, and systemic lymphoma in 3/11, adenoma/adenocarcinoma of the lung in 2/11, adenoma/adenocarcinoma of the liver in 1/11, and adenoma/adenocarcinoma in 1/11 for females.

[Su-polysaccharide skin test and immunotherapy with OK-432 in postoperative gastric cancer--the effect of a splenectomy].

A SU-polysaccharide skin test (SU-PS test) has been employed and studied as a parameter for monitoring the immunological status of OK-432 treated patients who had undergone a curative gastrectomy for a gastric cancer in stages II and III. The median diameter of the erythema (SU-PS value) was found to have no relation to the number of lymphocytes and the serum protein level and was depressed by combined chemotherapy. The SU-PS value was significantly higher in patients not given a splenectomy than in patients who had undergone a patients splenectomy. The spleen is considered the essential organ to focus on in OK-432 immunotherapy.

CO2 laser for treating glottic carcinoma.

Fifty cases of glottic carcinomas treated with CO2 laser were reviewed. The recurrence rate was fairly high (19/50), especially in T1b and T2 cases. The recurrences were successfully treated in all but 1 case who refused treatment. The 3-year crude survival rate was 92% and the 3-year determinate survival rate was 98%. The ultimate voice conservation rate was 100% for T1a, 76% for T1b and 71% for T2 cases. The results led us to the following conclusion: (1) glottic T1a carcinomas can be eradicated with laser excision biopsy preserving good vocal function; (2) for glottic T1b and selected cases of T2, laser debulking followed by radiotherapy is one of the best choices of treatment.

Effect of hydrocortisone on the development of embryonic chick duodenum--transmission electron microscope studies.

The effect of hydrocortisone on morphological changes in developing chick embryo was observed by transmission electron microscopy and immunohistologically with antiserum of actin. The morphological changes were more remarkable in the hydrocortisone-dosed 19th day embryonic duodenum. This hormone caused increase in the height and length of microvilli in absorptive epithelial cells. The development of organella such as Mitochondria, Golgi's apparatus and endoplasmic reticulum in the absorptive epithelial cells were enhanced. Injection of this hormone also induced an increase in microfilamentus bundles in the microvilli of absorptive epithelial cells. These morphological features of hydrocortisone-dosed 19th day embryonic duodenum indicated similar ones to the chicken at the stage of normal just hatching or 1st day after hatching. These results strongly support the previous suggestion that hydrocortisone may be one of the important factors regulating normal development in chick embryonic duodenum and may exert a major influence on the rapid growth and maturation of absorptive epithelial cells.

[Phase II study of carboplatin in malignant lymphoma].

A phase II study of carboplatin for malignant lymphoma was conducted. Thirty-nine patients with malignant lymphoma were entered into this study, and 6 of them were excluded from evaluation due to violation of selection criteria. The remaining 33 cases were studied to assess the effectiveness and safety of this drug. Two CR and 11 PR were obtained with this medication (effective rate 39%). The significant affecting factors were previous treatment, disease stage and P.S. score. Many cases suffered from gastrointestinal symptoms and reduced hematopoiesis. The former complication was self-limited without any fatal episodes. However, the latter included severe cases, especially thrombocytopenia, which is suggested to be a dose limiting factor (DLF) of this drug.

[Preservation surgery of head and neck cancer. Results and postsurgical function].

The purpose of conservation surgery is to eradicate the cancerous lesion while preserving the morphology and function of the affected organ as much as possible. Many important functions including swallowing, phonation and articulation are carried out by the organs in the head and neck. This paper presents the short-term (2 years) results of conservation surgery for carcinomas of the tongue, oropharynx, hypopharynx and larynx. Long-term results await further follow-up because of the fact that we started full use of conservation surgery rather recently. Three types of conservation surgery were applied for cancer of the tongue in 36 cases:laser surgery alone, laser debulking surgery followed by (chemo) radiotherapy and induction chemotherapy followed by laser debulking surgery and radiotherapy. The local control rate and survival rate were 97% and 88%, respectively. The basic principles of conservation surgery for oropharyngeal cancer were the same as those for cancer of the tongue. A total of 23 patients were treated, yielding a 65% local control rate and a 63% survival rate. For hypopharyngeal cancer, two types of conservation surgery were applied:partial pharyngectomy and partial laryngopharyngectomy. A total of 10 patients underwent these procedures. The local control rate was 60% and the survival rate was 70%. Two major groups of conservation surgery were applied for cancer of the larynx:laser surgery with or without radiotherapy in 74 cases and partial laryngectomy in 94 cases. The local control rate was 69% for laser surgery and 87% for partial laryngectomy, while the survival rates were 100% and 91%, respectively.

Analyses of azopigments obtained from the delta fraction of bilirubin from mammalian plasma (mammalian biliprotein).

Azopigments were obtained from the delta fraction of bilirubin (mammalian biliprotein) in cholestatic sera of men, rats and guinea pigs by diazo reaction with diazotized p-iodoaniline and analysed by t.l.c. Delta bilirubin of men and rats generated both unconjugated and glucuronide-conjugated azodipyrroles, whereas that of guinea pigs, in which the predominant form of conjugated bilirubin in serum was bilirubin monoglucuronide, generated only unconjugated azodipyrrole. We further analysed the azopigments by reversed-phase h.p.l.c. to distinguish their endovinyl and exovinyl isomers. The results indicated (a) that covalent binding of bilirubin to protein occurs exclusively on the conjugated dipyrrolic (either endovinyl or exovinyl) half of the parent conjugated bilirubin, (b) that both bilirubin monoglucuronide and bilirubin diglucuronide generate delta bilirubin, the latter yielding a 'conjugated' form of delta bilirubin that preserves the glucuronic acid moiety on the dipyrrolic half not bound covalently to protein, and (c) that therefore at least four forms of delta bilirubin exist in jaundiced sera of men and rats.
