Anterior esthetic rehabilitation on teeth and dental implants optimized with Procera technology: a case report.

With the latest developments of the Procera system, all-ceramic crowns have become an attractive solution to provide functional and esthetic rehabilitation on teeth and dental implants. The Procera AllCeram crown and Procera Abutment embrace the concept of computer-assisted design and computer-assisted machining (CAD/CAM) and can be used together for optimal esthetic result. The purpose of this case report was to illustrate the advantages of these new components for complex anterior rehabilitation. Three natural teeth and a Procera Abutment were restored using four Procera AllCeram crowns. Treatment planning and esthetic benefits are discussed.

[Endocrine complications of genetic hemochromatosis].

The authors report the prevalence and severity of endocrine complications in a cohort of 115 patients suffering form genetic hemochromatosis and followed since two decades. Already 40% of them had developed diabetes at the time of diagnosis of hemochromatosis, which was made at the age of 50 +/- 12 years (m +/- SD). Hypogonadism was evidenced in 42% of the patients. In most of them, it was considered as secondary to pituitary lesions as assessed by GnRH tests. No other endocrine complications, in particular thyroid disease, were evidenced during follow up. On the other hand, it was also of interest to note that liver cirrhosis was observed in 52% of the patients at the time of hemochromatosis diagnosis. A relationship between cirrhosis, diabetes and hypogonadism was also assessed in these patients. We conclude to the still high prevalence of endocrine complications in genetic hemochromatosis.

The dynamic behavior of the diastolic slope of monophasic action potential can be related to the occurrence and maintenance of delayed afterdepolarization dependent arrhythmias.

We have described the value of the diastolic slope of the MAP recording at the end of a pacing train as a qualifying marker for the induction of delayed afterdepolarization (DAD) dependent arrhythmias. In the present study (1) the behavior of the slope at different time points during a pacing train was quantified and related to the arrhythmogenic outcome (group A) and (2) termination of DAD dependent VT was related to changes in the slope steepness (group B). In dogs with chronic complete AV block, a MAP was recorded during (1) ventricular pacing, before and after ouabain administration (group A) and (2) 6 spontaneous and 6 lidocaine induced VT terminations (group B). During control (group A), the slope at the end of pacing train was 5 +/- 3 m V/s (mean +/- SD), independent of the pacing duration. During ouabain, this increased to 20 +/- 15 mV/s (P < 0.05), varying with the duration of pacing. The slope was steeper after pacing for 4 seconds, compared to 20 seconds (26 +/- 12 mV/s vs 16 +/- 13 mV/s, P < 0.05) which corresponded with more frequent VT induction. In spontaneously terminating VTs (group B), CL increased from 353 +/- 54 ms at the start to 434 +/- 78 ms (P < 0.05) before VT termination. This corresponded with a decreasing steepness of the slope from 19 +/- 10 mV/s to 6 +/- 5 mV/s (P < 0.05). In lidocaine induced VT termination, the CL and the steepness of the slope showed an identical behavior. There is a dynamic variation in the steepness of the diastolic slope during pacing, which depends on the duration of pacing and predicts arrhythmogenic outcome. Furthermore, a decrease in steepness of the slope during DAD dependent VT can be used to predict VT termination.

Primary low-grade B-cell lymphoma of MALT-type occurring in the liver: a study of two cases.

BACKGROUND/METHODS: Primary non-Hodgkin's lymphomas of the liver are rare. One specific clinico-pathological entity has been identified as hepatosplenic gamma/delta T-cell lymphoma. Recently, another distinct primary lymphoma of the liver has been recognised as primary low-grade hepatic B-cell lymphoma of mucosa-associated lymphoid tissue (MALT), based on a study comprising four cases. We analysed two additional cases of this particular non-Hodgkin's lymphoma, not only by morphology and phenotyping, but also by genotyping and cytogenetic analysis. RESULTS: This type of non-Hodgkin's lymphoma is characterised by a dense lymphoid infiltrate, localised in the portal tracts, and is associated with lympho-epithelial lesions of the bile ducts, thereby mimicking hepatitis or an inflammatory bile duct disorder. In one of our cases, translocation t(3;14)(q27;q32) was identified as the sole cytogenetic abnormality. A high incidence of trisomy 3 has been associated with marginal zone B-cell lymphomas, and fluorescence in situ hybridisation as well as comparative genomic hybridisation studies have shown frequent involvement of the long arm of chromosome 3. Nevertheless, t(3;14)(q27;q32) involving BCL6 gene, located at 3q27, has not yet been found. CONCLUSION: Our findings suggest a role for the BCL6 gene in the histogenesis of this particular lymphoma.

Autologous bone graft to augment the maxillary sinus in conjunction with immediate endosseous implants: a retrospective study up to 5 years.

This study reports the successful use of osseointegrated implants to replace posterior teeth combined with the autogenous bone graft technique in the sinus. Preliminary data (1 to 6 years experience) of this one-stage procedure are presented. A total of 33 patients were treated--44 sinus grafts were augmented with bone from the anterior iliac crest. Reconstruction was completed with ceramic fixed partial dentures, and there was a follow-up of 3 to 80 months (mean 40.2 months) after loading. None of the 44 sinus grafts was lost. Of the 121 implants placed, eight failed, resulting in a failure rate of 6.6% and a cumulative failure rate of 6.8%. Of the 44 prostheses placed in the 44 sinuses, one failed, resulting in a prosthesis stability of 97.7%. Complications were encountered in three patients (three sinuses, eight implants); treatments were administered, symptoms subsided, and the implants integrated in two patients (two sinuses, six implants). One patient lost the two implants and the prosthesis, but the graft integrated. No other complications have since occurred. The results compare favorably with previous reports in terms of implant survival and stability. The implant survival rate approached that of implants placed in uncompromised maxillary bone.

Marginal bone levels at Brånemark system implants used for single tooth restoration. The influence of implant design and anatomical region.

In this retrospective study of Brånemark system oral implants, 75 consecutive patients were treated with 84 implants for single-tooth replacement from 1988 to 1993. Two implants were lost and not replaced: the first one before the abutment connection, the latter during the first year in function. The cumulative failure rate reached 2.4% during the 5-year period. Seventy-one percent of the implants were located in the incisor/canine region of both jaws. Twenty-six per cent of the implants were inserted in special presurgical or surgical conditions. The mean annual bone loss as scored on radiograph, was 0.8 mm during the first year and 0.1 mm the following years. Different implant designs were used, a more pronounced bone loss was observed for the conical implant. The present data shows that the cumulative failure rate for single Brånemark implants and radiographic bone loss is similar to that found around implants used for the treatment of complete and partial edentulism.

Variations in fibrinolytic activity of human whole saliva.

The physiological variations of fibrinolytic activity (FA) in saliva were investigated in a population of 149 healthy volunteers including 12 pregnant women. An increase in FA could be observed with increasing age. Marked variations were observed during daytime with a peak value early in the morning. Higher FA values were observed in males as compared with females in the 20-39 yr age group. A clear decrease in FA was observed during pregnancy. A significant correlation existed between FA and suspended epithelial cell concentration, which seemed responsible for most of the physiological FA variations. It was concluded that age, sex, pregnancy, and the time of the day are important for salivary FA and should be taken into account in any study on salivary FA related to physiological or pathological processes.

Study of the possible association of HLA class II, CD4, and CD3 polymorphisms with schizophrenia.

In the present study the HLA-DRB and DPB1 alleles as well as CD4 and CD3 polymorphisms were tested in 100 Belgian schizophrenic patients and 204 controls. Our results indicate a significant negative association of the DPB1 0101 allele with schizophrenia (relative risk [RR] = 0.27). Furthermore a significant positive and negative association could be noticed for the CD4*A4 allele and CD4*A7/A8 genotype, respectively (RR 1.79 and 0.47, respectively). These findings suggest that some contribution of HLA class II and CD4 genes to an autoimmune-like pathogenesis in schizophrenia might exist.

Poly[d(C-A)].poly[d(G-T)] is highly transcribed in the testes of Drosophila hydei.

Microdissection of the lampbrush loops "threads" and "pseudonucleolus" of Y chromosomes from primary spermatocytes of Drosophila hydei and subsequent microcloning of the DNA yielded several recombinant DNA clones which cross-hybridized in screening the different clone banks. By DNA sequencing we found that the inserts of these cross-hybridizing clones contain blocks of poly[d(C-A].poly[d(G-T)]. Testis RNA contains a large fraction of transcripts with this simple repeated nucleotide sequence. With the aid of transcript in situ hybridization we discovered that the "cones" and "pseudonucleolus" lampbrush loops are the primary sites of transcription of poly[d(C-A)].poly[d(G-T)] in spermatocytes. In addition, we found a strand-specific transcription of (CA/GT)n. In both the "cones" and "pseudonucleolus" the (CA)n strand is transcribed, while in the "pseudonucleolus" (GT)n is also transcribed. Labelled (CA)n probes also react with the protein bodies in spermatid nuclei. These observations are discussed in the context of possible functions of (CA/GT)n transcripts in spermatogenesis.

Analysis of ward staff initiatives towards mentally retarded residents: clues for intervention.

Traditionally, efforts to improve the quality of residential care for the mentally retarded are focused on modifying particular ward staff behaviours. Several studies suggest, however, that ward staff behaviours are, at least partly, controlled by certain resident behaviours. This study investigated the relationship between resident behaviour and resident directed initiatives by ward staff. Recording occurred on four living groups with 39 residents during 32 weeks on the average for each group. Pearson product-moment correlations revealed that residents' level of ambulation, waking state, looking behaviours, adaptive-, maladaptive- and stereotype behaviours were related to the amount and quality of resident directed initiatives. The results suggest that modifying certain resident behaviours might offer a fruitful strategy for improving residential care for mentally retarded individuals.

Nutrient insulin secretagogues decrease 45Ca2+ efflux from islet cells by a mechanism other than the inhibition of the Na+-Ca2+ countertransport.

The mechanism whereby nutrient insulin secretagogues decrease 45Ca2+ efflux from islet cells is controversial. It was studied with mouse islets perifused with Ca2+-free solutions. In the presence of Na+, glucose and ketoisocaproate inhibited 45Ca2+ efflux by about 50%. Substitution of choline+ salts for Na+ salts decreased the efflux rate by 45%, but did not prevent glucose from decreasing it further. Ketoisocaproate also inhibited 45Ca2+ efflux, but less markedly than in an Na+ medium. Omission of Na+ decreased the efflux rate even when it was already lowered by glucose or ketoisocaproate. It is thus clear that nutrient insulin secretagogues decrease 45Ca2+ efflux from islet cells by a mechanism other than the inhibition of the Na+-Ca2+ countertransport, possibly by increasing sequestration of the ion in cellular organelles.
