[Permanent interstitial implantation of 125iodine in case of carcinomas of the oral cavity and the oropharynx. Preliminary report].

The authors present their first experiences gained during the last year with the permanent implantation of 125iodine seeds in thirty patients suffering from carcinomas of the oral cavity and the oropharynx. The properties and advantages of 125iodine, the method of implantation, the dosimetry and the problems of radioprotection are particularly underlined. In every case, we apply pre- or postoperative external radiotherapy, i.e. we expose the primary tumor to a dose of 50 Gy fractionated in the usual manner over four or five weeks. This combined treatment is intended to overcome the resistance to therapy of ORL malignomas and to obtain a high local rate of tumor control and a low rate of functional and cosmetic troubles.

[Current therapeutic principles as well as new therapeutic aspects of idiopathic hypertrophic subaortic stenosis. II. Chemotherapeutic and direct surgical therapy].

The severity of the functional obstruction of the outflow tract of an idiopathic hypertrophic subaortic stenosis is essentially determined by the contractility of the myocardium, the left-ventricular volume and the peripheral vascular resistance. Though in the acute experiment a clear reduction of the degree of stenosis by beta-receptor-blockers may be proved the long-term results of such a therapy are unsatisfactory, particularly in patients with obstruction in rest. The use of calcium antagonists seems to promise more favourable results. The progression of the disease is, however, scarcely influencible by pharmacological measures alone. Nitrobodies are not suitable for treatment and glycosides should be used only within a manifest insufficiency of the left heart. An effective reduction or removal of the ventriculo-ventricular pressure gradient as well as a decrease of the left-ventricular telediastolic pressure may be achieved on operative way. There is an indication to this, when the complaints increase despite medicamentous therapy. At present the transaortic myotomy under use of the heart-lung-machine is regarded as most favourable method. A simultaneous or only substitute of the mitral valve should be restricted only to those cases which, apart from the functional, still show an organically conditioned insufficiency of the mitral valve. The early and late results of the palliative surgical interventions are very favourable in experienced centres. However, secure long-term prognoses may not yet be derived from this.

[Current therapeutic principles as well as new therapeutic aspects of idiopathic hypertrophic subaortic stenosis. II. Pathophysiology, natural course and diagnosis].

Idiopathic hypertrophic subaortic stenosis is a noncompensatory hypertrophy of the myocardium which above all concerns the ventricular septum in its upper part near to the basis. The etiology is not yet sufficiently clarified, though a genetic component is regarded as ascertained on account of the proved familial accumulations. Haemodynamically the morphologic changes effect a systolic obstruction of the left-ventricular outflow, an insufficiency of the mitral valve and an aggravated diastolic filling of the left ventricle. Further pathophysiological symptoms of the functional subaortic stenosis are an extremely rapid ejection of blood in the early, not obstructive phase of the systole as well as a pathological forward movement of the mitral valve beginning in the midst of the systole. The disease has a bad prognosis. To be sure, the natural course is usually characterized only by a slow progression of the complaints, however, particularly in adolescents frequently sudden cases of death appear by disturbances of the cardiac rhythm or a relative coronary insufficiency. The disease which is already frequently to be diagnosed according to anamnesis, auscultation, ECG and form of the pulse curve may be ascertained using the ultrasound echocardiography as well as heart catheterization including angiography. Apart from a direct measurement of the ventriculo-ventricular pressure gradient during wuch examinations the size of the functional obstruction may further be characterized by certain pharmaca or physical methods which influence the volume of the ventricle or the afterload.

[Radiation therapy combined with local hyperthermia: a preliminary report of clinical experiences (author's transl)].

First clinical results from a total of 62 patients treated by localized hyperthermia combined with additional megavoltage therapy are reported. Evaluation of the treatment results was possible in 49 cases. Of these, 30 underwent additional treatment with hyperthermia. The local results of radiation therapy could be improved by hyperthermia, no intensification of radiation effects upon sound tissues being detected. This effectiveness of hyperthermia was impressive especially with tumors containing extended necrotic parts, e.g. glioblastomas, carcinomas of the oropharynx or hypopharynx, and tumors of the skin. Regarding routine application of localized hyperthermia, there is a series of essential problems still to be solved.

[Rare lipomatous lesions of the thyroid gland, the uterus and the heart (author's transl)].

Rare lipomatous lesions are described which were found in a thyroid gland with amyloidosis, in a leiomyoma of the uterus, and in a cicatrix of a myocardial infarction. Semithin and ultrathin sections revealed intracytoplasmatic fat droplets of varying diameter in smooth muscle cells of the uterine myoma as well as in fibroblasts/cytes of the thyroid gland. Regressive changes may be the reason for fat deposition in these cells thus that they cannot be distinguished from original fat cells by means of light microscopy.

Carcinogenicity study of lindane in the mouse.

Linadane (gamma-BHC) was investigated for its possible carcinogenic effect on 500 mice (250 males, 250 females) of the Chbi: NMRI (SPF) strain in a study lasting 80 weeks. A substance-related production of tumours was not evident at the dosage levels employed (12.5, 25 and 50 ppm). The tumours found corresponded in type and frequency to those occurring spontaneously in this strain. Electron-microscopical examination of livers, carried our at the same time, provided no evidence of Lindane-induced fine structural hepatocellular alterations.

Studies on the relation between latency relaxation and resting cross-bridges of frog skeletal muscle.

Latency relaxation of frog skeletal muscle (LR) was investigated with respect to its relation to resting cross-bridges. A decrease in the initial stiffness of the resting muscle (stiffness of the muscle during the beginning of a length-change) was found, when repeated triangular length-changes were imposed on the muscle. This decrease in the initial stiffness depends on the velocity of the length-change. It is interpreted that the decrease in the initial stiffness reflects a detachment of the resting cross-bridges from their binding-sites. The LR, induced immediately after the offset of the length-changes, i.e. when the cross-bridges are still detached, showed an increased depth, its time course remaining unchanged. There is a strong correlation between the increase in the depth of the LR and the decrease in the initial stiffness. The LR regained its original depth (depth without a preceding length-change) about 5 s after the offset of the length-changes (20 degrees C). It is suggested that the LR of skeletal muscle is not due to a detachment of resting cross-bridges.

Glycogenous hepatonuclear inclusions in the aged mouse--an electron microscopical study of the histogenesis of nuclear inclusions.

Fifty-three and 84 week old mice have been investigated for liver nuclear inclusions. Light microscopical investigations revealed a significantly higher incidence of intranuclear PAS-positive material in the 84 week old mice. Electron microscopical studies showed a continuous development of nuclear inclusions from invagination of the double nuclear membrane containing all cytoplasmic organelles in various conditions via isolation of the invaginated portion to intranuclear free glycogen as a visible final stage. During the whole process alpha-glycogen is degraded to beta- and gamma-particles. These nuclear changes are not accompanied by other pathological changes either in the cells concerned or in the surrounding liver parenchyma. These nuclear findings are attributed to specific metabolic conditions in the ageing mouse.

[Adrenal-like Leydig cells (author's transl)].

Adrenal-like lipoid-rich Leydig cells, which could be found in a cryptorchid testis, were investigated by light and electronmicroscopy. There were nodular and diffuse proliferation of these adrenal-like cells in the interstitium of the testis. Electronmicroscopically these cells are fasciculated and characterized by large liposomes, many tubulovesicular mitochondria, and a large smooth endoplasmatic reticulum. But the presence of crystals of Reinke in these cells underlined their relationship to Leydig cells. The clinical history of this case is characterised by an extreme adipositas (167 kg) and high urinary estrogenexcretion. This excretion could be suppressed with dexamethasone and stimulated with HCG. After orchiectomy estrogen excretion decreased for 4 months and then increased again, after ACTH stimulation performed by reason of adrenal insufficiency. At this time there is no evidence of adrenal tumor; in the contralateral, scrotal testis, spermiogenesis and Leydig cells are without pathologic changes as revealed by biopsy.

[Results following surgery of fibrous subvalvular aortic stenosis in childhood and adolescence].

From 1963 up to the end of June 1974 32 patients were operated on for fibrous subvalvular aortic stenosis in Leipzig (age at operation 6-17 years). 18 patients had concomitant cardiovascular malformations, 4 patients died. The results are discussed in detail. Contrary to other publications the authors adopt the attitude that the operation in subvalvular stenosis is to be recommended even with gradients as low as 50 mm Hg, particularly when haemodynamically significant concomitant malformations are present. Therefore, the measuring of the left ventricular pressure and the localisation of the stenosis by intracardiac diagnostics should be performed not too late. It may be possible that the measuring of the left ventricular pressure must be repeated some years later. Also for the exact evaluation of the post-operative results recatheterisations are necessary.

The non-teratogenicity of 2,4,5-trichlorophenoxyacetic acid in the rhesus monkey (Macaca mulatta).

The herbicide 2,4,5-trichlorophenoxyacetic acid (2,4,5-T) containing 0.05 ppm of tetrachlorodibenzo-p-dioxin, was administered to pregnant rhesus monkeys daily from Day 22 through Day 38 of gestation. At the dose levels administered, 0.05 mg/kg, 1 mg/kg, and 10 mg/kg, no evidence of toxicity was seen in the mother and no evidence of teratogenicity was seen in any of the offspring. Observations of the infants for 1 year following birth indicated that there was no toxicity due to the 2,4,5-T.

A contribution to the question of the possible hepatocarcinogenic effects of lindane.

The possible hepatocarcinogenic effect of lindane was studied in an 80-week study using 500 NMRI mice (strain Chbi : NMRI (SPF)). After giving doses of 12.5, 25 and 50 ppm in the food no compound-induced liver tumours were observed. Neither the occurrence of lymphatic leukemia, malignant haemangioendotheliomas, reticulum cell neoplasms nor liver cell adenomas could be related to the administration of the compound. In view of the actual residue level of lindane the probability of carcinogenic risk to man would appear remote.
