A randomized, double-blind therapeutic trial of 0.25% desoxymethasone and 0.1% hydrocortisone 17-butyrate in the treatment of varicose eczema.

A double-blind, randomized trial was carried out in 60 patients with varicose (hypostatic) eczema to compare the efficacy and tolerance of treatment with 0.25% desoxymethasone in an oily cream base, the oily cream base alone, and 0.1% hydrocortisone 17-butyrate cream. The creams were applied twice daily and patients' progress followed for up to 38 days. Clinical ratings based on an assessment of individual signs and symptoms, the area of skin involved and the physician's overall impression demonstrated a significant difference from the oily cream base in favour of both active treatments within the first 10 days. No significant difference between the two active treatments was shown. All three treatments were well tolerated by the patients.

Development of amphibian thymus. I. Morphological differentiation, multiplication, migration and lysis of thymocytes in the urodele Pleurodeles waltlii.

Different stages of thymus morphogenesis and thymocyte differentiation have been studied at the ultrastructural level in the urodele amphibian Pleurodeles waltlii Michah. From stage 38 to 42 the undifferentiated lymphoid stem-cells colonize the epithelial thymic buds. From stage 43 to 45 the lymphoid stem-cells differentiate into lymphoblasts and then transform into typical lymphocytes. A clasmatosis phenomenon seems to be involved in this transformation. From stage 46 to 52, a phase of intense proliferation occurs and relations between dense reticular epithelial cells and lymphocytes are described. At stage 53, numerous lymphocytes die in the thymic tissue and are phagocytosed by macrophages. At the same time, lymphocytes undergo migrations through the intra- and peri-thymic blood vessels. These lymphocytes should populate the peripheral lymphoid organs, according to the previous finding that stage 52 was the last developmental step for an efficient abrogation of cell-mediated immunity by thymectomy in Pleurodeles.

Prediction of multiple visits by a community mental health center's psychiatric emergency home visiting team.

Ten psychosocial characteristics of 442 patients treated by a psychiatric-emergency home visiting team were studied to determine whether or not those variables were related to further calls for emergency home visits. Changes in the patients' clinical conditions between first and second visits were also assessed. There were no significant changes between first and second visits, and only the display of bizarre behavior during the first visit was positively related to the number of subsequent visits. The implications of these findings for psychiatric-emergency team crisis intervention are discussed.

The mature mid-cycle follicle in the mare.

Oestrogen and progesterone concentrations in blood and follicular fluid and blood levels of LH were determined in 426 mares at different stages of the oestrous cycle. Mature follicles occur at all stages of the cycle; they ovulate readily in early metoestrus, occasionally in late metoestrus and very rarely in dioestrus. Maturation of a mid-cycle follicle is associated with intermediate levels of LH, which are less than those found during oestrus. This lower level of LH together with a high level of progesterone are probably responsible for the failure of ovulation and regression of most of the mid-cycle mature follicles found in the mare.

Malignant melanoma of the conjunctiva after 11 years.

A 76-year-old lady presented with a conjunctival malignant melanoma, which had been incompletely excised 10 years earlier. After rapid and massive growth of the tumor, which was later removed along with the globe, there was no evidence of local or systemic recurrence during 22 months of observation until the patient died of unknown causes. Pathological examination of the ocular tissues obtained at operation showed intraocular extension of tumor into Schlemm's canal and the presence of tumor cells in vascular channels in the limbal region.

["Pedunculated tumor" of the right ventricle in an infant].

A infant is reported who had pulmonary valve stenosis with a pedunculated throbus in the right ventricle. On angiocardiography this appeared as a tumour. The origin of the thrombus and the diagnostic problems interpreting the angiocardiographic appearances are discussed.

Case report of congenital tricuspid insufficiency.

Congenital tricuspid insufficiency is a rare disease secondary to malformations of the tricuspid valve and their chordae tendinae. Diagnosis is difficult but worthwhile, since surgery can be performed. We report one case repaired by surgery.

The rural nurse practitioner: a challenge and a response.

From a cohort of 525 employed nurse practitioners (NPs) who graduated between May 1975 and June 1976, 85 (16 per cent) reported practicing in rural areas. While 91 per cent of the rural NPs chose the short-term certificate programs rather than the master's degree for their NP preparation, 47 per cent had already earned a baccalaureate or master's degree prior to NP training. Family NPs comprised the most frequent specialty area chosen, followed by pediatric and adult specialties. Of the 85, 99 per cent were actually providing a broad array of primary care services to clients over a wide age range. Physicians were periodically available on site in about 80 per cent of the practices, by telephone in 97 per cent of the practices, and in 60 per cent of the practices provided additional consulation by record review. Major motivations of the NPs for entering rural practice were to participate in a creative approach to health care delivery (50 per cent of the practice setting were less than five years old), and for the opportunity for role autonomy. Over 95 per cent of these NPs and employers were satisfied with the NP role.

Overcoming barriers to the employment and utilization of the nurse practitioner.

A national longitudinal cohort study of nurse practitioners and their employers conducted during 1973--1976 provided data on barriers to the development of the nurse practitioner role in primary care. Nearly 90 per cent of the 500 primary care nurse practitioners responding and 75 per cent of the 407 employers responding reported encountering one or more barriers to the role development of the nurse practitioner in their practice setting. Nurse practitioners identified an average of 2.2 barriers and employers identified an average of 1.6 barriers each. Specific barriers identified by 20 per cent or more of the nurse practitioners and employers were legal restrictions, limitations of space and facilities, and resistance from other providers. The data and other evidence suggest that these barriers are not insurmountable and that progress is being made in overcoming these obstacles.
