[Evaluation of skin temperature of patients under spinal anesthesia during cesarean section].

Temperature changes of the first finger and toe were evaluated in patients under spinal anesthesia. One hundred and thirty female patients who were to undergo cesarean section were selected for this study. During spinal anesthesia the changes in skin temperature were classified in three patterns. The first pattern showed a rise of the first finger and toe temperature just after intrathecal injection of 0.3% dibucaine (hand and foot type, n = 77). The second pattern showed only a rise of the first toe temperature (foot type, n = 55), and the third pattern showed no change in the first finger and toe temperature (unchanged type, n = 3). The results of cold description tests were T5 +/- 2 (mean +/- SD) in hand and foot type, T8 +/- 2 in foot type and L1 +/- 6 in unchanged type. The skin temperature pattern was affected by the position of the patient. When the patients were placed in a head down position, 38 cases of foot type showed a rise of the first finger temperature, and 1 case of unchanged type showed a rise of the first finger and toe temperature. In these cases the operative procedure was done under spinal anesthesia. The remaining 4 cases of foot type and 2 cases of unchanged type needed another anesthetic method. In cesarean section under spinal anesthesia, skin temperature pattern of hand and foot type seems desirable. Simultaneous monitoring of hand and foot (first fingertip temperature) seems to be useful to evaluate the height of spinal anesthesia.

[Efficacy of long-term beclomethasone dipropionate inhalation therapy in pediatric patients with asthma, and its effect on height growth and adrenocortical functions].

Seventy-eight pediatric patients with moderate to severe chronic asthma, aged 6 to 16 years, who failed to respond well to inhaled DSCG and theophylline RTC, were treated with beclomethasone dipropionate inhaler (BDI) for 0.5 to 10 years (mean: 4.2 +/- 2.4 years). The efficacy rate gradually increased with time: 61.1% at one year after the start of the treatment and 89.5% at three years. Long-term BDI therapy over five years did not cause suppression of height growth or adrenocortical functions (early morning cortisol level and rapid ACTH test). However, the safety of long-term BDI therapy in children still needs to be studied more thoroughly and established. Further investigation will be necessary since some cases did not respond to BDI therapy while others died of exacerbated asthmatic symptoms soon after the start of BDI therapy.

Naltrexone in the treatment of alcohol dependence.

Seventy male alcohol-dependent patients participated in a 12-week, double-blind, placebo-controlled trial of naltrexone hydrochloride (50 mg/d) as an adjunct to treatment following alcohol detoxification. Subjects taking naltrexone reported significantly less alcohol craving and days in which any alcohol was consumed. During the 12-week study, only 23% of the naltrexone-treated subjects met the criteria for a relapse, whereas 54.3% of the placebo-treated subjects relapsed. The primary effect of naltrexone was seen in patients who drank any alcohol while attending outpatient treatment. Nineteen (95%) of the 20 placebo-treated patients relapsed after they sampled alcohol, while only eight (50%) of 16 naltrexone-treated patients exposed to alcohol met relapse criteria. Naltrexone was not associated with mood changes or other psychiatric symptoms. Significant side effects (nausea) occurred in two naltrexone-treated subjects, and one naltrexone-treated subject complained of increased pain from arthritis. These results suggest that naltrexone may be a safe and effective adjunct to treatment in alcohol-dependent subjects, particularly in preventing alcohol relapse.

[The application of the problem-solving method in a situation of surgical intervention; a case report].

This study used the problems solving method for directing nursing assistance to the hospitalized surgical patient. We present the elaborate instrument founded in the Bailey e Claus model which is analyzed and exemplified. The authors consider the available resources concluding that the proposed is possible to be established. They realize how necessary is in nursing all the taking decision process to reach the proposed objectives, toward the resolution of the patient problems.

[Coronary artery spasm immediately after the long-standing operation for cancer of the tongue].

A 72-year-old male underwent radical operation for cancer of the tongue. Anesthesia was maintained with the combination of enflurane-N2O-vecuronium and cervical epidural block. Five minutes after the cessation of the longstanding operation, VT and circulatory collapse occurred. After administration of lidocaine and ephedrine, VPC and ST elevation were noted, followed by VT and Vf. Cardioversion successfully restored sinus rhythm with no ST change, suggesting an episode of coronary artery spasm. The possible inducing factors in this case were hypotension and acute imbalance in autonomic nervous systems caused by hypovolemia, hypothermia, insufficient anesthetic depth, loss of surgical stress, neostigmine and epidural block. The authors reviewed case reports on coronary spasm, especially looking for possible inducing factors of coronary artery spasm during anesthesia.

Length of the human umbilical cord in multiple pregnancy.

The factors controlling the length of the human umbilical cord are uncertain. In the generally accepted 'tension theory', the length of the umbilical cord is proposed to increase the tensile force on it in the uterus. The greatest tensile force is supposed to be fetal movement. If this is so, the length of the umbilical cord should be shorter in multiple pregnancy than in single pregnancy. To examine this possibility we measured the umbilical cord lengths in 3314 singleton births, 82 twins births (164 infants), and 11 triplet or quadruplet births (35 infants). The cord lengths in multiple pregnancies (twins 51.3 cm, triplets and quadruplets 50.1 cm) tended to be shorter than that in singleton pregnancies (57.2 cm). However, no significant difference was found in the ratio of the umbilical cord length to the infant body weight in these groups. Therefore, we conclude that an important factor in determining the cord length is the size of the infant.

The primary structure of skeletal muscle myosin heavy chain: I. Sequence of the amino-terminal 23 kDa fragment.

Subfragment-1 was prepared from adult chicken pectoralis myosin by limited digestion with alpha-chymotrypsin, and an amino-terminal 23 kDa fragment of the heavy chain was obtained by digesting the subfragment-1 with trypsin. The 205-residue sequence of the fragment was determined by sequencing its cyanogen bromide, tryptic, and chymotryptic peptides. The amino-terminal alpha-amino group of the fragment was acetylated, and two methylated lysines; epsilon-N-monomethyllysine and epsilon-N-trimethyllysine were recognized at the 35th and 130th positions, respectively, as in rabbit skeletal myosin. Comparing the 205-residue sequence of the skeletal myosin with those of cardiac, and gizzard myosins from chicken, considerable differences are recognized, especially in the amino-terminal region, but strong homologies are observed around the reactive lysine residue, around the epsilon-N-trimethyllysine residue, and around the consensus sequence of GXXGXGKT for nucleotide-binding proteins. On the other hand, only 12 amino acid substitutions are recognized between adult and embryonic skeletal myosins, allowing for the post-translational methylation.

[Studies of free theophylline in asthmatic patients].

Total and free theophylline concentrations were examined by fluorescent immunoassay using serum from 48 asthmatic patients treated with sustained-release theophylline formulation. Total and free theophylline concentrations showed a good correlation. The mean % free theophylline (free theophylline concentration X 100/total theophylline concentration) was 44.7% in children and 43.6% in adults. The % free theophylline had no relation to age of the patients or total theophylline concentration. However, the % free theophylline increased correlatively when serum pH or albumin was decreased. As changes in free theophylline concentration influence therapeutic responses and side effects, an assessment of serum pH and albumin in patients treated with theophylline is necessary.

Application of a computer-assisted high-performance liquid chromatographic multi-wavelength ultraviolet detection system to simultaneous toxicological drug analyses.

An emergency drug screening system for the separation and identification of toxic drugs, MULTI-HPLC, is presented. Chromatographic peaks, which were impossible to identify with a conventional high-performance liquid chromatographic UV detection system, became distinguishable by the spectral search and retention prediction of the data-processing program MCASYST. Sixty-five toxic drugs, frequently identified in drug poisionings in Japan, were selected as references in the drug library. Retention time, optimum detection wavelength, detection limit and recoveries from serum and urine were listed. Possible applications of the system are demonstrated, using gastric contents, sera and urines in cases of multiple drug ingestion. Quantitative analysis was sufficiently sensitive and precise to permit clinical diagnosis with increased accuracy.

Two-phase system membrane reactor with cofactor recycling.

For the purpose of constructing a two-phase system reactor the enzymatic process of l-menthol production with cofactor recycling was studied as a model. The half-life of the menthone reductase immobilized onto activated carbon was 4 times as high as that of the free enzyme. The enzyme was capable of regenerating NADH when methyl isobutyl carbinol was used as a second substrate. Continuous production of l-menthol was achieved by using a reactor equipped with a hydrophobic microfiltration membrane. It was found that both NAD(H) and the enzyme could be retained in the reactor and the products, l-methanol and methyl isobutyl ketone, passed through the membrane. The production of l-methanol was maintained for 270 h at a rate of 46.1 g l-1 d-1, and had decreased by one-half at 607 h. The recycling number of NAD(H) was 2500 (max. 3020) during the operation. The number of theoretical plates was calculated to be 40 for the separation of l-menthol from other reactants.

Computer-assisted liquid chromatography for automated qualitative and quantitative analysis of toxic drugs.

A computer-assisted liquid chromatographic system (MCASYST) is evaluated for automatic qualitative and quantitative analysis of toxic drugs in poisoned human fluids. The system performance is confirmed by the actual analyses of several cases of poisoning. The results indicate that the accuracy of identification by retention prediction and UV spectral search is very high, and quantitation by the memorized calibration curves can be performed very conveniently. However, it is difficult at present to identify all peaks that appear in chromatograms for human urine and serum because the drugs may be changed to their metabolites in those fluids. Metabolite identification will be the next step to improve the performance of this system.

[Occurrence of breast cancer, renal cancer and multiple myeloma in a Nagasaki atomic bomb survivor].

A 60-year-old female, who was exposed to the Nagasaki atomic bomb at 18 years old, had renal cancer and subsequently was found to have multiple myeloma (IgG kappa). She underwent the left mastectomy for breast cancer at 43 years old but was not given chemotherapy and radiotherapy. The karyotype of bone marrow cells was 46, XX. The estimated radiation dose was under 10 rads. While the effect of such a low-dose of radiation is considered to be almost negligible, there would be a possibility that in this case the risk of carcinogenesis was enhanced as her age advanced.

Synthesis of "dihydroacarbose", an alpha-D-glucosidase inhibitor having a pseudo-tetrasaccharide structure.

"Dihydroacarbose" (2), an alpha-D-glucosidase inhibitor having a pseudo-tetrasaccharide structure, was synthesized by reductive coupling of 4(3)-amino-1(1),6(1)-anhydro-2(1),3(1),2(2),3(2),6(2),2(3),3(3)-hepta-O- benzyl-4(3),6(3)-dideoxy-beta-maltotriose and 2D-(2,4/3,5)-2,3,4-tris(benzyloxy)-5-(trityloxymethyl)cyclohexa non e with sodium-cyanoborohydride. The former intermediate was prepared from a partially benzylated 1(1),6(1)-anhydro-beta-maltotriose, and the latter was prepared from a chiral, penta-substituted cyclohexene derived from D-glucose. The synthetic 2 was found to be a strong, non-competitive inhibitor (Ki = 1.13 x 10(-6) M) against small-intestinal sucrase of rat.

Comparative effectiveness and costs of inpatient and outpatient detoxification of patients with mild-to-moderate alcohol withdrawal syndrome.

We compared the effectiveness, safety, and costs of outpatient (n = 87) and inpatient (n = 77) detoxification from alcohol in a randomized, prospective trial involving 164 male veterans of low socioeconomic status. The outpatients were evaluated medically and psychiatrically and then were prescribed decreasing doses of oxazepam on the basis of daily clinic visits. The inpatient program combined comprehensive psychiatric and medical evaluation, detoxification with oxazepam, and the initiation of rehabilitation treatment. The mean duration of treatment was significantly shorter for outpatients (6.5 days) than for inpatients (9.2 days). On the other hand, significantly more inpatients (95 percent) than outpatient (72 percent) completed detoxification. There were no serious medical complications in either group. Outcome evaluations completed at one and six months for 93 and 85 percent of the patients, respectively, showed substantial improvement in both groups at both follow-up periods. At one month there were fewer alcohol-related problems among inpatients and fewer medical problems among outpatients. However, no group differences were found at the six-month follow-up, nor were differences found in the subsequent use of other alcoholism-treatment services. Costs were substantially greater for inpatients ($3,319 to $3,665 per patient) than for outpatients ($175 to $388). We conclude that outpatient medical detoxification is an effective, safe, and low-cost treatment for patients with mid-to-moderate symptoms of alcohol withdrawal.

Effect of histamine on two different affinity sites in beta-adrenoceptors.

1. To study the effect of histamine on two different affinity sites in beta-adrenoceptors, we tested specific binding of [3H]befunolol to microsomal fractions from the guinea pig taenia caecum in the presence and absence of histamine (10(-5) M). 2. The Scatchard plot of data in the absence of histamine was recognized as two straight lines suggesting the existence of two different affinity sites: high and low. However, a curvilinear plot with upward concavity was observed in the presence of histamine (10(-5) M). 3. These results suggest the possibility that histamine alters the binding sites of beta-adrenoceptors into several classes of sites with lower affinity. 4. Further, an antagonism between isoprenaline and BFE-55, which interacts only with the high affinity site, was tested on the atria (beta 1-adrenoceptor predominant) and tracheae (beta 2-adrenoceptor predominant) of the guinea pig. The pA2-values in the atria were almost equal to those in the trachea, suggesting that both beta 1- and beta 2-adrenoceptors contained two different affinity sites.

Dose difference in beta-adrenergic blockers with intrinsic sympathomimetic action to block beta-adrenoceptors and induce sympathomimetic action.

1. Isoprenaline, a beta-adrenergic full agonist, and carteolol, befunolol and BFE-55, beta-adrenergic blockers with an intrinsic sympathomimetic action, induced dose-related increases in heart rate when administered intravenously (i.v.). 2. The beta-partial agonists administered i.v. shifted a dose-heart rate increased response curve of isoprenaline, suggesting a competitive antagonism. 3. In carteolol, doses to block beta-adrenoceptors were found to be 400-1000 times lower than that to induce agonistic action. In BFE-55, doses to block beta-adrenoceptors were equal to that to induce sympathomimetic action. The difference between doses of the befunolol to produce both actions was intermediate. 4. These results suggest that there may be a difference between doses blocking beta-adrenoceptors and inducing sympathomimetic action in some beta-adrenergic partial agonists and, therefore, that there may exist beta-partial agonists which do not induce sympathomimetic action in doses blocking beta-adrenoceptors.

Airway obstruction in a child with esophageal achalasia.

A 13-year-old Japanese boy who had respiratory symptoms and had been treated as a case of chronic bronchitis was found by us to have esophageal achalasia. He had complained of cough for one year and a half, and pulmonary function studies showed severe airway obstruction. The chest radiogram showed the trachea compressed by the dilated esophagus. After operation, the symptoms disappeared and marked improvement was found in the flow-volume curve. Esophageal achalasia should be considered as one of the differential diagnoses of airway obstructive disease.
