Cloning of a Drosophila cDNA encoding a polypeptide similar to the human insulin receptor precursor.

A Drosophila cDNA clone was obtained using the human insulin receptor cDNA sequence as a probe. The 3586 bp nucleotide sequence predicted a single polypeptide of 1095 amino acid residues which showed considerable homology (35.2%) with the human insulin receptor precursor. Although the cDNA was incomplete at its 5'-terminal region, it encodes a transmembrane glycoprotein as a single precursor of a two subunit molecule having a structural architecture similar to that of the human insulin receptor precursor. The presumptive beta subunit carries a well conserved Tyr kinase domain which showed 63.5% homology with that of human insulin receptor; however the protein of the alpha subunit is only weakly conserved (25%).

A case of simultaneous bilateral germ cell tumors arising from cryptorchid testes.

We report a rare case of simultaneous bilateral testicular germ cell tumors arising from uncorrected cryptorchid testes. Each side had a different histological type, which consisted of pure high grade seminoma on the left side, and teratocarcinoma with choriocarcinoma and yolk sac tumor elements in addition to seminoma on the right side. Patients with cryptorchidism are known to have a higher risk of germ cell tumors. Genetic factors also may have a role in the oncogenesis in our patient, since his older brother had had a seminoma in the left cryptorchid testis previously. Both patients had the HLA-Aw24 antigen. The characteristics of familial testicular tumors are discussed.

[Clinical effect of allylestrenol on benign prostatic hypertrophy].

Allylestrenol at the daily dose of 50 mg was administered to 45 patients with benign prostatic hypertrophy. The treatment was performed for more than 12 weeks in 40 patients, and improvements, marked and moderate, were observed in 22 patients (55%) in the overall judgement. As side effects of this drug, decrease of potency was observed in 3 cases, decrease of libido in 1 case, pigmentation on breasts in 2 cases, palpitation in 2 cases, short breath in 1 case, and gastrointestinal symptoms in 1 case. However, these side effects were not serious. Our trial suggests that the treatment of benign prostatic hypertrophy with allylestrenol can be useful in urological clinics.

[Concentration level of latamoxef sodium (LMOX) in prostatic tissue].

The concentration of latamoxef sodium (LMOX) in serum and prostatic tissue was evaluated. Thirty seven patients with benign prostatic hypertrophy were given 1 g LMOX intravenously prior to prostatectomy. Prostatic tissue and blood were sampled at 1, 2 or 3 hrs after administration of LMOX. The concentration of LMOX in prostatic tissue was 17.4 +/- 3.4 micrograms/g tissue, 11.1 +/- 1.3 microgram/g tissue and 8.9 +/- 1.3 microgram/g tissue 1, 2 and 3 hrs after injection, respectively (mean +/- S.E.). The penetration ratio of LMOX (concentration in tissue/in serum) to prostate was 30-45%. Therefore, the 80% MIC of LMOX is lower than 8 micrograms/ml for most gram negative bacteria, the results suggest that LMOX is very effective against prostatitis caused by these organisms.

[Recurrence following operative treatment of renal staghorn calculi and prophylaxis].

Residual stone and stone recurrence following operation for renal staghorn calculus were investigated in 58 kidneys of 55 patients. Residual stones were found in 12 kidneys (20.7%), but cases who underwent intraoperative radiogram using Kodak X-O mat KS film or extracorporeal renal surgery showed no residual stones. True recurrence was observed in 9 kidneys (19.6%). The rates of true recurrence after partial nephrectomy, nephrolithotomy, extended pyelolithotomy and autotransplantation with extracorporeal renal surgery were 29.2%, 8.3%, 33.3% and 0% respectively. Morphological study of renal tissue removed intraoperatively from patients with staghorn calculus suggested that calcium deposits in pelvic mucosa or collecting tube might play an important role in stone recurrence. However, despite the calcium deposits in removed specimens, no recurrences have been observed in patients who underwent extracorporeal surgery for complicated staghorn calculus. Therefore, this procedure might be useful to prevent stone recurrence.
