[Stage D1 prostatic cancer benefited by a single regimen of Estracyt].

One problem in the management of prostatic cancer is that about half of the patients with this disease have metastatic lesions at first diagnosis and therefore tend to be given palliative rather than radical therapy. We report here a patient with stage D prostatic cancer who was treated with a single regimen of estramustine phosphate (Estracyt). The patient was a 63-year-old man who was admitted to Keio University Hospital because of sudden onset of double vision. Under a presumptive diagnosis of brain tumor, he underwent thorough examination including brain CT, Ga and bone scan and basic blood tests, which revealed an extraordinarily high level of acid phosphatase. He was therefore referred to our urological division for investigation of possible prostatic cancer. On the basis of the results of urological examinations, a diagnosis of prostatic cancer, stage D, was confirmed. Accordingly, radical surgery was not indicated and instead he was started on oral Estracyt, to which he responded well. He has been enjoying a comfortable life to date, over one and a half years after initial referral.

[Clinical effect of terodiline hydrochloride (TD-758) in patients with urinary disturbances].

We evaluated the efficacy of terodiline hydrochloride (TD-758) in 23 patients with psychosomatic bladder, chronic prostatitis and chronic cystitis. The drug was administered at a dose of 24 mg once a day for 4 weeks. In 78% of the patients the symptoms such as urinary frequency and sense of residual urine improved. Only mild side effects such as thirst were noted. We concluded that terodiline hydrochloride was effective in this trial.

A new mutation involving the sublingual gland in NFS/N mice. Partially arrested mucous cell differentiation.

A new mutation in mice affecting the mucous cell differentiation of the sublingual glands is described. The normal mouse sublingual glands are mucus-secreting and virtually all the acinar cells differentiate to mucus-rich cells by the day of birth. In contrast, all endpieces of newborn mutant mice consisted of acini of immature cuboidal cells. However, normal mucous cells, staining intensively with mucin-specific stains such as Alcian blue at pH 2.5 or mucicarmine, appeared in the mutant mice from an early age singly or in groups in a small number of acini, and their number apparently increased with age to occupy over 30% of the total acinar cells. Ultrastructurally, irregular secretion granules of varying electron-density, distinct from ordinary sublingual mucin granules, were frequently observed in the cytoplasm of the immature acinar cells in the mutant phenotype. The genetic analysis showed that a single autosomal recessive gene determined the observed abnormality. This is the first salivary gland mutation and will provide a critical model for the study of salivary mucous cell differentiation.

1,25-Dihydroxyvitamin D3 receptors and their relationship to histological features in renal cell carcinoma.

The present studies were carried out in 11 human renal cell carcinomas to determine the presence of a receptor specific for an active form of vitamin D, 1,25-dihydroxyvitamin D3. Saturation and Scatchard analyses of the cytosol receptor for 1,25-dihydroxyvitamin D3 showed that nine tumors had a detectable level of the receptor (two fmol/mg. protein). The equilibrium dissociation constant of these receptors ranged between 46 and 380 pM and the binding capacity also ranged between 3.5 and 12.7 femtomol/mg. protein. Sucrose density gradient analysis of the specific binders revealed that the tumors had a receptor protein appearing as a single 3.6S peak. Two tumors which had only a trace of the receptor were high grade solid tumors consisting mainly of spindle or pleomorphic cells. Nine tumors possessing 1,25-dihydroxyvitamin D3 receptor consisted of clear and/or granular cells. Thus, the absence of the receptor was only accompanied by low differentiated sarcomatoid tumors with poor prognosis. However, so far, the amount of the receptor in the receptor-positive tumor did not relate to the other clinical and pathological features of the patients.

[Endoscopic ethanol injection treatment for bladder cancer].

One invasive and 4 superficial bladder cancers were treated by local injection of absolute ethanol through an endoscope. With the patient placed in the lithotomy position, an endoscope was introduced after mucosal anesthesia with xylocaine jelly. According to the usual manner of retrograde catheterization, a 23G syringe needle attached to a 5F ureteral catheter was advanced into the base of the tumor and through the needle absolute ethanol was injected. This treatment is characterized by endoscopic delivery of ethanol as with sclerotherapy for esophageal varices to obtain a potent necrotizing effect as achieved by transcatheter embolization of ethanol for the treatment of renal cell carcinoma. As a rule, this technique does not require any anesthesia other than mucosal anesthesia with xylocaine jelly and can be done repeatedly and very easily, thus having a high cost effectiveness. This treatment serves as a good palliative therapy for invasive bladder cancers, because of its rapid achievement of hemostasis and reduction of the tumor masses. Furthermore, this treatment is just as effective as standard transurethral resections to eradicate superficial bladder cancer, even though the treatment may have to be repeated.

[A case of renal leiomyoma].

A case of renal leiomyoma in a 35-year-old female is presented. Since 1948, fifteen cases including our own case of renal leiomyoma, have been reported in the Japanese literature. They were discovered either from clinical symptoms or incidentally by on image diagnostic study. Some diagnostic and therapeutic problems are discussed.

[A case of bilateral synchronous renal cell carcinoma managed with radical nephrectomy and ex-vivo tumor enucleation].

A 61-year-old female presented with gross hematuria and left flank pain. The arteriogram and CT scan revealed a large left renal tumor and multiple small right renal metastatic tumors. A chest X-ray and Ga scintigram showed no other metastatic lesions. A left radical nephrectomy and a right renal bench surgery were performed at the same time. The right multiple tumors were enucleated and the right kidney was autotransplanted into the right iliac fossa. The postoperative course was uneventful and the patient was free of tumor with good renal function. Renal preservation surgery was considered more beneficial for this patient than bilateral nephrectomy followed by hemodialysis.

[Cyclophosphamide, adriamycin and cisplatinum combination chemotherapy for advanced urothelial and prostatic carcinoma].

Between 1980 and 1985, 17 patients with advanced urothelial carcinoma and 13 with metastatic prostatic carcinoma refractory to hormonal therapy were treated with a combination chemotherapy of cyclophosphamide (CPM), adriamycin (ADM) and cis-diammine-dichloroplatinum (CDDP) to evaluate its antitumor effect and toxicity. ADM (1 mg/kg) on day 1, CPM (2 mg/kg) on days 2 through 5 and CDDP (1.5 mg/kg) on days 6 and 7 were administered every 3 weeks. Of the 17 patients with urothelial carcinoma, 13 were eligible for evaluation. One patient achieved CR with a disease-free interval lasting for 29 months, one showed PR (duration of response 2 months), 4 NC and 7 PD, with an overall response rate of 15% (2/13). Of the 13 patients with prostatic carcinoma, 11 could be evaluated. No patients achieved CR, one had PR (duration of response 5 months), 2 NC and 8 PD, with an overall response rate of 9% (1/11). No statistically significant difference in survival was noted between responders (CR + PR) and non-responders (NC + PD) to the combination chemotherapy, irrespective of whether they had metastatic urothelial or prostatic carcinoma. Myelosuppression was frequently noted, with sepsis occurring in one patient. No mortality attributable directly to this regimen was noted.
