[Clinical study of urothelial tumors of the upper urinary tract. 2: Primary ureteral tumors].

Twenty-five primary ureteral tumors treated at our University between 1963 and 1981, were reviewed retrospectively. The conclusions of this study are as follows. Sex and age distribution of the patients were 18 males and 7 females (2.6: 1), and average age was 63.04 years old. The major symptom was hematuria. The majority of the patients were admitted to our clinic within 6 months from manifestation of symptoms. The major finding of IVP was non-functioning kidney. The positive rate of urinary cytology was 63.2%. Total nephroureterectomy with bladder cuff was performed in 21 out of 23 cases. Histologically, 22 cases were transitional cell carcinoma and one case was squamous cell carcinoma. Simultaneous urothelial tumor was found in 13 cases in the bladder. Most of the ureteral tumors (63.6%) were found in the lower third segment of the ureter. Subsequent urothelial tumors were seen in 3 bladders and one urethra out of 22 cases receiving surgical treatment in the follow-up period. The 5-year survival rate by actuarial method was 39.4%. Among several factors, grade and stage of tumor were the most influencing factors for prognosis. An effective method of post-operative treatment could not be established.

[Studies on the appropriated interval of mass screening for cervical cancer].

In order to study the appropriate interval for cervical cancer screening, we investigated mainly the screening history of 1,086 cervical squamous cancer cases detected by mass screening. 1) The cancer detection rate (CDR) for the 2-year successively screened group who were class I in the first screening was 0.051% and all cases were carcinoma in situ (CIS). In the 2-year interval screened group, detection rates for CIS, microinvasive cancer and stage 1b cancer were 0.053%, 0.035% and 0.018%. But there is no significant difference in CDR between the 2-year successively screened group and 2-year interval screened group (p less than 0.05). 2) In the 2-year successively screened group who were class II in the first screening, the CIS and microinvasive cancer detection rates were 0.044% and 0.022%. There is no significant difference in CDR between class I group and class II group. In the group who were class I in the first screening, we detected most cases in the stage 0 and a few cases in the stage Ia and Ib by mass screening at 2-year intervals. If the detection of a few cases in stage Ib is permitted, we can enforce mass screening for cervical squamous cancer at 2-year interval and it is considered that class II group can be screened at same interval as class I group.

Loperamide in rat intestines: a unique disposition.

When everted sacs of rat duodenum, jejunum and ileum were incubated with [14C]loperamide in vitro, unchanged drug and its metabolites were found not only in tissues but also in media of the mucosal side with virtually no radioactivity in media of the serosal side. The amounts of metabolites found in media of the mucosal side were comparable to or larger than those in tissues. Di-desmethyl loperamide was more predominant in the media as compared with mono-demethylated one than in the tissues. Therefore, a portion of loperamide absorbed in intestines can be metabolized there and directly secreted back into lumen. Oral loperamide thus undergoes a unique disposition, likely constituting one of mechanisms for its distinct dissociation of central and antidiarrheal activities.

Glycolytic activity of rat aorta after exercise.

Activities of glycolytic enzymes in the aorta were investigated in female Wistar rats. There were two groups of rats; one served as the control (sedentary rats), while the other group was forced to run on a treadmill for 10 weeks. In the control animals, the activities of hexokinase, phosphofructokinase and aldolase were relatively lower than those of the other glycolytic enzymes (phosphoglucose isomerase, lactate dehydrogenase and pyruvate kinase). After exercise, the activity of phosphofructokinase increased by 15%, whereas the other enzymatic activities were much the same as in the controls. Within the limits of the experiments, the increased percentage of phosphofructokinase was statistically significant (p less than 0.05). Since phosphofructokinase is a putative rate limiting enzyme, this enzymatic activation may indicate that glycolytic activity in the rat aorta is enhanced during and after running exercise.

Human Ia-like-antigen-expressed cutaneous T cell lymphoma associated with Epstein-Barr virus infection.

2 cases of cutaneous T cell lymphoma are reported. In both cases immunological studies revealed that the tumor cells from the cutaneous lesions had human Ia-like antigen on the cell surface, which was considered to be alloantigen primarily present on B lymphocytes. Serum examination showed an extremely high titer of two types of antibody against Epstein-Barr (EB) virus in the 1st case and a relatively high titer of one type of antibody in the 2nd case, suggesting that the patients had EB virus infection. Recently, some hematologists found the significant correlation of T lymphocytosis and the appearance of human Ia-like antigen in infectious mononucleosis. Based on these findings, it is assumed that EB virus has the ability of the immunological transformation of cutaneous neoplastic lymphocytes, and so we should not overlook the susceptibility of phenotypical changes of the original cell by EB virus when the immunological classification of cutaneous malignant lymphoma is done.

Studies on a new immunoactive peptide, FK-156. II. Fermentation, extraction and chemical and biological characterization.

An interesting immunoactive peptide, FK-156, has been isolated from the fermentation broth of Streptomyces olivaceogriseus sp. nov. and Streptomyces violaceus. The compound was purified by column chromatography with activated carbon, ion exchange Sephadex and cellulose powder. FK-156, obtained as white powder, exhibits a wide variety of immunostimulatory activity in vivo and in vitro with experimental animals. Pretreatment of mice or rats with this peptide protected the animals against lethal infection with Escherichia coli and resulted in prolongation of life span of tumor bearing animals.

Studies on a new immunoactive peptide, FK-156. IV. Synthesis of FK-156 and its geometric isomer.

For the structural confirmation of FK-156, two possible structures, 1 and its geometric isomer 2, were synthesized. Di-Z-meso-diaminopimelic acid (4) was converted into 14 via a sequence of reactions involving, as key steps, an enzyme-mediated asymmetric hydrolysis (6 leads to 7), followed by carbobenzyloxylation using a copper chelate procedure (7 leads to 8). Condensation of 14 and the appropriately protected lactoyl dipeptide 17 and removal of the protecting groups of the resulting 18 afforded 1. Protection of 7 to 22, followed by coupling to glycine via an azide method, gave 25. Derivatization of 25 to 29 and condensation with 17 gave 30, which was deprotected to yield 2. Compound 1 proved to be identical in all respects with the natural product.
