A 74-year old female presented a giant tumor and serosanguineous nipple discharge from the left breast. The tumor was first recognized 26 years ago, and untreated. The resected breast contained several cysts and some small white nodules. Some of the cysts contained intracystic tumors. Histologically, the tumor consisted of a mixture of papillotubular carcinoma and intraductal papilloma. This case seems to represent a malignant change in intraductal papilloma from its long clinical course and by the microscopic findings. Review of this case seems to warrant clinical attention to the possibility that benign intraductal papilloma may transform itself into malignancy after many years.

A 37-year-old female, who had undergone a left standard mastectomy in May, 1983 for breast cancer, was treated with Tamoxifen and FT-207 postoperatively. The patient had no further evidence of disease until two and one-half years later, when a routine chest film showed a single, half-rounded density 6.5 cm in diameter in the hilus of her left lung. Bestrabucil, the benzoate of an estradiol-chlorambucil conjugate, was administered p.o. at a daily dose of 200 mg for four weeks. Thereafter, the tumor greatly decreased in size to 3.0 cm in diameter. When a left upper lobectomy was performed, the tumor was found to be necrotic tissue. Microscopically, a small cancer nest similar to the primary breast carcinoma was observed in the center of the necrosis. The necrotic tissue was surrounded with cellular infiltrate, markedly consisting of histiocytes.
Cathepsin B and L activities were determined with 7-amino-4-methyl-coumarin conjugates as substrates using small tissue specimens of gastric cancer and benign gastric ulcer. Both cathepsin B and L activities were higher in homogenates of gastric cancer tissue than in regenerative mucosa of benign gastric ulcer. Cathepsin B activity was much higher in cancer tissue than in tissue surrounding the tumor. In contrast, the activity in regenerative mucosa of benign gastric ulcer was not elevated as compared with the tissue surrounding the ulcer. Similar results were obtained with cathepsin L activity. These results indicate that cathepsin B and L are related to proliferation and evolvement of gastric cancer.

A case of mixed gonadal dysgenesis with anaplastic seminoma is reported. A 43-year-old, female, who was brought up as a female, was admitted to our hospital for swelling of left inguinal region. The physical examination revealed a short stature, ambiguous external genitalia but no Turner's stigmata. A child's head-sized tumor was noticed in the left inguinal region by CT scan. The histopathological diagnosis of this tumor was anaplastic seminoma. The right gonad was removed at laparotomy with lymph node dissection in the pelvis. Histological examination of right gonad revealed streak gonad and immature uterine with fallopian tube. In the Japanese literature, 7 cases including our case of mixed gonadal dysgenesis with gonadal tumor have been reported.
Forty-three cases of metastatic bone tumor were treated with Elcatonin. The agent was injected intramuscularly to each patient at a dose of 40 units twice daily. Twelve patients (28%) experienced pain relief within 4 days after treatment and after 4 weeks, twenty-eight patients (65%) had palliation of pain. In patients with hypercalcemia (4 cases), a decrease of serum Ca was observed one week after administration of Elcatonin. Improved bone scintigram was observed in 37.5% of cases, and radiographic improvement in 20% of cases. These data indicate that Elcatonin is effective for achieving pain relief and in improving the state of invaded bones when administered in combination with conventional treatment modalities for patients with metastatic bone tumors.

A specific, relatively sensitive, quantitative and standardized enzyme-linked immunosorbent assay (ELISA) procedure was developed for the detection of heterophile Hanganutziu and Deicher (HD) antibodies which are occasionally elevated in pathologic human sera. The HD antigen-active molecule used for the assay was a ganglioside (N-glycolylneuraminyllactosylceramide, abbreviated as NeuGc-LacCer) previously purified from horse erythrocyte membranes. The test used antigen-coated plastic microtiter plates and anti-human immunoglobulin G (IgG, Fab fragment) conjugated with alkaline phosphatase. Fifty-four normal human sera gave ELISA values ranging from -2 to 2%. Random sera from hospitalized patients were first screened by the horse erythrocyte hemagglutination (HA) test, whereby 5.7% (76 cases) gave abnormal HA titers of 128-4096 compared to titers in normal sera equal to or less than 64. Ninety-seven % of the patients' sera gave abnormal ELISA values (3-200%). They were classified into 3 groups: cancer (42 cases), infection (10 cases), and others (24 cases). The potential value of this ELISA method is discussed.
Three types of bone-single-crystal sapphire implant interfaces were histologically determined in submergible and supramergible implant: direct bone apposition (type I), thin connective tissue with poor collagen fibers (type II), and dense fibrous connective tissue (type III). Type I interface was predominant and type III interface was sparsely seen in submergible implants. In contrast to these findings, type III interfaces frequently appeared in supramergible implants, primarily at the apical region. These differences were considered to occur by the biomechanical variable of postoperative healing.
Infusion of an ammonium acetate solution into dogs during mannitol-induced reversible opening of the blood-brain barrier resulted in a marked rise in intracranial pressure (ICP). The preventive effect of a branched-chain amino acid (BCAA) solution on ammonia-induced brain edema was tested by measuring ICP and brain water content. The BCAA solution apparently prevented ammonia-induced brain edema, indicating that BCAA accelerated ammonia detoxification in the brain.
With canine disease models for aortic dissection, we performed comparative evaluations of several surgical procedures for the management of this dissection. From these experimental procedures, we developed a new operative technique, the Ivalon sponge occlusion method, designed to promote thrombus formation and to mesh (organize) effectively with the tissue ingrowth of the peripheral dilated, dissected aortic lumen. The details of the technique are described. It has been used successfully in 12 patients with DeBakey type I and type III aortic dissection. Based on our experimental and clinical evaluations, although the results are preliminary, we believe this operative technique is a simple, effective approach for the management of extended aortic dissection.
