[Two cases of spontaneous fistula between urinary tract and gastrointestinal tract].

This is a report of two cases of a fistula between the urinary and gastrointestinal tracts. One case was a 56-year-old female patient with a right pyeloduodenal fistula, and the other was a 38-year-old female with a left ureterodescending colon fistula. The chief complaint was right lumbago in the first case, and fever and left lumbago in the second case. Both cases were diagnosed by retrograde pyelography. In both cases, the ailing kidney appeared to be non-functioning, and the former was operated by nephrectomy, fistulectomy, and duodenectomy in the cuneate form, while the latter was operated by nephrectomy and fistulectomy. The cause of the formation of the fistula was considered to be chronic pyelonephritis accompanied by renal stones in the first case, and rupture of the colon diverticulitis in the second case. Postoperative prognosis was satisfactory in both cases.

[Statistics on operations in the Department of Urology, Kobe University School of Medicine (January, 1976 to December, 1980)].

Statistical observations on operations performed in our department from 1976 to 1980 were reviewed, especially in comparison with the statistics for the preceding 10 years. Operations on the kidney were the most frequent, as they were in the preceding 10 years, but operations on the bladder have increased remarkably, in 1980 accounting for more than those on the kidney. During this period, operations for benign diseases such as lithotomy of the upper urinary tract and prostatectomy tended to decrease. On the other hand, operations for malignant diseases of kidney, bladder and scrotal contents steadily increased. Transurethral operations are still increasing, especially for bladder tumors. It has been established that bladder tumors should be treated by either TUR-BT or total cystectomy and not by partial cystectomy. As to urinary diversion, ileal conduit was the main procedure used during this 5-year period instead of cutaneous ureterostomy.

[Bilateral complete deafness following head injury in a child].

A case of head injury is reported because it presented such an unusual symptom as sudden, bilateral complete deafness. An 11-year-old boy was involved in a traffic accident while riding on a bicycle. He was admitted immediately after the accident in semicomatose state. The pupils were isocoric and pupillary light reflex was normal. Conjugate deviation to the right was seen. Oculo-cephalic response was absent. X-rays of his skull showed two linear fractures in the occipital bone. CT scan of the head showed symmetrical ventricular dilatation which was suspected to be a sequela of asphyxia at his birth although there was no obvious finding of cerebral contusion. His mental and physical development had been uneventful. One week after admission, his consciousness improved to some extent and at that time right hemiparesis and right infranuclear facial palsy were revealed. One month after the accident, he became alert and able to eat and to read the books on the bed, and then bilateral deafness was noticed. Audiometry showed that hearing was lost completely to both high and low notes, and there was absence of bone conduction bilaterally. Auditory brainstem response test revealed no response from wave I through wave V bilaterally. Right hemiparesis and right infranuclear facial palsy completely subsided and his mental and intellectual state became normal. He was discharged ambulatorily in good condition except for bilateral complete deafness. Now he is leading a useful life and attending a prefectural deaf and mute school.(ABSTRACT TRUNCATED AT 250 WORDS)

Chemical modification of dipeptidyl peptidase iv: involvement of an essential tryptophan residue at the substrate binding site.

Inactivation of pig kidney dipeptidyl peptidase IV (EC 3.4.14.5) by photosensitization in the presence of methylene blue at pH 7.5 was observed to have pseudo-first-order kinetics. During the process, until over 95% inactivation was achieved, the histidine and tryptophan residues were decreased from 14.0 to 2.7 and 12.6 to 7.1, respectively, per 94,000-Da subunit, without any detectable changes in other photosensitive amino acids. Modification of four histidine residues per subunit using diethylpyrocarbonate resulted in only 30% inactivation of the enzyme, while N-bromosuccinimide almost completely inactivated the enzyme with the modification of only one tryptophan residue per subunit, as determined by absorption spectrophotometry at 280 nm. The protective action of the substrate and inhibitors such as Ala-Pro-Ala and Pro-Pro against the modification of tryptophan residues with N-bromosuccinimide was observed both fluorometrically and by measurement of activity. On the basis of these results it is suggested that one of the tryptophan residues in the enzyme subunit is essential for the functioning of the substrate binding site of pig kidney dipeptidyl peptidase IV.

Synthesis of 1,2,3,4-tetra-O-acetyl-5-deoxy-5-C-[(R) and (S) -methoxyphosphinyl]-alpha- and -beta-D-ribopyranoses.

Treatment of methyl 5-deoxy-5-C-( diethoxyphosphinyl )-2,3-O-isopropylidene-beta-D- ri bofuranoside with sodium dihydrobis (2- methoxyethoxy ) aluminate , followed by hydrogen peroxide, mineral acid, and hydrogen peroxide, gave 5-deoxy-5-C-( hydroxyphosphinyl )-alpha,beta-D- ribopyranoses in 40-45% overall yield. The structures of these sugar analogs were effectively established on the basis of the mass and 400-MHz, 1H-n.m.r. spectra of the title compounds, derived by treatment with diazomethane and then acetic anhydride in pyridine.

Promotion of spontaneous diabetes in non-obese diabetes-prone mice by cyclophosphamide.

Cyclophosphamide promoted the onset of overt diabetes in non-obese diabetes-prone mice of both sexes. Two injections of this agent at 2 weeks apart were necessary to obtain a constant high incidence, although even a single injection was effective in some animals. Clinical symptoms of the cyclophosphamide-induced diabetes were similar to those of the naturally occurring type. The same schedule of cyclophosphamide treatment failed to induce diabetes in non-diabetic mouse strains, such as DS/Shi,Jcl:ICR or in non-obese non-diabetes-prone mice, which suggests that the promotion of diabetes by cyclophosphamide in non-obese diabetes-prone mice is not due to the simple pharmacological action of this agent but to some immunopharmacological action.

Magnetic microcapsules for targeted delivery of anticancer drugs.

To achieve targeted distribution of anticancer drugs with sustained activity, ferromagnetic ethylcellulose microcapsules containing an anticancer drug, mitomycin C (FM-MMC-mc), were prepared by a method based on phase separation principles. Two prototypes of FM-MMC-mc were made: one with the drug as the core and zinc ferrite on its capsular surface (outer type); the other with both the drug and zinc ferrite as the core (inner type). Both preparations provided a sustained-release property and a sensitive response to conventional magnetic force, although certain differences in the release rate of drug, magnetic responsiveness, and particle size were found between the two dosage forms. Animal studies showed that the magnetic microcapsules could be magnetically controlled in the artery and urinary bladder. VX2 tumors in the rabbit hind limb and urinary bladder were successfully treated with magnetic control of FM-MMC-mc. Pharmacokinetic study revealed that the targeting of the microcapsules markedly enhanced the drug absorption into the surrounding tissues for a prolonged period of time. The results indicate the feasibility and effectiveness of the magnetic microcapsules as a targeted drug delivery system.

Mercury in feathers of wild birds from the mercury-polluted area along the shore of the Shiranui Sea, Japan.

Total mercury content in the feathers of 95 stuffed, wild birds collected all over the shore of the Shiranui Sea (where Minamata disease occurred in many towns and villages in the 1960's) was measured. They were collected over 25 years, from 1955 to 1980. They showed relatively high mercury levels till the late 1970's, although drainage of waste water containing methylmercury from the Chisso Corporation's Minamata Factory was stopped in 1968. In addition, a strong correlation between feeding habits and mercury content was observed; thus the five groups in order of diminishing mercury content were: fish-eating sea birds (mean +/- SD = 7.1 +/- 3.7 ppm, n = 14), omnivorous water fowl (5.5 +/- 5.6 ppm, n = 17), predatory birds (3.6 +/- 2.9 ppm, n = 16), omnivorous terrestrial birds (1.5 +/- 1.2 ppm, n = 31), and herbivorous water fowl (0.9 +/- 0.4 ppm, n = 17). Mercury content of the feathers of omnivorous terrestrial birds in the Shiranui Sea was exceedingly high compared with those in China and Korea. The findings in the present study suggest that stuffed, wild birds may be a good index of past environmental pollution.

Purification of human plasma alpha 1-proteinase inhibitor and its inactivation by Pseudomonas aeruginosa elastase.

Human alpha 1-proteinase inhibitor was purified according to a modification of the method of Kurecki et al. (Anal. Biochem. 99, 415 (1979) ), with Affi-Gel Blue treatment before Zn-affinity column chromatography. The inhibitor was inactivated in the presence of Pseudomonas aeruginosa elastase (1/2,000 molar ratio) for 2 h at pH 7.5 and 25 degrees C. The inactivated inhibitor was purified by column chromatography on Sephadex G-75 and DE-52. Little or no difference was observed between the native and inactive inhibitors in immunological response, amino acid composition or far-ultraviolet CD spectrum. On the other hand, a considerable difference was observed in the near-ultraviolet CD spectrum. Two amino-terminal sequences were found in the inactive inhibitor in almost the same ratio; one was the same as that of the intact inhibitor and the other was Met-Ser-Ile-Pro-. The two components were separated by high-performance liquid chromatography using 0.1% trifluoroacetic acid containing 30-70% CH3CN (gradient) as the eluent. Amino acid analysis and N- and C-terminal amino acid sequence analyses indicated that one fraction corresponded to the sequence of 1-357 of the alpha 1-proteinase inhibitor and the other to 358-394. We concluded that P. aeruginosa elastase can inactivate human alpha 1-proteinase inhibitor by splitting the peptide bond of Pro357-Met358, leading to local change near the active site but little change in the structure as a whole. The split carboxy-terminal fragment binds tightly to the rest of the inhibitor.

Incorporation of sialoglycoprotein containing lacto-series oligosaccharides into chicken asialoerythrocyte membranes and restoration of receptor activity toward hemagglutinating virus of Japan (Sendai virus).

Bovine erythrocyte sialoglycoprotein (GP-2) (1) containing lactoseries oligosaccharide chains, which showed highly specific inhibition of hemagglutination by HVJ (Hemagglutinating virus of Japan, Sendai virus), was incorporated into neuraminidase-treated chicken erythrocytes which had lost their biological responsiveness to the virus. The GP-2-incorporated erythrocytes were agglutinated and lyzed again by the virus. Incorporation of 1,900 molecules of GP-2 per asialoerythrocyte restored fairly well the susceptibility of the cells to HVJ-mediated agglutination and hemolysis. Treatment of the erythrocytes with neuraminidase again resulted in the complete abolishment of the response to HVJ. The above observations are consistent with the view that exogenous sialoglycoprotein, GP-2, can be functionally integrated into the surface membrane of asialoerythrocytes and serve as the receptor for HVJ during the initial adsorption-fusion phase of the virus infection of the target cells.

T lymphocyte reconstitution in long-term survivors after allogeneic and autologous marrow transplantation.

T cell subsets and their immune reactivities were studied in long-term survivors after bone marrow transplantation and the results of autotransplanted and allotransplanted patients were compared. These two groups of patients (4 autotransplants and 4 allotransplants) were roughly comparable in terms of their underlying diseases, pretransplant conditioning regimens, supportive care, and posttransplant sampling days for immunological studies. Significant differences were observed between autologous and allogeneic marrow recipients in the total number of OKT3-, OKT4-, OKT8-, and OKIa1 -positive cells. Similar differences were observed between transplant patients and normal controls. Decreased OKT4 cells and increased OKT8 cells resulted in inversion of the OKT4:OKT8 ratio, which was significantly lower in allotransplanted compared with autotransplanted patients, and both groups of transplant patients showed depressed responses in comparison with normal controls. In contrast, there were no significant differences in MLR reactivities between transplanted patients and normal controls. When mitogenic responses were analyzed in relation to T cell subsets, phytohemagglutinin responsiveness showed a significant correlation with OKT4:OKT8 ratios (P less than 0.01) and the proportions of cultured OKT4 cells (P less than 0.01). These observations suggest that T lymphocyte reconstitution is still incomplete or abnormal in long-term survivors regardless of the type of graft. Furthermore, abnormalities observed in these long-term survivors were characterized by an imbalance of T cell subsets that was more profound in allotransplanted than in autotransplanted patients.
