Adenocarcinoma arising at a colostomy site. Report of a case.

Primary adenocarcinoma arising at a colostomy or ileostomy site is very rare, and only three cases have been reported following surgical treatment of ulcerative colitis. A 38-year-old man who developed an adenocarcinoma originating at the site of a colonic stoma and producing a large fungating tumor mass in the surrounding abdominal wall is described. This occurred 19 years after Miles' operation for a carcinoma of the rectum.

Hyperplastic fundic gland polyp of the stomach.

A case of a solitary pedunculated gastric polyp occurring in the fundic mucosa of a 47-year-old woman is reported because of its unique morphology. The polyp showed a peculiar lobulated configuration supported by smooth muscle bundles, and its epithelial component was composed mainly of a proliferation of relatively normal but disorganized fundic glands containing variable numbers of mucous, parietal, argyrophile, and chief cells. Although these histologic features may suggest a hamartoma-like quality, the lesion probably represents a peculiar form of localized hyperplastic process of the gastric mucosa exhibiting fundic glandular cell differentiation.

Malignant transformation of intrathoracic ancient neurilemoma in a patient without von Recklinghausen's disease.

A case of intrathoracic ancient neurilemoma undergoing malignant change in a 48-year-old woman is reported. The lesion was first discovered incidentally during routine roentgenologic examination of the chest 15 years before its surgical removal. The gross and histologic examination of the tumor showed the features of ancient neurilemoma with extensive regressive changes as well as small preserved areas composed of Antoni type A and B tissues. In addition, obviously histologically malignant foci manifested by the presence of increased cellularity, active mitotic activity, and gradual loss of orginary neurilemomatous differentiation were present. The patient died 16 months after operation of recurrent and distant metastatic disease. At autopsy, there was no evidence of generalized neurofibromatosis or other primary cancer. A review of the available literature revealed only two other similar cases documenting malignant transformation of solitary benign neurilemomas in patienets without von Recklinghausen's disease.

A case report of myasthenia gravis associated with Hashimoto's thyroiditis.

An unusual case of myasthenia gravis is presented. The disease was clinically established in a 71-year-old Japanese woman by an electromyographic, pharmacologic test and anti-acetylcholine receptor (anti-AChR) antibodies demonstrated in the circulating blood. The coexistence of Hashimoto's thyroiditis was diagnosed by the presence of a diffuse thyroid enlargement, anti-thyroglobulin and anti-microsomal antibodies and a lowered thyroidal 131I uptake. A delusion of persecution developed during the treatment with pyridostigmine and ephedrine chloride, but the symptom disappeared after discontinuing the use of ephedrine chloride.

Multiple malignancies (squamous cell carcinoma and sarcoma) arising in a dermoid cyst of the ovary.

A case of multiple malignancies (squamous cell carcinoma and sarcoma) arising in an ovarian dermoid cyst of a 75-year-old woman is reported: this appears, to the best of our knowledge, to be the first well documented example of such a complex tumor. The two malignant components had a well-demarcated boundary except for at their junction, a feature more indicative of a collision tumor rather than carcinosarcoma of the ovary. The solid sarcomatous portion displayed a prominently myxoid and pleomorphic appearance with a focal whorling and storiform pattern, the features considered to be compatible with the myxoid varient of malignant fibrous histiocytoma recently defined by Weiss and Enzinger. The patient is alive and well 21 months after surgery.

Cholecystic granulomas in gallstone disease. A clinicopathologic study of 17 cases.

This paper concerns with 17 cases of chronic cholecystitis with various granulomatous changes of the wall. All materials were obtained from patients who underwent cholecystectomy for various complications of gallstone disease. The only significant sign attributable to the granulomas was a palpable nodular induration in the wall at operation in several cases, and this led to a suspicion of neoplasm in one. Pathologically, these lesions were characterized principally by nodular proliferation (0.5-3.0 cm in size) of various forms of histiocytic cells, including lipid-containing histiocytes (foam cells), PAS positive brown pigmented cells, and multinucleated giant cells. Deposition of bile pigment and/or cholesterin crystals were found deeply in the areas of granulomatous reaction in about two-thirds of the cases. This finding supports a bile-induced granulomatous process of these lesions. The granuloma in one of our series contained typical Michaelis-Gutmann bodies seen in a case of malakoplakia. It is suggested that bile materials, which are capable of eliciting intense phagocytic reaction, may also act as an initiating factor in formation of malakoplakic lesions.

Cystosarcoma phyllodes of the breast with features of malignant fibrous histiocytoma.

A case of a 33-year-old female with metastasizing malignant cystosarcoma phyllodes of the left breast showing histologic features of malignant fibrous histiocytoma is reported. Metastases consisting of only undifferentiated mesenchymal cells were found in the lungs, liver, pleurae, kidneys, ileum, and vertebrae. Seven cases of fibrohistiocytic tumors of the breast previously recorded in the literature were reviewed. Although rare, this type of tumor should be included in the differential diagnosis of mesenchymal mammary tumors.

Bu-2313, a new antibiotic complex active against anaerobes. I. Production, isolation and properties of Bu-2313 A and B.

An unidentified oligosporic actinomycete strain, No. E864-61, produced two new antibiotics, Bu-2313 A (C27H35NO9) and Bu-2313 B (C26H33NO9). Bu-2313 A and B each exhibited a broad antibiotic spectrum against Gram-positive and Gram-negative anaerobic bacteria, and showed in vivo activity against experimental infections produced by B. fragilis and C. perfringens. Bu-2313 also inhibited some aerobic bacteria such as streptococci. Bu-2313 B was approximately two-fold more active than Bu-2313 A.
