A new type of mobile dental clinic developed following the Lebanon War, 1982.

A two-tier emergency dental services delivery system was developed following the October 1973 War, based on large mobile dental clinics (definitive restorations) and portable field units (emergency treatments). The increased demand for definitive restorations at the front lines, the mountainous terrain, and the extreme climatic conditions encountered during the Lebanon War, 1982, necessitated the development of a third alternative. A standard, compact module, either truck-mounted or helicopter-lifted, was outfitted as a fully equipped dental clinic according to Dental Corps specifications.

Effect of binocular variations of Snellen's visual acuity on Titmus stereoacuity.

Thirty normal subjects were tested with the Titmus stereotest varying the binocular Snellen's visual acuity from 20/20 to 20/200. From the 36 possible combinations, a nomogram describing the relationship of binocular variations of Snellen's visual acuity and stereoacuity was constructed. The nomogram provides a useful reference for comparing expected and achieved clinical results of the Titmus stereotest in normal subjects. The effects of various pathologic conditions of the eye on the relationship between Snellen's visual acuity and stereoacuity needs to be studied.

Wechsler discrepancies and the Rorschach experience balance.

A higher Performance than Verbal score commonly is interpreted as indicating a more externalizing, action-oriented style, while those who display the reverse pattern of WAIS scores often are seen as better able to delay. Because very similar personality descriptions are developed from the Rorschach Experience Balance (EB), a relationship between Verbal-Performance discrepancies and the ratio of movement to color was predicted. Contrary to expectation, correlations between these two indices for a group of psychiatric inpatients (N = 188) and a group of non-referred children N = (47) were nonsignificant. The Wechsler discrepancy also was unrelated to other Rorschach indices and, for the children, to scores derived from a behavior checklist. These findings cast some doubt on the assumption of a common factor underlying the WAIS Verbal-Performance discrepancy and the Rorschach Experience Balance.

A dose-response study of the effect of levobunolol on ocular hypertension.

We conducted a randomized, double-masked, dose-response study of the ocular hypotensive effect of the beta-adrenergic blocker, levobunolol. A single drop of placebo or levobunolol (at concentrations of 0.03%, 0.3%, 0.6%, 1%, and 2%) was administered to one eye of each of 48 patients with ocular hypertension. The 0.3% and 0.6% concentrations decreased intraocular pressure significantly from baseline levels compared to placebo at one, two, and four hours after treatment. The 1% and 2% concentrations decreased intraocular pressure significantly from baseline compared to placebo at one, two, four, six, eight, and 12 hours after administration. No objective or subjective side effects were noted, and no substantial changes in visual acuity, pupil diameter, pulse rate, or blood pressure were recorded during the study. These results appear to justify long-term studies of levobunolol for the treatment of increased intraocular pressure.

Experience in the computer handling of clinical data for dialysis and transplantation units.

This paper describes a computer data handling system which includes a powerful interactive program for the presentation and rapid modification of graphs. The system was developed in a dialysis and transplantation center in its early phases and is now in use in 20 renal centers in Europe and North America. Recent developments are a response to the varying demands of different centers. Our work describes some of the wide range of functions for which the system is used and the favorable response of staff working with a system that is easy to use and flexible, and which is also responsive to the requirements of the clinical user without the intervention of a computer programmer. The versatility of the system has made it possible to transfer it to other clinical specialties. As well as retaining its primary functions of improving the day-to-day care of patients and increasing the understanding that doctors have about their patients, the computer is used for research and administrative functions.

Interpretation of white space responses in children's Rorschach protocols.

While most texts continue to assert that the Rorschach white space response (S) indicates hostility and oppositionality, several studies suggest that such responses are more accurately reflective of cognitive differentiation and autonomous strivings. In addition, there are no studies which have systematically explored the significance of S responses in children's protocols. To shed light on these issues, 47 children (7 to 13 yr.) were administered the Rorschach, Thematic Apperception Test, and the WISC-R, while their parents completed a behavioral checklist. Subjects with high S-response, independent of age, IQ, or perceptual accuracy, used the whole blot more often than those who made less frequent use of white space. They also had higher frequencies of shading determinants and blends and used more content categories. These results were interpreted as suggesting a tendency for subjects showing high S-response to construe the world in more complex terms. No differences were found between subjects high and low on S-response on either behavioral ratings or projective indices of hostility or negativism.

CO2 responses in infants: on-line display and analysis using a microcomputer and waveform analyzer.

This report describes a new system for measuring and recording ventilatory responses to inspired CO2 in infants, which consists of a nasal pneumotach, microcomputer, and a waveform analyzer. The waveform analyzer measures ventilatory responses on-line and displays them in real-time, breath-by-breath. Thirty-two ventilation studies were performed in 24 normal, full-term infants (3-300 days of age). The mean ratio of delta VE/ delta PACO2 was 44.3 +/- 22.9 (SD) ml/kg X min X mm Hg BTPS (range 15.0-121.2). We believe this new system is a significant improvement over previous methods for measuring and recording ventilatory responses of infants.

Fourth goal of perinatal medicine.

Reduction in maternal mortality, infant mortality, and infant morbidity have been successively the goals of perinatal medicine. The fourth is to reduce bonding failure. In July 1978 a preventive service was started in the John Radcliffe Maternity Hospital. A twice-weekly round is made. Midwives refer families who cause them concern. In the first year the referral rate ws 20.5 per 1000 liveborn babies. The referred sample differed from the hospital population in terms of maternal psychiatric history, marital state and babies' admission to special care. The main reasons for referral were: doubt about parenting ability (27%), psychiatric history (15%), disturbed behaviour in hospital (14%), and diffuse social and medical problems (17%). Long-term care was needed for only 14% of families. At their first birthdays, six babies were placed away from their natural parents; the sample had had a slightly higher than expected admission rate to hospital; the distribution of weights did not differ from the expected; doctors and health visitors were still concerned about one-quarter of the families. Seven cases of screening failure were found among those not referred to our service, but only one was seriously abused. No child referred in the first year has been seriously neglected or abused.

Timolol and epinephrine: long-term evaluation of concurrent administration.

Thirty-two ocular hypertensive subjects were treated for 90 days with either 0.5% timolol maleate or 2% epinephrine hydrochloride twice daily to one eye and both drugs to the fellow eye. The ocular hypotensive effects of timolol and epinephrine were partially additive throughout the course of this study. On the 91st day, eyes treated with epinephrine had a 25.8% mean reduction of intraocular pressure from baseline, whereas their fellow eyes treated with epinephrine and timolol had a 33.9% reduction. Eyes treated with timolol had a 27.2% mean reduction of IOP, whereas fellow eyes treated with timolol and epinephrine had a 29% reduction. These results suggest that the majority of patients being treated with either drug are unlikely to have a substantial long-term reduction in IOP when the other drug is added to their therapeutic regimen.

Timolol and acetazolamide. A study of concurrent administration.

A five-week clinical trial of the concurrent administration of topical timolol maleate and oral acetazolamide was conducted to determine the additivity of the ocular hypotensive effects of the two drugs. One drop of 0.5% timolol maleate, every 12 hours, produced a mean reduction in outflow pressure from baseline of 36.0%. When added to an eye pretreated with oral acetazolamide, timolol caused an additional mean decrease in outflow pressure of 27.6%. Acetazolamide, 500 mg orally every 12 hours, produced a mean reduction in outflow pressure from baseline of 48.6%. When added to an eye receiving topical timolol, oral acetazolamide caused an additional mean decrease in outflow pressure of 43.2%. Concurrent administration of oral acetazolamide and topical timolol reduced outflow pressure only slightly less than the predicted reduction assuming full additivity of the drugs. This study supported the clinical usefulness of concurrent administration of acetazolamide and timolol to lower intraocular pressure.
