Data from 17 states, drawn from the 1983 National Study on Child Neglect and Reporting, were used to compare families in which a daughter had been sexually abused by a natal father or stepfather. While broad comparisons are made in terms of age and race of victims, as well as household composition, the primary focus is on familial stress factors. Given the lower incidence of natal father abuse and the assumption that natal fathers have stronger emotional links to their children and greater commitment to the father role, we predicted that when they did engage in sexual abuse, it would be in a family environment characterized by relatively high levels of personal, social, and economic stress. This was confirmed to the extent that natal father abuser families showed significantly higher levels of drug and/or alcohol abuse, marital problems, and insufficient income than did stepfather abuser families.

The flexure strength of the joint between layers of different types of amalgam condensed consecutively one on to the other was evaluated in vitro. The data indicated no statistical differences in flexure strength between the combined samples (single composition spherical and lathe-cut eutectic amalgams) and the control (homogeneous lathe-cut eutectic amalgam).
Our experience and review of the literature demonstrate that spinal epidural lipomatosis is a rare but serious neurologic complication of iatrogenic Cushing's syndrome. Most patients develop slowly progressive paraparesis over months, but a subgroup exists which presents with acute, irreversible paraplegia. We consider therapeutic options.

We studied the effects of adding cytomegalovirus (CMV) in vitro to normal human bone marrow mononuclear cells (BM-MNCs), committed myeloid progenitor cells, primitive myeloid blast-colony forming cells, and pre-formed marrow stromal cell monolayers in order to shed light on the mechanism by which hemopoiesis is suppressed in patients who acquire systemic CMV infection after allogeneic bone marrow transplantation. Incubation of BM-MNCs or committed progenitor cells with laboratory strain AD169 or wild strain CMV had no significant effect on total colony numbers or the morphology of component cells. CMV mRNA was not identified by in situ hybridization. In contrast, incubating marrow stromal monolayers with CMV produced specific cytopathic effects in fibroblasts and adipocytes and reduced the capacity of the stromal layers to support the proliferation of primitive myeloid progenitor cells. We conclude that CMV infection may impair hemopoiesis in vivo by a direct effect on the cellular components of the marrow stroma.
The older patient requiring major orthopedic surgery for a hip fracture is at risk of poor outcome if special precautions are not taken during the perioperative period. Close attention must be paid to associated medical problems and the use of medications, especially during the postoperative period. Surgical success and rehabilitation with a return to a high level of function can be best assured through good cooperation between primary care physicians, internists, and orthopedic surgeons.

Data are presented on standardization and reliability of a behavior-based measure of attention and self-control. This computerized test system administers a series of continuous performance tests, as well as measures of response inhibition. The practicality and sturdiness of this approach have allowed for the development of normative data based on the protocols of 1266 children between 4 and 16 years of age. Test-retest reliability coefficients were high after short intervals (less than 45 days) and 1 year. Originally developed for use in the assessment of children referred for ADD/Hyperactivity, this system has been applied to the evaluation of children with other disorders, as well as to adolescents and adults. Findings are discussed in terms of multimodal assessment, computerization of psychological tests, and the need for cautious standardization.
The audiograms of 59 patients who had 15 different types of tubes implanted in a prospective study from 1982 to 1985 were reviewed. The audiograms were analyzed for changes in air and bone thresholds at 250, 500, 1000, 2000, 4000 and 8000 Hz. The first post-operative audiogram was tabulated for all the initial post-operative visits and for only the initial post-operative visits in which the tubes were functioning. The change in pure tone average air conduction showed a range in improvement from 22.1 dB for the Armstrong Grommet Tube to 1.9 dB for the Canoe Tube. A total of 8 tube types showed mean improvements of 10 dB or better. With the exception of the Reuter Bobbin, all mean air conduction thresholds in functioning tubes were below 20 dB. For 5 tubes the mean threshold was less than 10 dB. The Armstrong Beveled Grommet, Donaldson, and Richard's Wing tubes showed the largest improvements in air conduction threshold with mean decreases of 22.1, 18.7 and 18.3 dB, respectively. Pure tone average bone conduction for all functioning tubes improved 2.8 dB.

The thymus-dependent (T-dependent) limb of the immune response in protein malnourished mice was examined by using several tests of T cell function, both in vivo within the intact animal, and after removal of lymphocytes from the protein deprived host. The capacity to provide help for IgG antibody responses and the DTH response to antigen were diminished after short-term (3 weeks) deprivation. However, both these responses were normal in mice maintained on a protein deficient diet for 11 weeks. The depressed DTH responses of protein deprived mice were due, at least in part, to a failure to mount the inflammatory phase of the response. Finally, using a graft-versus-host reaction (GvHR) as an index of T lymphocyte function, we found that spleen cells from malnourished donors were fully capable of inducing the splenic and intestinal changes associated with a GvHR in unirradiated F1 animals. Overall, these results suggest that T cell function is not irreversibly damaged by protein deprivation. However we propose that cell-mediated immune responses are influenced indirectly by the restrictive environment which interferes with cell migration, and by the impaired capacity of protein-deprived animals to mount a non-specific inflammatory reaction.
