The effect of a single dose of DDT on thyroid function in male rats.

A single dose of 100 mg/kg of commercially available 1, 1, 1,-trichloro-2, 2, bis (p-chlorophenyl), ethane, (DDT), dissolved in corn oil, was administrated to adult male Sprague-Dawley rats by stomach tube and two indices of thyroid function were examined. The iodide transport function of the thyroid gland determined by the thyroid: serum radioiodide concentration ration, (T:S) was depressed, and complete inhibition in thyroidal radioiodine release occurred 8 to 10 hr following administration of DDT and continued for 24 to 30 hr. Subsequently the block in thyroidal 131I release was followed by a return to a rate not significantly different from that of control rats. Whether the acute effects of a single large dose of DDT on thyroid function are a consequence of a local action on the thyroid gland or on pituitary thyrotropin release, or of some action elsewhere is not clear.

Murine resistance to type III group B streptococci.

We examined the role of serum, complement and the reticuloendothelial system in mouse resistance to type III group B streptococci (GBS). Normal serum obtained from 21 day-old mice failed to protect chick embryos against a lethal intravenous (IV) inoculation of GBS-III-SS620/50. Mice that were decomplemented using cobra venom factor remained resistant to intraperitoneal (IP) inoculation of 10(6) colony-forming units (CFU) of GBS-III-22620/50, GBS III-Bell and GBS-III-Minnesota. In contrast, mice prepared with IP oleic acid were killed with 10(4) CFU of GBS-III-SS620/50 IP. However, when mice were injected IV with oleic acid, they remained resistant to IP inoculation of GBS-III-SS620/50. These results suggest that peritoneal macrophages play a role in murine defense against IP infection with GBS-III-SS620/50.

Multiple renal cell carcinoma in solitary kidney.

An unusual case of asynchronous bilateral renal cell carcinoma presenting as multiple tumors in a solitary kidney is presented. A literature review suggests that the prognosis is improved for those patients with a longer interval between tumors and for those who undergo surgical therapy.

Electrocardiographic poor R wave progression III. The normal variant.

Electrocardiographic poor R wave progression (PRWR) is found in patients with anterior myocardial infarction, left ventricular hypertrophy and right ventricular hypertrophy, and is also seen in apparently normal individuals. Technical and constitutional factors that might contribute to PRWP in normal subjects were examined. Prospective analysis of the effect of precordial lead misplacement and respiratory variation on the surface ECG was performed in 68 hospitalized patients. Both false positive and false negative PRWP were created with superior and inferior lead position change respectively. The incidence and mechansim of true PRWP were analyzed by retrospective analysis of ECGs and records of 100 individuals with normal findings, and 50 additional individuals with mitral valve prolapse, at cardiac catheterization with coronary angiography. PRWP occurred in 8% (8/100) of normals and was not related to age, sex, height, weight, body surface area, ponderal index, thoracic skeletal abnormalities, ECG frontal axis, serum cholesterol, arterial blood pressure or mitral valve prolapse. In view of the voltage changes produced by alteration of lead placement, one tail of a normal distribution of null planes may account for PRWP in subjects without disease.

Pulmonary arteriovenous fistula, pulmonary artery aneurysm, and other vascular changes of the lung from penetrating trauma.

Pulmonary arteriography was performed in 22 patients with penetrating injury of the lung(s) within the first 72 post-injury hours and no vascular lesion directly attributable to the injury was found. Pulmonary arteriovenous fistula or pulmonary artery aneurysm secondary to penetrating pulmonary injury, are extremely rare lesions, but they should be suspected in a patient with penetrating wound of the chest with signs and symptoms of pulmonary arteriovenous fistula or with chronic residual well circumscribed density on chest roentgenogram. The treatment of these lesions should be excision of the lesion with preservation of the lung, whenever possible, or resection of the involved lobe.

Intraaortic balloon counterpulsation with and without reperfusion for myocardial infarction shock.

Forty patients were treated for cardiogenic shock secondary to acute myocardial infarction. Twenty-one (group 1) were treated with intraaortic balloon counterpulsation and 19 (group 2) were treated with counterpulsation and coronary artery bypass grafting. The groups were similar in age, incidence of previous infarction, initial hemodynamics and coronary anatomy. The in-hospital mortality between group 1 (52.4%) and group 2 (42.1%) was not significantly different. The difference in long-term mortality between group 1 and group 2 was substantially different (71.4% vs 47.3%). The subset of group 2 (n = 12) that underwent reperfusion and counterpulsation within 16 hours from the onset of symptoms of infarction had a lower mortality (25.0%) than the subset (n = 7) that underwent operation more than 18 hours after the onset of symptoms (71.4%). The long-term mortality in the subset of group 2 patients operated on within 16 hours after the onset of infarction was significantly different from that in group 1 (25.0% vs 71.4%, p less than 0.03). The data suggest that reperfusion with counterpulsation is more effective when carried out early. Patients who develop shock more than 18 hours after the onset of symptoms of infarction appear to benefit most if treated with counterpulsation alone.

Nonresective treatment of abdominal aortic aneurysms. Use of acute thrombosis and axillofemoral bypass.

In good-risk patients, abdominal aortic aneurysmectomy can be accomplished with a mortality of 2% to 5%. However, in poor-risk patients, ie, those with severe reduction of cardiac, respiratory, and/or renal function, the mortality of this procedure has been reported to be as high as 60%. Fifteen poor-risk patients with abdominal aortic aneurysms have been treated with acute, induced thrombosis and simultaneous axillobilateral femoral bypass. Each patient had preoperative ultrasound and radionucleide flow studies and the runoff from the aneurysm was determined angiographically. Thrombosis, induced by interruption of the iliac outflow vessels, occurred in 12 patients within 72 hours. Flow persisted in three patients, and thrombosis was induced by transaxillary catheter deposition of bucrylate in the residual outflow vessels. There were two operative deaths (less than 30 days), both due to myocardial infarctions, and four late deaths, three of which were caused by the problems that initially contraindicated direct graft replacement of the aneurysm. Preliminary experience with this approach indicates that this is a simple and potentially effective method of treatment of abdominal aortic aneurysm where direct graft replacement is contraindicated.

Microbiologic evaluation of poly-HEMA root canal filling material.

This study was designed to determine whether poly-HEMA root canal filling material can support bacterial growth. Eight different microorganisms were tested against the polymerized material, as well as against the powder and paste components of the unpolymerized material. The results establish that this root canal filling material does not support bacterial growth.

Scanning electron microscope study of a new irrigation method in endodontic treatment.

The effectiveness of a newly developed perforated endodontic irrigation needle, with its distal aperture sealed, was compared to conventional endodontic irrigation procedures. In all instances 5 percent sodium hypochlorite was used since the delivery system, rather than the irrigant, was being evaluated. A base line group in which all soft tissue was described was also included. The results indicate that the perforated endodontic irrigation needle is a very effective instrument for the cleansing of instrumented root canals.
