The role of the Food and Drug Administration in the regulation of the products of recombinant DNA technology.

We emphasize again that the FDA will regulate each product according to the relevant statute and will apply those quality assurance standards necessary to assure safety for foods, and safety and efficacy for other products. We are also sensitive to continuing public concern about the environmental consequences of this technology. What-ever the outcome of our review, the FDA will attempt to regulate responsibly with appropriate concern for our two major constituencies--the public and the industry.

Secondary hyperparathyroidism manifesting as acute pancreatitis and status epilepticus.

A 36-year-old with end-stage renal disease secondary to hypertensive nephrosclerosis had a two-day history of epigastric pain and nausea. Soon after admission, multiple grand mal seizures uncontrolled by intravenous phenytoin sodium and diazepam developed. His calcium level was 14 mg/dL and his amylase level was 2,230 mg/dL; lumbar puncture was normal. Hemodialysis lowered his calcium level to 10.7 mg/dL but failed to control his seizures. Secondary hyperparathyroidism was thought to be the cause of his malignant hypercalcemia, and an emergency subtotal parathyroidectomy was performed. Postoperatively, his grand mal seizures resolved. Confusion and aphasia also developed, but they resolved over the ensuing three weeks. Microscopic examination of the parathyroid glands revealed diffuse chief cell hyperplasia. Preoperative parathormone level was 2,196 pg/dL (normal, less than 450 pg/dL). A review of the literature has failed to reveal a similar case.

[Role of secretion in the dynamics of the maturation of predentin. Morphological, autoradiographic and freeze fracture study in the rat; changes induced by vinblastine].

Morphological and autoradiographic data give evidence that secretion take place at the odontoblast process level according to a lateral axis, perpendicular to the maturation axis of the predentine. Induced modifications with vinblastine sulphate emphasize the importance of the pericellular space (150--200 nm) while the disorganization of the collagen fiber arrangement suggests the presence of flows. We propose an hypothesis which connects all these facts.

Esophagogastrectomy using the auto suture EEA surgical stapling instrument.

A stapling instrument, which creates end-to-end inverting anastomoses, is described for esophagogastrostomy. The inverted anastomosis is held together by a double staggered row of stainless-steel wire staples. Experience with 7 consecutive patients who underwent esophagogastrectomy for carcinoma of the distal esophagus or proximal stomach using this device is reported. Postoperative complications included atelectasis in 1 patient, cardiac failure in 1, and pneumonia in 1. A fibrotic stricture developed at the anastomotic site 5 months postoperatively in 1 patient. There were no anastomotic leaks. The stapled anastomosis takes 2 to 3 minutes to perform and seems to be at least comparable to handsewn anastomoses.

Atrioventricular sequential pacemakers: indications, complications, and long-term follow-up.

Our experience with 32 patients with atrioventricular (AV) sequential pacemakers and an average follow-up of 22 months is presented. The pertinent literature and physiology are reviewed. The indications, advantages, and complications of AV sequential pacemakers are analyzed. Half of the patients required bifocal pacing for control of arrhythmia alone, while half required control of arrhythmia associated with congestive heart failure due primarily to a noncompliant left ventricle. It is anticipated that the hemodynamic improvement occurring as a results of AV sequential pacing will increase the use of this mode of cardiac pacing in selected patients.

Effects of methyldopa on renal hemodynamics and tubular function.

To determine the effects of methyldopa on renal function, clearance studies were performed on hypertensive subjects during sustained steady-state water diuresis. The data reveal an acute fall in glomerular filtration rate and sodium clearance, whereas renal blood flow was unchanged. The antinatriuresis was the result of decreased filtration of sodium and possibly enhanced proximal tubular sodium reabsorption. These changes occurred before any demonstrable fall in systemic blood pressure; thus a direct effect of the drug on arteriolar resistance within the renal circulation is suggested. Chronic administration of methyldopa for 1 wk induced sustained reduction in blood pressure and resulted in the same changes in renal hemodynamics and sodium excretion noted after acute administration. These data suggest that methyldopa, like other antihypertensives, reduces glomerular filtration rate and increases sodium retention.

Evidence for and characterization of a liver cell proliferation factor from blood plasma of partially hepatectomized rats.

A boiled extract acidified to pH 5.5 from the blood plasma of partially hepatectomized rats was treated with neuraminidase and injected i.p. into untreated rats. The DNA-synthesis of the liver cells showed a four-fold increase in comparison with controls. Extracts from the plasma of partially hepatectomized rats without neuraminidase treatment showed no increase in DNA-synthesis. Injections of boiled acid extracts from plasma of normal rats, however, showed no comparable differences before and after neuraminidase treatment. The activity of neuraminidase treated boiled, plasma extract is lost after treatment both with trypsin-chymotrypsin and with beta-galactosidase. Gel chrmoatography of the factor gave a molecular weight of about 38,000 D. The specific activity of the active extract after chromatography was raised by a factor of 300. According to affinity chromatography the factor was shown to be a glycoprotein containing N-Ac-glucosamine. The factor is inert with respect to the proliferation of spleen and kidney, i.e. it is organ specific. According to these results a regulatory system of hepatopoiesis is proposed.

An analysis of the factors associated with respiratory distress syndrome in premature infants whose mothers had been given dexamethasone therapy.

The incidence of respiratory distress syndrome (RDS) in 259 premature infants whose 218 mothers had received antepartum dexamethasone therapy was 8.9 per cent. The main characteristics of those infants who developed RDS were a shorter gestational age and a lower Apgar score at delivery; all were delivered before 35 weeks gestation, and those with Apgar scores below 7 had a significantly higher incidence of RDS. The incidence of RDS in vaginal breech deliveries (15.4 per cent), Caesarean sections (14.6 per cent), and multiple births (16.0 per cent) was higher than in vaginal vertex deliveries (6.8 percent), and in singletons (6.0 per cent). The use of isoxsuprine and the length of time membranes were ruptured before delivery were unrelated to the incidence of RDS. The results of this study suggest that while maternal glucocorticoid administratioin is an important ancillary to the prevention of RDS in premature infants it cannot substitute for optimal delivery conditions.

Human lung transplantation.

The 38th attempt at allotransplantation of a human lung is described in a patient with injury due to smoke inhalation. The innovative features in the procedure included prolonged support with an extracorporeal membrane oxygenator during and for four days following transplantation, pharmacologic control of platelet function with sulfinpyrazone, continuous monitoring with a fiberoptic ear oximeter, and pretreatment of the transplanted lung with cytotoxic durgs. The patient survived until the 18th postoperative day, with no evidence of tissue rejection, but he died following ischemic disruption of the bronchial anastomosis. We conclude that the major determinant in the future of human lung transplantation is related to the establishment of a bronchial arterial supply to the transplanted bronchus.
