Intrarenal arterial stenosis. Saralasin acetate infusion test as an aid to diagnosis.

This report documents a case of hypertension caused by an intrarenal arterial stenosis in a ten-year-old girl. The renovascular origin of her hypertension was suggested by a positive saralasin acetate infusion test, and the lesion was identified by use of the subtraction technique in renal arteriography. Partial nephrectomy resulted in resolution of the patient's hypertension with good function in the remaining portion of the kidney.

Evaluation of low flow long dwell chronic intermittent peritoneal dialysis.

TWelve patients on chronic intermittent peritoneal dialysis for end-state renal disease were followed a total of eight months on high dialysate flow rate (four liters/hour) and low dialysate flow rate (two liters/hour). Creatine, blood urea nitrogen, serum electrolytes, albumin, calcium and phosphorous were recorded weekly. Interdialytic weight gain and intradialytic weight loss were recorded with each dialysis. No significant differences were noted when the mean values were compared for the two trail periods. The two liter/hour dialytic flow rate is less expensive, more convenient for the patients, and could result in a decreased risk for peritonitis when compared to the four liter/hour flow rate. For chronic intermittent peritoneal dialysis, the two liter/hour flow rate should be the preferred mode of therapy.

Serious adverse reactions associated with sulindac.

Sulindac is a nonsteroidal anti-inflammatory agent that has been associated with serious adverse reactions. We saw four patients with reactions associated with sulindac. Our patients, one of whom died, had high temperatures and involvement of one or more organs, including the skin, liver, CNS, lymph nodes, bone marrow, and lungs. Eight similar previously reported cases also are summarized. In view of these cases of sulindac-induced toxicity, six of which were proved unequivocally by drug rechallenge, we suggest that physicians be cautious in prescribing this agent.

The effects of vinblastine on the cell structure and activity in the rat incisor enamel organ during the secretory stage in vivo as shown by radioautography using 3H-proline, 3H-serine and 3H-fucose.

Four hours after injection of vinblastine sulphate (VBL) the cells of the enamel organ of the rat incisor showed: 1) loss of polarity in the tall secretory ameloblasts--secretory granules, which were lacking in the Tomes processes accumulated in the supranuclear area; condensing vacuoles also accumulated near dictyosomes; enlarged intercellular spaces without disruption of intercellular junctions; lysosomal structures and autophagic vacuoles. 2) an increase of lysosome-like structures in the outer enamel epithelium, the stellate reticulum and the stratum intermedium. 3) a severe reduction in number of free ribosomes in the stratum intermedium as well as an increase in size of the Golgi complex: numerous electron lucent vesicles appeared. These findings as well as radioautographic data demonstrated 1) the impairment of secretory activity of ameloblasts. 2) the restriction of permeability in the enamel organ suggesting strong interaction between VBL and the membranes.

[Use of dysprosium considered as an activable tracer in the study of tooth structures].

After 32 days of administration to rats of dysprosium in drinking water, this element has been studied by neutron activation analysis in the intra-oral part of the incisors as well as in the molar crowns. This element was absent in the intra-osseous parts of the incisors, the femurs and the liver. After intravenous injection of 13 mg or 26 mg of Dy in several doses over 32 days, this element was found in the incisor formation and maturation zones as well as in the part having erupted in the oral cavity. It was also found in the molars, the femurs and the liver. The structural study has shown that no cytotoxic effect was observed with the dysprosium doses used. Dentinogenesis and amelogenesis were not disturbed. The final structures were homogeneous. By comparing the two administration ways of dysprosium we have at our disposal a study model using an inducible tracer of the incorporation of adsorption of a mineral element in dental structures during early or late maturations.

[Histochemical and morphometric study of the changes induced by the absorption of sodium fluoride on the submaxillary and sublingual glands in the rat].

Three groups of Osborn Mendel rats received for two weeks either drinking water without addition of fluorides (control rats) or drinking water containing respectively 50 and 100 ppm of sodium fluoride. The histochemical study of the submaxillary and sublingual glands showed a very strong decrease in the polysaccharide and protein stainings in the 100 ppm group, whereas the 50 ppm group showed similar reactions to the control group. The morphometric study showed a strong decrease of the acinar and canalicular surfaces mainly in the submandibular gland. Thus the absorption of high doses of sodium fluoride induced in these two salivary glands morphological changes of the secretory cellular units. An indirect way of action can thus be exerted in the oral cavity.

[External adamantine epithelium, stellate reticulum and intermediate stratum of the enamel organ of the incisor in the rat: ultrastructural study of sections and after freeze fracturing].

A morphological study of the cells of the outer epithelium layer, of the stellate reticulum and of the stratum intermedium of the rat incisor on thin sections and after freeze-etching allowed the differentiation of an intracellular and an intercellular compartment. Both were implicated in transport and diffusion phenomenons. The structure of these cells suggested that they synthetize non exportable proteins. Nevertheless they could be implicated in some none precised activities of biosynthesis and secretion.

Successful revascularization of totally ischemic bronchial autografts with omental pedicle flaps in dogs.

In 10 dogs, the left lung was removed and a closed stump of bronchus, comprising the distal main and lobar bronchi, was reanastomosed to the main bronchus. This totally ischemic cul-de-sac of bronchus was wrapped by an omental flap in five of the 10 dogs. All dogs without an omental wrap died of graft necrosis within 5 days. Injection studies into the bronchial arteries failed to demonstrate any revascularization of the bronchial grafts. In contrast, those dogs with omental wraps, when put to death at 23 days, displayed healthy viable bronchial grafts with full revascularization from the omental vessels. In another three dogs the revascularization via the omentum was demonstrated as early as 4 days and well established at 8 days. The ability of the omentum to rapidly revascularize the bronchus may be of value in potentially ischemic bronchial anastomoses such as in lung transplantation.

Effects of phosphate administration on tubular calcium transport.

To evaluate the mode and tubular sites of action of PO4 administration on renal Ca reabsorption, micropuncture and clearance experiments were performed on acutely parathyroidectomized rats: (1) control (n=7), 2.5% saline expansion + furosemide infusion (0.5 mg/kg/hr); (2) PO4 infusion (n=6), treated as were the controls plus intravenous neutral PO4 (25 mg/kg/hr). CaCI2 was infused to keep plasma UF Ca comparable in both groups [2.68 +/- 0.08 mEq/l (mean +/- S.E.) vs 2.52 +/- 0.10, N.S.]. Po4 infusion increased plasma PO4 (14.0 vs 8.4 mg/dl, p less than 0.001) and FE PO4 (9.03% vs. 0.09%, p less than 0.001) but decreased FE Ca (1.83% vs. 5.95%, p less than 0.02) while GFR and FE Na were similar. FD of Na, PO4 and Ca were similar in late proximal and ED tubule punctures. In the LD tubule, PO4 infusion increased FD PO4 (18.5% vs. 7.9%, p less than 0.005) and reduced FD Ca (3.51% vs. 6.66%, p less than 0.01) and (TF/UF) Ca/Na (0.69 vs. 1.40, p less than 0.001). LD tubule FD Ca (y) was inversely related to TF/P PO4 concentration (X) in LD tubules: (y=7.2 - 0.12x, r=0.68, p less than 0.02). Significant Ca reabsorption (1.68% of filtered load) was observed beyond the superficial LD tubule only during PO4 infusion. These data indicate that phosphate infusion enhances Ca reabsorption independently of PTH, plasma Ca, and renal Na handling at a site within and possible beyond the distal convoluted tubule of the superficial nephron. [A multicriteria tool for the analysis of health status indicators of a population. Application to the elaboration of a typology for the use of health indicators].

The authors have designed a multicriteria framework for the analysis of health indexes in order to provide a tool for the comparison of population health status indexes and to establish a typology of these indexes regarding their use for public health. This framework allows for (a) shedding some light on the conceptualization of health underlying each index, (b) understanding the nature of the phenomenon which are effectively measured, (c) understanding the principles of the construction of the index, (d) giving some details concerning its main properties, (e) and finally understanding its possible uses. Using this multicriteria framework for analyzing and comparing 16 different population health status indexes, the authors suggest some guidelines for choosing an index regarding its future utilization (identification of priorities, resources allocation, evaluation). Finally, the limits of the utilization of health status indexes within the process of decision making in public health are underlined.
