Chronic pain patients and the nonorganic physical sign of nondermatomal sensory abnormalities (NDSA).

The nonorganic physical sign of nondermatomal sensory abnormalities (NDSA) is frequently found in chronic pain patients. The presence of NDSA qualifies the patient for a potential DSM-III diagnosis of conversion disorder. DSM-III (Axis I and II) diagnoses and personality styles were assigned to 283 mixed chronic pain patients. Discriminant analysis was used to identify diagnoses and personality styles significantly related to NDSA. The demographic variables of workers' compensation (WC) status, age, sex, and race were used as control variables. The discriminant function comprised all control variable, and psychiatric diagnoses accounted for only 22.1% of the variance. Age and WC status, i.e., the demographic variables, explained most of the variance (16.8%), with psychiatric diagnoses adding little (5.3%). The vast majority of the variance is unexplained, indicating the presence of "unexplained" factors for the presence of NDSA in chronic pain patients.

Completed suicide in chronic pain.

Although convergent lines of evidence indicate that one can expect a high rate of suicide completion for chronic pain patients, this problem has not previously been investigated. Follow-up data from our pain center revealed three chronic pain patients (two men and one woman) who completed suicide. These three cases are presented. The sequential nature of the data enabled us to calculate suicide rates for our chronic pain population and subsamples of this population: 16.5 women per year; 29.3 men per year; 57.1 white men and 34.9 white women in the age range of 35-64 years per year; and 78.6 white worker compensation men in the age range of 35-64 years per year. Calculation of the 95% confidence interval and comparison of these suicide rates to the general population of the United States using the Z statistic indicated that all chronic pain patient suicide rates were significantly greater than that of the general population. White men, white women, and white worker compensation men with chronic pain in the age range of 35-64 years are twice, three, and three times as likely, respectively, as their counterparts in the general population to die by suicide. Although no firm conclusions can be drawn because of the small suicide sample, these case reports indicate a need for further studies of chronic pain patient suicide rates at other pain centers.

More Munchausen with chronic pain.

Munchausen's syndrome is a popular name for a factitious disorder with physical symptoms. We have identified four Munchausen patients within a sample of 2,860 chronic pain patients for a frequency of 0.14%. These patients are described and an attempt is made to develop a profile of the Munchausen patient presenting with chronic pain.

Impact of a humanmade disaster on the utilization pattern of a psychiatric emergency service.

In May 1980, Liberty City Miami, a black area, suffered a humanmade disaster: a riot. The effects of the riot on the utilization pattern of the only psychiatric emergency service (PES) for the Liberty City catchment area were studied in two ways: autocorrelation and cross-correlation analyses were used to determine whether PES presentations demonstrated a 7-day cycle and whether this cycle was disturbed by the riot; and analyses of variance (ANOVAs) were used to compare numerically the riot week to the preceding week, the riot week to the following 12 weeks, and the 12 pre-riot weeks to the 12 post-riot weeks. Black PES presentations did not demonstrate a statistically discernable cycle, and none of the ANOVAs were statistically significant. It was concluded that the riot did not have a significant psychiatric morbidity effect as measured by PES presentation data. These results are discussed in reference to previous disaster studies.

Calcium in bacteria: a solution to which problem?

Calcium and calcium-binding proteins including those resembling calmodulin are implicated in numerous diverse processes in bacteria. These processes include chemotaxis, sporulation, virulence, the transport of sugars and proteins, phosphorylation, heat shock, the initiation of DNA replication, septation, nucleoid structure, nuclease activity and recombination, the stability of the envelope, and phospholipid synthesis and configuration. That such varied processes should have a common factor, calcium, suggests major underlying principles of calcium metabolism which have yet to be discovered.

Concentrations and patterns of released pterins of various animal cell cultures.

Neopterin, biopterin, pterin, 6-hydroxymethylpterin and isoxanthopterin were determined in the supernatants of various animal cell cultures. The different cell lines showed distinct variations in the concentrations as well as in the pattern and total amount of the released pterins. In cells from the same organ but from diverse species differences in pattern and release were found. Cell lines derived from primates had a high release of neopterin. A further difference resulted from the origin of fetal or adult organs. Neoplastic cell lines showed different patterns of pterins, dependent on the type of tumour. Primary cultures from embryonic bovine lung had the highest total amount of released pterins in all cell lines examined.

[Effect of rotary instruments on composites for coronal restoration. A proposed evaluation protocol for the effect of rotary instruments and surface conditions].

Numerous core composites are used to reconstruct abutments for fixed partial dentures. The preparation is carried out with various type of burs. The purpose of this study is to facilitate the choice among them. Six chemical composites, one glass ionomer cement and two light cured composites have been selected. Calibrated samples have been made, divided into four series and prepared with six different burs used for fixed partial dentures, three of them as finishing burs. In the first part of the investigation, the samples were examined with a S.E.M., in order to visualize the surface texture. According to the materials used, variations may exist. In the second part of the investigation, a tridimensional study was performed using a mechanical scanner microscope which permitted the quantification of some roughness characteristics, thus, an objective comparison of the surfaces became possible. Due to the complementary nature of the two investigation techniques, a protocol is presented for the study of burs instrumentation and prepared tooth surface.

[Pteridine pattern in the urine of swine of different breeds and halothane genotypes in relation to nutrition and stress].

Urinary pteridine patterns and creatinine concentrations were determined in pigs of the breeds Piétrain, Deutsche Landrasse and their crossbreds. Also the influence of the halothane genotypes, of different feeding and of treadmill exercise was examined. We conclude that creatinine concentration in pig urine is not a reliable reference parameter for the pteridine excretion. Creatinine values were significantly influenced by breed and feeding. The pteridine pattern of pigs' urine depends on the breed and correlates with other metabolic parameters. Stress is reflected by altered pteridine concentrations.

[Surface treatment of mineralized tissue: different approaches and prospects].

This paper review the effects of orthophosphoric acid and other acidic agents on dental enamel. The effects of other treatments such as proteolytic agents or of mixture able to remove the lipids are also described. Specific problems are related to the dentine treatment. Acids and chelators should be avoided, because of the dentine tubules, the odontoblasts and pulp. We review also the problem of the smear layer, the surface of the carious dentine and the consequences for dental treatment. Researches which are still in progress are reported including the use of enzymes and alcali gels submitted to feeble electrolysis, this latter being able to prepare dentine, cementum and bone surfaces in order to increase the adhesion of biomaterials.

Autometallographic visualization of glycosaminoglycans in the tongue mucosa of rats using cuprolinic blue and enzymatic digestions.

In order to visualize by light microscopy the glycosaminoglycans (GAGs) in the rat tongue mucosa, the tissue was fixed with cuprolinic-blue (CB)-aldehyde and the staining enhanced by autometallographic (AM) procedure. As other polyanions were also detected, enzymatic digestions with hyaluronidase, chondroitinase ABC and pronase were performed on these tissues in order to test the specificity of the staining. Chondroitinase ABC caused a dramatic decrease of silver grains in the lamina propria whereas hyaluronidase and pronase induced only discrete or no modification. This supported the concept that the GAGs visualized by CB and autometallography in this area as dermatan sulphate. The other polyanions (mostly DNA and RNA) seen in the epithelial layers were unaffected by these enzyme treatments.

Immunolocalization of a 110 kD molecule and a 150 kD molecule in rat incisor and mandibular bone.

In the present study, antibodies against rat dental proteoglycans were used to characterize and localize the proteoglycans in rat incisor and mandibular tissues. Polyclonal rabbit antibodies were raised against a CPC-precipitated fraction of a sulfated dental extract. In unpurified dental extract these antibodies recognized two molecules of 110 kD and 150 kD. The 150 kD molecule was susceptible to chondroitinase ABC digestion but the 110 kD molecule resisted this enzymatic degradation. Immunocytochemically these two molecules were seen to be located in the pulp, the enamel organ and the mandibular bone. In each tissue only the periphery of the cells was stained and not the intracellular compartment. In the mineralized area of bone, dentin and forming enamel no staining was seen. These results indicate common epitopes in the proteoglycans from pulp, predentin, enamel organ and bone. Some differences were found in the nature of tooth and bone proteoglycans.

[Pilot study of the relationship of free amino acids in serum and in the cerebrospinal fluid of horses].

In a blind study serum and cerebrospinal fluid (CSF) of control horses and of horses in hepatic coma after chronic food intoxication with Senecio alpinus were collected simultaneously and the composition of free amino acids was determined. The hepatic encephalopathy index in serum (less than 1.65) and in CSF (less than 1.11) of liver patients was distinctly less than to the control values in serum (greater than 2.42) and in CSF (greater than 1.49). The serum concentrations of glutamic acid in hepatic coma were elevated five-fold in comparison to the controls. An indication of ammonia decontamination was that nearly ten-fold higher values of glutamine were found in the cerebrospinal fluids of patients than in serum. In comparison to controls the serum levels of glutamine in horses with hepatic encephalopathy were decreased by the factor 0.7.

Scanning electron microscopy of the forming enamel of rat incisor: influence of fixative and treatments interacting with the organic matrix.

The forming enamel of rat incisor was either treated before fixation with one of these five treatments: ultrasonication, NaOCl, chloroform/methanol, NaOH or tween 20; or the incisors were first fixed and then submitted to one of these treatments. The structure of the forming enamel, compared with sections fixed with aldehyde and submitted to osmic maceration, depends on the way the samples were treated and on the fixative used. An amorphous material, removed by osmium tetroxide but not by aldehyde solution was seen to be located between fiber-like structures. In the region of inner enamel secretion the thickness of interrod septa was fairly constant, whereas in the region of outer enamel secretion, transverse septa of interrod enamel gradually became thinner and finally disappeared. The longitudinal septa did not display such changes. Longitudinal grooves and ridges were seen at the end of enamel completion. These longitudinal septa finally merged and formed a flat surface.

Multiple ubiquitination of calmodulin results in one polyubiquitin chain linked to calmodulin.

In the presence of Ca2+ and ATP/Mg2+ mammalian calmodulin can be covalently coupled to ubiquitin by ubiquityl calmodulin synthetase (uCaM-synthetase). Three ubiquitin derivatives 125I-CT-ubiquitin (prepared by the chloramine-T method), 125I-BH-ubiquitin (prepared by the Bolton-Hunter method) and methylated forms of ubiquitin were tested with native calmodulin. Alternatively native ubiquitin was tested with the Bolton-Hunter derivative of calmodulin (125I-BH-calmodulin). Up to three molecules of ubiquitin can be incorporated into one molecule of calmodulin. Since both native forms of ubiquitin and calmodulin are good substrates of uCaM-synthetase, ubiquitination is not a result of an altered conformation (i.e. denaturation) of either protein. With 125I-BH-calmodulin it is demonstrated that calmodulin is also present in the higher molecular weight ubiquitin conjugates. If methylated ubiquitin is employed as substrate for uCaM-synthetase only one conjugate corresponding to the mono-ubiquitination product of calmodulin is formed. This demonstrates that only a single lysine residue in calmodulin is conjugated to ubiquitin. All other higher molecular weight ubiquitin-calmodulin conjugates must therefore be composed of one molecule of calmodulin to which an oligo- or poly-ubiquitin chain is linked. Since it can be shown that the mono-ubiquitination product of calmodulin still contains ca. 1 mol trimethyllysine/mol calmodulin, the poly-ubiquitin chain is not linked to lysine 115 of calmodulin. In addition a demethylation of trimethyllysine 115 by enzymes in reticulocyte lysate or the DEAE-enriched enzyme fraction with subsequent ubiquitination at this site of calmodulin can also be excluded.
