Lung cytochrome P450-dependent benzyloxyphenoxazone debenzylase and ethoxyphenoxazone deethylase activities in total microsomal and isolated alveolar type II cells: responses to changes in assay conditions with special reference to non-linear dependence at low enzyme concentration.

1. Responses of cytochrome P450-dependent ethoxyphenoxazone deethylase (EtOPx'ase) and benzyloxyphenoxazone debenzylase (BzOPx'ase) activities to changes in assay conditions were measured in total microsomal and isolated alveolar type II (tII) cells from rats pretreated with beta-naphthoflavone. 2. Whereas microsomal EtOPx'ase activity was unaffected by storage at -80 degrees C for up to 4 months, BzOPx'ase activity began to decline after only 1 month. 3. The microsomal and type II activities were unaffected by changes in pH (7.2-8.0) or salt content. 4. The type II activities increased after sonication 2.3-2.7-fold or in the presence of 10 microM dicumarol 1.7-1.9-fold. 5. Type II BzOPx'ase was sensitive to metyrapone (MP) whereas EtOPx'ase was sensitive to alpha-naphthoflavone (ANF). I50 values for the tII activities were calculated as: 0.63 microM--MP (BzOPx'ase), 80 microM--MP (EtOPx'ase), 0.024 microM--ANF (EtOPx'ase). At the highest concentration of ANF (10 microM), 50% inhibition tII BzOPx'ase was not observed. The results were similar to those obtained with the total lung microsomal fraction. 6. Microsomal and tII BzOPx'ase activities exhibited non-linear dependence at low enzyme concentration. Linearity was restored by 0.5 mM dimyristoylphosphatidylcholine.

Hemodynamic changes in a term newborn piglet model of patent ductus arteriosus.

Patent ductus arteriosus (PDA) is a common form of congenital heart disease in full-term infants. To investigate the morbidities associated with a left to right PDA shunt, we produced a PDA in six full-term newborn piglets (less than 36 hours old) by bathing the ductus arteriosus with prostaglandin E (PGE) and infiltrating it with formalin. In five age-matched piglets, the ductus arteriosus was ligated to serve as controls. Microsphere determinations of left ventricular output (LVO) and regional blood flow (Q) were made on three consecutive days. We produced left to right shunts of 36 to 47% (mean) in the experimental piglets. The experimental piglets had a 22 to 36% increase in LVO with a one- to twofold reduction in percentage of LVO to the kidneys and carcass (p less than 0.05). Although percentage of LVO to the gastrointestinal tract was reduced (p less than 0.05), no reduction of absolute Q to the gastrointestinal tract was observed. Brain and heart Q were similar in both groups. We conclude that significant hemodynamic changes result from left to right shunting in the full-term newborn piglet with PDA. These hemodynamic changes, such as reduction in renal blood flow, are relevant information that will help explain the morbidities observed in infants with a hemodynamically significant PDA with a left to right shunt.

Lack of sex differences with the Baldwin illusion.

Responses of college students (16 men and 16 women) to the Baldwin illusion showed a significant effect for size of square but not for sex. Findings are consistent with those reported previously by Porac, Coren, Girgus, and Verde for adults and for the oldest group of children tested by Pressey and Wilson.

Platelet-activating factor-induced aggregation of rat alveolar macrophages.

Isolated rat alveolar macrophages aggregate in the presence of PAF in a dose- and cell-dependent manner. Saturation was achieved at 80 microM PAF. The response was increased linearly with cell number up to a concentration of 3 x 10(6) cells/ml, but decreased at higher cell concentrations. The stereoisomer, enantio-PAF, and the C2-acetyl hydrolyzed product, lyso-PAF, each caused aggregation of isolated rat alveolar macrophages in a manner similar to that for PAF. The PAF-induced aggregation of alveolar macrophages may be mediated through non-specific binding sites and may represent a toxic response to relatively high levels of PAF that released at localized sites.

The role of economics in the evaluation of hospice care.

The hospice movement has developed rapidly in the past decade and the range of services offered to the terminally ill has proliferated accordingly. There has, however, been little systematic evaluation of either the costs and effectiveness of hospice care as opposed to traditional services for the terminally ill; or the relative costs and outcomes of the alternative modes of provision available within the hospice sector. This paper examines the role of economic evaluation of the costs and effectiveness of hospice services in order to obtain maximum patient benefit from the limited resources available for terminal care. The potential policy implications of such research are outlined.

Tocainide-associated neutropenia and lupus-like syndrome.

Neutropenia in association with a lupus-like illness that developed after the introduction of tocainide therapy is described. The mechanism of drug-associated neutropenia and the manifestations of drug-associated lupus are briefly discussed.

Intracellular elemental concentrations in resting and secreting rat parotid glands.

Electron probe x-ray micro-analysis was used to study the elemental concentration changes that occur during pilocarpine-stimulated saliva secretion. Quantitative x-ray micro-analysis of elemental concentrations in intracellular compartments of rat parotid glands stimulated in vivo with pilocarpine showed that Na concentration was significantly increased, while K concentration was significantly reduced. The magnitude of these changes was consistent with values obtained in other tissues with the x-ray micro-analysis method, and in the same tissue with other experimental methods. Comparisons with results from studies utilizing dispersed acini suggest that acinar dispersion procedures may affect intracellular elemental concentrations. Total electrolyte concentrations in cytoplasm and secretory granules were estimated to increase on a dry-weight basis following pilocarpine stimulation. The former change is consistent with the notion of a transcellular route of salivary fluid flow, while the latter change may be important in the exocytosis of secretory granules.

Short-term effects of calcium carbonate, lactate, and gluconate on the calcium-parathyroid axis in normal elderly men and women.

We evaluated the impact of three calcium salts on the calcium-parathyroid axis in healthy elderly volunteers. Fasting subjects were administered a standardized 1-g calcium load on three occasions, using calcium carbonate, lactate, and gluconate in random sequence with 8 oz milk or orange juice as carrier. Blood and urine were collected at baseline and for 6 h following the calcium load. Each salt rapidly increased the serum-calcium concentration and urinary-calcium excretion. Response duration was shorter with gluconate than with the other salts, but peak responses were similar for all preparations. Urinary-calcium excretion was slightly lower when orange juice replaced milk, but calcemic responses were not different. Changes in immunoreactive-parathyroid hormone and renal-phosphorus handling did not differ among calcium salts. Except for a shorter duration of effect with gluconate, all three calcium salts provided similar short-term suppression of the parathyroid axis.

Menstrual function and bone mass in elite women distance runners. Endocrine and metabolic features.

Bone mass and metabolic features were studied in 17 women distance runners. Eleven of the women had secondary amenorrhea for 1 to 7 years. Six women have maintained regular menses since menarche. Both groups were matched for aerobic capacity, body fat, exercise intensity, and age of menarche. Mineral density of lumbar spine in the amenorrheic runners was lower than that in the cyclic women and age-matched controls, but higher than that in runners with secondary amenorrhea who are less physically active. Mineral density of the radius was normal in both groups. Running-related fractures were more frequent in amenorrheic women. Metabolic assessment showed no differences between groups, except that serum triiodothyronine was lower in the amenorrheic group, perhaps reflecting low calorie intake. Intense exercise may reduce the impact of amenorrhea on bone mass; however, amenorrheic runners remain at high risk for exercise-related fractures.

How long do compression bandages maintain their pressure during ambulatory treatment of varicose veins?

Compression bandages are normally applied for 6 weeks after sclerotherapy. The changes in pressure exerted by such bandages have been measured over 8 h. The results show that different surgeons applied bandages over a wide range of pressures but the range for each individual surgeon was surprisingly narrow. The pressure falls with time and approaches zero at 6--8 h of normal activity by the subject, but more slowly if the knee and ankle joints are excluded from the bandage. The initial pressures were higher when STD compression pads were used, although the rate at which the pressure fell was the same.

Control of distension of varicose veins achieved by leg bandages, as used after injection sclerotherapy.

A study was performed to determine whether the pressures routinely produced by bandaging for compression sclerotherapy of varicose veins are adequate to maintain the superfical veins almost empty of blood. The results suggest that well-applied bandages can provide sufficient support to combat the high distending pressures found in varicose veins. The large variation among different surgeons, however, indicates that any clinical assessment of compression sclerotherapy should include measurement of the pressure at which the bandages are applied.

Support for varicose veins.

A method has been devised to allow reliable comparison of different strengths and constructions of support hosiery. Five garments were evaluated for the compression they exerted on the leg and their ability to limit the distension of a model varicose vein. Stockings and tights which provide modest compression can achieve worthwhile control of vein distension.
