Analysis of soluble Entamoeba histolytica antigen using enzyme-L inked electroimmunotransfer blots.

Soluble antigens of ten strains of E. histolytica were studied by sodium dodecylsulphate-polyacrylamide gel electrophoresis (SDS-PAGE) and enzyme-linked electroimmunotransfer blots (EITB). No relations of immune replicas to virulence, geographical origin and method of cultivation (xenic or axenic culture) were found. Antigens of all ten strains tested precipitated with anti-E. histolytica human serum in the area of 30-43 kD. Antigen of HK-9 strain created in this area a characteristic pattern with all sera containing the specific anti-E. histolytica antibodies and, therefore, EITB can be used for excluding false positive results in ELISA.

Response of Kampuchean strains of Plasmodium falciparum to antimalarials: in-vivo assessment of quinine and quinine plus tetracycline; multiple drug resistance in vitro.

Forty-three patients were enlisted in the in-vivo test for sensitivity of Kampuchean strains of Plasmodium falciparum to quinine. The mean parasite density count on day 0 was 32,398 asexual parasites per microliter of blood. With a dosage of 1.5 g quinine base daily for 10 days the average parasite clearance time was 5.6 days, and the average duration of fever 3.4 days. The in-vivo test was evaluated at 7 and 10 days after the start of therapy. After 7 days only 16 patients were parasite negative by microscopic examination (S); 20 patients had an RI recrudescence, four patients responded at the RII level and three at the RIII level. Evaluating the in-vivo test at 10 days, the number of patients parasite negative increased to 18, the number of those with an RI level of resistance increased to 21, two patients gave an RII response and two had an RIII level of resistance. Twenty-two adult males were included in an in-vivo test of the sensitivity of P. falciparum to quinine plus tetracycline. The course of treatment was: quinine 3 x 500 mg daily for 10 days, tetracycline 3 x 500 mg for 7 days. Parasite density counts on day 0 averaged 11,393 asexual parasites per microliter of blood. The average parasite clearance time was 5.9 days, and the average duration of fever was 3.8 days. After 7 days of treatment, 81.8% of patients were parasite negative, while one patient had a recrudescence after 17 days (RI). Three infections were resistant at the RII level. By prolongation of the observations to day 10, the parasitaemia was cleared in all patients, i.e. all infections were sensitive to quinine plus tetracycline. Thirty-four patients with falciparum malaria were screened for in-vitro resistance to chloroquine, mefloquine and quinine using the WHO standard micro-test.(ABSTRACT TRUNCATED AT 250 WORDS)

Entamoeba polecki: morphology, immunology, antigen study and clinic of the first infections in Czechoslovakia.

Entamoeba polecki Prowazek, 1912 was recorded for the first time in Czechoslovakia in two students from Kampuchea. Uninucleated cysts of diameter 14.2-15.7 micron with nuclei diameter 3.2-4.2 micron were found-repeatedly in stool samples taken from them. The nucleus accounted in average for 24.3% of the cyst diameter. The isolation and two following subinoculations on Dobell-Leidlaw medium were achieved, more abundant growth was recorded on medium with pig serum. Sera of both students were positive in serological tests using the E. histolytica antigen. No serious clinical symptoms were observed, both patients were cured successfully by metronidazole and ornidazole. Electroimmunotransfer blots were used to characterize E. polecki as a separate species. The antigenic structure of polyxenically grown E. polecki was compared with the antigenic structure of E. histolytica (axenically grown HK-9 strain and 3 polyxenically grown strains). Different blot patterns of both species were obtained, but common fractions of 30-40 kD probably responsible for serological cross-reactions were found.

[The morphology of the eggs of Paragonimus westermani (Trematoda)].

The entire shape, surface structure and the details in operculum of the eggs of Paragonimus westermani imported to Czechoslovakia by one Korean and three Vietnamese were examined by light and scanning electron microscopy with emphasis on differential diagnostics. Differences distinguishing eggs of P. westermani from other species of this fluke parasitizing in man are briefly discussed.

Malaria in Kampuchea: clinical course of falciparum malaria in Chemin de Fer Hospital, Phnom Penh.

To receive actual information about the clinical course of falciparum malaria case history of 373 patients hospitalized in Chemin de Fer Hospital in Phnom Penh from May 1 till October 1, 1985 were evaluated. No patients were infected in Phnom Penh. We estimated that only 8.3% of patients had higher parasitaemia than 100,000 or more asexual parasites in microliter of peripheral blood which is considered as a heavy infection. Complicated malaria was found in 39 patients (10.4%). The most frequent complications were cerebral complications (80.0%), renal failure (23.3%), and liver failure (16.6%). Ten patients had multiple organ complications and they represented 25.6% of patients with complications. The 100% mortality was observed in those in coma stage III; with haemoglobinuria, epistaxis and melena. Only coma stage III as a single symptom caused the death. In other cases death resulted from multiple organ complications.

Histopathological diagnostics of liver amoebiasis in Cambodia.

Results of histopathological examination of a solitary chronical amoebic abscess in the liver of a patient from Cambodia are described. The abscess was at a proliferously reparative phase and Entamoeba histolytica was detected at the border between the necrosis and inner wall of abscess, as well as in the proper non-specific granulation tissue of the inner layers of abscess capsule. A differential diagnosis of the amoebic abscess from similar parasitary or pseudoparasitary liver lesions and differential diagnosis of amoebae in histological sections are given. For orientation examinations of the liver tissue for the presence of amoebae the authors recommend the impregnation after Grocott and staining with Goldner's trichrome for a more detailed evaluation of histological sections.

Malaria in the province of Takeo, Cambodia.

Malaria was studied in the province of Takeo, Cambodia. In the hyperendemic region of Kirivong district, 49 (46.0%) of 105 suspected patients were found to be infected, 98% of them with Plasmodium falciparum and 2.0% with P. vivax. The highest prevalence (85.7%) was recorded in the group of 15-20-year-old patients. A total of 296 patients were examined in the hospital of Takeo during one year (1983-1984) and 77 (26.0%) of them were positive. P. falciparum infection was found in 76.6% and P. vivax in 23.4% of cases. The highest prevalence (42.3%) was also in the age-group of 15-20 years. The "7-day test" was used in vivo in 15 patients in order to detect the sensitivity of P. falciparum to Fansidar. The asexual forms of parasites disappeared within 4 days, while the gametocytes survived in two patients until 7th and 8th day, respectively. The observations could not be terminated, since the two patients will fully left the hospital.

Intestinal parasites in Kampuchea, Takeo Province.

Fresh stool samples obtained from 1407 adult patients who sought treatment in Takeo province hospital and 332 Takeo preschool and school-age children from 3 to 18 years of age were examined for the presence of intestinal parasites using the technique of native preparation and the flotation method of Faust with subsequent staining with Lugol solution to demonstrate cysts. In hospital patients, a total of 13 protozoan and 9 helminth species were diagnosed. The prevalence of Entamoeba histolytica (cysts and trophozoites) was highest in the age group 15-18 years (18.3%), the peak prevalence of Gairdia lamblia (27.6%) occurred in children of the age group 6-9 years. The highest frequency distribution of Pentatrichomonas hominis (20.1%) was recorded in 3 to 5 years old, that of Enteromonas hominis (12.8%) in 6 to 9 years old. The predominant helminth was Ancylostoma duodenale, with the peak prevalence (65.2%) in patients older than 18 years, followed by Ascaris lumbricoides and Strongyloides stercoralis. Almost half of children patients under 6 was infected with at least two species of parasites, patients over 6 were infected simultaneously with two or more intestinal parasites in an absolute majority of cases. In Takeo preschool and school children the spectrum of diagnosed protozoan and helminth species was somewhat narrower than seen in hospital patients, but their prevalence rates were higher, except for the flagellate Pentatrichomonas hominis. The highest prevalence rates recorded were for E. histolytica 29.5% (age category 10-14 years), for G. lamblia 34.8% (age category 6-9 years), for P. hominis 19.3% (age category 3-5 years), for E. hominis 10.5% (age category 3-5 years), for A. duodenale 85.9% (age category 15-18 years), for A. lumbricoides 26.1% (age category 6-9 years), and for S. stercoralis 18.8% (age category 6-9 years). As many as 70% of children at the age between 6 and 15 years were simultaneously infected with two or three species of intestinal parasites.

Giardiasis and ABO blood groups.

The relationship between giardiasis and ABO blood groups was studied in a group of 136 children from Eastern Slovakia, with a special attention to ethnic specifics of gipsy children. The study showed no statistically significant differences in the distribution of ABO blood groups between the subgroup of Giardia-infected gipsy children and the gipsy population minority living in the area. On the other hand, the ABO distribution pattern in subgroup of non-gipsy children infected with Giardia intestinalis was found to differ from that in the general non-gipsy Eastern Slovakian population by the absence of blood group AB among children with giardiasis. There was indicated a tendency towards an increased frequency of blood group B in subgroup of Giardia-infected gipsy children and towards group A in subgroup of nongipsy children with giardiasis, but the differences were not statistically significant.

Detection of Giardia intestinalis in duodenal aspirate and in the stool.

Compared was the detection of Giardia intestinalis in duodenal contents and in the stool. During endoscopic examination duodenal contents were recovered by means of a tube attached to the mucosa and by aspiration from the region of pars verticalis. In 102 persons, duodenal and stool examinations revealed four and five cases of Giardia infection, respectively. The results showed that both diagnostic methods are equivalent and demonstrated that parasitologic examinations of patients suffering from dyspeptic disorders are very useful.
