Surgical treatment of recurrent and stomal peptic ulcers.

Twenty-three patients with endoscopically proved recurrences after a primary operation for peptic ulcer disease performed in the course of a 7-year period are reviewed. The time elapsed between the operation and the recurrence ranged from 1 year to 17 years. The patients were mainly males. During the period studied 451 primary operations for peptic ulcer disease were performed in the same department. In 12 out of 23 recurrences the primary operation was also performed in the department of the author. In cases of small duodenal or stomal ulcer with two or more previous laparotomies transthoracic truncal vagotomy is preferred to any kind of abdominal intervention. A useful combination of duodenal exclusion, truncal vagotomy and a reflux-preventing type of GEA for recurrences after B 1 type partial gastrectomies was developed. In cases of late recurrences with mild symptoms the patients have to be first treated with H2-receptor blockers (cimetidine, ranitidine, etc.). The cases resistant to medical management have to be considered for surgery. Their results after a secondary surgical intervention proved to be fairly good, probably due to methods adequately selected for the given patient and to reflux-preventing GEA used in some cases. The consideration of an optimal individual solution for each patient is emphasized.

[Transmission of Marek's disease to wild feathered game].

The following wild feathered game were tested from 6 to 12 months of age: common pheasant (Phasianus colchicus), common gray partridge (Perdix perdix), European quail (Coturnix coturnix), and Greek partridge (Alectoris graeca). Only pheasant was found to be susceptible to infection with a virulent virus of Marek's disease. In this bird the first clinical symptoms of the disease (nervous changes) were observed in 28 days from infection. Immunoprecipitation antibodies to the Marek's disease virus occurred in the serum on the 75th day from infection, and patho-morphological changes, typical of acute Marek's disease, were observed on the 75th to 85th day from infection. The remaining species of wild feathered game appeared not to be susceptible to infection with the Marek's disease virus. In these birds, like in pheasants, immunoprecipitation antigens of the Marek's disease virus were not found to be present in the epithelial cells of feather follicles. In the infected Greek partridge, common gray partridge and quail, serum was not found to contain any antibodies. Laying quail had no antibodies in the yolk of the eggs they laid. The results indicate a different oncogenesis of Marek's disease in the domestic fowl and pheasant. It is concluded that wild feathered game play no important role in the epizootology of Marek's disease.

[Extensive intestinal necrosis without obstruction caused by so-called low flow state].

Medical history of a 36 year old woman with severe combined valvular deformities and frequent failure of the circulation inspite of the treatment with digitalis is reported. Impairment of the circulation gradually lead to the so called "low flow state" which resulted in infarction of the small intestine without vascular occlusion. Inspite of the urgent removal of the necrotized segment of the intestine in an extent of 4 metres, the patient died. In connection with the case reported pathologic processes resulting in infarction of the small intestine are discussed. Emphasis is made on the reported less frequently encountered factor and on the effect of digitalis on the circulation of visceral organs.
