[Treatment of urinary and intestinal bilharziosis with praziquantel. Apropos of 182 cases].

182 patients representing 188 cases of schistosomiasis (including 152 S. haematobium, 34 S. mansoni, and 2 S. intercalatum cases) were treated with praziquantel using 40 mg/kg in a single intake. Biological check-up did not reveal any abnormality that could have been related to the treatment. Clinical side-effects appeared in 25% cases and were essentially gastrointestinal symptoms, headaches, dizziness. Efficiency was assessed by direct parasitological examinations of the feces or urine that were performed after treatment. It reached 91% for S. haematobium and 95% for S. mansoni schistosomiasis.

[Early syphilitic hepatitis. Two cases].

Two patients are presented as cases of secondary syphilitic hepatitis. Clinical and biological signs coincided with usual description of this affection. Syphilitic serological tests were all very positive. These two patients were homosexual men. In one case, we have found treponema in hepatic biopsy, underelectron microscope only, which confirmed the diagnosis. Evolution of both patients was improved by anti-syphilitic penicillin the rapy. Thus, this is an important diagnosis, which should not missed, because of the good prognosis of this affection with specific treatment, and because of the increasing frequency of syphilis for 20 years.

High titres of anti-T antibodies and other haemagglutinins in human malaria.

The prevalence of antibodies against (i) human red blood cells (RBC) of A and B groups, (ii) trypsinized O Rh+ RBC and (iii) neuraminidase treated O Rh+ RBC were investigated both in sera of Africans from a malaria endemic area of Upper Volta and in sera of Europeans with acute malaria from a Paris hospital. An increased frequency of high titres of haemagglutinins was observed against A and B as well as O Rh+ trypsinized human RBC, thus confirming previously published results. In addition, agglutination of neuraminidase treated RBC showed that the titres were increased in about 40% of Africans studied and in about 80% of patients with acute malaria. Using agglutination with a specific anti-T lectin and inhibition with two ligands, it was found that sera of malarious patients contain high titres of antibodies directed against the T antigen of neuraminidase treated RBC. The mechanisms of appearance of high titres of autohaemagglutinins in malaria and their possible interference in the anaemia associated with this disease are discussed.

[Dracunculus medinensis infestation following holidays in West Africa. Epidemiological implications (author's transl)].

All 50 patients with dracunculiasis admitted to a Paris hospital during a 3-year period were West African immigrant workers, 70% of whom came from a very limited geographical area. Only 6 of the workers were "first arrival"; the others had contracted filariasis during holidays in their home country. The mean duration of stay in hospital was 40 days. In 40 of the patients who had spent 6 months or less in their home country, the exact dates of first arrival in France, departure to, and return from, Africa and emergence of adult worms were available, so that the duration of the Dracunculus medinensis cycle (mean: 12 +/- 1,7 months) and the period of local transmission could be determined. These epidemiological data account for the seasonal character of infestation and for the occurrence of dracunculiasis in France. They also exclude a diagnosis of filariasis based on more or less suggestive symptoms when the delays after possible infestation are either too long or too short.

Litomosoides carinii infection in cotton-rats: evolution of microfilaraemia before and after treatment with diethylcarbamazine and suramin.

The activity of diethylcarbamazine (DEC) and suramin (S) on Litomosoides carinii filariasis was assessed by the study of microfilaraemia in 30 cotton-rats (Sigmodon hispidus). DEC (per os) and S (injected subcutaneously) were administered alone or in combination, at doses of 50 mg/kg/day for five consecutive days. 15 rats were treated at day 91 after infection and 15 others at day 184. The immediate but temporary efficacy observed with DEC was in contrast to the delayed and long-lasting effect of S. With combined therapy there was neither antagonism nor synergism. The demonstration of an apparently smaller reduction of parasitaemia in the rats treated with DEC before the plateau phase of the microfilaraemia was consistent with the lack of action of this drug on microfilariae outside the blood system.

Imported malaria in a hospital in Paris.

The number of cases of imported malaria diagnosed by the Parasitology Laboratory of the Pitié-Salpêtrière Hospital Group in Paris has shown a significant increase in recent years. From 1970 to 1979, 443 cases were diagnosed and the annual number recorded during this period rose from seven to 93 cases. This is causing anxiety since Plasmodium falciparum is the principal species observed, affecting two out of three patients. The principal epidemiological characters of malaria imported into France, the diagnostic difficulties and the therapeutic schedules are shown in a study of 192 case histories of patients observed by the authors.

Blood group active substances in Paragonimus westermani.

Lyophilized crude extracts of Paragonimus westermani were investigated for the presence of human blood group active substances, using an automated haemagglutination inhibition technique. This sensitive method allowed the detection of A, B, H, Lewis (a), Lewis (b) and P1 blood group specificities, in the fluke's extracts. No inhibition was observed with C, c, D, E, e, K, Kpa, Kpb, Fya, Fyb, Jka and Jkb blood group antibodies.

Comparative automated assay of anti-P1 antibodies in acute hepatic distomiasis (fascioliasis) and in hydatidosis.

An automated assay of anti-P1 allohemagglutinins has been carried out on 133 P2 subjects: 13 with acute hepatic distomiasis (Fascioliasis), 20 with hydatidosis and 100 healthy blood donors. Anti-P1 were detected in 100% of the distomatid sera, in 50% of the hydatid and in 29% of the healthy individuals. When compared to a reference serum test (anti-P1 standard), their concentration was found to be weak in healthy subjects, moderate in hydatidosis and exceptionally increased in acute distomiasis (up to 5-6 times the standard anti-P1 level). The 2-mercaptoethanol treatment showed that even the increased distomatid anti-P1 sera were of an IgM nature. The respective origin, synthesis and strength of these allohemagglutinins are discussed and the advantages of using distomatid IgM anti-P1 as human sera test are emphasized.
