Prediction of blood cyclosporine concentrations in haematological patients with multidrug resistance by one-, two- and three-compartment models using Bayesian and non-linear least squares methods.

The blood cyclosporine (CsA) concentration-time profile in each of 24 adult haematological patients with multidrug resistance taking the first course of CsA treatment was fitted by one-, two- and three-compartment models to obtain relevant pharmacokinetic parameters. The pharmacokinetic parameters obtained were implemented into the PKS program (Abbottbase Pharmacokinetic System) as the population pharmacokinetic parameters used to predict blood CsA concentrations in adult haematological patients with multidrug resistance. The predictions of blood CsA concentrations by one-, two- and three-compartment models using the Bayesian method (BM) and the non-linear least squares method (NLLSM) were evaluated employing 11 patients who took the second course of CsA treatment. While the Akaike's information criterion (AIC) favoured the two-compartment model to describe CsA concentration-time profiles in patients taking the first and second courses of CsA treatment, the predictive performance analyses showed that both two- and three-compartment models were better than the one-compartment model for prediction, but the three-compartment model was slightly superior to the two-compartment model. The results also show that the predictions using BM were slightly better than those using NLLSM. Several factors affecting BM predictions and the possible difference among AIC, BM and predictive performance analyses were also addressed.

The teaching of pharmacology in Italian medical schools: the point of view of Italian doctors.

OBJECTIVE: A survey was carried out to gather the opinions of doctors about the current method of teaching pharmacology in Italy. METHODS: A questionnaire was mailed to 3860 doctors, who were asked a series of questions regarding the teaching of pharmacology to medical students in Italian universities. RESULTS AND CONCLUSIONS: The great majority of those who replied considered the teaching they received to be mainly theoretical. The doctors thought that much more time and attention should be dedicated to those pharmacology subjects that are more closely connected to physiopathology and clinical practice (clinical pharmacology).

Steady-state serum concentrations of imipramine, its main metabolites and clinical response in primary enuresis.

Thirty-seven children (6-13 years old), receiving a flexible dosage of imipramine (IMI) for nocturnal enuresis, were evaluated. After a mean time of 8.5 +/- 7.0 weeks of therapy, 40.5% no longer wet the bed; 32.4% had a mean benefit of 80%; 27.01% had a negligible response. The best relationship observed was between clinical effect and drug serum concentrations rather than with drug daily dose, the most satisfactory being that with IMI seric values (P = 0.019). Responders (effect higher than 50%) had higher IMI serum concentrations (P less than 0.05) than poor responders. At 3 and 6 months after stopping the drug, over 90% of the responders maintained the maximum response reached during treatment. The side-effects observed were irritability, reduction of appetite, headache, a mild increase of blood pressure.

The pharmacokinetics of tricyclic antidepressant drugs in the elderly.

Antidepressants, especially tricyclic agents (TCAs), are increasingly used in geriatric patients since depression is a common mood disorder in the elderly and the size of elderly population is increasing. Notwithstanding the importance of kinetics to better use of drugs, its study in the elderly (regarding TCAs) is not sufficiently developed. The present paper briefly reviews the available data on amitriptyline, nortriptyline, protriptyline, imipramine, desipramine and clomipramine kinetics in the elderly.

[Clinical evaluation of clotiazepam in the ambulatory treatment of anxiety states].

To assess the therapeutic efficacy and tolerability of Clotiazepam in the treatment of anxiety, the drug was administered to 20 subjects suffering from anxiety (12 cases) or from nervous depression (8 cases), all of whom were without psychopharmacological drugs for at least one month prior to the trial. Treatment with Clotiazepam improved the anxiety and brought about a statistically significant reduction in both groups of patients regards the total score in the Hamilton Scale and in the two sub-scale scores (somatic and psychological symptoms). Tolerability to the drug was very satisfactory.
