The usefulness of bone-marrow scintigraphy in the detection of bone metastasis from prostatic cancer.

We used a combination of bone and bone-marrow scintigraphy to study 25 patients with prostatic cancer. Of the 18 cases whose 99mTc-methylene diphosphonate (MDP) bone scans showed hot spots in the lower lumbar region of the spine and/or the pelvic bone, 8 had normal bone-marrow scintigrams. These 8 patients were subsequently shown to have senile, degenerative changes of the spine. On the other hand, in 9 of the 10 patients whose bone-marrow scintigrams showed accumulation defects, follow-up study and characteristic X-ray findings confirmed the presence of metastases. In all 6 cases with extensive bone metastases shown by 99mTc-MDP bone scintigraphy, 99mTc-sulphur-colloid bone-marrow scintigraphy showed multiple accumulation defects. In conclusion, bone-marrow scintigraphy was found to be useful in distinguishing metastatic lesions from benign degenerative changes in the cases with suspected bone involvement, as well as in evaluating equivocal lesions in the pelvis.

A case of synovial sarcoma with bone metastasis identified by bone marrow scintigraphy.

In a patient with synovial sarcoma, routine bone survey showed no abnormality, while bone marrow scintigraphy with Tc-99m sulfur colloid revealed a defect in the fifth lumbar vertebra. At surgery, tumorous invasion was noted in the fifth lumbar vertebra and the surrounding tissues. It was suggested that the bone marrow scintigraphy was particularly useful in the detection of tumorous invasion into the bone marrow at the early stage before the destruction of skeletal tissue.

Moyamoya-like diseases associated with ventricular hemorrhages: report of three cases.

Three rare cases of moyamoya-like diseases with moyamoya type vessels caused by spontaneous internal carotid artery occlusion, spontaneous middle cerebral artery occlusion, and internal carotid artery occlusion due to cervical irradiation are presented. They resulted in ventricular hemorrhages. One patient died and two survived. Postoperatively, the collateral circulation of the survivors was evaluated by single photon emission tomography using N-isopropyl-[123I]-p-iodoamphetamine. The effectiveness of reconstructive surgery is shown, and moyamoya-like diseases that have been reported are reviewed.

Metastatic medullary thyroid cancer: localization with iodine-131 metaiodobenzylguanidine.

A patient in whom metastatic medullary thyroid cancer was diagnosed underwent a scintigraphic examination using [131I]MIBG. Multiple hot lesions and diffuse hepatic uptake were noted corresponding to bone and liver metastases. Iodine-131 MIBG may prove to be useful for scintigraphic localization and for the treatment of medullary thyroid cancer as in pheochromocytoma and neuroblastoma.

[Clinical study on the measurement of squamous cell carcinoma (SCC)--related antigen in SCC].

By using a RIA Kit for squamous cell carcinoma (SCC) antigen, extracted and purified this substance from liver metastasis of SCC of the uterine cervix, a fundamental and clinical study has been done. Based on the fundamental study, it was shown that this kit was useful clinically. High concentrations of serum SCC related antigen were observed in squamous cell carcinoma of the lung, head and neck, and uterus. It was recognized that the measurement of serum SCC related antigen was useful in not only diagnosis, but also in the evaluation of effectiveness of therapy or prediction of recurrence. In particular, as there is a lack of reliable tumor markers in head and neck tumors, good results can be anticipated from the measurement of SCC related antigen in such cases.

[Clinical study of the measurement of tissue polypeptide antigen in patients with breast cancer].

Tissue polypeptide antigen (TPA) concentrations in patients with breast cancer were measured using a radioimmunoassay (RIA) kit. Although this RIA kit presented some problems in the dilution test of high-concentration samples, it was found to be adequate for clinical use. It was shown that the measurement of serum TPA concentration was useful in monitoring the local recurrence, for the early diagnosis of distant metastasis and for evaluating the therapeutical effect after surgery. TPA concentration showed no correlation with CEA concentration. Therefore, in order to raise the rate of diagnostic success, simultaneous measurement of both TPA and CEA was essential.

[Mutagenesis of amino and/or methyl analogs of acridine in Salmonella and yeast: a comparison between the spot-test and the pre-incubation methods].

The relationship between mutagenic activities and chemical structure of acridine derivatives was examined by using Salmonella typhimurium strains TA 1537 and TA 1977 and yeast Saccharomyces cerevisiae. Most analogs with amino and/or methyl group(s) caused frameshift-type mutation in Salmonella without mammalian microsomal enzyme activation. The 9-amino analogs were strong mutagens and mutagenicity was also increased when 10-position was methylated in 1-amino and 2-amino compounds. However, 9-amino derivatives did not cause mitochondrial mutation in yeast, where 3,6-diamino and/or 10-methyl groups were structural requisites for significant mutagenic activity. In comparison with the pre-incubation method, the spot-test method was shown to be less sensitive. The mutagenicity of some compounds could not be detected by the spot-test method. Mutagenic activities of these drugs were revealed and increased markedly with an increase in pre-incubation period up to 20 min, suggesting that the pre-incubation of tester bacteria with a compound prior to a mutagenicity assay is necessary for the detection of mutagenesis of these compounds.

Two- and three-dimensional ultrastructural observations of angiogenesis in juvenile hemangioma.

Two- and three-dimensional electron microscopic observations by a serial sectioning method revealed endothelial sprouts with intracytoplasmic vacuolization in rapidly growing human juvenile hemangioma. A large vacuole bounded by a single unit membrane was enclosed in the cytoplasm and on the inner aspect of the vacuolar membrane several short microvilli were demonstrated. These appearances have not been reported before. The presence of microvilli in the vacuole indicates that the endothelium has reached the point of differentiation when a vascular lumen forms. In the cytoplasm adjacent to the vacuolar membrane, a significant number of 7 to 10 nm microfilaments were identified. These intracytoplasmic microfilaments are assumed to play a mechanical role in the development of the cytoplasmic vacuole and/or the sprout. The formation of a vacuole observed in the endothelial sprout is similar to the findings of Sabin (1920) by light microscopy in endothelial sprouts in the blood island of the chick embryo. The active endothelial sprout in juvenile hemangiomas is considered to be at least partially responsible for the capillary proliferation and enlargement of the tumor.

Petite induction in Saccharomyces cerevisiae by ethidium analogs. Action on mitochondrial genome.

Petite induction of ethidium analogs was examined in both resting and growing yeast cells. All of the analogs used in these experiments were active in dividing cells of Saccharomyces cerevisiae; only the parent ethidium bromide was mutagenic under resting conditions. Incorporation of adenine into mitochondrial DNA appeared to be prevented completely by ethidium and partially inhibited by other analogs. Treatment of growing cells with analogs affected fragmentation of pre-existing DNA as seen by the loss of a mitochondrial antibiotic resistance marker. The rates of elimination of the marker were different; ethidium generated greater loss than the monoamino analogs (3-amino and 8-amino-); and the deaminated analog was least effective. However, in resting yeast the marker was partially eliminated only with treatment of the parent ethidium. The degradation of the mitochondrial DNA by exposure to ethidium compounds was confirmed by agarose gel electrophoresis. Electrophoretic patterns of the mitochondrial DNA treated with each of the analogs under growing conditions and only with ethidium under resting conditions showed degradation of the mitochondrial DNA.

Production of frameshift mutations in Salmonella by phenanthridinium derivatives: enzymatic activation and photoaffinity labeling.

The effect of metabolic activation on the mutagenic potential of some phenanthridinium compounds was examined in Salmonella typhimurium strains TA1538 and TA1978 . All of the compounds tested were mutagenic in TA1538, a DNA excision-repair-deficient strain, when metabolizing enzymes were included in the assay. Reversions were not detected when these compounds were examined under the same conditions in TA1978 , the isogenic strain of TA1538 proficient in DNA repair. The mutagenic activity of an azido analog of propidium iodide was also examined using photoactivation and enzymatic activation, and with both conditions, reversions were observed in TA1538 but not in TA1978 . Furthermore, the ranking of mutagenic activity of propidium azide relative to ethidium azide analogs was comparable for both types of activation. The evidence from several studies suggests that the structural requirements for mutagenic activity for this series of phenanthridinium compounds appear to be the same whether mutagenesis is induced via photoactivation or metabolic activation. The interaction with DNA resulting in covalent alteration of the DNA is implicated as the mutagenic mechanism whether the active species is generated by metabolic- or photo-activation.

Liposome entrapment enhances the hypocalcemic action of parenterally administered calcitonin.

The hypocalcemic effect of liposomal-entrapped calcitonin (CT) was evaluated in rats. Salmon CT and human CT were entrapped in liposomes composed of egg phosphatidylcholine with or without an equimolar amount of cholesterol. The liposomes were separated by Sepharose 4B chromatography into fractions consisting of large multilamellar vesicles and small unilamellar vesicles. The incorporation of CT was monitored by counting [125I]CT and by specific RIA. Liposomal entrapment enhanced the hypocalcemic potency of parenterally administered salmon CT and human CT. After iv administration, the large multilamellar vesicles were more potent than small unilamellar vesicles in their hypocalcemic effect; cholesterol inclusion in the MLV liposome preparation prolonged the hypocalcemia. However, with im administration, the cholesterol-free liposomes were more potent than their cholesterol-containing counterparts regardless of size. These studies demonstrate that liposomal entrapment can be used to enhance the hypocalcemic potency of CT. It appears that both the size and composition of the liposome preparation are important in this effect, as is the route of administration. It may be possible to produce liposome-CT preparations with advantageous pharmacological characteristics.
