Pancreatic arteriovenous malformation in a patient with gastrointestinal hemorrhage.

A 39-year old woman, with a pancreatic arteriovenous malformation (AVM) which ruptured into the pancreatic duct, was treated successfully with distal pancreatectomy and splenectomy. Histological examinations revealed an AVM with extravasation into the pancreatic ducts. A review of the cases reported to date emphasizes the importance of physical signs, individual histories and angiographic studies for early diagnosis and surgery.

[Activity of vertebrate collagenase in the human ovary (author's transl)].

Participation of collagenolytic activities in the ovulatory process had been recently studied in the experimental animals, though their existence in the human ovary remains obscure. In the previous report, we demonstrated the collagenolytic activity by using synthetic substrates N-carbobenzoxy-Gly-Pro-Gly-Gly-Pro-Ala and the cathepsin B1 activity in human ovarian follicles. To investigate the activity for degradation of native collagen in the human ovary, we used 14C-labeled collagen prepared from young rat skin in the present study. Collagenolytic activity for synthetic substrate DNP-Pro-Gln-Gly-Ile-Ala-Gly-Gln-D-Arg OH supposed to be specific for vertebrate collagenase was also measured. Our results clearly demonstrated that there existed vertebrate collagenase activity and the collagenolytic activity measured by using DNP-peptide in the human ovarian follicles. The correlation coefficient between both enzymatic activities was 0.56. The DNP-peptide-degrading activity seemed to partly represent the collagenolytic activity from vertebrate collagenase. It was concluded that vertebrate collagenase besides collagenolytic activities did exist in the human ovary and they seemed to be involved in the degradation of collagen fibers in the follicular apex at the time of ovulation.

Acrosome reaction in ascidians induced by a calcium ionophore.

Ca ionophore A23187 induces an acrosome reaction in Ciona intestinalis and Halocynthia roretzi spermatozoa. This process occurs by the fusion of the acrosomal outer membrane along its peripheral margin with the plasma membrane overlying the acrosome. The fusion process leads to the exocytosis of the acrosomal contents and results in the formation of a vesicle bounded by membrane made up of the acrosomal outer membrane and the overlying plasma membrane that is eventually shed. The acrosomal inner membrane is exposed and becomes a part of the plasma membrane enclosing the anterior region of the sperm head. The chemical nature and the precise role of the acrosomal contents at fertilization must await further studies.

Damped oscillation analysis of natural and artificial periodontal membranes.

The mechanical mobility of human teeth was studied by using a wave data treatment system with an accelerometer attached to a personal computer. The healthy teeth in different generations of men and women, who ranged in age from their teens to their forties, showed similar damped oscillation patterns. The spring constant and coefficient of viscous damping were estimated by using a viscoelastic model of damped oscillation with 1 degree of freedom. The coefficient of viscous damping increased dramatically with increases in the damping factor, whereas the spring constant scarcely varied. Silicone membranes were used as model periodontal membranes. The coefficient of viscous damping decreased with increases in silicone membrane thickness. Collagen membranes, which are expected to function as artificial periodontal membranes, were adhered to the model tooth by drying collagen gel and fixed with apatite-collagen composites. The collagen membranes showed damped oscillation wave patterns similar to those of human periodontal membranes.

Evaluation of the intratumoral vasculature of hepatocellular carcinoma by power doppler sonography: advantages and disadvantages versus conventional color doppler sonography.

BACKGROUND: To determine whether a difference exists in the relative ability of power Doppler sonography and conventional color Doppler sonography to detect the intratumoral vasculature of hepatocellular carcinoma based on lesion size and location. METHODS: Sixty patients with 88 hepatocellular carcinoma lesions that showed tumor staining on angiography and were enhanced on dynamic computed tomography were evaluated. Power Doppler sonography and color Doppler sonography were used to detect the intratumoral vasculature, and their sensitivity to blood flow was evaluated. RESULTS: Power Doppler sonography showed a superior detection rate for lesions smaller than 2 cm and located 4-8 cm from the abdominal surface in the right hepatic lobe as compared with color Doppler sonography (p < 0.01). Neither power Doppler sonography nor color Doppler sonography depicted the intratumoral vasculature of lesions located more than 8 cm from the abdominal surface (n = 14). Both color Doppler imagings exhibited a low detection rate for lesions in the left hepatic lobe (n = 31, p < 0.01). CONCLUSIONS: Power Doppler sonography should be applied in the evaluation of small or intermediate depth lesions because it is more sensitive to these lesions than color Doppler sonography, but it is not useful for left lobe and deep lesions.

Patient-administered nitrous oxide/oxygen inhalation provides effective sedation and analgesia for colonoscopy.

In a double-blind, randomized, placebo-controlled study of patients undergoing colonoscopy, sedation with an inhaled mixture of nitrous oxide/oxygen was compared with conventional intravenous sedation (pethidine 50 mg, midazolam 2.5 mg). In the patients studied, no significant differences were noted in number of pain episodes, need for additional intravenous sedation, or patient pain scores between the group receiving the nitrous oxide/oxygen mixture (n = 30) and those managed with conventional benzodiazepine/opiate injection (n = 29). Both methods were significantly more effective than placebo (n = 30). Six patients in the benzodiazepine/opiate group had oxygen desaturation, whereas none did in the nitrous oxide/oxygen group. Duration of stay after the procedure was significantly shorter in the gas inhalation group than in those receiving conventional intravenous sedation. Except for patients with severe chronic obstructive pulmonary disease, nitrous oxide/oxygen inhalation is a safe and acceptable alternative method of sedation and analgesia during colonoscopy.

The fate of homologous nasal septal cartilages in tympanoplasty.

We have used homologous nasal septal cartilage for tympanoplasty for the last 8 years and obtained satisfactory results. In order to demonstrate the fate of homograft cartilage implanted into the middle ear, mucopolysaccharides have been studied by means of enzyme digestion. The matrix of normal septal cartilage was divided into three regions: 1) pericellular region; chondroitin sulfate B, 2) distal interstitial region; hyaluronic acid and chondroitin sulfates, 3) peripheral interstitial region; collagen. In preserved cartilage, chondroitin sulfate B was lacked out, but hyaluronic acid and collagen remained intact though the amount of mucopolysaccharides diminished slightly when compared with normal septal cartilage. Homograft cartilages evidenced depletion of mucopolysaccharides. Homograft cartilages should be used for the purposes mentioned, though not as material for columella, nor for reconstruction of large bone defect.

Conservation treatment intensified with tamoxifen and CAF chemotherapy for subareolar breast cancers.

Formerly, patients with subareolar breast cancers have not been good candidates for breast-conservation treatment (BCT) in Japan. However, recently it was reported that patients with subareolar cancers were good candidates for this treatment. In order to improve both cosmetic results and survival rates, we performed BCT for 27 patients with subareolar breast cancers following pre-operative CAF chemotherapy and tamoxifen administration. From August 1989 to June 1998, we performed BCT for 200 out of 206 patients who visited our hospital with the desire of breast-conservation. Of these patients, 27 presented with tumors within 2 cm of the nipple areolar complex. For 26 of these 27 patients, breast-conserving surgery was performed following two to four times of CAF chemotherapy, and another one patient did not undertake any surgical resection. Following the surgery, patients were treated with radiation therapy to the intact breast and ipsilateral axilla to a total dose of 4,400 cGy with electron conedown to a total dose of 5,300 cGy. Their primary tumors were at T4bN0 in 1 case, T2N1b in 5 cases, T2N0 in 18 cases, and T1cN0 in 3 cases. Tamoxifen was administered to all the patients. CAF chemotherapy was performed six times for stage I patients, and eight times for stage II patients in total. For one patient with direct tumor invasion to the nipple, intraarterial infusion chemotherapy was performed following radiation therapy and CAF chemotherapy, without any surgical resection. For another 26 patients, breast-conserving surgery was performed, and 5 of them underwent lumpectomy under local anesthesia on an outpatient basis. For these 26 patients, the microscopic surgical margin was positive for 8 patients, close for 6 patients, negative for 10 patients, and unknown for 2 patients. All patients are alive with no evidence of disease after a follow-up of approximately 47 months on average, and all of their cosmetic results are estimated as excellent or good. It is concluded that BCT intensified with preoperative CAF chemotherapy and tamoxifen for subareolar breast cancers is a satisfying treatment modality in terms of both cosmetic results and survival rates.

Unrecognized hepatic steatosis and non-alcoholic steatohepatitis in adjuvant tamoxifen for breast cancer patients.

Adjuvant tamoxifen has become the treatment of choice against estrogen receptor-positive breast cancer. Adverse effects are rarely observed and since symptoms of hepatic steatosis, non-alcoholic steatohepatitis and cirrhosis are usually negligible, such effects are not well characterized despite large cohort studies of adjuvant tamoxifen. This issue remains to be systematically studied. The present study consisted of 136 breast cancer patients treated with or without tamoxifen. Patients had laboratory tests once each month and underwent abdominal computed tomography (CT) annually for 5 years. The extent of hepatic steatosis was assessed by CT as the liver/spleen ratio. While receiving adjuvant tamoxifen, 40 of 105 patients developed hepatic steatosis (liver/spleen ratio <0.9) without obvious changes in body mass index. Twenty-one had a liver spleen ratio of <0.5, whereas none of the 31 patients treated without tamoxifen had a ratio <0.9 or <0.5 (p<0.0001 and p<0.0001, respectively). Hepatic steatosis was recognized in 35 of the 40 patients within the first 2 years of receiving adjuvant tamoxifen and 21 of the 40 had increased transaminase levels. Liver biopsy revealed NASH in 6 of 7 patients among the 21 with a liver/spleen ratio of <0.5. A subset of individuals given adjuvant tamoxifen developed progressive hepatic steatosis without significant changes in the body mass index. We suggest a liver/spleen ratio of <0.5 as a criterion upon which liver biopsy should be recommended since NASH frequently occurred in such patients.

Loss of heterozygosity of nucleotide excision repair factors in sporadic oral squamous cell carcinoma using microdissected tissue.

The deficiencies of nucleotide excision repair (NER) factors are involved in rare genetic diseases such as xeroderma pigmentosum (XP) with increased risk of developing cancer on sun-exposed areas of the skin. However, the abnormality of NER factors in human sporadic carcinoma remains unclear. Loss of heterozygosity (LOH) analysis, using the microdissected tissues, for the XPA, XPB, XPC, XPD, XPE, XPF, XPG and the transcription-coupled repair factor, Cockayne syndrome B (CSB) revealed that NER factors were abnormal in 30.0% (3/10 cases) of oral squamous cell carcinomas. Furthermore, 10.0% of oral carcinomas exhibited LOH for NER factors without LOH for tumor suppressor genes such as p53, FHIT, APC, BRCA1, BRCA2 and DCC. These observations raise the possibility that alterations of NER factors may be involved in carcinogenesis in human oral squamous cell carcinoma.
