Clinical study on the control of dental plaque using a photo energy conversion toothbrush equipped with a TiO2 semiconductor.

Adult female subjects used a toothbrush equipped with a cylindrical TiO2 semiconductor for 3 weeks. The subjects were divided into two groups, one which used the semiconductor toothbrush and the other which used a conventional toothbrush. The pl-I of the experimental group was the same as that for the control group, but showed a tendency to decrease week by week during the experimental period. The PMA-I of the experimental group during the experimental period showed a tendency to decrease, with a significant difference being observed compared to the control group after the third week. CPITN in the experimental group showed no obvious tendency towards an improvement when compared with the PMA-I value. It is suggested that improvements in gingivitis and oral cleanliness can be expected through application of the present toothbrush which is equipped with a TiO2 semiconductor.

Radioimmunoassay of glycated serum protein using monoclonal antibody to glucitollysine and coomassie-brilliant-blue-coated polystyrene beads.

A radioimmunoassay for glycated serum protein (GSP) was developed using monoclonal antibody to glucitollysine and polystyrene beads coated with Coomassie-Brilliant-Blue (CBB) as adsorbent for serum protein. The monoclonal antibody was raised by immunizing BALB/c mice with reduced glycated LDL and fusing their spleen cells with mouse myeloma cells. CBB-coated polystyrene beads were introduced to absorb a constant amount of serum protein. The protein adsorbed on the CBB-coated beads was reduced by NaHB4, and after treatment with radiolabeled antibody, the radioactivity of each bead was counted with an automatic gamma-counter. The standard glycated protein used was reduced glycated human serum albumin, in which 8 of 59 lysine residues were glycated. The intra- and interassay coefficients of variation of GSP were 4.8-6.5% and 1.6-6.0%, respectively. The GSP level of diabetic patients was significantly higher than that of normal controls (1.97 +/- 1.23 vs. 0.47 +/- 0.21 nmol/mg-protein; mean +/- SD, p less than 0.001). The GSP levels of patients with insulin-dependent and non-insulin-dependent diabetes mellitus were 3.03 +/- 1.05 and 1.51 +/- 1.00 nmol/mg-protein, respectively. A good correlation was found between the levels of GSP and hemoglobin A1c (HbA1c) (r = 0.85, p less than 0.001). In patients admitted to the hospital for diabetes education and glycemic control, the GSP level decreased 43 +/- 12% with the decrease in the fasting plasma glucose level (39 +/- 13%) and the mean daily plasma glucose level (MPG, 47 +/- 15%) in a four week period after admission, whereas the HbA1c level decreased only 13 +/- 6% during this period.(ABSTRACT TRUNCATED AT 250 WORDS)

Leukosialin, a major sialoglycoprotein defining leucocyte differentiation.

We have isolated a major sialoglycoprotein on leucocytes and found that this glycoprotein, termed leukosialin, is ubiquitously present on various human leucocytes (granulocytes, monocytes/macrophages and T lymphocytes). Our studies showed that leukosialin is significantly glycosylated by O-linked oligosaccharides (70 chains/molecule). The polypeptide portions of these molecules are, however, apparently the same, with a molecular mass of 38.5 kDa. The amino acid sequence derived from cDNA shows tandemly repeated O-glycan attachment sequences, and about 70% of the serine or threonine residues in the external domain are modified by O-glycans. The structures of those O-linked oligosaccharides are characteristic of each cell lineage and maturation stage. In particular, we have shown that O-glycans of leukosialin are converted from NeuAc(alpha 2-3)Gal(beta 1-3) [NeuAc(alpha 2-6)]-GalNAc to NeuAc(alpha 2-3)Gal(beta 1-3) [NeuAc(alpha 2-3)Gal(beta 1-4)GlcNAc(beta 1-6)] GalNAc during T cell activation.

Phase II study of carboplatin in non-small cell lung cancer.

Forty four patients with advanced non-small cell lung cancer (NSCLC) were treated with carboplatin (CBDCA; cis-diammine-1, 1-cyclobutane dicarboxylate platinum II) at a dose of 400-450 mg/m2 intravenously every four weeks in a phase II study. Forty three patients were evaluated for response and toxicity. Two patients achieved responses resulting in an overall response rate of 4.7% (95% confidence limits: 0.6-15.8%); one of these had not been treated previously and the other had been treated previously with vinblastine. The durations of their responses were 5 and 7 months, respectively. The response rate in 28 previously untreated patients was 3.6% (1/28; 95% confidence limits: 0.1-18.3%). Myelosuppression was the most commonly found toxicity, and thrombocytopenia especially was dose-limiting. Thrombocytopenia (less than 75,000/mm3) was observed in 12 patients (28%). Leukopenia (less than 3,000/mm3) was observed in 14 patients (33%). No serious infection or bleeding occurred, however. Treatment with 400-450 mg CBDCA/m2 was well tolerated in the good risk patients in this study. Mild to moderate emesis was observed in 28 patients (65%). No renal toxicity, neurotoxicity or ototoxicity was seen. It was demonstrated that CBDCA had little efficacy in NSCLC at the dose and schedule given in the present study.

Prognostic factors in small cell lung cancer: multivariate analysis in the National Cancer Center Hospital (Japan).

Consecutive series involving 172 patients with small cell lung cancer were analyzed retrospectively using eight pretreatment and two treatment-related prognostic factors in respect of their influence on survival. All the patients received chemotherapy with or without chest irradiation, according to phase II or phase III trial protocols of the National Cancer Center Hospital, Tokyo, from 1970 to 1987. The influence on survival of the various factors was investigated using univariate methods and Cox's proportional hazards model. In patients who survived for more than one year, a performance status of 0-1, limited disease, an age greater than 60 years, the absence of liver metastasis, radiotherapy to primary site and response to chemotherapy were determined by univariate analysis to be favorable prognostic factors. By multivariate analysis, performance status (P = 0.005), age (P = 0.026) and response to induction chemotherapy (P = 0.0001) proved to be valuable prognostic survival factors. The extent of disease which had been considered one of the most significant prognostic factors, was shown not to be a significant independent variable by multivariate analysis. Staging procedures may influence the prognostic analysis. Although classifying the limited or extensive stage of disease is still recommended, the current staging system lacks stringency and may not, in fact, reflect the tumor burden accurately. A simpler and meaningful staging system needs to adopted universally in order to continue the build-up of data for comparison from all institutions.

[A case of acute spinal epidural hematoma with spontaneous resolution and its MRI].

A case of non-traumatic spinal epidural hematoma (SEH) with spontaneous resolution was reported. A 80-year-old woman was admitted to our hospital for her paraplegia of acute onset. She had had no specific previous history, and she did not either receive any drug or suffer from hypertension. On admission, general status was unremarkable. Flaccid paralysis of lower extremities and bilateral sensory disturbance of all modalities below the level of Th4 were observed. Deep tendon reflexes were normal in upper extremities, while absent in lower extremities. Babinski's sign was not elicited. From the findings of CT and metrizamide CT myelography, SEH of ventral type was presumed. MRI revealed hematoma compressing spinal cord over two vertebral segments, and widely spreading even to C7 rostrally and to Th10 caudally. Urgent surgical intervention was taken into consideration, but was not performed because of her rapid improvement: sensory disturbance alleviated from day to day, and she became able to walk within the 3rd day of hospitalization. Almost complete recovery from motor and sensory dysfunction was achieved in about 7 days after admission. Only 4 cases of spontaneous recovery of SEH have been reported so far, and this patient is the 5th such case. Although CT and metrizamide CT myelography are useful in diagnosing SEH, MRI is also proved to be an accurate and efficacious method for evaluation of its size, location and extent in the spinal canal.

[Clinical management of diabetic retinopathy].

Factors to be checked concerning local and systemic condition were studied statistically in order to clarify the methodology for clinical management of diabetic retinopathy. NIDDM (n = 1517) and IDDM (n = 30) persons participating in baseline and follow-up examinations were included. The vitreous fluorophotometric values were selected for local check factors. Glycosylated hemoglobin was selected for systemic check factors. To determine the retinopathy status at both the baseline and follow-up examinations, all fundus photographs were graded in a masked fashion, using the author's classification scheme (1983) which specified six levels of retinopathy for each excepted from the interrupted proliferative retinopathy. Level 0: no retinopathy, Level 1: microaneurysms only (AI), Level 2: microaneurysms and retinal hemorrhages (AII), Level 3: preproliferative retinopathy (soft exudates, increased capillary occlusion and intraretinal microvascular abnormalities) (BI), Level 4: neovascularization elsewhere (BII), Level 5: neovascularization of the disc (BIII), Level 6: vitreous hemorrhages or proliferative tissue (BIV, V). A positive correlation between the progression of retinopathy and glycosylated hemoglobin or vitreous fluorophotometric values were observed. The coefficient of correlation was 0.67 between posterior vitreous fluorophotometric values and levels (scores) of retinopathy. The coefficient of correlation was 0.41 between glycosylated hemoglobin and levels of retinopathy. These data suggest that these two factors can predict the progression of diabetic retinopathy.

Multi-organ damage (MOD) induced by cancer cachexia and its pathogenesis.

The intraperitoneal implantation of the ascitic hepatoma cells, AH-130 to rats induced marked atrophy of the systemic organs within 2 weeks, resulting in the animal death. By the method of Feulgen hydrolysis curve analysis, the amount of single-stranded DNA and the degree of DNA instability were shown to be increased in these atrophic organs. In keeping pace with the progression of the cachectic multi-organ damage (MOD), the amount of lipidperoxide and the activity of superoxide dismutase (SOD) as measured by electron spin resonance (ESR) were increased in the ascites toward the end stage of chachexia. The increased lipidperoxide and SOD induction reflect the increased production of active oxygens, especially superoxide. The marked systemic organ damage induced in cancer cachexia seems to be due to DNA damage by active oxygens.

[Prolonged thrombocytopenia after autologous bone marrow transplantation].

Thirty two patients with hematologic malignancies and solid tumors were treated with intensive therapy and autologous bone marrow transplantation. In nine out of 32 patients, it took more than 50 days to achieve a sustained platelet count of 50,000/microliter or greater. Significant associations with poor platelet recovery were found for patient age, diseases, period of cryopreservation, the kinds of eradicative therapy and in vitro purging. But most of these factors overlapped each other in the same patients. No correlation was found between platelet recovery and number of cells or CFU-GM infused.

[Release of peripheral blood stem cells following chemotherapy in childhood malignancies].

Peripheral blood stem cells released following chemotherapy were examined in 21 children with malignancies in early remission. In order to obtain more than 1 x 10(5) CFU-GM per liter which is the minimal concentration to achieve autologous blood stem cell transplantation by cytapheresis, myelosuppressive chemotherapy which reduced leukocyte count below 1,000/microliters or neutrophil count below 200/microliters was necessary. Because the repetition of chemotherapy reduced the release of CFU-GM in peripheral blood, blood stem cells should be collected early after the beginning of chemotherapy. By long term culture method, peripheral blood stem cells seemed to contain pluripotent stem cells. Using our therapeutic protocol, more than 1 x 10(5) CFU-GM per liter could be obtained in malignant lymphoma and acute non-lymphoblastic leukemia, thus autologous blood stem cell transplantation seemed to be possible in these diseases.

Correlation of acrosomal status and sperm performance in the sperm penetration assay.

The sperm of some infertile men are unable to penetrate zona pellucida-free hamster oocytes but gain that ability after treatment with human follicular fluid (hFF). We asked whether altered incidences of acrosome reacted sperm explained these observations. Patient sperm failing to penetrate oocytes had fewer acrosome reactions than did healthy males, but the percentage reacted was not correlated with oocyte penetration. Sperm incubated 3 hours, then exposed to hFF, exhibited increased penetrations for 7 of 10 males, without an increase in percentage reacted sperm. Sperm incubated 22 hours before hFF treatment had penetrating ability enhanced 250- to 1000-fold, but the percentage reacted increased only sixfold. We conclude that factors other than the percentage reacted sperm are the major determinants of penetration capacity.

Trial of IgG therapy for lupus nephritis--report of two cases with a successful response.

Two cases with systemic lupus erythematosus (SLE) were treated with high-dose intravenous IgG. In patient 1, the IgG therapy dramatically improved both the lupus nephritis and serological titers for SLE. In patient 2, intravenous IgG yielded a partial remission in the lupus nephritis as well as a satisfactory decrease in proteinuria and disappearance of the skin rash characteristic of SLE. IgG therapy is suggested as possibly beneficial for the treatment of SLE based on different mechanisms from those of corticosteroid action. Further studies are needed to determine the detailed efficacy of IgG therapy for SLE.

Lectin histochemistry in rat thyroid tumours.

The thyroid tumours and background goiterous and adenomatous lesions induced in rats with diisopropanolnitrosamine (DIPN) plus methylthiouracil (MTU), and regenerative thyroid tissues after wounding were studied by lectin histochemistry. Ten weeks after cessation of the carcinogen treatments, carcinomas invading the surrounding tissues and blood vessels (13/20) and papillary micronodules (11/20) were formed in the thyroid tissues. In general, the carcinoma lesion was solitary, and the papillary micronodules were multiple in a single thyroid gland. Among the lectins tested, Maclura pomifera (MPA) and Solanum tuberosum (STA) showed specific binding with both carcinoma and papillary micronodule lesions, but not with the background goiterous and adenomatous lesions and regenerative thyroid tissues. The former both lesions showed higher labelling indices with BUdR or 3H-thymidine and poorer thyroglobulin accumulation than the latters, thereby indicating their enhanced proliferative capability and depressed potency of cyto-differentiation. The common cytological and histochemical properties of carcinoma lesions and papillary micronodules allow us to regard the latter as pre-invading carcinoma lesions. The lectins MPA and STA may be, therefore, used as the specific markers of malignancy in rat thyroid carcinogenesis.

Survival of recombinant erythropoietin in the circulation: the role of carbohydrates.

Recombinant human erythropoietin produced in transfected Chinese hamster ovary cells is glycosylated much the same way as the erythropoietin present in human urine. To determine the role of carbohydrates in the stability of recombinant human erythropoietin in vivo, [125I]-labeled recombinant erythropoietin was intravenously infused into rats. The erythropoietin was slowly cleared from the blood with a half-life of approximately two hours. Asialoerythropoietin, which was produced by treatment of recombinant human erythropoietin with sialidase, was found to be cleared rapidly from circulation within ten minutes. These data suggest that the galactose binding protein of hepatic cells is involved in the clearance of asialoerythropoietin. Erythropoietin also contains N-glycans with a few N-acetyllactosamine repeats, which can be enriched by tomato lectin affinity chromatography. The lectin-bound fraction was cleared to a larger extent than was the unfractionated erythropoietin, while the component that did not bind the lectin was found to be stable in the circulation. Authentic N-acetyllactosamine repeats (polylactosaminoglycans) prepared from erythrocytes were similarly rapidly cleared from the circulation to the liver, and this clearance was inhibitable with asialo-alpha 1-acid glycoprotein. These results suggest that (a) the sialic acid of the recombinant erythropoietin is necessary for this glycoprotein hormone to circulate stably and (b) glycoproteins with more than three lactosaminyl repeat units may be cleared by the galactose binding protein of hepatocytes.
