[Cerebral infarction in a boy with subdural abscess].

A 13-year-old boy was reported with a subdural abscess in the left anterior cranial fossa and an infarction in the lesion supplied by the left middle cerebral artery. Cranial computed tomography revealed a gradual increase of a low density area with surrounding capsule formation, and later another low density area which indicated in occlusion of the left middle cerebral artery. Cerebral angiography disclosed a stenosis at the trifurcated portion of the left internal carotid artery near the abscess. These findings suggested the occurrence of infarction due to angiitis of the internal carotid artery.

Detection of ornithine decarboxylase gene expression in 12-O-tetradecanoylphorbol-13-acetate-treated mouse skin using in situ hybridization.

The localization of ornithine decarboxylase gene expression in mouse skin by tumour promoter, 12-O-tetradecanoylphorbol-13-acetate, was investigated using in situ hybridization. After 4 h of treatment with tumour promoter, the grains representing ornithine decarboxylase mRNA increased remarkably in number in the epidermis, especially in the follicular region. When the cells containing more than 5 grains were counted as positive, the number of positive cells increased by about 52-fold in the follicular epidermis, by about 19-fold in the interfollicular epidermis and by 4-fold in the dermis, as compared with controls. These results indicate that the epidermal cells are mainly responsible for the activation of ornithine decarboxylase by 12-O-tetradecanoylphorbol-13-acetate.

A case of prurigo pigmentosa considered to be contact allergy to chromium in an acupuncture needle.

A 53-year-old male developed prurigo pigmentosa on his back, after undergoing acupuncture for 3 years. The eruptions were ceased on discontinuing the therapy but recurred with its resumption. The acupuncture needle contained 18.12% chromium. Erythema was induced by patch testing with potassium dichromate, and a flare-up was observed in the area of the patch test on resumption of acupuncture. We consider that the eruptions were induced by contact allergy to the chromium component of the acupuncture needles.

[Adult type of idiopathic paroxysmal cold hemoglobinuria].

A 78-year-old female suffered from idiopathic paroxysmal cold hemoglobinuria (PCH). Her symptom occurred immediately after she worked outside in the cold early morning. This case is characterized by the eldest case with non-syphilitic PCH reported in Japan and by the most advanced anemia (Hb 4.2 g/dl) possibly due to prolonged hemolysis after cold exposure. Analysis of the serum revealed positive Donath-Landsteiner antibody of IgG type, which could react to hemolysis not only below 15 degrees C but also at 15-20 degrees C in vitro.

[ECG changes associated with PGF2 alpha injection into the basal cistern of rabbits].

In the previous studies by the authors, it was shown that various types of changes in electrocardiograph were produced by injection of a small dose of PGF2 alpha into the basal cistern of rabbits which were studied to examine the mechanisms of those in humans with subarachnoid bleeding. In this study with reference to elucidating nerve pathways of PGF2 alpha produced stimulation, changes in electrocardiograph were examined in three groups of the animals which underwent dissection of the spinal cord either at C2-3, or at Th7-8, or bindings of bilateral suprarenal artery and vein, prior to the administration of PGF2 alpha into the basal cistern. The administration of PGF2 alpha decreased more significantly appearance rate of electrocardiographic alterations in the animals following spinal cord dissection at C2-3, and bindings of the suprarenal vessels than the animals with spinal cord dissection at Th7-8. It was concluded that the development of these electrocardiographic abnormalities was caused by direct autonomic discharges to the heart as well as by increased circulating catecholamines.

[Qualitative and quantitative evaluation of CFU-E colonies in myelodysplastic syndromes].

Hemoglobin (Hb) synthesis in colony-forming-unit erythroid (CFU-E) colonies from the bone marrow of 10 normal subjects and 6 patients with myelodysplastic syndromes (MDS) was studied using microcytofluorometry. Hb content was determined utilizing a photochemical reaction in which the intracellular Hb is converted into fluorescent porphyrin. Briefly, the CFU-E in plasma clots was assayed according to Tepperman's method. Bone-marrow mononuclear cells were then dispensed onto petri dishes containing the plasma clots. The cells in plasma clots were cultured for seven days and then air-dried. The samples were then fixed with pure methanol. Thereafter, the dishes were exposed to ultraviolet light (lambda = 405 nm) in the presence of an SH-donor (0.2 M mercaptoethylamine hydrochloride). The intensity of porphyrin fluorescence was measured using a microcytofluorometer. When the photochemical reaction was carried out for 50 min, the intensity of fluorescence was found to be proportional to the mean corpuscular Hb (MCH) level, suggesting that the intracellular Hb level can be determined as a function of the intensity of this fluorescence. The number of CFU-E colonies in MDS patients was smaller than that in normal subjects. In addition, intracolony Hb level was markedly lower in the MDS cases than in normal subjects. Our findings suggest that the maturation process of CFU-E is impaired in MDS patients.

Histamine and its synthesis in mammalian retinas.

Histamine (HA) content and histidine decarboxylase (HDC) activity were studied in the sensory retinas of the rat, guinea-pig, rabbit, dog, monkey, bovine and pig. Using a highly sensitive and specific method developed for the determination of histamine using high-performance liquid chromatography, HA was detected in the retinas of all species examined. HA content ranged from 11 (bovine) to 540 pmole/g wet tissue (dog), and HDC activity ranged from 2 (rabbit) to 150 (dog) fmol HA synthesized/min/mg protein. With the exception of the dogs, the retinal values were one order lower than those in the brain in all species examined. The canine retinas had the high HA and HDC values among the species examined, and were approximately equal to those in the brain. However, the ratios of HDC activity to HA content ranged from 0.04/min to 0.51/min. These values resembled those in the brain and were much higher than those in mast cells. This suggests that histamine in the retinas exists mainly in the neurons even though the actual amounts are small and may be involved in the physiological and the pathological responses in mammalian retinas.

[Pharmacokinetic and clinical studies on flomoxef in mature and premature infant].

Pharmacokinetic and clinical studies of flomoxef (FMOX) in neonates and premature infants were conducted, and the results obtained are summarized below. 1. Plasma concentrations of FMOX at 15 minutes after one shot intravenous injection of 20 mg/kg to 6 cases were in a rang of 33.0-69.9 micrograms/ml and half-lives (T 1/2's) were between 0.68 and 4.89 hours. The plasma concentration of FMOX at 15 minutes after one shot intravenous injection of 40 mg/kg to 1 case was 79.9 micrograms/ml and the half-life (T 1/2) was 2.45 hours. Drug concentrations in plasma upon 1-hour intravenous drip infusion were 71.1-114.0 micrograms/ml and T 1/2's were 1.64-3.41 hours. T 1/2 tended to be couse shorter as ages of babies increased. 2. Urinary excretion rates in the first 6 hours after one shot intravenous injection of FMOX 20 mg/kg to 1 case and 1-hour intravenous drip infusion of FMOX 40 mg/kg to 2 cases were 60.4%, and 27.2 and 55.3%, respectively. 3. Clinical effects of FMOX against 12 cases of bacterial infections were excellent in 6 cases, good in 5 cases and poor in 1 case, thus the clinical efficacy rate was 91.7%. FMOX was also given to 6 cases for prophylaxis and prophylactic effects were observed in all the cases. 4. No adverse effects were observed in the 21 cases examined, but elevations of S-GOT and S-GPT were found in 1 case. The abnormal laboratory test results were probably due to this drug.

Atypical inheritance of hemogenetic markers: seven cases of disputed parentage.

The present paper reports seven families with an atypical segregation pattern which we have recently found during paternity testing. In one family, heterozygous mother-child incompatibility (1B2A and 1A2B) in the PGM1 system was observed. This could be explained by an intragenic recombination between the PGM1*1B and PBM1*2A alleles giving rise to the PGM1*2B allele. In the other families, two types of new GC variants with two pairs of double band were identified. These variants (GC*1S1A2 and GC*1F1A2) were presumed to have arisen from the duplication of two GC*1 alleles.

[Complete response provided by new antitumor agent, MST-16 (sobuzoxane), in a breast cancer with multiple cutaneous metastases].

A case of recurrent breast carcinoma was successfully treated with MST-16 in an early phase II study. A 60-year-old woman developed skin metastases on the forehead, neck, abdomen and back 8 months after receiving standard radical mastectomy for right breast carcinoma. MST-16 was given p.o. at the dose of 1,200 mg/body/day initially, and the dosage was modified thereafter depending on the grade of leukopenia. All metastatic lesions disappeared completely 38 weeks after the initiation of the treatment. The CR thus obtained has continued for 8 months up to the present. Although leukopenia was observed during the administration of MST-16, it could be well managed by dose modification. Further trials with this drug for breast carcinoma should be conducted, since complete responses are expected with few adverse effects as in the case under study.

[Pathophysiological study of corona radiata infarcts by clinically available diagnostic methods].

The authors evaluated 18 patients who presented with corona radiata infarction, one of the 'water-shed infarctions', on CT and/or MRI to determine its etiology and pathophysiology using cerebral angiography, single-photon emission computed tomography (SPECT), and tests of hemostatic function including hematocrit, platelet aggregation and adhesiveness. On angiography, 8 of these 18 patients had ulcerative lesions in the common carotid artery bifurcation with or without minimal stenosis and exhibited no or a minimal area of hypoperfusion localized to the corona radiata on SPECT. In these, microembolism from the lesions at the common carotid bifurcation seemed play an important role in the genesis of corona radiata infarction. In 7 of the remaining 10 patients, cerebral angiography showed occlusive lesions of the internal carotid artery around its origin in 3, more than 90% stenosis of the internal carotid artery in 1, severe stenosis of the M1 segment of the middle cerebral artery in 2, and M1 occlusion in 1. In 5 of these 7, SPECT demonstrated a larger area of hypoperfusion than the corona radiata in the involved hemisphere. In the remaining 2, SPECT demonstrated a hypoperfusion area localized to the corona radiata. In all 7, the hematocrit was elevated. A collateral blood supply was visualized in 5 of 7 on cerebral angiography. In these 7 patients, hemodynamic disturbance was considered to contribute to the pathogenesis of infarction in the corona radiata. In the final three patients, cerebral angiography showed significant occlusive lesions in the main trunk of the cerebral arteries.(ABSTRACT TRUNCATED AT 250 WORDS)

[6 cases of contralateral atelectasis immediately after thoracotomy in lateral position--its mechanism and treatment].

Six cases of atelectasis developing in the unopened contralateral lung immediately after thoracotomy in the lateral position were presented, and its cause and treatment were discussed. Atelectasis due to retention of secretion showed various atelectatic X-ray shadows which differed in size by the segment or lobe unit. On the other hand, cases of atelectasis whose occurrence was surmised to be related to the lateral position and anesthesia showed mainly infiltrative shadows which extended from the outer region of the middle-lower lung field to the basal segments of the lung, although some cases were accompanied by atelectatic shadows of various size in terms of the segment or lobe unit. Therefore in order to prevent atelectasis, it is desirable to move sputum by aspiration and appropriately apply ventilation, intermittent pressurization and PEEP, while paying careful attention to the unopened lung. In the treatment of atelectasis, selective endobronchial pressurization using a bronchofiberscope equipped with a cuff was effective. It was also very effective to place an endotracheal tube for ordinary anesthesia near the target bronchus with the aid of a bronchofiberscope and perform selective endobronchial pressurization via that tube.

[Drug-induced blood dyscrasias in Tohoku district].

One hundred and thirty-four cases were identified in seven prefectures of the Tohoku district. Fifty-seven per cent of them were over 50 years old, and the incidence had no differences between sexes. They consisted of 51 leukopenia, 32 thrombocytopenia, and nine anemia cases which included seven hemolytic anemia and two pure red cell aplasia. Agents associated with blood dyscrasias were those acting on the central nervous system (CNS) (49 cases), cardiovascular drugs (25 cases), and antimicrobial agents (48 cases). Leukopenia occurred to five patients with cinepazidemaleate (Brendil) administration, 11 with thiamazole (Mercazole), and 14 with antimicrobial agents which were mainly penicillins and cephalosporins. Thrombocytopenia was induced by H2-receptor antagonists (six cases) and antimicrobial agents mostly of penicillins and cephalosporins (12 cases). Anemia contained seven cases of hemolytic anemia, including two patients caused by methyldopa (Aldomet) and two by dried human immunoglobulin (Glovenin). Among 15 cases, 12 out of 19 agents were those affecting the CNS. Six cases were dead from drug-induced blood dyscrasia.

[Inhibitory effect of arbutin on melanogenesis--biochemical study using cultured B16 melanoma cells].

Inhibitory effect of arbutin (hydroquinone-beta-D-glucopyranoside) on the melanogenesis was studied biochemically using cultured B16 melanoma cells. The maximum arbutin concentration lacking an inhibitory effect on cell growth was 5 X 10(-5) M. At this concentration, melanin content per cell was decreased significantly to about 39%, compared with that of arbutin untreated cells. Also, tyrosinase activity of arbutin treated cells was decreased significantly. When arbutin was added to B16 melanoma cell suspension, arbutin was not hydrolyzed to liberate hydroquinone. Further, tyrosinase activity in crude preparations from B16 melanoma cells was inhibited by arbutin. From these results, it is suggested that arbutin can inhibit the melanogenesis by affecting not only the synthesis but also the activity of tyrosinase rather than by killing melanocytes B16 melanoma cells. Also, it is suggested that hydroquinone is not responsible for the inhibitory effect of arbutin on the melanogenesis.

[Semi-quantitative evaluation of atherosclerotic lesions of the thoracic aorta with high resolution transesophageal ultrasound].

This study was designed to assess semi-quantitative ultrasonic evaluation of atherosclerotic lesions with high resolution transesophageal ultrasound. Various types of atherosclerotic lesions were differentiated using transesophageal ultrasound in 51 samples from 20 specimens of fresh descending aorta of humans or other animals in vitro, and in 62 patients in vivo. Microscopic examination of normal, fatty plaque, fibro-fatty plaque, fibrous plaque, calcified lesion and ulcerative lesion apparently coincided with transesophageal ultrasound images of normal (0), intimal irregular thickening (degree of I), intimal protuberance with low and high echo density in the plaque (II), high echo density in the plaque having an additional echo layer in the medial layer (III), irregular intimal thickening with calcification and ulcerative lesion with low and high mixed echo (IV). The quality of ultrasound image obtained from the patients in vivo proved similar to that obtained during the in vitro studies, and in addition six ulcerative lesions including two with torn intima were detected with transesophageal echocardiography. These results demonstrate that high resolution transesophageal ultrasound is an excellent approach to detect semiquantitatively atherosclerotic lesions of the aorta in vitro and in vivo.

[Usefulness of high resolution transesophageal echocardiography for diagnosis of ruptured chordae tendineae of the mitral valve: clinical and echocardiographic characteristics].

Transesophageal echocardiography with high resolution (TEE; 5 MHz, convex type probe) revealed ruptured chordae tendinae of the mitral valve (RCT) in seven patients. However transthoracic echocardiography (TTE) was diagnostic only in two patients. These all seven patients had mitral valve prolapse and mitral regurgitation documentated by TTE. Six of seven patients were 59 years or older, and four patients had only mild symptoms. TEE showed the systolic appearance of an edge echo, suggesting an edge of ruptured chordae tendineae within the left atrium. These results show that TEE is a useful method for detecting RCT of the mitral valve, especially in the aged patients with MVP and mitral regurgitation. It is because of the availability of high resolution and nearfield in TEE.
