Connections of the fronto-parietal operculum and the postcentral gyrus with the posterior ventral thalamic nucleus, especially its medial nucleus, in monkeys.

In 22 crab-eating monkeys (Macaca irus), either lesions or HRP injections were made in the frontol parietal operculum and, for the purpose of contrast, in the postcentral gyrus to observe their fiber connectionl with the posterior ventral nucleus (VP). The exposed opercular part, reported to consist of areas 3b, 1, and 2 showed scanty fiber connections with VP. The buried parietal operculum had connections with the media (VPM) and lateral (VPL) nuclei of VP, corresponding to the second somatic sensory cortex (SII). This cortica area was connected, in VPM, to its ventromedial part, while the base of the postcentral gyrus, the ordinary face area of SI, was connected to its lateral-most part. This suggests that the ventromedial part of VPM is more intimately related to SII, and its lateral-most part to SI. In contrast, the SI and SII areas of VPL overlapped more extensively with each other. The cortical area which received fibers from the parvocellular part of VPM (VPMpc) lay in the frontal operculum, especially in its buried part. This area extended caudally beyond the precentral dimple and included areas 1 and 2 of the buried frontal operculum, indicating that its posterior extent was wider than the pure taste cortical area. Spinothalamic terminations in VPL and the PO nuclear group were discussed in relation to cortical connections of these thalamic nuclei. It was suggested that spinothalamic input could be relayed to both postcentral and opercular cortices. The relation of the spinothalamic input to the PO group appeared to be minor.

[Multidisciplinary treatment of head and neck cancer].

A new multidisciplinary treatment for head and neck cancer was shown by focusing on the maxillary and oropharyngeal carcinoma. Neo-adjuvant chemotherapy which consists of cisplatin and peplomycin was incorporated into it. In terms of maxillary carcinoma, 2 courses of chemotherapy were given with an interval of 2 weeks, followed by radiotherapy (40 Gy) combined with intraarterial chemotherapy (5-FU). When no cancer cells were detected at the completion of this therapy, adjuvant chemoimmunotherapy was given. When an apparent tumor was still revealed by CT, radical surgery was performed. When cancer cells were detected only by histological examination, additional radiotherapy up to 60 Gy was given by Linac. The results obtained were analysed in 14 cases. We also presented another type of multidisciplinary treatment in oropharyngeal carcinoma.

[Preoperative chemotherapy for head and neck cancer].

The authors have studied preoperative chemotherapy for head and neck cancer since 1963, focusing on intra-arterial chemotherapy. The results obtained revealed that preoperative chemotherapy played an important role in the improvement of 5-year survival in maxillary sinus carcinoma and tongue carcinoma. However, functional and cosmetic damage after radical surgery has newly energed as problems to be resolved. The appearance of cisplatin has raised an important the level of effectiveness of cancer chemotherapy in recent years. Since the combination of cisplatin and bleomycin analogs revealed a remarkable synergistic effect in experimental chemotherapy, it has been used clinically as preoperative or preradiation chemotherapy for advanced Stage III and IV head and neck cancer. Under the above-mentioned circumstances, treatment for head and neck cancer has undergone various changes over the past twenty years. At present it is no exaggeration to say that the period in which dependence was placed on only surgery and/or radiation is over. The authors would like to emphasize that preoperative chemotherapy should be carried out as a part of multidisciplinary approach under a new concept of neo-adjuvant chemotherapy which includes not only preoperative chemotherapy but also preradiation chemotherapy. The role of preoperative chemotherapy, its current status and future prospects are discussed in this paper, looking back on the history of chemotherapy for head and neck cancer.

Effects of nipradilol (K-351) on alpha-adrenoceptor mediated responses in various isolated tissues.

Nipradilol competitively antagonized norepinephrine- or phenylephrine-induced contractile responses of guinea-pig thoracic aorta. These actions of nipradilol were about 6 times less potent than those of phentolamine. Nitroglycerin showed a non-competitive antagonistic action on norepinephrine-induced contractions of aorta. Furthermore, nipradilol competitively inhibited norepinephrine-induced contractions of rat vas deferens and dose-dependently reduced the phenylephrine-induced inhibitory responses in rabbit ileum. These antagonistic actions of nipradilol were 30 to 100 times less potent than those of phentolamine. Nitroglycerin did not appreciably affect these alpha-adrenoceptor mediated responses in rat vas deferens and rabbit ileum. The inhibitory action of clonidine on the twitch contraction of rat vas deferens produced by intramural stimulation was only slightly antagonized by nipradilol (pA2 = 5.4). Nipradilol and nitroglycerin showed a non-competitive antagonistic action on clonidine-induced contractions of canine saphenous vein after the exposure to phenoxybenzamine, while phentolamine competitively inhibited the clonidine responses. These results suggest that nipradilol possesses an alpha 1-adrenoceptor blocking action; it possesses very weak or practically no presynaptic alpha 2-blocking activity but shows a non-competitive antagonistic action on postsynaptic alpha 2-adrenoceptor mediated contractile responses.

[Surface marker studies on lymphoma and leukemia cells with microcytotoxicity test using the UCLA Monoclonal Antibody Tray 5].

Surface marker studies of lymphoid malignancies are important in predicting the prognosis. We have investigated surface marker studies on lymphoma and leukemia cells with a new method of a microcytotoxicity test using monoclonal antibodies. The microcytotoxicity test (UCLA Monoclonal Antibody Tray 5) has the advantages of being performed in a few hours with an easy technique. The results showed that T-cell lymphomas and B-cell lymphomas could be clearly differentiated by monoclonal antibodies specific to T or B cells, respectively. Leukemia cells of ALL and AML were also tested for several monoclonal antibodies. However, the problem that most of the monoclonal antibodies used in this tray have not been characterized for specificity has yet to be solved.

[A study of sputum beta 2-microglobulin and carcinoembryonic antigen in patients with primary lung cancer].

Sputum beta 2-microglobulin (beta 2MG) and carcinoembryonic antigen (CEA) levels were measured by enzyme immunoassay and radioimmunoassay, respectively. Sputum beta 2MG levels in patients with primary lung cancer are significantly higher than in those with benign lung disease or metastatic lung cancer. Sputum CEA levels in patients with primary lung cancer are significantly higher than those in patients with benign lung disease. These results suggest that sputum beta 2MG and CEA may be useful in the diagnosis of primary lung cancer.

[Phase II study of UFT for head and neck cancer].

A Phase II study of UFT for head and neck cancer was conducted in 10 institutions. UFT is a mixture of Futraful and uracil. Eighty-four patients entered this trial, of which 60 were evaluable. UFT was administered orally at a daily dose of 600 mg/day. Eight patients achieved complete response and 10 achieved partial response with an over-all response rate of 30.0 %. Evaluating response according to by histology, the response rate was 30.9% for cases of squamous cell carcinoma. Complete response was observed in one case of undifferentiated carcinoma. Response rate according to primary site was 33 to 40% for the nose & paranasal sinuses, mesopharynx, hypopharynx and larynx. The response rate was 28.9% for the group of patients treated previously, and 33.3% for the group previously untreated. The mean time for 50% or more regression of the tumor was 4.3 weeks. Toxic effects appeared in 40.3% of 67 evaluable cases as anorexia, nausea, vomiting, stomatitis, diarrhea etc. In one case of maxillary carcinoma, severe bone marrow suppression was observed. We concluded that UFT therapy was markedly effective for head and neck cancer.

Cell surface phenotypes and expression of viral antigens of various human cell lines carrying human T-cell leukemia virus.

Human T-cell leukemia/lymphoma virus (HTLV)-carrying cells from various origins were characterized by cell surface markers and expression of HTLV antigens. Eight cell lines named TCL were obtained by transformation of peripheral blood leukocytes (PBL) of healthy donors or HTLV carriers in cocultures with HTLV-producer MT-2 cells. Nine cell lines named ILT were interleukin 2 (IL2)-dependent cell lines cloned from PBL of ATL patients and healthy HTLV-carriers. Tc-Kan9 cell line was also an IL2-dependent cell line clonally established from PBL culture stimulated with autologous TCL cells. Five cell lines named TL were established in vitro directly from PBL of an adult T-cell leukemia (ATL) patient and from ILT cells of an ATL patient and three HTLV-carriers, respectively, to grow autonomously without IL2. All the TCLs, ILTs, TLs and Tc-Kan9 possessed Leu-I antigen, a pan-T-cell marker. Leu3a antigen, a helper/inducer T-cell marker, was expressed on five of eight TCLs and all of the ILTs and TLs. Leu-2a, a cytotoxic/suppressor T-cell marker, was detected only on Tc-Kan9 but not others. Fresh ATL leukemic cells of patients had a helper/inducer T-cell marker. Ia, OKT9 and Tac antigens, markers for activated and differentiated T cells, were strongly expressed on all of the cell lines tested and fresh ATL leukemic cells were weakly positive for these antigens. Expression of HTLV antigens detected by mouse monoclonal antibodies and an ATL-patient serum varied among these cell lines. One TL, two ILTs and most of the fresh ATL leukemic cells did not express HTLV antigens on the cell surface. The other cell lines were all positive for the surface viral antigens. However, molecular species of antigens defined by radioimmunoprecipitation with an ATL-patient serum were not always identical among the cell lines. Molecular weights of polypeptides detectable in most of the cell lines were 62K, 46K, 40K, 24K, 21K and 19K which could never be detected in several control T-cell lines. 68K and 28K polypeptides were frequently detected in MT-2 and TGLs. GIN14, a mouse monoclonal antibody against HTLV core protein (p19) detected not only p19 in various cell lines but also p28, p29, p31 or p40 in certain cell lines tested. B-cell lines named LCL were established and cloned from PBL of two HTLV-carriers by EB-virus-induced transformation and they also expressed HTLV antigens, Ia, OKT9 and Tac antigens.(ABSTRACT TRUNCATED AT 400 WORDS)

Cytochemical changes in lactate dehydrogenase isoenzymes in human brain tumours.

The lactate dehydrogenase (LDH) isoenzyme patterns in benign and malignant brain tumours were determined by means of electrophoresis of the cell extracts and selective cytochemical stain of the smears. The LDH isoenzyme distribution of the cell extracts showed a pronounced cathodal shift in the malignant gliomas and metastatic carcinomas. Normal brain tissues and histologically benign gliomas, however, showed an anodal pattern with a dominance of the H-type LDH. Schwannomas and meningiomas had a midzone isoenzyme pattern with a dominant LDH3 fraction. Pituitary adenomas usually showed the LDH pattern similar to that of the normal cerebrum. The LDH M fraction could be cytochemically verified using an inhibitory effect by 2.6 M urea in staining. Astrocytomas grades 3-4 and metastatic carcinomas were characterized by loss or marked reduction of stainability by urea treatment, while astrocytomas grades 1-2 and oligodendrogliomas were resistant to urea inhibition.
