New amino acid derivatives formed by alkaline treatment of proteins.

Intense heat treatment of proteins at high pH favors formation of transient, reactive intermediates derived from serine and cystine. The postulated dehydroalanine intermediate reacts further with the epsilon-amino groups side chains and sulfhydryl groups of cysteine residues to form derivatives of lysinoalanine and lanthionine. Recent studies indicate that besides these crosslinked products, several others are formed, by reaction of histidine, arginine, and possibly other residues. For this reason, caution should be exercised in assigning the prelysine peak(s) of an amino acid chromatogram to lysinoalanine. These results suggest that unnatural amino acid derivatives, other than lysinoalanine, may also contribute to the toxic effects reported from studies in which alkali-treated protein was fed to rats.

Relationships between fluoride in enamel, DMFT index and fluorosis in high- and low-fluoride areas in South Africa.

The DMFT Index, degree of fluorosis (DEGF), and enamel fluoride concentration (F) were determined in 88 children living in a high- and 79 children living in a low-fluoride area in South Africa. The following interrelationships between these parameters were found: in both areas there was a significant negative correlation between log F and age; in the high-fluoride area there was a significant positive correlation between log F and DEGF; and in the combined data from the two areas there were significant correlations between log F and area, log F and age, and log F and DEGF, and a significant association between DEGF and area. There were no significant correlations between DMFT and log F and between DMFT and DEGF.

Intramuscular salbutamol in treatment of acute exacerbations of childhood asthma.

Salbutamol was given by the intramuscular route to children aged 3 to 16 years with an attack of asthma. Results with a dose of 8 microgram/kg were favourable but suggested that a higher dose might be more so. 16 children thereafter were treated with 20 microgram/kg which produced a greater mean increase in peak expiratory flow rates (PEFR) without increase in side effects. This dose was then used in a double-blind crossover trial of salbutamol against a saline placebo. Half of 36 children treated with 20 microgram/kg showed rapid clinical improvement, the maximum rise in PEFR occurring within the first 5 minutes. A rise in pulse rate and occasionally a tremor were the only side effects noted. We conclude that intramuscular salbutamol 20 microgram/kg is a safe and useful initial medication in the management of the asthmatic child suffering an acute exacerbation.

Volume characteristics of extra- and intraparenchymal segments of the canine pulmonary artery.

The volumes of the extraparenchymal segment (VpaEP) and intraparenchymal segment (VpaIP) of the pulmonary arterial tree were determined in intact anesthetized dogs during room air breathing and acute hypoxia. Total pulmonary arterial blood volume (Vpatotal) was calculated as the product of pulmonary blood flow and pulmonary arterial circulation time. An angiographic technique was used to estimate VpEP. VpaIP was calculated by subtracting VpaEP from Vpatotal. During room air breathing at functional residual capacity, mean +/- SD of VpaEP was 17.1 +/- 5.1 ml and of VpaIP was 31.7 +/- 20.8 ml, representing 40% and 60%, respectively, of Vpatotal. Vpatotal increased 22.2 +/- 10.5 ml during lung inflation, with proportional increases in VpaIP and VpaEP. VpaEP was found to be influenced equally by changes in transmural pulmonary arterial and transpulmonary pressures. Acute hypoxia was accompanied by an increase in pulmonary vascular resistance and a decrease in volume distensibility of the extraparenchymal segment. Vpatotal increased 76% without changes in the relative volume distribution of VpaEP and VpaIP. These findings can be best explained by active vasomotion with an increase in down-stream pulmonary vascular resistance.

A new roentgenographic method for estimating mucous velocity in airways.

A new roentgenographic method is described for the estimation of mucous velocity in airways. Radiopaque discs of teflon mixed with bismuth trioxide were deposited in the trachea, and their motion was observed by a fluoroscopic image intensifier. Simultaneous measurements of disc movement were obtained by a previously reported cinebronchofiberscopic method and by the roentgenographic method in 6 anesthetized dogs. Mean +/- SD tracheal mucous velocity was 8.5 +/- 7.3 mm per min by the roentgenographic method, and 7.6 +/- 7.1 mm per min by the cinebronchofiberscopic method. Discrepancies between the 2 methods for individual disc velocities could be accounted for by the cumulative errors of both methods. The roentgenographic method was also used in 7 normal volunteers who did not have topical anesthesia of the tracheobronchial tree. The discs were blown through the inner channel of a bronchofiberscope, the tip of which was located just above the vocal cords. Placement of the discs on the tracheal mucosa generally did not produce coughing. Tracheal mucous velocity ranged from 7.4 to 19.4 mm per min as estimated from videotape recordings of the image intensifier images. There was no consistent difference in velocity between the erect or supine position, or after topical anesthesia with lidocaine.

Study of the pulmonary circulation in a canine asthma model.

Measurements of respiratory mechanics, gas exchange, and pulmonary hemodynamics were made before and serially for 2 hours after inhalation challenge with Ascaris suum extract in dogs with natural sensitivity to this antigen. Nine of 14 dogs randomly exposed to an inspired oxygen fraction of 0.21 or 0.60 to 0.70, responded to the challenge with bronchospasm reflected by a transient decrease of mean specific respiratory system conductance to 50 per cent of baseline after 5 to 15 min. This was accompanied by a decrease in alveolar ventilation in all and acidosis in some animals. No changes were observed in transmural pulmonary arterial pressure, cardiac output, pulmonary vascular resistance, and pulmonary blood volume in all 9 dogs who responded with bronchospasm, whether or not arterial hypoxemia was allowed to develop. In contrast, a significant increase in pulmonary vascular resistance occurred in control dogs in whom comparable degrees of arterial hypoxemia were produced by 10 per cent oxygen breathing or mechanical bronchial obstruction by placing beads in lobar and segmental airways. No changes in any parameter measured occurred in the 5 dogs who did not develop bronchospasm after antigen challenge. The results indicated that moderately severe, antigen-induced bronchospasm in anesthetized dogs has no effect on the pulmonary circulation even when associated with arterial hypoxemia. Because nonuniformity of alveolar hypoxia allowing redistribution of blood flow is probably not responsible for this failure to develop increases in pulmonary vascular resistance in induced asthma, it is suggested that the elaboration of chemical mediators may have interfered with the hypoxic pulmonary vasoconstrictor response.

Acute effects of an aerosol hair spray on tracheal mucociliary transport.

The acute effects of a commercially available aerosol hair spray preparation and a Freon propellant on various pulmonary function tests and tracheal mucociliary transport were studied in 12 normal nonsmokers. Tracheal mucous velocity was estimated by a roentgenographic method. In 7 subjects exposed to hair spray by directing the aerosol to the hair for 20 sec, no significant changes occurred in any of the various pulmonary function parameters, whereas mean tracheal mucous velocity decreased by 57 per cent (P less than 0.001) 1 hour after exposure. This effect was transient and could no longer be demonstrated after 3 hours. No significant changes in tracheal mucous velocity or pulmonary function tests were observed in the 5 control subjects exposed to the Freon propellant alone. These observations suggest that acute exposure to aerosol hair spray produces a transient impairment of a pulmonary defense mechanism, and that measurements of mucociliary transport is a more sensitive indicator of this type of airway irritation than conventional pulmonary function tests.

Tumors of the major salivary glands.

Tumors of the major salivary glands are reviewed according to classification, location, surgical procedure and end results. Our data of the incidence of benign and malignant tumors show that the most commonly involved area is the parotid gland and the most frequent is of the mixed variety. In the parotid region 80 percent are benign and 20 percent are malignant; whereas, in the submandibular gland, the malignant and benign tumors are equally distributed. The need for an extensive surgical attack and inclusion of contiguous structure is dictated by the nature of the malignant disease. The role of postoperative irradiation is discussed as is the indication for neck dissection. Management of the facial nerve, relative to malignant tumors of the parotid gland, is considered in detail.
