Twenty-two patients with adenocarcinoma metastatic to the liver were treated with rapid fractionation whole-liver irradiation (1350-2100 rads in 300-rad fractions) with simultaneous intrahepatic 5-fluorouracil (10-15 mg/kg/day) and intrahepatic Adriamycin 2.5-10 mg/m2/day) as part of a Phase I-II study. Of the 21 patients who completed therapy, 19 had colorectal carcinoma and 2 had metastatic adenocarcinoma of unknown origin. Objective response was judged by measurement of liver size, evaluation of liver function tests, and by liver scan or CAT scan of the liver. Ten of the 21 evaluable patients responded, yielding an overall response rate of 47.6%. The response rate in patients with colorectal carcinoma was 55% (10/19). At this time, median duration of response is 14+ weeks and median survival from onset of therapy is 15+ weeks. Hematologic and gastrointestinal toxicity were tolerable. No hepatic toxicity was documented. This combined modality therapy was found to be a safe effective method for the palliation of liver metastasis.
Cast films composed of different ratios of polyethylene glycol and ethylcellulose containing salicylic acid, caffeine, and tripelennamine as model dispersed drugs were prepared and exhibited sustained release. The drug content of the film declined at an apparent first-order rate initially, whereas the drug quantity released was proportional to the square root of time. Data analysis validated the latter treatment, which is in accordance with the diffusional matrix model, and disproved the validity of the apparent first-order conformity. The release rates were independent of film thickness and proportional to drug concentration in pure ethylcellulose films; in polyethylene glycol--ethylcellulose films, a positive deviation from linearity was observed. The logarithm of the rate constant was proportional to the fraction of polyethylene glycol in the film. Unlike in pure ethylcellulose films, the release rate in mixed films was altered by a change in the external fluid pH.

Chemotherapy was administered in the immediate postoperative period to seventy patients (52 with melanomas and 18 with sarcomas) after a total of eighty-seven major operations, with no morbidity or mortality traceable to the chemotherapy. There was no apparent interference with wound healing or what would be considered a normal postoperative course. Fourteen of these patients (5 with melanomas and 9 with sarcomas) received a combination of radiation anc chemotherapy initiated in the postoperative period, and it was tolerated well. This combination appears to be safe, provided the field of radiation is not so large that is may add significantly to the myelosuppressive effect of chemotherapy and the dosage of concomitantly administered radiopotentiating agent(s) is reduced. Sixteen patients had Bacillus Calmette-Gúerin (BCG) immunotherapy in the immediate postoperative period without complications. This policy of a tight interweaving of modalities is safe, has the theoretic advantage of an earlier concerted attack on microscopic residual tumor, and appears particularly promising in sarcomas.
The caries inhibitory effect of optimally fluoridated drinking water is well documented. Communities are often exposed to naturally fluoridated water with F content exceeding the optimal level. The purpose of this study was to determine the relationship between enamel fluoride, degree of fluorosis (DEGF) and caries incidence (DMFT) in an endemic fluoride area. Eighty-five 14- to 16-year-old children (37 males and 48 females) who were born and had been living continually in the town of Kenhardt in South Africa were selected for this study. The fluoride content of the common water supply is 3.2 ppm F. The DMFT and DEGF were determined clinically. Acid etch biopsies were carried out on both maxillary central incisors of each subject. Ca and F analysis were carried out and mass enamel expressed in microgram and mass F in pg. Data transformations to loge mass F (1nF), loge mass En (1nEn) and square root DMFT were made. The 1nF values were corrected to a standardized depth of 10 micrometer (208.6 microgram enamel equivalent mass) by covariance analysis. Correlations between the parameters square root DMFT, 1nF ad DEGF were calculated, adjusting for the sex of the individual. The correlations were as follows: (formula: see text). A positive association among the three parameters was established.
Two children who presented with severe failure to thrive were found to have Type III glycogen storage disease. They both also had defects of tubular acidification, an association not previously described. The nature of the tubular lesion is characterized and the explanation and therapeutic implications are discussed.

A drug delivery system is proposed constituted of spherical placebo granules as cores with polymeric surface films containing drug. This timed release dosage form has been prepared by means of a fluidized bed coating technique using ethyl cellulose as the polymeric film and caffeine and salicylic acid as model drugs. The release of the drugs from the dosage form (a) at different drug concentrations and (b) into solutions of different pH showed that drug release was linearly related to the square root of time. Good agreement was found between the theoretical release rate of caffeine, calculated according to Higuchi's equation for a homogenous matrix using membrane permeation parameters measured on linear films, and the experimental results in the case of low drug concentrations. Deviation of the release rate from the homogenous model at high drug concentrations could be explained by crystallization of the drug from the film.
Seventeen normal teenagers and eight asthmatic children were studied during submaximal exercise on a bicycle ergometer. A modification of a noninvasive multiple gas rebreathing method was utilized for the estimation of pulmonary capillary blood flow (Qc), diffusing capacity (DL), oxygen consumption (VO2), functional residual capacity (FRC), and pulmonary tissue plus capillary blood volume (VTPC). Significant increases in Qc, DL, VO2, and heart rate occurred in both groups of subjects. Regression equation of Qc equaled 6.0 + 0.0078 VO2 (ml/min) in the teenagers and 3.39 + 0.0082 X VO2 (ml/min) in the asthmatic children. The values of DL as a function of VO2 in both groups were similar to previously reported data. No significant changes in FRC or VTPC occurred in either group during exercise. This method appears to be an acceptable noninvasive way of studying exercise physiology in healthy or sick children.

Ovarian hyperstimulation syndrome was produced in rabbits by administration of human menopausal gonadotropin (hMG) and human chorionic gonadotropin (hCG). Histamine levels in the animals' plasma were determined by an enzymatic-isotopic assay. The results of this study show that there is no statistically significant difference between histamine levels in ovarian hyperstimulated animals as compared with control animals. Furthermore, no differences in the number of mast cells in the ovaries could be demonstrated between the 2 groups. It is concluded that histamine probably does not play a role in the pathogenesis of this syndrome. The relevance of this suggestion to other proposed mechanisms on the etiology of ovarian hyperstimulation syndrome is discussed.
To determine the age-related response of the remaining pulmonary parenchyma to lung resection, a matched set of 24 purebred beagle dogs underwent pneumonectomy at 6 to 10 weeks of age (group I) or at 1 year of age (group II). Eight unoperated adult beagles served as controls (group III). One year after pneumonectomy, pulmonary hemodynamics in group I were the same as those in older animals shortly after pneumonectomy and in normal control animals. Functional residual capacity, total lung capacity, single breath diffusing capacity for carbon monoxide, and static lung compliance were measured in all three groups while anesthetized and intubated. Lung volumes and diffusion capacity for carbon monoxide in group I did not differ significantly from those of control animals. Group I animals did have a lower lung compliance than the control group, but significantly greater than the group II adult pneumonectomy dogs. The adult pneumonectomy dogs (group II) had significantly lower values for lung volumes, diffusion capacity, and compliance when compared to the control group. Arterial blood gases were not significantly different among the groups. It is concluded that beagles undergoing early pneumonectomy have an increase of alveoli beyond the normal complement in the remaining lung with concomitant remodeling of the pulmonary capillary bed resulting in normal lung volumes, diffusing capacity, and gas exchange.
Although telephone calls comprise almost one fourth of all childhood patient-physician contacts, the content of telephone care is not emphasized in most educational and service programs. In response to the need to improve management of telephone calls to our pediatric emergency room, we developed 28 protocols to deal with the 25 most common complaints presented by phone. This paper describes the content of these protocols, the training of the health assistants who administered them, and the measures we took to assure their safety and general utility in pediatric practice settings. The study demonstrates the feasibility of an organized system for telephone care based on protocols which include: (1) basic data to be collected for each chief complaint category; (2) a range of appropriate dispositions; and (3) advice for home management when the patient does not require an immediate medical visit. Potential uses of these protocols for medical and nursing education and for clinical service needs are discussed.
The kinetics of sickling of malaria-infected red cells from humans with sickle cell trait were studied in vitro in an attempt to obtain direct experimental evidence for a selective advantage of the hemoglobin S heterozygote in a malarious region. The sickling rates of cells infected with Plasmodium falciparum and of non-infected cells were studied both in the total absence of oxygen (by dithionite addition) and at several different concentrations of oxyhemoglobin which might obtain in vivo. In all cases, red cells containing small plasmodium parasite forms (ring forms) sickled approximately eight times as readily as uninfected cells. Cells containing large parasitic forms (trophozoites and schizonts) appeared to sickle less readily than uninfected cells, by light microscopy criteria, but electron micrographs demonstrated the presence of polymerized deoxyhemoglobin S with a high frequency. It is concluded that enhanced sickling of plasmodium-infected AS cells may be one mechanism whereby the hemoglobin S polymorphism is balanced in favor of the heterozygote.
Treating wheat gluten, soy protein, and lactalbumin under alkaline conditions at 65 degree C for various times destroys part of the threonine, cystine, lysine, tyrosine, and arginine residues in these proteins. The losses were accompanied by the appearance of lysinoalanine and unidentified ninhydrin-positive substances. Treating wool under somewhat milder alkaline conditions destroys part of the cystine and lysine residues, but not the other amino acids cited. In this case, loss of cystine and lysine was accompanied by the appearance of lanthionine and lysinoalanine. Amino acid analysis of alkali-treated acylated proteins revealed that acylation by acetic and succinic anhydrides prevents or minimizes destruction of lysine residues and the formation of lysinoalanine in wheat gluten and soy protein and, in wool, minimizes destruction of cystine and lysine residues and the formation of lanthionine lysinoalanine. Mechanisms are proposed to explain the observed inhibiting effects of protein acylation and oertain additives on lysinoalanine formation.
