[Premature termination--a multifactoral phenomenon].

Twelve drop out cases in psychodynamic short term therapies according to the focus of cyclic maladaptive pattern by Strupp and Binder (CMP 1984) are reviewed regarding initial diagnostics, tape-recorded therapy sessions, and supervision records. Predictors of premature termination of therapy such as initially rated patients' therapy motivation, narcissistic personality traits, but also characteristics of interpersonal behavior of both patient and therapist are examined and discussed. Particularly important are the therapeutic alliance and narcissistic patients' difficulties to relate personally to their therapists instead of enacting projective dynamics of idealization and devaluation. Therapists frequently tend to respond to them by subconscious display of hostile countertransference. Four patterns of dynamics of therapy drop outs are described.

Nutritional management in diabetes and pregnancy.

During pregnancy it is necessary for the mother with diabetes to maintain careful control of the diabetes in order to lessen both fetal and maternal morbidity and mortality. The diet must take into account not only the metabolic requirements of the mother, but also those of the developing fetus. A total weight gain of 22 to 26 pounds is recommended, with the pattern of weight gain being more important than the total amount. Pregnancy is not a time for weight reduction. Caloric requirements can be met by the addition of 300 kcal. per day to the prepregnancy meal plan, or about 36 kcal. per kilogram. This can be met by the addition of 50 gms., carbohydrate and 30 gms. protein. Diets will contain approximately 1800 to 2500 calories a day. Calcium, iron, folic acid, and sodium also require attention in meal planning. The importance of regular meals and snacks must be emphasized. In particular, bed-time snacks are important because of the tendency towards overnight hypoglycemia and ketosis.

[Psychotherapy motivation and psychotherapy acceptance. Initial results of the Mannheim intervention study].

In our controlled longitudinal study of a representative high-risk sample of psychogenically impaired individuals, within a multidimensional model of mental health, the variables "life event", "social support" and "personality" are examined concerning their analysis of variance and their interdependence. We assume that a therapeutic influence on the variable "personality" causes a modified risk of becoming psychogenically impaired, and intend to prove this in terms of the altered maturity of defenses, the improved ego-functions, but also in terms of the improvement of the social network and the rarer constellation of critical life events. The modified effect on the variable "personality" is a differential offer of psychoanalytic therapy. By means of our instruments of investigation we tried to clarify if the acceptance of psychotherapy offered to resp. the motivational situation of our probands in regard to a psychotherapy covariates with certain sociodemographically or depth psychologically measurable personality patterns. There was a trend towards accepting the offer of therapy in case of high psychical and socio-communicative psychogenic impairment and in case of relatively high frankness on the FPI scale. The acceptance of an offer of therapy correlated highly significantly with the frankness scale of the Bond questionnaire extended for several defense items. The implications of this finding are discussed.

Dairy product analysis: identification of microorganisms by mid-infrared spectroscopy and determination of constituents by Raman spectroscopy.

Identification of microorganisms by traditional microbiological methods is time consuming. The German Federal Health Office has developed a method using mid-infrared spectroscopy to identify microorganisms rapidly. This method has been modified for application to microorganisms important in the dairy industry. Mid- and near-infrared spectroscopies are well-established methods for quantitative measurements of fat, protein, lactose, and solid content in a variety of products. A disadvantage of both methods is the huge absorption due to water; extraction of other components is complicated and can be achieved only statistically. With Raman spectroscopy, water causes less absorption. We investigated the use of Raman spectroscopy as a quantitative method for milk powder.
