[Physical urticaria].

The different types of physical urticaria are triggered by mechanical and thermal stimuli, as well as electromagnetic waves. Localized forms restricted to the skin and mucous membranes are most common, but generalized urticaria with variable extracutaneous manifestations can also occur. Physical urticaria is usually sporadic but may rarely have a familial form; it is often associated with chronic urticaria. In most instances, the short time interval between the physical stimulus and reaction points to a causal relationship, but in delayed types the exact diagnosis may be missed without provocation tests. The clinical implication of physical urticaria is demonstrated by investigations showing a greater degree of disability in affected patients as compared to other types of urticaria. There is still an incomplete understanding of the crucial pathophysiological aspects; most likely inflammatory reactions involving leukocytes, endothelial cells and nerves stimulated by various mediators play an important role in this form of urticaria.

The influence of atracurium, cisatracurium, and mivacurium on the proliferation of two human cell lines in vitro.

We tested the influence of atracurium and cisatracurium (final concentrations: 0, 0.96, 3.2, 9.6, 32, and 96 microM) on proliferation of human cells (hepatoma HepG2 cells and human umbilical vein endothelial cells) in vitro. In additional experiments, glutathione, N-acetylcysteine, or carboxyl esterase was added before the addition of either relaxant. The number of cells counted after 72 h of incubation was expressed as a percentage of the mean cell number in wells incubated without additives. Atracurium and cisatracurium progressively decreased cell proliferation in a concentration-dependent pattern. With human umbilical vein endothelial cells, atracurium or cisatracurium (3.2 microM) decreased the cell count to 67.7 % (SD, 14.8%) and 50% (SD, 8.6%), respectively. Cell proliferation was not inhibited by mivacurium. The results were similar to those with HepG2 cells. Glutathione, N-acetylcysteine, and carboxyl esterase partially reversed the effects of atracurium and cisatracurium. When incubated in a buffer with glutathione, atracurium decreased the number of glutathione-sulfhydryl groups. The findings that atracurium and cisatracurium inhibit proliferation of human cell lines in vitro, but that mivacurium does not, and that this effect is alleviated by glutathione and N-acetylcysteine, as well as by the carboxyl esterase, indicate that the inhibition may be caused by the reactive acrylate metabolites.

Ionized magnesium supplementation in critically ill patients: comparing ionized and total magnesium.

PURPOSE: The purpose of this study was to assess the effect of magnesium supplementation on total magnesium, ionized magnesium, ionized calcium, potassium, and pH in critically ill cancer patients and to compare the validity of the measurements. MATERIALS AND METHODS: Thirty-three consecutive critically ill patients receiving magnesium supplementation were placed in this prospective observational study at the Comprehensive Cancer Center, University Hospital. One gram (4.1 mmol) magnesium in 50 mL D5W was administered to critically ill patients, and the following were measured: total magnesium, ionized magnesium, ionized calcium, potassium, albumin, pH, BUN, creatinine, creatinine. RESULTS: Total magnesium and ionized magnesium increased by a mean of .11 +/- .02 and .05 +/- .01 mmol/L, respectively, after supplementation with 4.1 mmol of magnesium sulfate (P = .0001). Total magnesium, ionized magnesium, albumin, ionized calcium, potassium, and pH did not change significantly by the administration of 1 g of magnesium sulfate. CONCLUSION: The mean ionized magnesium (IMg+2) relationship to total magnesium (TMg) cannot be predicted before the supplementation with the available technology. After supplementation of 4.1 mmol/L the ionized magnesium level increased by .05 +/- .01 mmol/L. Magnesium supplementation had no significant effect on ionized calcium, potassium, and pH. TMg and IMg+2 should be followed independently.

[Adhesins of Acinetobacter strains].

One of the important factors contributing to the pathogenicity of bacteria is the presence of adhesins on cell surface, which facilitate colonisation in the macroorganism. The presence and type of adhesins occurring in four species of the genus Acinetobacter: A. baumannii (184), A. junii (59), A. lwoffii (65) and A. haemolyticus (22) was determined by haemagglutination test with a 3% suspension of fresh, tannic acid-treated of guinea pig, cow and human group O and AB erythrocytes, with or without the addition of one of sugar inhibitors (D-mannose, alpha-methylmannopyranoside, D-galactose-N-acetyl-D-glucosamine, L-fucose and D-ribose). In strains from all species, adhesines of the mannose-resistant (MR) type dominated. The mannose-sensitive (MS) type was present solely on the surface of one A. lwoffii strains. A. baumannii (36), A. junii (8), A. lwoffii (11) and A. haemolyticus (4) exhibited mannose-resistant hemagglutination in relation to fresh erythrocytes and that reaction was restrained by D-galactose, D-galactose and L-fucose (no other inhibitor used restrained it). The results achieved prove that cell adhesines other than those of MR type must be present on the cell surface. Additional adhesines occurred mainly in strains isolated from the respiratory and urinary tract infection simples, but were not found in isolates from blood cultures.

Local anesthesia is superior to spinal anesthesia for anorectal surgical procedures.

In this prospective study we compared local with spinal anesthesia for anorectal surgical procedures with regard to pain control, recovery time before unassisted ambulation, incidence of postoperative complications, length of hospital stay, and cost effectiveness in 80 consecutive patients. Patients were allocated in two groups: group 1 (n = 52) received local anesthesia, and group 2 (n = 28) had spinal anesthesia. There were no intraoperative complications related to the anesthetic technique, and there was no difference between groups in the number of doses of narcotics required to control postoperative pain (1.2 +/- 1.5 vs 1.8 +/- 1.7 in group 1 and 2 respectively, P > 0.05). Recovery time before unassisted ambulation was significantly longer in group 2 (139 +/- 96 minutes in group 2 vs 82 +/- 62 minutes in group 1, P < 0.05). There were 21/52 complications in group 1 in contrast to 21/28 in group 2, (P < 0.05). There was no difference between groups in the postoperative incidence of nausea, vomiting, headache, weakness, and constipation; however, the incidence of postoperative urinary retention was significantly higher in group 2 (5/52 in group 1 vs 9/28 in group 2, P < 0.05). As a result of urinary retention, more patients in group 2 required overnight hospitalization (12/52 in group 1 vs 21/28 in group 2, P < 0.05). Patients in group 2 required 36 hospital days in contrast to 21 days for patients in group 1, P < 0.05. The difference in hospital days resulted in $18,000 greater cost for patients in group 2.(ABSTRACT TRUNCATED AT 250 WORDS)

[Hemolytic properties of bacteria from the genus Acinetobacter].

Hemolytic activity of 370 strains of Acinetobacter was investigated. They represented species A. baumanii (195), A. junii (70), A. lwofii (71) and A. haemolyticus (27) and were isolated from hospitalized patients. Direct and intermediate hemolysins were tested in the plate test with application of human, sheep, bovine and rabbit blood, incubating cultures at 22 and 37 degrees C for 3 days. Hemolysins of A. lwoffii were more active at 22 degrees C against rabbit and sheep erythrocytes. Direct hemolysins produced by remaining species of Acinetobacter hemolyzed most easily human erythrocytes. Out of 27 strains of A. haemolyticus, 26 produced hemolysins. Intermediate hemolysins, produced only by A. junii (43% of strains) and A. haemolyticus (37%), were active only against animal erythrocytes. A. baumanii and A. lwofii produced only intermediate hemolysins and percentage of strains producing them was, respectively, 3.6 and 7.0.

[Occurrence of species of the Acinetobacter genus in material from patients and in the hospital environment].

370 clinical strains of genus Acinetobacter were classified in accordance of taxonomy of this group of bacteria introduced by Nouvet and Grimont (1986). Most frequent species isolated was A. baumanii (195), followed by A. lwoffii (71), A. junii (79) and A. haemolyticus (27). A. calcoaceticus and A. jonsonii were present only sporadically. A. baumanii was isolated from bronchial secretions (36.9%), sputum (20.5%), blood (8.7%), pus (8.2%) and ur@ine (8.2%). A. lwoffi was isolated above all from throat smears (26.8%), sputum (16.9%), bronchial secretions (16.9%) and urine (16.9%). A. junii was present in sputum (25.7%), blood (17.6%), throat smears (17.6%), bronchial secretions (13.5%) and urine (12.2%).

Susceptibility of isolates of Acinetobacter anitratus and Acinetobacter lwoffii to the bactericidal activity of normal human serum.

The bactericidal activity of normal human serum against the Gram-negative coccobacilli Acinetobacter anitratus and Acinetobacter lwoffii was studied; 12% and 84%, respectively, of the tested strains appeared to be sensitive. Thus, serum resistance may be an important factor contributing to the pathogenic potential of A. anitratus strains. Three types of bactericidal action were shown. In the first, the strains were killed when the alternative complement pathway was activated. In the second, some strains required both the classical and alternative pathways. In the third variant, the strains needed either the alternative or classical activation pathway.

[Sensitivity of bacteria of the genus Acinetobacter to antibiotics and ofloxacin].

Between 1989-1989 276 strains of Acinetobacter genus were isolated which contained: Acinetobacter calcoaceticus subsp. anitratus (n = 167), Acinetobacter calcoaceticus subsp. Iwoffi (n- = 83), Acinetobacter haemolyticus (n = 26). Their sensitivity to aminoglycoside antibiotics, beta-lactams, tetracyclines, chloramphenicol, colistin, and ofloxacin was tested. More than 90% of strains were sensitive to colistin and ofloxacin. The sensitivity to remaining antibiotics differentiated depending on species. Acinetobacter anitratus were highly resistant to Ist and IInd generation of cephalosporins, and moreover to penicillins, tetracyclines, and chloramphenicol. Cephalosporins of IIIrd generation were active against 70% of strains with exception of cefoperazone what was also the case for representatives of aminoglycosides as netilmicin and amikacin. Strains of Acinetobacter Iwoffi were in majority sensitive to all antibiotics with exception of cephalothin, cephradine and cefoperazone. More than 90% of Acinetobacter haemolyticus strains were sensitive to gentamicin, carbenicillin, azlocillin, ceftriaxone, cefotaxime and tetracyclines.

Detection of terminal deoxynucleotidyl transferase (TdT) by flow cytometry in leukemic disorders.

We applied a new technique to the detection of intracellular TdT in 26 leukemic patients, including 16 non-T acute lymphoblastic leukemia (ALL), four T-ALL, one T-lymphoblastic lymphoma in leukemia phase, one undifferentiated leukemia, one de novo lymphoblastic phase of chronic myeloid leukemia, and three acute monocytic leukemias (AMOL). Mononuclear cell suspensions were incubated in saponin to permeabilize the cell membrane. The cells were then stained by indirect immunofluorescence (IF) using anti-human TdT monoclonal antibodies and were analyzed by flow cytometry (FCM). The TdT results were compared with those obtained by biochemical TdT assay (26 cases), immunoperoxidase determination (PAP) (12 cases), and fluorescence microscopy (seven cases). The results obtained by PAP and fluorescence microscopy were 100% concordant with those obtained by FCM and biochemical assay. TdT determination by FCM allows the analysis of large numbers of cells in a fast, objective, and reliable manner, as compared with biochemical assay, PAP, and fluorescence microscopy determinations.

Evaluating the implementation of health policy: pre-hospital care in Pennsylvania.

A statewide study of ambulance services was conducted to inform executive and legislative decision making regarding Emergency Medical Services in Pennsylvania. The study focused on the training status of ambulance personnel, turnover, and coordination between state and local levels. The study found indications that adequate numbers of trained personnel were often not making ambulance trips. A variety of issues that might cause and solve this problem are discussed, as are the dissemination and utilization of the study's results.

Simultaneous colpo-recto-sacropexy.

The literature concerning coexisting massive genital and intussuscepting rectal prolapse is reviewed, two cases in point are presented, and the rationale for concurrent surgical management is offered. The technique of combined colporecto-sacropexy is described in detail.

[A method for the determination of oxygen uptake and other parameters in the conscious rabbit].

An equipment for the continuous recording of the oxygen consumption and other parameters in the awake rabbit is presented. Construction, function, advantages, and failings were described. More than 300 successful animal experiments were carried out by means of this equipment till now. It is characterized by a clear structure and a simple operation and has a stable function. Important indications are the course and the therapy of the shock, the energy transformation in the postoperative respectively the posttraumatic phase, and the influence of drugs on the metabolism.

Quantitative analysis of intramembranous particles in the membranous system of rat liver cells at different stages of development and aging.

The density of intramembranous protein particles was studied by freeze-fracture. Particle density on the fracture faces of the plasmalemma and the rough endoplasmic reticulum (RER), as well as the outer and inner membranes of the nucleus and the mitochondria in rat hepatocytes were quantified. Comparison among different age groups sampled days postcoitum (dpc), days postpartum (dpp), and months postpartum (mpp) showed age-related changes in particle density in each membrane system. With the exception of the RER, particle densities increased after the 16th dpc, reached a maximum at birth, and then decreased with increasing age. Simultaneously, the number of nuclear pores shows a positive correlation with the particle density of the nuclear membranes. The particle density on the membranes of the RER shows a maximum on the 16th dpc and on the 6th dpp. Thereafter, the density of the RER decreases slightly. In all membrane systems, the density of the particles on the external fracture faces is more variable than density of the particles on the protoplasmic fracture faces.
