A new technique for hemodilution, preparation of autologous platelet-rich plasma and intraoperative blood salvage in cardiac surgery.

We have developed a new system for the production of autologous platelet-rich plasma and red blood cell concentrates to be used in autologous transfusion support of cardiac surgery patients. In 15 operations no homologous blood products were required. Costs were diminished since with the same harness it was possible to carry out the intraoperative blood salvage and concentrate the erythrocytes contained in the oxygenator and its lines. Indirect costs were also reduced since no infective complication was observed due to homologous blood products.

[Esophagogastroplasty in the treatment of squamous carcinoma of the esophagus (our experience)].

The authors report on their experience with 42 cases of squamous carcinoma of the oesophagus subjected to oesophagectomy and oesophagogastroplasty. The patients' mean age was 55.2 years (range: 41-70) with a 13:1 male/female ratio. A positive history of cigarette smoking and of alcohol consumption was found in 87% and 80% of patients respectively. The sites of the neoplasms were as follows: 24 in the mid 3rd of the oesophagus, 9 in the lower 3rd, 6 in the upper 3rd of the thoracic oesophagus and 3 in the cervical oesophagus. In 39 patients the operation was performed in two stages according to Lewis and in 2 cases it was performed in 3 stages according to McKeown, while one patient was treated with a pull-through operation according to Akiyima. Six patients died during the first 30 days postoperatively as a result of circulatory complications or ARDS. Fistulization of the anastomosis as observed in 5 cases, though this was clinically manifest only in two. Mean survival was 5.9 months, 18.6 months and 10.5 months according to the period considered.

Cultivation of a pig parvovirus in various cell cultures.

The susceptibility of several established cell lines of pig (LLC-PK1 = pig kidney; MPK = minipig kidney; PK15 = pig kidney; ESK = embryonic swine kidney), bovine (EBTr = embryonic bovine trachea), monkey (MA-104 = fetal rhesus monkey kidney) and human (HEL-299 = embryonic human lung) origin to porcine parvovirus was studied. The primary pig kidney cell cultures (pPK) were included in the study as the reference cell system. From the results it appeared that the virus only replicated in cell lines originated from swine. In particular the MPK and ESK cell lines showed a susceptibility similar to that observed for pPK cell cultures. Intranuclear inclusions and plaques were also induced in these cell systems. It appeared therefore that MPK and ESK cell lines both possess all the requirements for use in pig parvovirus studies.

[Preoperative evaluation of the patient candidate for surgical intervention on the chest: methods of study and risk factors].

The authors review the main methods designed to evaluate operability and surgical risk in candidates for chest surgery. They also report on their own experience with 135 cases operated on for thoraco-pulmonary diseases requiring surgery; in these cases, postoperative complications were studied in relation to various respiratory parameters evaluated differentially and as a whole prior to surgery. This study enabled the investigators to establish that patients with CV, FEV1, FEF25-75 and Tiffffeneau Index values below 60% of the respective theoretical reference values run a very substantial risk of postoperative complications, with a post-surgical morbidity rate of more than 60% in such patients. Similar risks are run by patients with VR values below 60% or above 120% of predicted values or with Emphysema Index values above 120% of predicted values. The author's experience with fractional bronchospirometry revealed that, in the high-risk patient sample, a predicted postoperative FEV1 value of only 800 ml/sec is a poorly selective limit.

Temperature effect on subclasses of muscarinic receptors in rat colon, heart and cerebral cortex.

The high-affinity muscarinic antagonist /3H/-Quinuclidinyl benzilate (/3H/-QNB) has been used to label muscarinic receptors in a crude membrane fraction of rat cerebral cortex, colon and heart. The inhibition of /3H/-QNB binding by Atropine, Oxotremorine and Pirenzepine was investigated at three temperatures: 37 degrees C, 22 degrees C and 10 degrees C. The IC50 values and the proportion of high (Rt1) and low (Rt2) affinity binding sites were determined for the three compounds. When the temperature were lowered from 37 degrees C to 10 degrees C, in the agonist and antagonist dissociation constants decreased in all tissues. Changes in temperature did not modify Rt1 or Rt2 values for Oxotremorine and Pirenzepine. The results show marked temperature-dependent modifications of IC50 values for muscarinic receptors of high- and low-affinity sites in rat cerebral cortex, colon or heart.

Antigen specific, suppressor cells induced by the immunomodulator FCE 20696 in aged NZB/WF1 mice.

FCE 20696 is a new synthetic immunomodulating agent, active on some manifestations of the autoimmune disease of NZB/WF1 mice, in which a disorder of T suppressor cells (SC) has been described to be of pathogenetic relevance. The ability of FCE 20969 to induce SC in NZB/WF1 mice, which are then able to specifically inhibit the production of anti autologous red blood cells antibodies (aRBC Ab), was investigated. NZB/WF1 mice were chronically treated with the compound starting from the ninth week of age, sacrificed at different times and their spleen cells transferred to 9 weeks old, syngeneic mice. In donor animals SC were induced by rat RBC, whereas in the recipients the suppressive activity of transferred cells was evaluated from the appearance of aRBC Ab induced by the same antigen. The results show that antigen specific SC were present in both FCE 20696 treated and control young animals. In older controls, SC couldn't be induced, whereas in treated animals SC were present and able to transfer suppression into the recipients. FCE 20696 was active at 1.5 mg/Kg.

Peripheral neuropathy in Cockayne syndrome.

Two siblings with Cockayne syndrome are reported. In one case a sural nerve biopsy showed a demyelinating peripheral neuropathy with occasional inclusions in Schwann cells made up of electron dense finely granular material intermingled with vacuoles or lamellar structures. The significance, if any, of this accumulated material remains unclear. The presence, in addition, of small finely lamellar intra-axonal osmiophilic bodies suggests an associated axonal involvement.

Plasma and tissue levels of flecainide in rats.

The time-courses of flecainide plasma and tissue levels were studied in Wistar male rats after i.v. administration (4 mg/Kg). Drug assay in plasma and tissue was performed with a specific and accurate HPLC technique. The final half lives in plasma and tissues were about 4 hours, except in the brain where the half life value was 9.8 hours. The mean tissue/plasma (T/P) ratios in myocardial, kidney, liver, skeletal and muscle tissues were 9.11, 13.8, 14.37, 6.31 respectively, while in the brain the T/P ratio rose progressively over the sampling time to 10.0. These data suggest that flecainide may accumulate in the central nervous system during prolonged treatment. Flecainide levels in adipose tissue were very low. Finally, there was an early "bulge" in the concentration curve, possibly reflecting enterohepatic circulation or non-linear elimination kinetics.

A study of a herpesvirus isolated from dairy cattle with a history of reproductive disorders.

Three strains of herpesvirus were recovered from cows with vulvovaginitis. The three isolates (85/BH 16TV, 85/BH 17TV, 85/BH 18TV), when compared by cross serum neutralization (SN) tests, were found to be antigenically identical. They were serologically distinct from infectious bovine rhinotracheitis (IBR) virus and Bovid herpesvirus 2 (BHV2), while they cross reacted with bovine herpesvirus DN-599. Besides the serologic aspects, the three isolates appeared to share common biological, physical and morphological properties with the newly recognized bovine herpesviruses, of which DN-599 is a representative strain.
