A variation of the median nerve at the level of brachial plexus.

We report a variation of the median nerve at the level of brachial plexus in a 38 years old male cadaver. The median nerve was formed by the fusion of three branches; two of them coming from the lateral cord, and one from the medial cord. The abnormal root coming from the lateral cord had a very close oblique course over the axillary artery. These kinds of variations are more prone to injury in radical neck dissections and in other surgical operations of the axilla. Secondly, the very close course of the unusual branch with the axillary artery may lessen the blood supply of the upper extremity by compressing the vessel.

An abnormal digastric muscle with three bellies.

During the gross anatomy dissection of the submental region, an anatomical variation of the right digastric muscle was found. This muscle had three bellies. Whereas the anterior and posterior bellies had their normal origin and course and were joined by an intermediate tendon, the accessory anterior belly originated from the digastric fossa and a thin tendon together with the anterior belly was inserted onto the hyoid bone. This kind of variation can easily be confused with pathological conditions. For this reason, knowledge of the muscular anatomy and variations of the floor of the mouth is helpful when evaluating the base of the skull with CT and MR imaging.

The in-vitro and in-vivo characterization of PLGA:L-PLA microspheres containing dexamethasone sodium phosphate.

Dexamethasone sodium phosphate (DSP) is a widely used corticosteroid in the treatment of brain oedema associated with brain tumours. DSP has many side effects that limit its usage at an effective concentration. The objective of this study was to minimize these side effects by encapsulating DSP using biodegradable synthetic polymers, to extend the release time from microspheres and to evaluate the effectiveness in the treatment of brain oedema. Microspheres containing 5% DSP were formulated by the solvent evaporation method by using a 1:1 mixture of two synthetic polymers, poly(lactic-co-glycolic acid) and L-polylactic acid (PLGA and L-PLA). The surface morphologies and particle size distribution of the microspheres were investigated. The in-vitro release studies were performed in pH 7.4 phosphate buffer solution. For determining the effectiveness of microspheres in the treatment of brain oedema, Sprague-Dawley rats weighing 200-250g were used as an animal model. Brain oedema was generated by the cold lesion method, and the effectiveness of the microspheres in treatment of oedema was investigated by the wet-dry weight method, lipid peroxidation ratios and histological evaluations. The average particle size of the microspheres was 13.04 +/- 2.05 microm, and the in-vitro release time of the microspheres was 8 h for 100/release. The degree of oedema was significantly different from the control group for the wet-dry weight method and lipid peroxidation ratio (p < 0.05). Similarly, histological evaluation of the tissues shoved that degree of oedema was significantly decreased with respect to the control group. All these results showed that implantation of microspheres was significantly more effective with respect to the systemic administration of DSP.

Microleakage of two polyacid-modified resin composites under different conditioning methods in primary teeth.

This study evaluated the effect of different conditioning methods on micro-leakage in primary teeth. Fifty-one cervical cavities were prepared on the twenty-six extracted primary molars and divided randomly into five groups. In Group I and IV phosphoric acid, in group 11 non-rinse conditioner (NRC) was applied. In group III and V no pretreatment was used. Groups I-III were restored with Prime & Bond NT and Dyract AP and Group IV-V restored with Syntac SC and Compoglass F. The restored teeth were stored for 24 hours in distilled water at 37 degrees C, immersed in 0.5 percent basic fuchsin for 24 hours. Dye penetration was determined using a stereomicroscope. The results revealed that although enamel microleakage was not affected by the conditioning method used in both restorative materials, pretreating the cavity with phosphoric acid had a positive effect on reducing the dentin microleakage compared with conditioning by NRC.

Primary incisor triplication defect.

Conjoining abnormalities occur in less than 1 percent of the population. Triplication of primary teeth is even a rarer phenomenon with a prevalence of 0.02 percent. This study describes the clinical features and the light microscopic appearance of a case of triplicated primary teeth. The teeth with the conjoined defect were examined by light microscopy to evaluate the exact level and appearance of the fusion area. The light microscopic examination revealed grooves and cracks consisting of enamel and dentine in the fusion area, fusion was irregular and both dentin and cementum were confluent. It is concluded that the clinician should identify children were conjoined anomalies and should be aware of the oncoming problems.
