Fine structure of the human coccygeal body: a light and electron microscopic study.

There is only limited data on the microscopic structure of the coccygeal body in classical textbooks. Although there are several articles describing the light microscopic structure of the organ, the data presented are rather conflicting especially when comparing glomus tumors and normal coccygeal bodies. We therefore examined the pericoccygeal soft tissues histologically with the aim of obtaining further evidence. At the light microscopic examination, coccygeal bodies were distinguished as sections of several blood vessels encapsulated by a connective tissue capsule. Small arteries within the coccygeal bodies had an unusual endothelial lining, resembling pseudostratified epithelium or neuro-epithelium. We observed increased numbers of glomus cells in a few samples. A highly tortuous course run by the small arteries was observed in the serial sections. At their electron microscopic examination, endothelial cells exhibited the features of columnar epithelial cells, though some appeared to be vacuole rich. The basal surface of the cells showed basal infoldings, but not as extensive as those of absorptive cells. External lamina, subplasmalemmal vesicles, bundles of microfilaments, groups of membrane bound organelles including mitochondria and endoplasmic reticulum were observed in glomus cells. In conclusion, it is rather difficult to use the term tumor for the coccygeal bodies when relying solely on the number of cells observed, because some segments of the coccygeal bodies were rich in glomus cells. It is more likely that the coccygeal bodies show variability in different individuals. The characteristics of the endothelium of the small arteries in coccygeal bodies needs to be examined in a wider range of specimens in order to be validated as a unique entity.

Early protective effects of Iloprost after experimental spinal cord injury.

This investigation was undertaken to study the early protective effects of Iloprost, a stable analogue of prostacyclin, after spinal cord injury in rabbit. Sixteen adult male rabbits (New Zealand Albino) were injured by application of epidural aneurysm clip. Eight rabbits received an intravenous (i.v.) infusion of 30 micrograms kg-1 Iloprost, and eight rabbits received an infusion of saline (SF). Treatment with Iloprost started immediately after spinal cord injury and continued for one hour. Evoked potentials were recorded for each rabbit at one, 15, and 60 minutes after the spinal cord injury. Twenty-four hours later, all the rabbits were deeply anesthetized and spinal cords were removed for histopathological examinations. There was no meaningful statistical difference between cortical somatosensorial evoked potentials (CSEP) of the saline and Iloprost group. However, light and electron microscopic studies showed that the Iloprost treated group had moderate protection of myelin and axons; and limited edema. These results suggest that intravenous Iloprost treatment after spinal cord injury has a highly protective effect without any side effects.

Comparative study between corpus cavernosum-electromyography findings and electron microscopy of cavernosal muscle biopsies in erectile dysfunction patients.

BACKGROUND: Biopsy and electrical activity recordings of the corpus cavernosum are 2 new diagnostic methods for the evaluation of impotent men. We evaluated the corpus cavernosum ultrastructure and electromyography (EMG) recordings from patients with erectile dysfunction. METHODS: Twenty erectile dysfunction patients with veno-occlusive dysfunction underwent a detailed history, physical examination, biochemical tests, hormonal analysis, injection of an intracavernous vasoactive agent (60 mg papaverine-HCl), color penile Doppler ultrasonography, cavernosometry/ cavernosography and corpus cavernosum electromyography (CC-EMG). Thirteen patients underwent total vein ligation and 7 had penile prosthesis implantations. Tissue samples were obtained during surgery from both corpora cavernosa and examined by transmission electron microscopy. Control corporal tissue samples were taken from 3 cadavers. RESULTS: In 15 patients, CC-EMG recordings were 15.6 +/- 0.65 microV in the flaccid state, which decreased in 13 patients after papaverine (5.61 +/- 0.25 microV; P < 0.001). Five patients with diabetes mellitus had low amplitudes in the flaccid state (5.26 +/- 0.45 microV), which did not vary significantly after a papaverine injection (4.99 +/- 0.75 microV). The pathology of the corpus cavernosum biopsy specimens revealed a smooth muscle cell thickened basal membrane, dilated rough endoplasmic reticulum, and increased numbers of fibroblasts, but ultrastructurally normal endothelial cells lining the sinusoids. There was no difference between samples from diabetic or nondiabetic patients, or from either side of the corpora cavernosa. The only pathologic change observed in the controls was mitochondrial swelling. CONCLUSION: CC-EMG is less invasive and a valuable method in patients with erectile dysfunction, whereas no specific findings were observed from penile biopsy specimens.

Ultrastructure of the cornea epithelium in keratoconus.

In this study, 20 keratoconus corneal tissue were examined. In TEM examination of the cornea, the epithelium was irregular in thickness. In these epithelial cells, the number of microvilli were decreased and they showed an irregular arrangement. Vesicular degeneration and swelling of the mitochondria were observed in the cytoplasm of these cells. Interepithelial relation was normal and the desmosomes did not show any pathology. In the electron microscopic examination of the stroma, focal degeneration was observed in the collagen fibers. In SEM examination of the cornea, depressions and elevations were found on the epithelial surface. Additionally, degenerated epithelial cells were observed on focal areas. To our knowledge, the decreased number of microvilli and their irregular arrangement which we observed in TEM examination of the cornea epithelium were not reported in the literature.

Gross anatomic and scanning electron microscopic studies on the mandibular condyles of the human cadavers.

The mandibular condyles of 30 cadavers were examined bilaterally. The greatest medial-lateral lengths and greatest anterior-posterior widths of these condyles were measured with a millimeter caliper. The mean size of the greatest medial-lateral length of the superior surface of the condyle between males and females was significantly different. On the other hand, the mean size of the greatest anterior-posterior width of this surface between males and females was not significant. In gross anatomic examination, 28 of the 60 condyles (46.6%) had a convex articulating surface without having any surface irregularities. However, 32 of the 60 condyles (53.4%) demonstrated irregular articular surfaces with characteristic flattening and eburnation. Half of these surface irregularities were found in the lateral part and a few of them were located in the medial part and superior surface of the condyles. Short, irregular undulations, surface depressions and sharply demarcated microcraters were observed in the SEM examination of mandibular condyles having macroscopic abnormalities. These osseous abnormalities were present in the mandibular condyles of people, who were over the age of 50 years and they were very prominent especially in the ones who were completely edentulous. Therefore remodeling of the articular surface might occur in response to alteration of functional forces within the joints of these aged individuals.

Unreported anatomical variation of septum pellucidum.

During gross anatomy dissections of the brain, a developmental abnormality of the septum pellucidum was found in a 31-year-old male cadaver. Other parts of the central nervous system in this cadaver were normal in every aspect. Histological samples were taken from the neighboring areas of this abnormality, and they were examined under light microscope and scanning electron microscope. In this abnormality of the septum pellucidum, the two laminae of the septum pellucidum were fused together and there was a hole located 1 cm anterior to its apex. The maximum diameter of the hole was 0.5 cm in the sagittal plane and 0.6 cm in the vertical plane. In the light microscopic and scanning electron microscopic examinations, the free margin of this foramen was regular, and the surrounding tissue was intact and histologically unique to the septum pellucidum. Ependymal cells were present at the free margin of the foramen. Cavum vergae, cavum septum pellucidum, and agenesis of the septum pellucidum are described in the literature. These three abnormalities are seen in cadavers usually with histories of schizophrenia and other psychiatric or neurologic disorders. In a retrospective study, the cadaver with this abnormality had a history of schizophrenia and no history or signs of any kind of brain or head operation. As far as we could ascertain, the abnormality described here has not been reported previously.

Double superior gemellus together with double piriformis and high division of the sciatic nerve.

We report a case with double superior gemellus and double piriformis mm. associated with the sciatic n. dividing high and passing between the two piriformis mm. in the same lower extremity. This abnormality has not previously been described in the literature. As many musculoskeletal structures may be involved in sciatica, the supernumerary superior gemelli and piriformis mm. may exert pressure on the sciatic n. and this should be taken into account by clinicians.

An anatomic study of the lateral femoral cutaneous nerve.

In this study, the course of the lateral femoral cutaneous n. was examined bilaterally in 22 cadavers. Seven of these 44 lateral femoral cutaneous nn. showed variations in their course, especially in their number of branches under the inguinal ligament. During operations where the lateral femoral cutaneous n. may be demaged or in its decompression surgery, the nerve is to be found under the inguinal ligament. 1.52 +/- 0.84 cm medial to the anterior superior iliac spine. This nerve can be found passing through the inguinal ligament in as many as four branches.

The marginal mandibular branch of the facial nerve.

The peripheral, extraparotid course and localisation of the marginal mandibular branch of the facial n. is described, with variations, based on the dissection of 40 cadaver half heads. Its anatomical relationships with the ramus of mandible and facial a. are studied and morphometric features are reported. Knowledge of the accurate course and relationship of the marginal mandibular branch should help to protect this nerve from surgical injury.

Electron microscopy of the senile changes in lens epithelium.

Lens epithelia obtained from 37 patients who had undergone cataract surgery operation were examined by using transmission electron microscope. Their ages varied between 60-79 years and mean age was 70.2 years. Additionally, the lens epithelia of 14 patients, which were obtained in the same way were examined by using scanning electron microscope. Their ages varied between 58-78 years and mean age 70.7 years. In transmission electron microscopy, the normal appearing epithelial cells were intermingled with abnormals and the abnormal cells increased in number and in degree of abnormality with aging. The three dimensional structure of the interdigitating interlocking processes of lens cuboidal cells were visualised by using scanning electron microscope.

Histopathologic effects of mitomycin-C on endoscopic transnasal dacryocystorhinostomy.

BACKGROUND AND OBJECTIVES: The antimetabolite mitomycin-C is now being used in endoscopic transnasal dacryocystorhinostomies to prevent the closure of osteotomies. PATIENTS AND METHODS: A 0.5-mg/ml solution of mitomycin-C was applied to the osteotomy site for 2 1/2 minutes intraoperatively. Specimens from four patients were collected during surgery and at 15 days, 1 months, 3 months, and 6 months after surgery. The specimens were examined under light and electron transmission microscopy and were compared with control specimens. RESULTS: Light microscopy showed attenuated epithelium with intracytoplasmic vacuoles. Subepithelial connective tissue was looser and hypocellular. Electron microscopy confirmed these findings and demonstrated swelling of mitochondria, dilatation of endoplasmic reticulum, and chromatin-dense granules in nuclei of fibroblasts. CONCLUSION: By causing a decrease in density and cellularity of mucosa, topical use of mitomycin-C may enhance the success of surgery; however, further studies are necessary to determine the ultimate potential of this agent for this procedure.

A scapula with an unreported canal.

During the practical lectures in our department, we encountered an abnormality on the left scapula of an adult human. In this case, an elongated canal was found just posterior and inferior to the coracoid process and the scapular notch was not observed on this bone. Knowing about the anatomic variations of this region may have an importance for the surgeons.

An unusual origin for the accessory head of biceps brachii muscle.

During the superficial dissection of the pectoral region and the arm, an abnormal biceps brachii muscle was observed unilaterally. This muscle had three heads. Whereas the short and long heads had their normal origin, the accessory head originated from the anterior surface of the distal part of the pectoralis major muscle, which formed the anterior axillary fold. To our knowledge, this variation has not been previously described. Innervation and vascular supply of this accessory head was from the musculocutaneous nerve and the brachial artery, respectively.

Examination of joint space by magnetic resonance imaging in anatomically normal knees.

The medical and lateral knee joint spaces of 184 patients who had anatomically normal knees were measured by using magnetic resonance imaging (MRI) method. The findings were compared according to age, sex, height, and body mass index changes of the individuals and the mean values of medial and lateral knee joint spaces were calculated in every group. The results show that in an anatomically normal population, all the individuals have larger lateral knee joint spaces than medial knee joint spaces. The patients lose knee joint space regularly with increasing age. Children have larger knee joint spaces than adults. Men have larger knee joint spaces than women. The knee joint space size of the patients increase regularly with increasing height up to 180 cm. The patients, who are taller than 180 cm do not show any marked difference in joint space size, when compared with the patients whose heights differ in between 171-180 cm. Knee joint space size is not related to the body mass index of the individual.

An anomalous course of the interazygos vein.

During an anatomic dissection of the posterior mediastinum, an unusual course of the interazygos v. was noted. The hemiazygos v. ascended to the 8th thoracic vertebra, in front of the vertebral column. It left the column, ascending obliquely, and passing in front of the aorta and behind the oesophagus. This transverse segment, crossing the midline, is called the interazygos v. It may be confused with different lesions situated at this level: aneurysm, tumour or lymph node hypertrophy, during CT or MRI studies. Secondly, the surgeon must be aware of this azygos variation as a possible cause of haemorrhage during a procedure in this region.

A complicated variation of the upper extremity vascularisation.

In the right upper extremity of a cadaver, the radial artery originated from the medial side of the brachial artery at a higher level than usual, which was 16.7 cm below the coracoid process. Then, this artery crossed over the brachial artery, passed to its lateral side and ran to the forearm in front of the aponeurosis of the biceps brachii muscle. The brachial artery ended 1 cm distal to the elbow joint by dividing into the ulnar, anterior interosseous and posterior interosseous arteries. The common interosseous artery was absent. As the radial and ulnar arteries may be used as microvascular donor or recipient vessels, a good knowledge of the arterial vascularisation of the upper extremity is essential in planning surgical and reconstructive procedures in this area. Secondly, the course of the radial artery in our cadaver may be mistaken for a vein and an intravenous injection in such an artery can result in the loss of the hand or forearm.

High brachial artery bifurcation: a report of 2 cases.

High division of the brachial artery was observed in two cadavers, during routine dissection of upper extremities. In the first case, the brachial artery of the right upper extremity was divided into its two terminal branches immediately after passing between the lateral and medial roots of the median nerve and just below the origin of the profunda brachii artery. The lateral branch was the radial artery, located in the space normally occupied by the brachial artery and the medial one was the ulnar artery. In the second case, the brachial artery was divided into its two terminal branches just below the origin of the profunda brachii artery. Accurate knowledge of the relationships and course of these major arterial conduits and particularly of their variational patterns, is of considerable practical importance in the conduct of reperative surgery in the arm, forearm and hand.

Bitendinous palmaris longus muscle.

The palmaris longus is one of the most variable muscles and this should be considered in surgery of the wrist where good orientation is essential. This short note describes an unusual variant which was discovered by Hacettepe University medical students, during routine dissection of the upper limb. Since the palmaris longus provides a very useful graft in tendon surgery, the surgeon should be fully aware of its anatomical variation.
