The Beckwith-Wiedemann syndrome in a six year old boy.

A 6 year-old boy who has many of the physical manifestations of the Beckwith-Wiedemann syndrome has been reviewed regularly because of the increased risk of developing intra-abdominal neoplasms, hormonal and biochemical disorders and facial deformity.

Beckwith-Wiedemann Syndrome↗

Covert muscle injury in aged patients admitted to hospital following falls.

Ten patients (mean age 85 years) had elevation of serum creatine kinase and other enzyme levels when admitted to hospital after falling at home and lying on the floor. No patient had definite clinical evidence of muscle injury but isoenzyme studies showed that the creatine kinase originated from skeletal muscle. A high ratio of peak creatine kinase to aspartate transaminase levels also helped distinguish skeletal from cardiac muscle damage. Several patients had minor disturbance of renal function and one developed acute renal failure.

Evaluation of the morphanalysis machine in a clinical setting.

The experience of installing and using an Analytical Morphograph in a plastic surgical clinical setting is described. Initial problems have been resolved in safety and routine functioning. Although we have not managed to obtain the consistency of results claimed in the research field we have found the unit useful in the pre-operative, operative and follow-up phases of clinical patient management.

Cephalometry↗

The Chepstow feeding aid.

A specially designed spoon or scoop has been developed to enable controlled amounts of semi-solid food to be accurately and easily inserted into the mouth. This device simplifies feeding following major surgery for intra-oral cancer. Its use and other possible applications are described.

Serum pancreatic enzymes in the elderly.

Immunoreactive trypsin (IRT) and pancreatic lipase were measured in serum from 37 geriatric in-patients (median age 77.5 years) without history of diarrhoea, pancreatic disease, diabetes or acute or chronic alcoholic intake. IRT and pancreatic lipase concentrations/activity were strongly correlated and were markedly elevated when compared with a control population of 22 subjects (median age 27 years). Such elevations in pancreatic enzymes indicate a subclinical damage of exocrine pancreatic tissue. Assessment of exocrine pancreatic function in the geriatric age group should not be based on pancreatic enzyme levels in serum.

A one-stage correction of mandibular defects using a split sternum pectoralis major osteo-musculocutaneous transfer.

This paper describes a technique of mandibular reconstruction using the sternal skin and the outer table of the sternum transferred as a compound osteo-musculocutaneous flap based on the pectoralis major muscle. The technique has been used for immediate reconstruction at the time of cancer ablation but it can equally well be used as a delayed reconstructive procedure.
