[A new diagnostic approach in lymphangiomatosis. Apropos of a case].

Lymphangiomyomatosis is a rare disease affecting exclusively women usually in child-bearing age. It is characterized by proliferation of an atypical smooth muscle involving the pulmonary, mediastinal and retroperitoneal lymph system. The case reported here concerns a 53-year old woman presenting with an interstitial pulmonary lesion giving a characteristic image at CT. The patient also had recurrent chylothorax and a renal angiomyolipoma explored by CT and magnetic resonance imaging. The diagnosis of lymphangiomyomatosis was made on a cluster of arguments, without having recourse to surgical lung biopsy.

[Transrenal ureteral occlusion using detachable balloons. An alternative to cutaneous ureterostomy in the treatment of vesicovaginal fistula].

The treatment of urinary fistulae is particularly delicate when they occur in the course of a neoplastic disease. The development of less invasive, but variably successful, percutaneous techniques seems essential in these advanced patients. The authors report a case of ureteric occlusion by releasable balloon allowing the simple and rapid cure of a very disabling vesico-rectal-vaginal fistula associated with recurrent pelvic tumour.

Primary lung cancer staging: prospective comparative study of MR imaging with CT.

Forty-four patients with bronchogenic carcinoma were studied prospectively by both computed tomography (CT) and magnetic resonance (MR) imaging of the thorax during the week preceding thoracotomy. Transaxial MR imaging sequences included T1- and T2-weighted sequences. Coronal and sagittal T1-weighted sequences were added according to tumor location. CT and MR studies were reviewed separately, and the results were compared with surgical and pathologic findings on the basis of TNM classification. No statistically significant differences were found between the two imaging methods for the evaluation of tumor extent or node involvement. T2-weighted sequences did not yield further information on tumor extent or node involvement. Additional imaging planes (coronal or sagittal) appeared useful to study chest wall invasion. Analysis of concordances and discordances did not indicate whether one modality could be substituted for the other, or which diagnostic strategy might be more useful.

[Pulmonary digital subtraction angiography. A comparative study of 2 technics. Electrocardiographic servo-assistance versus 3 images per second].

A prospective study in 60 consecutive patients evaluated gain in quality of image using ECG servo-assistance during pulmonary digital subtraction angiography (PDSA). Two groups of 30 comparable patients were randomly allocated to examination with ECG servo-assistance or three images per second technique. Criteria for assessment of quality of image were defined and used to compare results. No significant difference were noted and ECG servo-assistance failed to improve images during PDSA.

[Magnetic resonance imaging in the study of pulmonary hilar involvement].

A retrospective analysis of results of magnetic resonance imaging (MRI) was conducted in 28 patients with uni- or bilateral pulmonary hilum lesions and findings compared with those of computed tomography (CT). All abnormal hilar masses examined were between 1 and 5 cm in diameter and were easily detected by transverse axial imaging. Contrast of their images was very high in relation to hilar bronchovascular elements in the two types of spin echo sequence with a short TR (0.5-0.7 s) and a long TR (1.5-2 s). Differentiation between tumoral or adenopathies and vessels was simpler than with CT with intravenous contrast. For screening of possible extension of hilar tumors. MRI appears to be superior to CT for providing data on vascular relations of lesions but inferior to CT for determining bronchial relations. Mediastinal extension of hilar tumors is easily defined by the short TR sequence but MRI supplies data of greater value than CT only in a low proportion of cases and then partly as a result of complementary coronal and sagittal imaging.

[Radiologic aspects of acute invasive pulmonary aspergillosis in 18 immunodepressed patients].

A retrospective study of 18 immunodepressed patients with acute invasive pulmonary aspergillosis enabled the radiologic picture of this increasingly frequent opportunist affection to be described, and different manifestations of the disease as a function of underlying conditions to be determined. Two groups could be distinguished: the first of patients with malignant blood diseases (n = 10) when the "aplasia" factor was dominant; the second of patients without blood diseases (n = 8) when the immunodepression was usually induced by a recent increase in corticoid therapy. Neutropenia was not a finding in any of these patients. The most frequent radiologic finding was single or multiple round excavating foci, these corresponding to zones of bronchopneumonia containing aspergillus filaments with central tissue necrosis forming the "target" lesion. The aplasia was distinguished by the multiplicity of lesions and their excavation following aplasia in the shape of a crescent due to sequestrum formation. Prognosis was related to severity of subjacent lesions. In the 2nd group, lesions could be single and nodular; prognosis was improved by the possibility of reducing immunodepression factors.

[Role of thoracic x-ray computed tomography in the pretherapeutic evaluation of bronchial cancer. A prospective study of 60 patients].

The value of computerized tomography (CT) was compared with that of conventional exploratory methods (standard radiology, bronchial tomography and endoscopy) in 60 patients with non-anaplastic bronchial cancer evaluated preoperatively. After CT examination thoracotomy was avoided in 14 (23%) patients with local or distal extension. CT proved more sensitive than conventional methods in the assessment of pleural and/or parietal extension, and of direct or lymph-node mediastinal invasion (67% vs 33%, 50% vs 33%, 64% vs 45% respectively). Similar results (75%) were obtained in cases with hilar lymph-node involvement. However, caution must be exerted before excluding thoracotomy, since CT may give false-positive results.

[Percutaneous drainage of intra-abdominal abscesses guided by real-time ultrasonography].

From October 1982 to October 1984, a percutaneous drainage under realtime ultrasound guidance was performed in 53 patients with abdominal abscesses. The location of the abscesses was subphrenic (23), retroperitoneal (16), and intrahepatic (14). A safe access route was found by using ultrasound and fluoroscopy in 53 out of 55 patients (96 p. 100). Percutaneous drainage failed in 8 patients and 3 of these patients died. The causes of death were: cerebral abscess (1), renal failure after surgery for correction of a duodenal fistula (1), and pancreatic abscess (1). The other five patients were cured by surgical drainage. Two complications were observed: one case each of pneumothorax and purulent peritonitis. Forty-five patients were healed by percutaneous drainage without operation. The duration of the catheter drainage was 14 days +/- 13 (m +/- 1 SD). Our results suggest that percutaneous drainage under realtime ultrasound guidance is an efficient and safe way to treat abdominal abscesses.

[Legionnaires' disease: 21 cases observed over 2.5 years in a Parisian respiratory intensive care unit].

The incidence of Legionnaire's disease is probably underestimated in France. Its clinical presentation is very suggestive, especially after the failure of a 48 hour therapeutic trial of beta-lactams, when a pneumococcal infection is initially suspected. This one sign is sufficient to orient the diagnostic survey and constitutes an indication for a therapeutic trial of macrolides for at least 72 hours. In fact, the delay in the diagnosis appears to be the determinant factor in the fatal outcome of the disease.

[Sclerosis of varicoceles. Apropos of 132 cases].

Endovascular sclerosis of the spermatic veins would seem to be the first step in the management of varicoceles, if these are considered to be responsible for hypofertility. In a series of 132 cases, the success ratio was 94 per cent on the right and 96 per cent on the left. The recurrence rate of 2 to 4 per cent would seem lower than in the case of surgical treatment (6 to 10 per cent). Moreover, the social and economic price of this ambulatory treatment is far lower. Surgical management would thus seem only indicated in the event of failure, and in the event of anatomical variations in the veins.
