The effect of cholecystectomy on the induction of colorectal tumors in mice by 1,2-dimethylhydrazine.

The influence of previous cholecystectomy (CCE) on the 1,2-dimethylhydrazine (DMH)-induced colorectal tumorigenesis was examined in 497 female NMRI mice after 14, 20, 24, 28, 33, and 36 weeks of induction time. There were only minor differences between the cholecystectomized and the non-cholecystectomized animals with regard to the percentage of tumor-bearing animals, number of adenomas, and location of the tumors. Only after an induction time of 20 weeks, the operated group had significantly more carcinoma-bearing animals than the non-operated group (P less than 0.01, chi 2-test). The implication of these results on known data of epidemiologic studies in human colorectal cancer are discussed.

[Cholecystectomy, cholelithiasis and cancer of the large intestine].

In a retrospective post-mortem study the connection between the post-cholecystectomy state, cholelithiasis and carcinoma of the large intestine was evaluated in 333 cases and compared with an age and sex matched control group and a group with gastric carcinoma. It was established that cholelithiasis (without cholecystectomy or contracted gallbladder) occurred significantly more frequently (24.9%) in patients with colonic carcinoma than in the control group (15.3%) or the gastric carcinoma group (12.6%) (P less than 0.001). In contrast the rate of cholecystectomy in patients with colonic carcinoma was not significantly increased (6.3%) when compared with both control groups (4.6 and 4.8%). Separate assessment of males and females showed that females had cholecystectomy and contracted gallbladder significantly more frequently (P less than 0.05). These figures suggest that the clinically observed association between cholecystectomy and colonic carcinoma reflects only a - not necessarily causative - connection between gallstone disease and colonic carcinoma.

[Clinical findings in thoracal functional disturbances (author's transl)].

Functional disorders of the thoracal motor structures show connections with visceral functions through the different forms of pain symptoms, but also not seldom without these, or else appear as their precursors. By means of conventional examination parameters, corresponding interconnection procedures could be demonstrated and the theoretical models for further discussion of the segmental reflector complexes are substantiated.

Nerve cells of the rabbit, cat, monkey and human caudate nucleus: a Golgi-study.

The Golgi architecture of the nucleus caudatus in rabbit, cat, monkey and human material was analysed. The brains were prepared by Golgi--Kopsch impregnation. The fairly divergent types of neurons practically belong to two main groups: projective or efferent neurons and local- or interneurons. The efferent neurons -- as it has been confirmed by experimental studies -- establish far contacts with various regions of the brain. According to the Golgi picture, their axon emits several collaterals, which produce local connections. The interneurons have only locally arborizing axon. The are mainly responsible for intrinsic connections of the nucleus. In both main groups of caudate neurons, several types and subtypes were observed. The number of the types and subtypes of the neurons show an increasing tendency in the phylogenetic series.

Fibrous histiocytoma: an analysis of the storiform pattern.

A storiform pattern is an important structural feature of fibrous histiocytoma (FH). In this analysis reconstructions of histological patterns were carried out from drawings. Highly FH with only a suggestion of storiform pattern and small star-formations were seen, there were other lesions with more pronounced fiber formation which showed more distinct and larger storiform stars. These structures can frequently be followed for only 3 sections of 5 micron thickness. In contrast to leiomyomas or meningioma, no regular or consistent orientation of these structures with respect to vessels is evident. Storiform structures apparently develop at the periphery of adjacent proliferating cells groups. They show a typical and diagnostically significant histological pattern, which was found to some degree in all FH examined.

Early sequential lesions during development of experimental gastric cancer with special reference to dysplasias.

The early sequential development of gastric cancer was studied with experimental animals and examined with respect to what conclusions can be drawn for understanding carcinogenesis in man. After limited oral administration of N-methyl-N'nitro-N-nitrosoguanidine to 174 rats carcinomas developed in most cases directly from the otherwise unchanged mucosa through various successive stages of transformation, without passing through a benign-appearing proliferative or neoplastic epithelial lesion. Focal dysplasia grade I was the first recognizable change observed by light microscopy, followed by dysplasia grade II, and subsequently dysplasia grade III. In spite of very similar morphological characteristics, the experimentally induced dysplasias cannot be simply equated in their etiology and biological behavior with the dysplasias of the human stomach. Dysplasias of grade I and II commonly found in man are usually associated with a chronic gastritis; they are located in the upper third of the mucosa and are for the most part reversible. The experimental dysplasias occuring in the proliferative zone of an otherwise undisturbed mucosa must be considered potentially premalignant, as they are irreversible and develop progressively. This finding points out that in man dysplasias grade III within the regenerative zone of non-inflammatory mucosa should be considered particularly as possible precursors of gastric carcinomas.

[Pathology of colonic polyps].

Recent developments in the field of polyps of the colon are discussed. The WHO classification represents a definite improvement. Apart from standardization, non-neoplastic polypoid changes of varying etiology and hamartomatous polyps are clearly distinguished from neoplastic, non-malignant adenomas. The various changes are outlined briefly in their essential characteristics. Clear-cut differentiation from early invasive cancer is of considerable importance for practical therapeutic purposes. Focal carcinoma within an adenoma is referred to only if invasion through the muscularis mucosae is established, while all other changes confined to the mucosa are termed adenomas with focal epithelial atypia. With regard to the relation between adenoma and cancer of the colon, recent results on topographical distribution of adenomas through the colon are of considerable significance. From serial studies of biopsy and autopsy specimens on the one hand, and from assessment of colon carcinoma incidence compared with mortality rates in carcinomas of different localizations on the other, it is evident that special attention should be given henceforward to adenomas, possible precursors, and carcinomas in the upper segments of the colon.

A rationale for treating hepatic adenoma.

Hepatic adenomas are being seen in young women taking oral contraceptives with alarming frequency. In the United States it is estimated that at least five million women are taking oral contraceptives. Physicians should consider hepatic adenoma in the differential diagnosis of all menstruating females with right upper quadrant pain. An early diagnosis and proper treatment could be life-saving.
