[Importance of plasma (histamine and tryptase) and urinary (methylhistamine) in peri-anesthetic anaphylactic and/or anaphylactoid reactions].

Histamine and tryptase, released during anaphylactoid reactions in anaesthesia, can be measured out by radioimmunoassay, provided that their own pharmacokinetic is respected. For two years, we have used sample kits in order to realize the measuring out of these mediators. The aim of this study was to evaluate the interest of these mediators within investigational procedures for anaphylactoid reactions. Eleven anaphylactoid reactions were observed (0,03%). The early blood samples (the first ten minutes following onset of the reaction) were made only in 36% of the cases. Within the serious reactions (grade III), the raising of tryptase indicates the involvement of mast-cell activation. Within minor clinical reactions (grade I), plasma histamine and urinary methylhistamine were the only mediators detected. In an anaphylactic reaction of grade II, which happened after the administering of vecuronium, tryptase was not detected. Therefore, these mediators give the anaesthetists the possibility to prove quickly the severity of the reactions and to direct the investigations very early towards the right way.

[Western blotting or immunoblotting: application of the Alastat-Alablot to the study of cross reactions between cow's milk and goat's milk].

The Alastat-Alablot was used as immunoblot technique for detecting cross-reactivity between CM and GM. The study was conducted on 9 patients presenting with a food allergy well-documented on the basis of clinical history and elimination diet and re-exposure tests. The Alastat-Alablot technique proved to be sensitive in detecting low levels of specific IgE not detectable by the Cap System. The most frequently encountered proteins that are shared between CM and GM have a molecular weight corresponding to beta-lactoglobulin and casein in the majority of patients.

Quantification and molecular analysis of plasmatic HLA-BCw molecules with a locus B and Cw HLA specific mAb (B1.23.2 mAb).

Plasmatic HLA class I molecules were analysed quantitatively and qualitatively with a B and Cw locus-specific mAb, B1.23.2. sHLA-BCw were detected among all individuals with a wide range of concentration. The HLA-A29(19) allele was significantly associated with an increased sHLA-BCw mean concentration. Three different isoforms of relative molecular weights of 44, 39 and 36kDa were detected. The 39 kDa class I heavy chain was not detected after immunoprecipitation with mAb B1.23.2 in 17% of tested plasma, but was immunoprecipitated with W6/32 mAb.

[Occupational eosinophilic lung in a grape grower: role of sulfites].

Observation of a patient with a history of asthma who presented a lung pathology in a context linked with the general condition, radiology of a biological inflammatory syndrome and hypereosinophilia, suggested a diagnosis of Chronic Pneumopathy of Eosinophils (PCE). The chronology of the symptomatology favours an occupational role of sulphites, to which the patient is exposed by the respiratory pathway and the reproduction of the symptoms in an identical exposure confirmed the hypothesis.

[Allergy associated with pepper and latex: new cross reaction?].

The aim of this study was to point out the frequency of associated sensitization to rubber latex and to sweet pepper. The study included three populations of patients: Patients allergic to rubber latex, patients sensitized to rubber latex and patients without any clinical symptoms for rubber latex and sweet pepper (negative controls). We pointed out a high frequency of patients sensitized to sweet pepper among the patients allergic or sensitized to rubber latex which lead to the hypothesis of a cross reaction between rubber latex and sweet pepper.

[The pork-cat syndrome or crossed allergy between pork meat and cat epithelia (2)].

Is there a port-cat syndrome in food allergy? Many clinical observations have revealed a frequent association between allergy to cat epithelia in subjects who have allergy to pork meat. This second part, from clinical tests such as skin tests and laboratory tests such as CAP RAST, electrophoresis, Western blot and chromatography, confirms the existence of a crossed reaction between pork meat and cat extract. This crossed reaction is linked with a common epitope. A common protein has been found that has a molecular weight of 67,000 d. for subjects who are sensitive to cat extracts and pork meat.

[Latex and pollinosis].

Through the multidetection test for specific IgE to pneumoallergens (Aero-Matrix Plus, Abbott) our attention has been drawn to polysensitization to pollens. Amongst these pollen tests, plantain seems to be frequently associated with sensitization to latex. The aim of this work at first, is therefore to verify the reality of the frequency of the association between latex and plantain sensitization. However, the conclusions are difficult, since there are no patients who are only sensitive to plantain as sensitivity to this allergen is always found in a pollen polysensitization. at least associated with grass pollens; at most associated with a number of other pollens. The double profile (perhaps plantain + / grass pollen +, or perhaps plantain + / grass pollen + / other pollens +) is determinant in prediction of sensitivity to latex, which is found essentially in the group plantain + / grass pollen + / other pollens +. This study has also shown that amongst the population who have specific IgE to latex (IgEs) (sensitized or allergic): the patients who have a clinical history of allergy to latex have no profile of pollen polysensitization; the non-allergic but sensitive patients (IgE positive, but no clinical signs on contact with latex) show an extreme pollen polysensitization. This poses a problem of different antigenic determinants or perhaps two different pathways to sensitization to latex.

[The pork-cat syndrome: RAST inhibition test with Feld One].

This work confirms that the common allergen in subjects who are doubly sensitized to cat and pork is quite different from that of subjects who are mono-sensitized to cat. The common protein is Albumin MW 67 Kda in the pork/cat syndrome, while the major allergen Feld One is characteristic in patients who are monosensitized to cat.

[Diagnostic value of the sulfite skin test].

Nearly 1,200 patients were patch tested with sulfites in Angers during the last two years. A positive patch test reaction was noticed in 2% of cases. The diagnosis of sulfites sensitivity was then established (or not) either with provocation test or sulfites withdrawal during four weeks. The result is a high specificity of the test, 12 patients among 13 having a positive patch test and a sulfite sensitivity.

[The pork-cat syndrome or crossed allergy between pork meat and cat epithelia (1)].

This work is a study of the frequency of the association between sensitivity to pork meat and cat epithelia. Comparison with a reference population that was not sensitized to pork meat, shows that this association is highly significant. This suggests therefore the possibility of a crossed allergenicity. If there is a real crossed allergenicity between pork meat and cat epithelia, we suggest that this crossed allergenicity is without doubt much greater and concerns more the meats and epithelia of mammals.

[Survey of pathologies caused by sulfites in allergology].

Increased use of sulphites as a preservative for foods and drugs is partly the origin of increasing prevalence of allergy to this anti-oxidant. All organs that are concerned with allergic problems may be affected, especially skin and the digestive tract. We have noted a frequent association between the pathology to aspirin (ORL, bronchial, skin) and that of sulphites: 30.8%. The history is fundamental and compensates for the lack of effectiveness of skin tests.

[Diagnostic criteria for latex allergy].

The author has made an observation which seems to illustrate his proposition perfectly. A patient who was the victim of many acute episodes of rhino-conjunctivitis and severe Quincke's oedema has found benefit from two specialised assessments which established a diagnosis of allergy only to latex. The key to this diagnosis rested on unusual reasoning based on a search for a sufficiently "virulent" allergen to be responsible for the severity of the clinical signs. This idea and recent understanding of crossed allergy materials should aid detection of subjects who are allergic to natural latex.
